Association of Individual- and Neighborhood-Level Social Determinants of Health with Post-Hospitalization Acute Kidney Injury Care.

KEY POINTS: Only one third of intensive care unit patients who developed severe AKI received Kidney Disease Improving Global Outcomes–recommended follow-up care within 3 months of AKI. Both individual-level and neighborhood-level socioeconomic disadvantages are associated with decreased post-AKI follow-up care. BACKGROUND: Individual-level and neighborhood-level social determinants of health (SDOH) measures have been associated with higher incidence of AKI, lower likelihood of recovery, and higher risk of mortality after AKI. The association of SDOH measures with posthospitalization AKI follo

Racial and Economic Neighborhood Segregation, Site of Delivery, and Morbidity and Mortality in Neonates Born Very Preterm.

OBJECTIVE: To assess the influence of racial and economic residential segregation of home or hospital neighborhood on very preterm birth morbidity and mortality in neonates born very preterm. STUDY DESIGN: We constructed a retrospective cohort of n = 6461 infants born <32 weeks using 2010-2014 New York City vital statistics-hospital data. We calculated racial and economic Index of Concentration at the Extremes for home and hospital neighborhoods. Neonatal mortality and morbidity was defined as death and/or severe neonatal morbidity. We estimated relative risks for Index of Concentration at th

Emergency response to the explosive growth of health care wastes during COVID-19 pandemic in Wuhan, China.

During the Coronavirus Disease 2019 (COVID-19) as a worldwide pandemic, the security management of health care wastes (HCWs) has attracted increasing concern due to their high risk. In this paper, the integrated management of HCWs in Wuhan, the first COVID-19-outbreaking city with over ten millions of people completely locking down, was collected, investigated and analyzed. During the pandemic, municipal solid wastes (MSWs) from designated hospitals, Fangcang shelter hospitals, isolation locations and residential areas (e.g. face masks) were collected and categorized as HCWs due to the high in

Ecosystem services and urban heat riskscape moderation: water, green spaces, and social inequality in Phoenix, USA.

Urban ecosystems are subjected to high temperatures--extreme heat events, chronically hot weather, or both-through interactions between local and global climate processes. Urban vegetation may provide a cooling ecosystem service, although many knowledge gaps exist in the biophysical and social dynamics of using this service to reduce climate extremes. To better understand patterns of urban vegetated cooling, the potential water requirements to supply these services, and differential access to these services between residential neighborhoods, we evaluated three decades (1970-2000) of land surfa

Health disparity by neighbourhood income.

BACKGROUND: Canadian cities are becoming more segregated by income. As such, investigation is required into the magnitude of health disparity between low-, average- and high-income neighbourhoods in order to quantify the level of health disparity at the scale of an urban city. METHODS: A cross-sectional ecological study design was used to review all hospital discharges, physician visits, medication utilization, public health information and vital statistics for an entire city by neighbourhood income status. Postal code information was used to identify six existing contiguous residential neigh

Mass Incarceration, Maternal Vulnerability, and Birth Outcomes Across U.S. Counties.

OBJECTIVES: To examine the associations among mass incarceration, maternal vulnerability, and disparities in birth outcomes across U.S. counties, utilizing an ecological model and reproductive justice perspective was used. This study tests whether mass incarceration is associated with infant mortality and low birthweight across U.S. counties, and whether maternal vulnerability explains the relationship between mass incarceration and birth disparities. METHODS: Data were derived from a variety of public sources and were merged using federal FIPS codes. Outcomes from the CDC Vitality Statistics

Social determinants of breast cancer risk, stage, and survival.

PURPOSE: Social determinants of health that have been examined in relation to breast cancer incidence, stage at diagnosis, and survival include socioeconomic status (income, education), neighborhood disadvantage, unemployment, racial discrimination, social support, and social network. Other social determinants of health include medical distrust, immigration, status, inadequate housing, food insecurity, and geographic factors such as neighborhood access to health services. Socioeconomic factors influence risk of breast cancer. For all racial/ethnic groups, breast cancer incidence rates tend to

The Political Future of Social Medicine: Reflections on Physicians as Activists.

The academic discipline of social medicine has always had a political and policy advocacy component, in addition to its core functions of research and teaching. Its origins lie in the 18th and 19th centuries, in the work of Johann Peter Frank and Rudolph Virchow, among others. Virchow's dictum that "politics is nothing else but medicine on a large scale" highlights that most social determinants of health are politically determined and shape population health. Yet despite intense epidemiological and sociological research on the social determinants of health, less attention has been paid to this

Exceptions to the rule: healthy deprived areas and unhealthy wealthy areas.

In general, inhabitants of low socio-economic areas are unhealthier than inhabitants of high socio-economic areas, but some areas are an exception to this rule. These exceptions imply that other factors besides the socio-economic level of an area contribute to the health of the inhabitants of an area, e.g. environmental factors. In our study we concentrate on areas within the Netherlands that are healthier or unhealthier than could be expected based on their socio-economic level. This study first identifies these areas and secondly determines which area characteristics distinguish these areas

The Association between State-Level Structural Racism and Alcohol and Tobacco Use Behaviors among a National Probability Sample of Black Americans.

BACKGROUND: Structural racism is how society maintains and promotes racial hierarchy and discrimination through established and interconnected systems. Structural racism is theorized to promote alcohol and tobacco use, which are risk factors for adverse health and cancer-health outcomes. The current study assesses the association between measures of state-level structural racism and alcohol and tobacco use among a national sample of 1,946 Black Americans. METHODS: An existing composite index of state-level structural racism including five dimensions (subscales; i.e., residential segregation a

Intersectionality and the Association Between State-Level Structural Racism, Binge Alcohol Consumption, and Smoking Status Among Black Americans.

BACKGROUND: Structural racism is associated with alcohol and tobacco use among Black Americans. There is a need to understand how this relationship differs within varying groups of Black Americans. This study assessed the moderating roles of age, gender, and income in the association between structural racism and binge alcohol consumption and tobacco smoking status among Black Americans. METHODS: A state-level index of structural racism was merged with data from a national probability sample of 1946 Black Americans. Hierarchical linear and logistic regression models tested associations betwee

Neighborhood mobility and racial disparities in preterm birth: A sibling study in California.

Recent work finds that upward neighborhood mobility-defined as reductions in neighborhood socioeconomic disadvantage due to moving-may improve birth outcomes. Less work, however, explores whether changes in socioeconomic context differentially impact birth outcomes by maternal race and ethnicity. In the US, mothers of minoritized racial and ethnic identity often experience worse neighborhood conditions and pregnancy outcomes than White mothers. Using a sibling-linked dataset, we examined whether neighborhood mobility corresponds with changes in preterm birth risk among Asian (N = 130,079), Bla

Neighborhood Deprivation and Privilege: an Examination of Racialized-Economic Segregation and Preterm Birth, Florida 2019.

The Black-White disparity in preterm birth persists and is not fully explained by individual-level social, behavioral, or clinical risk factors. Consequently, there is increasing emphasis on understanding the role of structural and area-level factors. Racialized-economic segregation measured as the index of concentration at the extremes (ICE) simultaneously captures extremes of deprivation and privilege. Our objective was to examine associations between preterm birth (PTB) and the index of concentration at the extremes (ICE). In this cross-sectional study, we analyzed 193,957 Florida birth rec

Association Between Social Vulnerability and Rates of HIV Diagnoses Among Black Adults, by Selected Characteristics and Region of Residence - United States, 2018.

During 2018, Black or African American (Black) persons accounted for 43% of all new diagnoses of HIV infection in the United States (1). The annual diagnosis rate (39.2 per 100,000 persons) among Black persons was four times the rate among all other racial/ethnic groups combined, indicating a profound disparity in HIV diagnoses (1,2). Community-level social and structural factors, such as social vulnerability, might help explain the higher rate of HIV diagnoses among Black persons. Social vulnerability refers to the potential negative health effects on communities caused by external stresses (

Casting a Health Equity Lens on Endocrinology and Diabetes.

As endocrinologists we have focused on biological contributors to disparities in diabetes, obesity and other endocrine disorders. Given that diabetes is an exemplar health disparity condition, we, as a specialty, are also positioned to view the contributing factors and solutions more broadly. This will give us agency in contributing to health system, public health, and policy-level interventions to address the structural and institutional racism embedded in our medical and social systems. A history of unconsented medical and research experimentation on vulnerable groups and perpetuation of eug

Nocturnal Mosquitoes of Pará State in the Brazilian Amazon: Species Composition, Habitat Segregation, and Seasonal Variation.

Mosquitoes (Diptera: Culicidae) are one of the most important disease vector species in the world. Many species have a high degree of anthropophilia and are often found in human habitations. In the present study, we have inventoried the nocturnal mosquito assemblage in intra-, peri-, and extradomicile environments in four municipalities in Pará, Brazil. At each municipality, a residence was selected and the mosquitoes were sampled using the protected human attraction capture and Shannon trap methods in April (rainy season) and August 2018 (dry season). We have collected a total of 696 mosquito

Measuring skin cancer risk in African Americans: is the Fitzpatrick Skin Type Classification Scale culturally sensitive?

OBJECTIVE: Fitzpatrick's Skin Type Classification Scale often is used to assess sun sensitivity and skin cancer risk. Because the scale was developed with Whites, its utility and validity with Blacks may be limited by its reliance on the European-cultural terms suntan and sunburn. We tested the hypothesis that most Blacks would be unable to classify their skin into the four Fitzpatrick skin types. DESIGN, SETTING, PARTICIPANTS: A random, statewide sample of 2085 California Black adults were administered a survey to categorize their skin into the Fitzpatrick types of always burn/never tan (I),

Racial and ethnic inequities to noise pollution from transportation- and work-related sources in the United States.

BACKGROUND: Racial and ethnic inequities in environmental noise exist in the US, partially attributable to historical structural racism. However, previous studies have not considered the totality of people's exposures. Since people spend most of their waking time at work, there is a need to consider cumulative exposure to noise both in and out of the workplace to understand who is most at risk of noise pollution-related adverse health outcomes. OBJECTIVES: To (1) investigate whether racial and ethnic minority communities are disproportionately burdened by transportation- and workplace-related

Associations between historical redlining, neighbourhood deprivation and chronic health in a cohort of people with a history of injection drug use.

BACKGROUND: Historical redlining, carried out by the Home Owners' Loan Corporation (HOLC), assigned US neighbourhoods grades A ('best') to D ('hazardous'), institutionalising residential segregation and leaving lasting effects on neighbourhood development and health outcomes. There is limited research examining the association between residence in historically redlined areas, present-day neighbourhood socioeconomic status and chronic disease in Baltimore, Maryland. METHODS: We used 2015-2019 data from the AIDS Linked to the IntraVenous Experience (ALIVE) cohort, an observational study of adul

Spatial disparities and environmental justice in PM2.5 exposure among urban E-bike Commuters:A case study of Changsha.

Amid increasing residential segregation, disparities in resource allocation and environmental health faced by vulnerable groups have drawn widespread attention. Electric bicycles (E-bikes), favored by low-income populations due to their affordability, convenience, and eco-friendliness, have gained widespread popularity, particularly in China. However, whether E-bikes can reduce daily air pollution exposure and mitigate associated environmental health inequalities remains unclear. This study integrates land use regression and space syntax to quantify spatial disparities in PM2.5 exposure among
