Evidence-Based Behavioral Health in Value-Based Primary Care: Aligning Incentives to Accelerate Adoption of Collaborative Care.

Behavioral health (BH) is a critical component of value-based primary care. However, access to BH services is a challenge, with a limited, geographically concentrated supply of providers. The Collaborative Care Model (CoCM) is an evidence-based, cost-effective way for value-based primary care leaders to deliver high-quality BH services. Despite a robust clinical and implementation science evidence base, real-world adoption of CoCM remains limited. A key barrier to CoCM adoption is a one-size-fits-all fee-for-service reimbursement methodology, which fails to adequately capture the value of CoCM

Investing in CenteringPregnancy™ Group Prenatal Care Reduces Newborn Hospitalization Costs.

OBJECTIVES: CenteringPregnancy™ group prenatal care is an innovative model with promising evidence of reducing preterm birth. The outpatient costs of offering CenteringPregnancy pose barriers to model adoption. Enhanced provider reimbursement for group prenatal care may improve birth outcomes and generate newborn hospitalization cost savings for insurers. To investigate potential cost savings for investment in CenteringPregnancy, we evaluated the impact on newborn hospital admission costs of a pilot incentive project, where BlueChoice Health Plan South Carolina Medicaid managed care organizati

The impact of removing financial incentives and/or audit and feedback on chlamydia testing in general practice: A cluster randomised controlled trial (ACCEPt-able).

BACKGROUND: Financial incentives and audit/feedback are widely used in primary care to influence clinician behaviour and increase quality of care. While observational data suggest a decline in quality when these interventions are stopped, their removal has not been evaluated in a randomised controlled trial (RCT), to our knowledge. This trial aimed to determine whether chlamydia testing in general practice is sustained when financial incentives and/or audit/feedback are removed. METHODS AND FINDINGS: We undertook a 2 × 2 factorial cluster RCT in 60 general practices in 4 Australian states tar

Increasing the Delivery of Preventive Health Services in Public Education.

The delivery of prevention services to children and adolescents through traditional healthcare settings is challenging for a variety of reasons. Parent- and community-focused services are typically not reimbursable in traditional medical settings, and personal healthcare services are often designed for acute and chronic medical treatment rather than prevention. To provide preventive services in a setting that reaches the widest population, those interested in public health and prevention often turn to school settings. This paper proposes that an equitable, efficient manner in which to promote

Bundled-rate legislation for Medicare reimbursement for dialysis services: implications for anemia management with ESAs.

With the incidence of ESRD on the rise, there is a continuing need to control anemia-related treatment costs in dialysis patients receiving reimbursement through Medicare. Currently, erythropoiesis-stimulating agents (ESAs) are billed separately from dialysis services, potentially creating little financial incentive for more efficient use. The Medicare Improvement for Patients and Providers Act, passed by the U.S. Congress in July 2008, includes provisions intended to address this concern. Under this act, dialysis services will be reimbursed using a fully bundled, comprehensive payment system

Challenges and progress in access to rare cancer drugs in China: Insights from clinical trials, availability and affordability.

This study evaluated the development and accessibility of targeted therapies for 23 rare cancers in China. Clinical trials increased between 2013 and 2025 but have recently plateaued, with 12% terminated or suspended. Domestic enterprises now dominate trial activity, while trials of imported drugs have declined. As of 31 December 2025, 63 targeted-therapy indications had been approved globally, compared with 40 indications for 15 rare cancers in China, and eight rare cancers still lacked targeted treatments. The median monthly treatment cost was US$2165, substantially exceeding monthly disposa

Longitudinal Reliability of Rheumatology Patient Safety Measures and Implications for CMS Case Minimums.

BACKGROUND: Metrics with acceptable reliability are necessary to ensure that quality measures reward performance. Low metric reliability may result from a lack of differences between reporting entities or low numbers of eligible patients. The Merit-based Incentive Payment System (MIPS) incentivizes high-performing practices to report, which may lower practice-to-practice variation and lower reliability. This study evaluated the impact of voluntary participation in MIPS and measure denominator counts on practice-specific reliability estimates. METHODS: Data came from the Rheumatology Informati

Assessing the uptake of incentivised physical health checks for people with serious mental illness: a cohort study in primary care.

BACKGROUND: People with serious mental illness are more likely to experience physical illnesses. The onset of many of these illnesses can be prevented if detected early. Physical health screening for people with serious mental illness is incentivised in primary care in England through the Quality and Outcomes Framework (QOF). GPs are paid to conduct annual physical health checks on patients with serious mental illness, including checks of body mass index (BMI), cholesterol, and alcohol consumption. AIM: To assess the impact of removing and reintroducing QOF financial incentives on uptake of t

The effect of pay for performance on risk incidence of hip fracture in type 2 diabetic patients: a nationwide population-based cohort study.

OBJECTIVES: Diabetes mellitus (DM) increases the risk of hip fracture. The literature rarely discusses the importance of pay-for-performance (P4P) programs for the incidence of hip fractures in patients with type 2 DM (T2DM). This study aimed to examine the impact of the P4P program on hip fracture risk in patients with T2DM. METHODS: This retrospective cohort study focused on data from T2DM patients aged 45 and older between 2001 and 2012. We continued to track these data until 2013. The data were collected from the National Health Insurance Research Database in Taiwan. To minimize selection

Impact of the HITECH financial incentives on EHR adoption in small, physician-owned practices.

BACKGROUND: Physicians in small physician-owned practices in the United States have been slower to adopt EHRs than physicians in large practices or practices owned by large organizations. The Health Information Technology for Economic and Clinical Health (HITECH) Act of 2009 included provisions intended to address many of the potential barriers to EHR adoption cited in the literature, including a financial incentives program that has paid physicians and other professionals $13 billion through December 2015. OBJECTIVE: Given the range of factors that may be influencing physicians' decisions on

Effect of nonpayment for hospital-acquired, catheter-associated urinary tract infection: a statewide analysis.

BACKGROUND: Most (59% to 86%) hospital-acquired urinary tract infections (UTIs) are catheter-associated urinary tract infections (CAUTIs). As of 2008, claims data are used to deny payment for certain hospital-acquired conditions, including CAUTIs, and publicly report hospital performance. OBJECTIVE: To examine rates of UTIs in adults that are coded in claims data as hospital-acquired and catheter-associated events and evaluate how often nonpayment for CAUTI lowers hospital payment. DESIGN: Before-and-after study of all-payer cross-sectional claims data. SETTING: 96 nonfederal acute care Mic

A Healthcare Finance Model for the United States: A Conceptual Workflow for Episode-Based Pricing, Risk Sharing, and Value-Based Accountability Using Bundled Payment as the Core Mechanism.

The United States healthcare system is increasingly moving toward bundled payments as a model for value-based accountability. This approach functions as a healthcare finance model in which the episode of care serves as the central unit of payment design. Rather than reimbursing individual services separately, it organizes costs, risk, incentives, accountability, and quality measurement around the full clinical episode, making the episode the core framework for both financial and care delivery decisions. Under traditional fee-for-service reimbursement, services are paid separately, which can fr

The Race to the Bottom: A High-Volume Hospital Experience With Comprehensive Care for Joint Replacement.

BACKGROUND: Comprehensive Care for Joint Replacement (CCJR) is a value-based system designed to increase quality and decrease cost for the total joint arthroplasty (TJA) episode of care. The creation of this program created financial incentives and risk for both hospitals and physicians during the TJA episode of care. Herein, we describe our results during the program. METHODS: We created a CCJR team consisting of care coordinators, nurse navigators, physicians, and a financial team. Quarterly meetings were instituted to monitor for changes and review outliers. Multiple subgroups were created

End-Stage Renal Disease Treatment Choices Model and Use of Home Dialysis and Kidney Transplant.

IMPORTANCE: To increase the use of home dialysis and kidney transplant, the Centers for Medicare & Medicaid Services launched the End-Stage Renal Disease Treatment Choices (ETC) model, a mandatory, randomized pay-for-performance program applied to 30% of US hospital referral regions. Its impact after 4 years of implementation is uncertain. OBJECTIVE: To assess the ETC model's impact on home dialysis, kidney transplant, and transplant waitlist, as well as measure the rate of financial penalties. Improving Care for Critical Care Patients by Strategic Alignment of Quality Goals With a Physician Financial Incentive Model.

BACKGROUND AND OBJECTIVES: The passage of the Affordable Care Act has ignited a shift from the pay-for-performance model to value-based care with a particular relevance in critical care settings. Provider incentive programs are widely considered as a means to reward providers based on the achievement of preset quality metrics. This article aims to demonstrate the effects of a provider incentive program in the critical care delivery system in a large academic center in the Northeastern United States. METHODS: This article describes the results of a retrospective analysis of a performance-drive
