A rapid, inexpensive diagnostic for bacterial pathogen and resistance detection in resource-limited settings.

Antimicrobial resistance is a major global health threat, with disproportionate impact in regions with limited diagnostic infrastructure. To address this challenge, we developed BADLOCK (Bacterial and AMR Detection by SHERLOCK), a rapid, low-cost molecular diagnostic platform for direct detection of bacterial pathogens and resistance genes from clinical samples. BADLOCK operates as a one-pot CRISPR-Cas13a reaction capable of detecting nine bacterial species and four major resistance genes directly from positive blood culture. It requires only a heat block and a basic fluorometer for operation.

Burden of Neurological Disorders in Resource-Limited Settings: Lessons from Pakistan for Global Neurology.

Neurological disorders (NDs) are a leading global cause of disability and mortality, with their burden falling disproportionately on low- and middle-income countries (LMICs). Pakistan exemplifies the systemic challenges faced in resource-limited settings, including delayed diagnosis, limited specialist access, and fragmented care pathways. Despite this, comprehensive spectrum-based data remain scarce. This study was aimed to quantify the burden and characterize the spectrum of NDs presenting to a major Pakistani tertiary-care center and to identify associated predictors and geographic disparit

Harnessing Sunlight Safely for Newborn Care: Balancing Risks and Benefits in Resource-Limited Settings.

Growing global interest in solar energy has renewed attention to sunlight exposure and its health effects. Although sunlight contains harmful ultraviolet radiation (UVR), controlled exposure has preventive and therapeutic benefits. In many low- and middle-income countries, particularly in sub-Saharan Africa and parts of Asia, newborns are traditionally exposed to early-morning sunlight to prevent vitamin D deficiency and help treat neonatal jaundice. Vitamin D deficiency can lead to nutritional rickets, while untreated severe jaundice may cause serious complications such as brain damage, deafn

Diagnosis and Management of Adult Status Epilepticus in Resource-Limited Settings: A Systematic Review.

BACKGROUND AND OBJECTIVES: Status epilepticus (SE) is the leading cause of death in patients with epilepsy, and it affects people in low/middle-income countries (LMICs) at a much higher rate. There is likely a significant gap between the recommended diagnosis and treatment of SE and current practices in resource-limited settings. We conducted a systematic literature review to determine how convulsive and nonconvulsive SE in adults is diagnosed and managed in LMICs. METHODS: All relevant articles from Embase, Medline, PubMed, and the Virtual Health Library Regional Portal databases, published

Guidelines for the Medical Management of Congenital Diaphragmatic Hernia: Implications for Resource-limited Settings.

For neonates with congenital diaphragmatic hernia (CDH), the challenges start immediately after birth. Respiratory distress and hemodynamic compromise set in with the herniation of the contents of the abdomen into the thoracic cavity. Though definitive surgical correction and long-term follow-up are required, the immediate postnatal stabilization is of paramount importance. Depending on the extent of herniation, the associated physiological impairment is variable. With extensive research over the past few decades and a better understanding of the disease physiology, coupled with newer modaliti

Tailoring clinical management after embryo transfer using β-hCG levels in resource-limited settings.

Cleavage-stage embryo transfers are often the best option for patients with limited oocytes or low fertilization rates due to medical or financial constraints. This study analyzed 424 women undergoing β-hCG testing 14 days after embryo transfer at a tertiary care center in India. Pregnancy outcomes were classified as no live births (biochemical pregnancies, ectopic pregnancies, miscarriages) or live births (single/multiple births). Higher β-hCG levels were associated with greater chances of live birth but also an increased risk of complications like preterm birth and preeclampsia, particularly

Innovations in kidney stone management: mini-PCNL for staghorn calculi in resource-limited settings.

INTRODUCTION: This study evaluates our initial experience with miniaturized percutaneous nephrolithotomy (mini-PCNL) in Vietnamese patients with staghorn calculi, using an 18F metal access sheath. This technique addresses the challenges of complex kidney stone management in resource-limited settings. METHODS: A multi-center retrospective review of 236 patients with staghorn calculi who underwent mini-PCNL with high-power Ho laser lithotripsy (Lumenis 100 W) was conducted at four provincial hospitals in northern Vietnam from January 2020 to December 2023. RESULTS: Among the 236 patients (mean

Respiratory distress syndrome management in resource limited settings-Current evidence and opportunities in 2022.

The complications of prematurity are the leading cause of neonatal mortality worldwide, with the highest burden in the low- and middle-income countries of South Asia and Sub-Saharan Africa. A major driver of this prematurity-related neonatal mortality is respiratory distress syndrome due to immature lungs and surfactant deficiency. The World Health Organization's Every Newborn Action Plan target is for 80% of districts to have resources available to care for small and sick newborns, including premature infants with respiratory distress syndrome. Evidence-based interventions for respiratory dis

Treating early-stage Hodgkin lymphoma in resource-limited settings: InPOG-HL-15-01 experience.

BACKGROUND: Hodgkin lymphoma (HL) in childhood is an eminently curable disease. Excellent outcomes can be achieved even in resource-limited settings and increasingly, the focus is on limiting long-term toxicity. Contemporary treatment incorporates a risk-stratified, response-adapted approach using multiagent chemotherapy with or without low-dose radiotherapy (RT). Many developing countries continue to use ABVD (adriamycin, bleomycin, vinblastin, and dacarbazine)-based regimen owing to limited acute toxicity, cost, and ease of delivery. We report outcomes of children with early-stage HL using l

Challenges in management of atypical hemolytic uremic syndrome: Bottle neck in resource limited settings.

Treatment of atypical hemolytic uremic syndrome cases is challenging right from establishing correct and timely diagnosis to execution of management protocol. A seven-year-old male child from poor socioeconomic status was admitted with chief complaints of fever, 3 episodes of vomiting and passage of cola coloured urine. Based on clinical and laboratory findings, diagnosis was established. However, ADAMTS13 levels and genetic studies required for diagnosis could not be performed due to financial constraints and non-availability of these tests. TPE kits were arranged from charitable organization

The development of a smartphone application to help manage epilepsy in resource-limited settings.

PURPOSE: Epilepsy is treatable but in low- and middle-income countries (LMICs) it goes untreated with dire consequences for people with it and their families. There are not enough available doctors to treat it so it has been suggested that non-physician health workers (NPHWs) take a role in diagnosis and management. Tools will be essential to help them. A smartphone application (app) for episode diagnosis has proved safe and effective; this paper describes an app for epilepsy management. METHODS: Questions were devised which captured temporal characteristics of episodes, diagnosis of episodes

Barriers to Effective Transfusion Practices in Limited-Resource Settings: From Infrastructure to Cultural Beliefs.

BACKGROUND: Surgery and anesthesia are indivisible parts of health care, but safe and timely care requires more than operating rooms and skilled providers. One vital component of a functional surgical system is reliable blood transfusion. While almost half of all blood is donated in high-income countries (HICs), over eighty percent of the global population lives outside of these countries. High-income countries have on average 30 donations per 1000 people, and the average age of transfusion recipient is over 65. Most low-income countries (LICs) have fewer than five donations per 1000 people, w

Feasibility and Safety of Peripheral Intravenous Administration of Vasopressor Agents in Resource-limited Settings.

BACKGROUND: Vasopressors are conventionally administered through a central venous catheter (CVC) and not through a peripheral venous catheter (PVC) since the latter is believed to be associated with increased risk of extravasation. Placement of a CVC requires suitably trained personnel to be on hand, and in resource-limited settings, this requirement may delay placement. Because of this and in cases where suitably trained personnel are not immediately available, some clinicians may be prompted to utilise a PVC for infusing vasopressors. The objective of this study is to assess the feasibility

Pooled nucleic acid testing strategy for monitoring HIV-1 treatment in resource limited settings.

BACKGROUND: Virological monitoring (VM) and drug resistance (DR) analysis are crucial for effective HIV management. Due to the high cost of commercial assays, VM and DR analysis is not performed in resource-limited-settings. OBJECTIVE: The objective of this study is to develop a pooling based algorithm for the combined identification of virologic treatment failure (VTF) by nucleic acid testing (NAT) and DR by sequencing - NAT+DR assay. STUDY DESIGN: We enrolled 559 participants on first-line therapy and analyzed for VTF. The virologically suppressed participants were followed-up to see the V

Assisted HIV partner notification services in resource-limited settings: experiences and achievements from Cameroon.

INTRODUCTION: In 2007, the Cameroon Baptist Convention Health Services (CBCHS) initiated an assisted partner notification services (aPNS) public health programme to increase HIV case identification and reduce HIV incidence in the most affected regions of Cameroon. We describe large-scale implementation of aPNS and overall programmatic achievements in a resource-limited setting through 2015. METHODS: CBCHS trained health advisors (HAs) from 16 CBCHS facilities and 22 non-CBCHS facilities to integrate aPNS into their existing jobs in five of the ten Cameroon regions. HAs recorded basic demograp

Introducing a multifaceted approach to the management of diabetes mellitus in resource-limited settings.

Globally diabetes mellitus (DM) and its complications are placing an enormous burden on individual patients and countries alike. South Africa is a developing country already under enormous pressure from communicable diseases such as HIV and tuberculosis. Added to this is DM, which serves to fuel the interactions between communicable and non-communicable diseases. Data from KwaZulu-Natal Province (KZN) have demonstrated that the majority of patients with DM in the public healthcare sector are diagnosed and started on treatment at their local resource-limited healthcare clinics. Method for identification of Cryptococcus neoformans and Cryptococcus gattii useful in resource-limited settings.

AIMS: The high HIV/AIDS burden in Sub-Saharan Africa has led to cryptococcosis becoming a public health concern. In this resource-limited setting, conventional identification methods are mainly used to diagnose cryptococcal infections. However, these methods are often inconsistent, and importantly, cannot discriminate between the aetiological agents, Cryptococcus neoformans and C. gattii. Therefore, there is a need for an alternative reliable method to identify these species. METHODS: We examined the usefulness of a PCR method, including a restriction digest, in identifying clinical C. neofor
