Palliation of malignant rectal obstruction with self-expanding metal stents.

BACKGROUND: Surgical management of patients with metastatic or recurrent rectal cancer remains controversial. Self-expanding metal stents are increasingly used for palliative treatment of advanced tumors, although long-term results are not yet available. METHODS: Between 1996 and 2003, 521 patients underwent surgery for rectal neoplasms. In the same time period, self-expanding metal stents were used for palliation of 34 patients with malignant rectal obstruction and incurable disease. The outcome of the patients was analyzed retrospectively. RESULTS: Rectal stents were successfully placed in

Endoscopic Full-Thickness Resection for Gastrointestinal Subepithelial and Epithelial Neoplasia: Current Status, Indications, and Technical and Oncological Challenges-A Multispecialty Perspective.

Endoscopic full-thickness resection (EFTR) has emerged as a minimally invasive treatment for small gastrointestinal subepithelial neoplasms (SETs). In particular, the development of novel closure methods such as clip-line and various endoscopic suturing techniques has enabled the secure closure of any resultant defects. During the Advanced Therapeutic Endoscopy session at the Endoscopic Forum Japan 2025, we summarized the current status of EFTR for SETs and discussed future clinical applications for epithelial neoplasms from oncological and technical perspectives. Although predominantly docume

Identification of a Novel RUNX1::HMGN1 Fusion in Therapy Acute Myeloid Leukemia.

Therapy acute myeloid leukemia (AML) is a heterogeneous malignant hematopoietic disease that arises either from an antecedent hematologic disorder, including myelodysplastic syndromes, myeloproliferative neoplasms, aplastic anemia, or as a result of exposure to genotoxic chemotherapeutic agents or radiotherapy (therapy-related AML). In this study, we describe a case of therapy AML after treatment for breast, uterine, and rectal malignancies in a patient with a special fusion gene-RUNX1::HMGN1.

[A man with abdominal pain, diarrhoea and rectal bleeding].

A 49-year-old man came to the emergency room with abdominal pain, vomiting, diarrhoea and rectal bleeding. Both colonoscopy and CT of the abdomen showed cecocolic intussusception caused by an appendiceal mucocele. An ileocecal resection was performed via a laparoscopic approach and microscopy of the tissue showed a low-grade mucinous neoplasm.

Endoscopic fenestration in the diagnosis and treatment of delayed anastomotic submucosal abscess: A case report and review of literature.

BACKGROUND: Abscess formation is one of the complications after radical resection of rectal cancer; cases with delayed postoperative anastomotic abscess are rare. Here, we report a rare case of postoperative anastomotic abscess with a submucosal neoplasm appearing after rectal surgery. Ultimately, the patient was diagnosed and treated by endoscopic fenestration. In addition, we review the literature on the appearance of an abscess as a complication after rectal cancer surgery. CASE SUMMARY: A 57-year-old man with a history of rectal malignancy resection complained of a smooth protuberance nea

Rectal tonsil mimicking a rectal neuroendocrine tumor.

Rectal tonsil is an uncommon reactive lymphoid proliferation that may mimic a rectal neuroendocrine tumor endoscopically and a low-grade lymphoid neoplasm histologically. A 43-year-old woman underwent colonoscopy for abdominal pain, which revealed a smooth, hemispheric subepithelial lesion measuring approximately 22 mm in the lower rectum, 8 cm from the anal verge. Endoscopic ultrasonography demonstrated a well-demarcated hypoechoic lesion predominantly confined to the submucosal layer, with preservation of the muscularis propria. Because a rectal neuroendocrine tumor was the leading preoperat

Infectious proctitis due to syphilis and chlamydia: an exuberant presentation.

Infectious proctitis by sexually transmitted diseases are increasing in incidence and should be considered in men who have sex with men with rectal symptoms. In this case, we show an exuberant form of syphilitic and chlamydial proctitis that could be diagnosed as another anorectal disease such as inflammatory proctitis or even as a neoplasm if the clinical history is not taken into account.

Syphilitic proctitis mimicking rectal cancer: A case report.

Syphilitic proctitis is a rare disease. It usually presents as proctitis, ulcer and neoplasm but lacks pathognomonic clinical symptoms. It is, therefore, difficult to diagnose and is occasionally treated inappropriately. We report the case of a 51-year-old man who had a hard, ulcerated mass, which occupied the circumference of the rectal wall and which mimicked a rectal tumor. Fortunately, positive finding from routine toluidine red unheated serum test and treponema pallidum particle agglutination tests made us reevaluate the patient and led us to suspect syphilitic proctitis. This diagnosis w

[A rare cause of bleeding intestinal intussusception in adult: jejunal lipoma].

Small bowel neoplasms are usually diagnosed when patients are referred with complications such as hemorrhage or obstruction. Intestinal lipomas are the third most frequent benign tumors among all intestinal neoplasms and their definite diagnosis is usually obtained after histopathological examination of the resected specimen. A 76 year-old male patient with partial intestinal obstruction and rectal bleeding at admittance was found to have an abdominal mass on ultrasound. The density pattern of the mass at abdominal CT was supporting a jejunal lipoma leading an intussusception. Then an enterocl

Primary adrenal gland carcinosarcoma associated with metastatic rectal cancer: a hitherto unreported collision tumor.

In this report we describe the case of a young woman with familial adenomatous polyposis who developed metastatic rectal cancer during pregnancy. At diagnosis, we decided to perform a transabdominal laparoscopic adrenalectomy, because of the high risk of bowel obstruction, and to define the origin of the adrenal gland lesion, suspected to be primary on the basis of imaging results. The histological specimen showed a collision tumor between an adrenal metastasis of a rectal tumor and a primary adrenal gland carcinosarcoma. The peculiarity of the case is due not only to its clinical presentation

Incidental Rectal Neuroendocrine Tumor Detected During Screening Colonoscopy Managed With Transanal Minimally Invasive Surgery (TAMIS): A Case Report.

Rectal neuroendocrine tumors (NETs) are uncommon neoplasms that are increasingly detected due to the widespread adoption of colorectal cancer screening programs. They typically present as small submucosal nodules, facilitating early diagnosis and minimally invasive treatment. We present the case of a 58-year-old woman with a history of type 2 diabetes mellitus who underwent screening colonoscopy while asymptomatic. During the procedure, an 18-mm non-polypoid submucosal lesion was identified on the anterior rectal wall, 8 cm from the anal verge. Contrast-enhanced computed tomography of the ches

Rectal localization of metastatic lobular breast cancer: report of a case.

The incidence of extrahepatic gastrointestinal metastases from breast cancer is reported in the literature only as necroscopy studies (6-18 percent); they usually originate from lobular or a mixed ductal-lobular subtype. Nonspecific presenting symptoms, death of the patients caused by other more frequent metastases, and variable radiographic features mimicking primary neoplasms cause a clinical underestimation of this pathology. We report here a case of rectal metastasis from a lobular carcinoma eight years after mastectomy.

Endoscopic Ligation with O-Ring Closure for Mucosal Defects after Rectal Endoscopic Submucosal Dissection: A Feasibility Study (with Video).

INTRODUCTION: Mucosal defect closure after colorectal endoscopic submucosal dissection (ESD) may prevent post-ESD adverse events. Delayed bleeding is a particular concern in the rectum due to the presence of numerous blood vessels. However, rectal defect closure often fails due to the thick rectal wall. This study aimed to examine the feasibility of our newly developed endoscopic ligation with O-ring closure (E-LOC) for defects after rectal ESD. METHODS: This was a prospective observational study conducted at a single institution. After excluding 2 patients with tumors mostly extending into t

Late Recurrence Matters: Delayed Hepatic Metastasis After Rectal Neuroendocrine Tumor.

Rectal neuroendocrine tumors (NETs) are rare, typically indolent neoplasms arising from enterochromaffin cells that may recur locally or distantly after long disease-free intervals. We report a case of robotic partial hepatectomy for delayed hepatic metastasis from a rectal NET. Methods: A 68-year-old woman was referred for evaluation of a liver lesion suspicious for metastatic rectal NET. Ten years earlier, a 4 mm rectal NET had been identified on screening colonoscopy and treated with endoscopic mucosal resection, with no residual malignancy. Surveillance endoscopy showed no recurrence. Ten

Robotic transanal minimally invasive surgery (R-TAMIS): current evidence in the treatment of early rectal neoplasia.

INTRODUCTION: Robotic transanal minimally invasive surgery (R-TAMIS) was introduced in 2012 for the excision of benign rectal polyps and low grade rectal cancer. Ergonomic improvements over traditional laparoscopic TAMIS (L-TAMIS) include increased dexterity within a small operative field, with possibility of better surgical precision. We aim to collate the existing data surrounding the use of R-TAMIS to treat rectal neoplasms from cohort studies and larger case series, providing a foundation for future, large-scale, comparative studies. METHODS: Medline, EMBASE and Web of Science were search

Cancer-Associated Mutations in Normal Colorectal Mucosa Adjacent to Sporadic Neoplasia.

INTRODUCTION: Colorectal cancer arises in a multistep process of carcinogenesis from normal mucosa. The earliest precursor might be a morphologically inconspicuous precancerous field, harboring cancer-associated mutations. METHODS: We systematically analyzed genetic alterations in 77 tissue samples from 30 patients with sporadic colorectal neoplasms (18 large adenomas and 12 adenocarcinomas) and matched adjacent normal mucosa (N = 30), as well as normal rectal tissue (N = 17). We profiled mutations associated with colorectal cancer by targeted sequencing of 46 genetic loci using 157 custom am

Value of a risk questionnaire in screening for colorectal neoplasm.

The value of a postal questionnaire and of Hemoccult II (Smith Klein Diagnostic Inc., Sunnyvale, California, USA) testing in screening for colorectal neoplasms was compared. In the questionnaire, the subjects were asked about previous treatment for colorectal neoplasm and rectal bleeding during the previous 6 months, specified as to type. All participants were asked to perform Hemoccult II blood testing over 3 days. Of 13,759 randomly selected subjects 9040 (66 per cent) performed the test and returned the questionnaire.
