Optimization of glucocorticoid administration in patients with sepsis using reinforcement learning: a multicenter retrospective study.

BACKGROUND: The use of glucocorticoids in sepsis remains controversial due to heterogeneous treatment responses across patient populations. More individualized treatment strategies are needed to guide glucocorticoid administration. METHODS: An offline reinforcement learning (RL) model was developed using the MIMIC-IV database and externally validated in the eICU-CRD. Adult patients with sepsis were included, and clinical trajectories were constructed using 24-hour time steps. Least absolute shrinkage and selection operator (LASSO) regression was applied for feature selection, and missing data

Automatic treatment planning using reinforcement learning for high-dose-rate prostate brachytherapy.

Purpose. In high-dose-rate (HDR) prostate brachytherapy procedures, needle placement solely relies on physician experience. We investigated the feasibility of using reinforcement learning (RL) to provide needle positions and dwell times based on patient anatomy during pre-planning stage. This approach would reduce procedure time and ensure consistent plan quality.Materials And Methods. We trained an RL agent to adjust the position of one selected needle and all the dwell times on it to maximize a pre-defined reward function after observing the environment. The RL agent moves on to the next needl

Cognitive neuroscience can support public health approaches to minimise the harm of 'losses disguised as wins' in multiline slot machines.

Video slot machines are associated with both accelerated transition into problematic forms of gambling, as well as psychosocial harm above and beyond other forms of gambling. A growing body of evidence is uncovering how key design features of multiline slot machines produce an inflated experience of reward, despite the fact that these features offer no overall financial benefit to the player. A pernicious example of this are 'losses disguised as wins' (LDWs), which occur when simultaneous bets placed on multiple lines result in a winning combination that returns an amount greater than zero, bu

Dynamic changes of brain networks during feedback-related processing of reinforcement learning in schizophrenia.

Previous studies have reported that schizophrenia (SZ) patients showed selective reinforcement learning deficits and abnormal feedback-related event-related potential (ERP) components. However, how the brain networks and their topological properties evolve over time during transient feedback-related cognition processing in SZ patients has not been investigated so far. In this paper, using publicly available feedback-related ERP data which were recorded from SZ patients and healthy controls (HC) when they performed a reinforcement learning task, we carried out an event-related network analysis

ZEST: Zero-shot embodied skill transfer for athletic robot control.

Achieving robust, humanlike whole-body control on humanoid robots for agile, contact-rich behaviors remains a central challenge, demanding heavy per-skill engineering and a brittle process of tuning controllers. We introduce ZEST (zero-shot embodied skill transfer), a streamlined motion-imitation framework that trains policies via reinforcement learning from diverse sources-high-fidelity motion capture, noisy monocular video, and non-physics-constrained animation-and deploys them to hardware zero-shot. ZEST generalizes across behaviors and platforms without relying on contact labels, reference

De novo covalent drug generation with enhanced drug-likeness and safety.

Covalent drugs have long played an essential role in therapeutics, yet computational design approaches remain largely confined to virtual screening of existing libraries. Despite recent advances in deep generative models for drug discovery, methods specifically tailored to de novo covalent drug generation are still lacking. Here we introduce CovaGEN, a conditional latent diffusion framework for the de novo design of covalent inhibitors with enhanced drug-likeness and safety. CovaGEN generates ligands from a drug-like latent space while conditioning on target sequences and employing a classifie

AutoPlan: Autonomous Surgical Planning Framework for Neurosurgery Interventions With Steerable Needles.

BACKGROUND: Accurate path planning for minimally invasive neurosurgery is necessary to navigate complex anatomy that contains critical structures, such as blood vessels, ventricles and white-matter tracts. Planning with nonholonomic kinematics is difficult, and typical planners fail to generate kinematically feasible paths. METHODS: AutoPlan is a deep reinforcement learning (DRL) framework that generates a needle trajectory on a magnetic resonance imaging (MRI) patient dataset. AutoPlan implements kinematic control strategies, variable curvature with fixed insertion (VCFI) and variable insert

Distributed cortical learning through LEC-mediated γ-synchrony.

Despite the well-established theoretical and experimental foundations of dopamine-driven reinforcement learning, how the reward prediction error (RPE) teaching signal modifies specific cortical memory networks remains unclear. Neural oscillations are crucial for the temporal binding of activities across distributed cell ensembles. Here, we identify that mouse lateral entorhinal cortex layer 5 (LEC5) mediated intercortical γ-synchrony is a neural correlate of the RPE signal derived from dopamine neurons in the ventral tegmental area (VTADA). The VTADA-LEC5 circuit-based intercortical γ-synchron

AdaPR: Adaptive Plane Reformatting for 4D flow MRI using deep reinforcement learning.

BACKGROUND AND OBJECTIVE: Plane reformatting for Four-dimensional Phase Contrast Magnetic Resonance Imaging (4D flow MRI) is time-consuming and prone to inter-observer variability, which limits fast cardiovascular flow assessment. Deep reinforcement learning (DRL) trains agents to iteratively adjust plane position and orientation, enabling accurate plane reformatting without the need of detailed landmarks, making it suitable for images with limited contrast and resolution, such as 4D flow MRI. However, current DRL methods assume that test volumes share the same spatial alignment as training da

CNRein: an evolution-aware deep reinforcement learning algorithm for single-cell DNA copy number calling.

Low-pass single-cell DNA sequencing technologies and algorithmic advancements have enabled haplotype-specific copy number calling on thousands of cells within tumors. However, measurement uncertainty may result in spurious CNAs inconsistent with realistic evolutionary constraints. We introduce evolution-aware copy number calling via deep reinforcement learning (CNRein). Our simulations demonstrate CNRein infers more accurate copy-number profiles and better recapitulates ground truth clonal structure than existing methods. On sequencing data of breast and ovarian cancer, CNRein produces more pa

ABC'S of AI: proposing a clinician's framework for the interpretation of artificial intelligence studies.

Artificial intelligence (AI) and machine learning have emerged as transformative analytic methods capable of identifying patterns and proposing treatment strategies from complex, heterogeneous data generated in patient care settings. However, translating reports of AI tools into clinical decision-making requires careful interpretation and contextualization. Here, we present a structured framework (ABCDEFG framework) for clinicians to critically appraise studies reporting AI tools using a high-profile reinforcement learning analysis of vasopressin initiation in septic shock as a case example. [A Review on Automatic Detection Algorithm for Patient-Ventilator Asynchrony during Mechanical Ventilation].

This study summarizes the application of automatic recognition technologies for patient-ventilator asynchrony (PVA) during mechanical ventilation. In the early stages, the method of setting rules and thresholds relied on manual interpretation of ventilator parameters and waveforms. While these methods were intuitive and easy to operate, they were relatively sensitive in threshold setting and rule selection and could not adapt well to minor changes in patient status. Subsequently, machine learning and deep learning technologies began to emerge and develop. These technologies automatically extra

A safe-enhanced fully closed-loop artificial pancreas controller based on deep reinforcement learning.

Patients with type 1 diabetes and their physicians have long desired a fully closed-loop artificial pancreas (AP) system that can alleviate the burden of blood glucose regulation. Although deep reinforcement learning (DRL) methods theoretically enable adaptive insulin dosing control, they face numerous challenges, including safety and training efficiency, which have hindered their clinical application. This paper proposes a safe and efficient adaptive insulin delivery controller based on DRL. It employed ten tricks to enhance the proximal policy optimization (PPO) algorithm, improving training

Machine learning as an artificial intelligence application in management of chronic hepatitis B virus infection.

Let's review the role of gut microbiota in pathogenesis of chronic hepatitis B infection as addressed in by Zhu et al. Zhu et al used high-throughput technology to characterize the microbial ecosystems, which led to an explosion of various types of molecular profiling data, such as metagenomics, metatranscriptomics, and metabolomics. To analyze such data, machine learning (ML) algorithms have shown to be useful for identifying key molecular signatures, discovering potential patient stratifications, and, particularly, for generating models that can accurately predict phenotypes. Strong evidence

Machine-Learning-Driven Discovery of N-Phenylbenzenesulfonamides as a Novel Chemotype for Lactate Dehydrogenase A Inhibition with Anti-Pancreatic Cancer Activity.

Lactate dehydrogenase A (LDHA) is a promising target for cancer therapy due to its crucial role in aerobic glycolysis. Despite extensive efforts, the structural diversity of LDHA inhibitors remains limited. Here, we utilized machine learning techniques, particularly reinforcement learning models, along with molecular dynamics simulations and biological validation, to identify N-phenylbenzenesulfonamides as a novel chemotype for LDHA inhibition. Compound 9 was generated and identified as a hit (IC50 = 720 nM), with its sulfonamide moiety forming crucial hydrogen-bonding interactions with LDHA.
