The long-term effect of a positive pressure respirator on the specific antibody response in pigeon breeders.

A group of pigeon fanciers with Pigeon Breeder's Lung wore a positive pressure respirator with a full mask and hood when working with pigeons. Serial measurements of specific IgG antibody to pigeon globulin (ELISA) were made in 22 subjects. Thirteen wore a respirator (monitored over 14 months) and nine did not (monitored over 3 months). Following introduction of the respirator, the serum antibody to pigeon globulin significantly decreased when compared with the non-respirator group. This fall in antibody persisted over 14 months reflecting the reduction of antigen inhalation. Although circulat

100 Years of Medical Countermeasures and Pandemic Influenza Preparedness.

The 1918 influenza pandemic spread rapidly around the globe, leading to high mortality and social disruption. The countermeasures available to mitigate the pandemic were limited and relied on nonpharmaceutical interventions. Over the past 100 years, improvements in medical care, influenza vaccines, antiviral medications, community mitigation efforts, diagnosis, and communications have improved pandemic response. A number of gaps remain, including vaccines that are more rapidly manufactured, antiviral drugs that are more effective and available, and better respiratory protective devices.

Noninvasive ventilation with a helmet in patients with acute respiratory failure caused by chest trauma: a randomized controlled trial.

Noninvasive ventilation (NIV) is beneficial in acute respiratory failure (ARF) caused by chest trauma; however, NIV-related complications affect the efficacy. We evaluated whether NIV with helmet decreases the incidence of complications and improves its effects in a single center. Patients with ARF after chest trauma were randomized to receive NIV with helmet or face mask. The primary outcome was the rate of NIV-related complications. Secondary outcomes were PaO2/FiO2, patient's tolerance, intubation rate, length of intensive care unit (ICU) stay, and ICU mortality. The trial was terminated ea

Evaluation of embolic protection devices for fat emboli prevention.

BACKGROUND: Patients with acutely treated femoral shaft fractures with reamed intramedullary nailing are at risk for acute respiratory distress syndrome due to liberation of bone marrow fat particles that travel to the lung and cause damage to the parenchyma. The purpose of this study was to demonstrate: (1) the ability of clinically applicable embolic protection devices to capture such particles; (2) how such a device affects cardiopulmonary function after reamed intramedullary nailing; and (3) evaluation of lung pathology to determine whether filtration affects pulmonary embolic load. Repurposing mucosal delivery devices for live attenuated tuberculosis vaccines.

Tuberculosis (TB) remains one of the most lethal infectious diseases globally. The only TB vaccine approved by the World Health Organization, Bacille Calmette-Guérin (BCG), protects children against severe and disseminated TB but provides limited protection against pulmonary TB in adults. Although several vaccine candidates have been developed to prevent TB and are undergoing preclinical and clinical testing, BCG remains the gold standard. Currently, BCG is administered as an intradermal injection, particularly in TB endemic countries. However, mounting evidence from experimental animal and hu

Reusable and Recyclable Graphene Masks with Outstanding Superhydrophobic and Photothermal Performances.

The 2019 coronavirus outbreak (COVID-19) is affecting over 210 countries and territories, and it is spreading mainly by respiratory droplets. The use of disposable surgical masks is common for patients, doctors, and even the general public in highly risky areas. However, the current surgical masks cannot self-sterilize in order to reuse or be recycled for other applications. The resulting high economic and environmental costs are further damaging societies worldwide. Herein, we reported a unique method for functionalizing commercially available surgical masks with outstanding self-cleaning and

One-Year Outcomes in Patients With Acute Respiratory Distress Syndrome Enrolled in a Randomized Clinical Trial of Helmet Versus Facemask Noninvasive Ventilation.

OBJECTIVES: Many survivors of acute respiratory distress syndrome have poor long-term outcomes possibly due to supportive care practices during "invasive" mechanical ventilation. Helmet noninvasive ventilation in acute respiratory distress syndrome may reduce intubation rates; however, it is unknown if avoiding intubation with helmet noninvasive ventilation alters the consequences of surviving acute respiratory distress syndrome. DESIGN: Long-term follow-up data from a previously published randomized controlled trial. PATIENTS: Adults patients with acute respiratory distress syndrome enrolle

Special article: personal protective equipment for care of pandemic influenza patients: a training workshop for the powered air purifying respirator.

Virulent respiratory infectious diseases may present a life-threatening risk for health care professionals during aerosol-generating procedures, including endotracheal intubation. The 2009 Pandemic Influenza A (H1N1) brings this concern to the immediate forefront. The Centers for Disease Control and Prevention have stated that, when performing or participating in aerosol-generating procedures on patients with virulent contagious respiratory diseases, health care professionals must wear a minimum of the N95 respirator, and they may wish to consider using the powered air purifying respirator (PA

Helmet ventilation for acute respiratory failure and nasal skin breakdown in neuromuscular disorders.

Noninvasive ventilation (NIV) has been widely used to decrease the complications associated with tracheal intubation in mechanically ventilated patients with neuromuscular diseases in acute respiratory failure. However, nasal ulcerations might occur when masks are used as an interface. Helmet ventilation is a possible option in this case. We describe two patients with acute respiratory failure due to Duchenne muscular dystrophy who developed nasal bridge skin necrosis during NIV. Helmet pressure support ventilation caused significant patient-ventilator asynchrony, leading to NIV intolerance. T

Personal protection during resuscitation of casualties contaminated with chemical or biological warfare agents--a survey of medical first responders.

INTRODUCTION: The threat of mass casualties caused by an unconventional terrorist attack is a challenge for the public health system, with special implications for emergency medicine, anesthesia, and intensive care. Advanced life support of patients injured by chemical or biological warfare agents requires an adequate level of personal protection. The aim of this study was to evaluate the personal protection knowledge of emergency physicians and anesthetists who would be at the frontline of the initial health response to a chemical/biological warfare agent incident. METHODS: After institution

Report of the investigation committee into the cyanide poisonings of Providence firefighters.

In the afternoon of March 23, 2006 a Providence firefighter was diagnosed as having cyanide poisoning after working at a building fire. In the aftermath of three fires at commercial and residential sites that day, eight additional firefighters (out of 28 tested) were found to have elevated levels of cyanide. Numerous other members reported symptoms consistent with cyanide poisoning, including headaches, weakness and fatigue, nausea, and shortness of breath. The Providence Fire Department (PFD) established a joint union management committee to review the situation.

Extracorporeal CO2 reduction for COVID-19: hypercapnic respiratory failure post extracorporeal membrane oxygenation.

COVID-19-induced acute respiratory distress syndrome (ARDS) has challenged medical providers. In severe cases, patients present with poor lung compliance, requiring not only lung protective mechanical ventilation strategies, but also extracorporeal support. Due to the nature of the pandemic, the extracorporeal carbon dioxide removal device called Hemolung Respiratory Assist System became available under the Food and Drug Administration Emergency Use Authorization for patients with COVID-19-induced ARDS. This allowed application of the device to treat patients with recrudescent ARDS following a

Call to action: Youth and filtering facepiece respirator (FFRs).

The COVID-19 pandemic significantly heightened public awareness of respiratory protection, yet a critical gap persists in evidence-based guidance and appropriately designed filtering facepiece respirators (FFRs), such as N95s, for youth (≤18 years of age) in the U.S. This commentary, informed by the author's international experiences training agricultural youth living and working on farms and insights gleaned from a systematic review, highlights an urgent need for better FFR solutions for this population. Acknowledging the sensitive nature of this topic and the early stage of the science, the

Filtering facepiece respirator use among farm youth in the US: A review.

The COVID-19 pandemic underscored the critical need for protective equipment, such as filtering facepiece respirators (FFRs), particularly for youth. This systematic review addresses the significant gap in evidence-based guidance for FFR use among US farm youth, a group potentially exposed to diverse respiratory hazards. Current FFR designs and protocols for FFR use are largely adult-centric. Adhering to PRISMA 2020 guidelines, a multidisciplinary panel reviewed 31 publications published between 1990 and 2023. An independent working group of agricultural safety professionals also contributed b
