Laser assisted tooth replantation case report.

BACKGROUND AND AIM: Although intentional replantation for extraoral treatment is a solution for complicated endodontic cases, it is accompanied with risk of root resorption which is most likely due to extraction trauma and infected remnants. Laser therapies have long been proved for bactericidal and biostimulation effects. In the present case, a pulpitis combined severe periodontal destruction molar was extracted with an Er:YAG laser for thorough degranulation and disinfection. Before the extraction and right after the treatment, low level laser therapy (LLLT) with 810nm diode was applied for

Periodontal tissue engineering after tooth replantation.

BACKGROUND: Blood-derived products, platelet-poor plasma (PPP) and platelet-rich plasma (PRP), constitute an approach in the enhancement of tissue healing. PRP has also been used as a scaffold for bone marrow stem cells in tissue engineering. This study evaluates the effect of PPP, calcium chloride-activated PRP (PRP/Ca), calcium chloride- and thrombin-activated PRP (PRP/Thr/Ca), and bone marrow mononuclear cells and PRP/Ca (BMMCs/PRP/Ca) on the healing of replanted dog teeth. METHODS: After 30 minutes of extraction, teeth were replanted with 1) no material (control); 2) PPP; 3) PRP/Ca; 4) PR

Dermal pocketing following distal finger replantation.

Replantation is an ideal technique for reconstruction following fingertip amputation as it provides 'like for like' total reconstruction of the nail complex, bone pulp tissue and skin with no donor-site morbidity. However, fingertips are often not replanted because veins cannot be found or are thought to be too small to repair. Attempts at 'cap-plasty' or pocketing of replanted tips with and without microvascular anastomosis have been done in the past with varying degrees of success. We prospectively followed up a group of patients who underwent digital replantation and dermal pocketing in the

Long-term results of transmetacarpal replantation.

UNLABELLED: The aim of this study was to evaluate subjective, functional and occupational outcome after transmetacarpal amputations. PATIENTS AND METHODS: Forty-three patients (mean age 34, range 1-71) with a transmetacarpal injury devitalizing three or more finger rays were treated with revascularization (22 patients) or replantation (21 patients). Most of the injuries (60%) were caused by a circular saw. Altogether 174 rays were injured of which 14 were doomed unsalvageable. An attempt was made to save the rest (n=160). Metacarpals were shortened on average 8 (range 0-22)mm. The number of a

Actualities in big segments replantation surgery.

Replantation of an amputation is no longer a difficult technical problem. Indeed, the experience gathered over the last few decades, right from the first concepts posed by the pioneers up to the present era and the improved technical aids, all go to suggest that the majority of amputated segments may now be reconstructed. However, what we really want from a replant is not just survival but function. Indications for replantations must follow careful and objective patient selection together with the evaluation of type and site of lesion and possible complications. Furthermore, the important role

Single-tooth osteotomy for intention replantation.

The conventional replantation technique has many drawbacks. The tooth may break during extraction; thus, teeth with diverged or curved roots are not good candidates for such a procedure. The occurrence of root ankylosis after replantation, with subsequent resorption, does not permit the replanted tooth to support any additional load, such as an orthodontic appliance or to be used as a bridge abutment. The procedure described here is designed to eliminate these drawbacks. The technique is based on leaving the delicate periodontal ligament fibers intact and transferring the wound created to the

[Partial replantation following proximal limb injury].

PURPOSE OF THE STUDY: Proximal replantation is a technically feasible but life-threatening procedure. Indications must be restricted to patients in good condition with a good functional prognosis. The goal of replantation must be focused not only on reimplanting the amputated limb but also on achieving a good functional outcome. For the lower limb, simple terminalization remains the best choice in many cases. When a proximal amputation is not suitable for replantation, the main aim of the surgical procedure must be to reconstruct a stump long enough to permit fitting a prosthesis preserving th

Postoperative ramifications of total ear replantation.

Complete avulsion of the external ear, much less common than other minor ear trauma, presents a complex and difficult problem. The authors present a 35-year-old male cigarette smoker who suffered traumatic avulsion of the right ear, and describe the replantation procedure and the patient's postoperative course. Aesthetic positioning of the replanted ear, treating the external auditory meatus, protecting the ear against exposure to the sun, and counseling the patient about issues related to sensation are important factors in the care of patients who have undergone such a procedure.

Above-elbow arm replantation during pregnancy.

We report a successful replantation of an above-elbow amputated left arm in a 26-year-old female with a good, functional, long-term result. The patient was at 18 weeks estimated gestational age at the time of surgery. Major upper extremity replantation during pregnancy has not been previously described in the literature. Current operative and perioperative aspects, including considerations that favored microsurgical replantation in this unusual case, are discussed.

[Measuring microcirculation after replantation and revascularization].

Eighty-four patients were evaluated following replantation or revascularization. Extremity function in daily and professional life was altogether satisfactory. Microcirculation investigations in eight patients included thermography, laser-doppler-flowmetry and photoplethysmography. The rewarming period and postocclusive hyperemia after standardized cooling of the upper extremity were determined. In areas of hypesthesia, the rewarming period was clearly delayed. In addition, reactive hyperemia after occlusion of circulation was significantly reduced.

Replantation of cryopreserved human parathyroid tissue.

Twenty-five patients with permanent postoperative hypoparathyroidism received cryopreserved parathyroid autografts. Twelve patients had undergone cervical re-operations due to persistent or recurrent hyperparathyroidism and 10 patients had malfunction of a fresh autograft after total parathyroidectomy. Hypoparathyroidism occurred in 2 patients after subtotal parathyroidectomy and in 1 after the resection of a solitary adenoma following previous thyroid resection. The viability of the tissue was examined histologically prior to replantation in 22 patients and the amount of tissue needed for tra

[Decrease in disability pension following replantation].

Between 1977 and 1986, 201 patients underwent replantations at the Accident Hospital in Salzburg. Of these 201 cases, 112 (55.7%) were work-related accidents. The first 47 cases were evaluated by two independent experts, comparing the actual occupational disability, i.e. impairment of earning capacity after replantation with the probable occupational disability payment had the replantation not been performed. In thirty-nine out of forty-seven cases, replantation had led to lower disability benefits.

[Cross-forearm replantation in bilateral amputation].

We report the case of a 27-year-old patient who sustained a traumatic amputation of his 4 limbs. For the right upper limb, the site of amputation was at the level of the upper third of the forearm. For the left upper limb, the site of amputation was at the level of the Cower third of the forearm. In both cases, the loss of soft tissue was so extensive that hand replantation was impossible in the initial position. In such an unusual case, replantation of the right hand to the left side was attempted in this young patient. Osteosynthesis of the radius to the ulna in supination was performed, and

Proximal upper limb replantation in children.

Proximal upper limb amputation not only leads to serious local disability but carries with it substantial systemic implications. The replantation of an avulsed limb is all too frequently followed by disappointing functional results. In children, however, with secure bony stability, vascular reconstruction and immediate appropriate nerve repair the results of replantation can be most gratifying. Two such cases have been seen at The Hospital for Sick Children and are reported in detail. The late functional results as illustrated make this complex procedure a very worthwhile undertaking.

Thermoregulatory control after upper extremity replantation.

Four patients with complete forearm amputations between the wrist and elbow were analyzed prospectively to determine the interrelationships of thermoregulation to pain, cold intolerance, arterial integrity, venous competence, motor nerve recovery, sensory nerve recovery, and time. Patients were examined at 6 weeks, 3 months, 6 months, 1 year, and 2 years after replantation. Vasomotor regulatory capacity was assessed by isolated cold-stress testing. Pain had diminished in all patients by 6 months to 1 year, regardless of other factors. Cold tolerance and normal thermoregulatory response, as ind

Ear replantation combined with local flaps.

Various techniques of reattachment of the amputated ear have been described. Some of these reattachments may be successful, and some may survive but without good aesthetic results owing to changes in the cartilaginous framework or its skin cover, while many others may result in complete failure. To improve the blood supply to the reattached ear, especially the venous return, local flaps have been devised to cover part of the ear. Eight patients with partial or complete amputation of their ears have been treated with this technique. They have had good postoperative results and at least 6 months

Forearm replantation--long-term functional results.

The replantation of amputated extremities is justified only by the ultimate good function and appearance of the replanted part. This paper is a critical review of our clinical experience with forearm replantation and revascularization in 6 patients over the past ten years. All replanted parts survived. The long-term functional results of the forearm replants are analyzed. Return of function depends on nerve regeneration. The functional outcome in the first patient was unsatisfactory due to poor motor and sensory return. Specific attention directed to careful primary neurorrhaphy in subsequent
