Acute stress induced by physical restraint can interfere with the validity of laboratory findings. Sedation could minimize such stress. However, it is not known whether sedation can affect hematologic and hemostatic parameters in cats. The purpose of this study was to evaluate hematologic and hemostatic parameters in domestic cats subjected to physical restraint in addition to one of two sedation protocols. In total, 50 cats were subjected to physical restraint and were then randomly divided into two groups of 25 animals, receiving dexmedetomidine (5 µg/kg) and butorphanol (0.3 mg/kg; DB group

Children with autism spectrum disorder (ASD) often experience significant distress during medical procedures, which can limit access to optimal treatments such as biologic therapy for atopic dermatitis (AD). However, practical strategies for initiating injectable biologic therapy without physical restraint in this population remain poorly described. We report a case demonstrating a structured and reproducible approach that enabled restraint-free initiation of dupilumab in a child with ASD. A 6-year-old boy with moderate-to-severe AD and comorbid ASD had a history of severe procedural avoidance

BACKGROUND: Use of physical restraint is a common practice in mental healthcare, but is controversial due to risk of physical and psychological harm to patients and creating ethical dilemmas for care providers. Post-incident review (PIR), that involve patient and care providers after restraints, have been deployed to prevent harm and to reduce restraint use. However, this intervention has an unclear scientific knowledge base. Thus, the aim of this scoping review was to explore the current knowledge of PIR and to assess to what extent PIR can minimize restraint-related use and harm, support car

For patient and personnel safety, agitated and violent individuals are sometime physically restrained during out-of-hospital ambulance transport. We report two cases of unexpected death in restrained, agitated individuals while they were being trans-ported by advanced life support ambulance. Both patients had been placed in hobble restraints by law enforcement. At autopsy, toxicologic analysis revealed nonlethal levels of amphetamines in one patient and nonlethal levels of ethanol, cocaine, and amphetamines in the other. In both cases the cause of death was determined to be positional asphyxia

Impaired hand function is among the most functionally disabling symptoms of unilateral cerebral palsy. Evidence-based treatment approaches are generally lacking. However, recent approaches providing intensive upper extremity training appear promising. In this review, we first describe two such approaches, constraint-induced movement therapy (CIMT) and bimanual training (hand-arm bimanual intensive therapy). We then summarize findings across more than 100 participants in our CIMT/bimanual training studies since 1997. We show that (1) at high intensities, CIMT and bimanual training improve dexte

OBJECTIVE: Despite the clinical relevance of enteral and parenteral tube feeding and restraint use in anorexia nervosa (AN), national-level data on their use have remained limited, particularly for total parenteral nutrition (TPN) and percutaneous endoscopic gastrostomy (PEG). This study systematically characterizes the national prevalence and temporal trends in intensive nutritional interventions, defined as nasogastric tube feeding (NGTF), TPN, and PEG, and physical restraint among hospitalizations for AN in the United States (US).
METHOD: We analyzed 66,785 hospitalizations for AN from the

OBJECTIVE: Although pneumonia is the leading cause of death among patients with dementia, the specific underlying causes remain unclear. In particular, the potential connection between pneumonia risk and dementia-related daily living difficulties, such as oral hygiene practice and mobility impairment, and the use of physical restraint as a management practice, has not been extensively studied.
METHODS: In our retrospective study, we included 454 admissions corresponding to 336 individual patients with dementia who were admitted to a neuropsychiatric unit due to behavioral and psychological sy

Death by suffocation due to prolonged thoracic compression represents a significant forensic issue with critical implications for the management of physical restraint interventions. However, it remains challenging for forensic pathologists to ascertain the exact cause of death when the individual is a habitual substance abuser and in an altered psychophysical state (Excited Delirium Syndrome, EDS). This case report describes the death of a male individual following an episode of psychomotor agitation and physical restraint by multiple individuals. The autopsy revealed findings consistent with

The phenomenon of 'conditioning', whereby prior exposure to moderate-level non-traumatic sound reduces the permanent injury to subsequent high-level sound exposures, is suggestive of protective effects mediated by stress-induced gene expression in other systems. To test the role of stress pathways in acoustic injury, this study investigated the effect of mild physical restraint, a classic non-acoustic elicitor of stress, on vulnerability to subsequent noise-induced hearing loss. CBA/CaJ mice were divided into groups (control, restraint-only, restraint pre-trauma, and trauma-only), and cochlear

BACKGROUND: There is some evidence to support approaches to reduce restrictive practices in settings for people with a learning disability who may also have a diagnosis of autism or mental health problems. However, there is a significant knowledge gap in how and why such approaches work and in what contexts.
AIM: This study aimed to understand how, why, for whom, and under what circumstances approaches used by healthcare staff to prevent and reduce the use of restrictive practices on adults with learning disability or autism work (or do not work).
DESIGN: The review followed a realist approa

The anterior cingulate cortex (ACC) is vulnerable to stress. Its dysfunction is observed in psychiatric disorders manifested as alterations in network oscillations. Mechanisms linking stress load to disturbed emotional-cognitive behaviors are of essential importance to further elucidate therapeutic strategies for psychiatric diseases. Here, we analyzed the effects of chronic restraint stress (CRS) load in juvenile mice on kainic acid (KA)-induced network oscillations in ACC slice preparations and on the forced swim test (FST). The immobility time (IT) was shortened at the beginning of the FST

The association of the two terms 'isolating' and 'protecting', seems counterintuitive. The practice of isolation conjures up images of an asylum or even prison, rather than a notion of safeguarding. Isolation is defined within caregiving institutions by a vast range of practices, neither rare nor recent. It is used in a variety of departments, from psychiatric isolation to septic isolation or a form of isolation common to both: social isolation resulting from institutionalisation.

Obtaining baseline hormone samples can be challenging because circulating hormone levels often change rapidly due to the acute stress of capture. Although field protocols are established for accurately sampling baseline glucocorticoid concentrations, fewer studies have examined how common sampling techniques affect androgens levels. Indeed, many studies focused on understanding the functional significance of baseline androgen levels use sampling methods known to activate the endocrine responses to stress. To understand how different field sampling protocols affect plasma androgen levels, we me

Coercive measures in mental health care can cause considerable stress for those affected. The long-term consequences for mental function have received little attention to date. Here, we present the case of an adolescent user who was frequently subjected to mechanical restraint. From a learning theory perspective, repeated restraint created an association between coercive measures and reducing internal tension that was maladaptive. This case study demonstrates that frequent coercive measures can have a negative impact on emotional regulation.

A psychiatric nurse since 2013, who became a clinical psychologist in 2022, I have had the opportunity, on numerous occasions, to use isolation and therapeutic restraint as part of my nursing practice, mainly in a closed psychiatric admissions service. These therapeutic tools, specific to psychiatry, are used in a very specific theoretical and legislative framework. Their use always leads to reflection, both individually and as a team. Indeed, their use must remain the last therapeutic bulwark to be used because it can be experienced with difficulty or even in a traumatic way by the patient, w

We describe an apparatus for immobilization of the rattlesnake head for brain experimentation. Its design also allows its use as a stereotaxic instrument for electrophysiological studies employing microelectrodes. Attachment of the head is at the two orbital ridges and the hard palate. The bregma provides a zero reference for the frontal, lateral, and depth coordinates.
