BACKGROUND: Melioidosis is a serious infection caused by soil-dwelling Gram-negative bacillus Burkholderia pseudomallei. It is most commonly reported in Northern Australia, Southeast and Southern Asia, China, and Taiwan.
METHODS: A case report and short review of the literature are presented. Presentation, diagnosis including genomic sequencing, and acute and long-term management are discussed.
RESULTS: A 58-year-old female presented with chronic rhinosinusitis secondary to melioidosis. This is the third reported incidence of sinusitis secondary to melioidosis, which occurred in an otherwise…

Rhinosinusitis is a prevalent disorder in the pediatric population. Unfortunately, during the past two decades, guidelines related to definitions, diagnostic procedures and management have been much more focused on adult than on pediatric rhinosinusitis. First consensus document on management of pediatric rhinosinusitis was published in 1998, followed by several documents related only to acute rhinosinusitis. The most extensive consensus document on rhinosinuistis, including pediatric rhinosinusitis, is European position paper on rhinosinusitis and nasal polyps, EPOS, updated in 2012.

Chronic inflammation of nasal mucosa and sinuses are of increasing prevalence. Patients suffering from chronic rhinosinusitis (CRS) are characterised by nasal obstruction and secre- tion, impaired sense of smell, head and facial pain, causing impact on quality of life as well as tremendous socioeconomic effects. Therefore, effective and specific diagnostics as well as therapies are essential, which have to be selected from state of the art, evidence based guidelines. According to EP3OS guidelines from 2007 the CRS is defined as chronic inflammation of the nose and nasal sinuses, with or withou…

A growing interest in rhinosinusitis in recent years resulted in two European Position Papers in Rhinosinusitis and Nasal Polyposis (EP3OS) documents, published in 2005 and 2007, respectively. The latter is intended to be a state-of-the art review for specialists and general practitioners, updating the current knowledge on rhinosinusitis and nasal polyposis, proposing the guidance for definitions and outcome measurements in research and providing an evidence-based review of the available treatments. The present article briefly discusses the guidelines for acute rhinosinusitis (ARS) and chronic…

UNLABELLED: Contemporary cohort cross-sectional study.
INTRODUCTION: Despite its importance for an accurate diagnosis, histology differences among nasal polyps and its clinical implications are rarely reported in the literature. The existing papers classify polyp samples without concern for prior treatments, which could influence the results attained.
AIMS: carry out a morphological study, through light microscopy, of nasal polyps' structural alterations in the absence of any type of prior treatment and histologically classify it in relation to studies published in the literature.

The patient referred to the otolaryngologist for the treatment of CRS has received many therapies for the condition. Newer therapies available focus on the anti-inflammatory therapies and local application of antimicrobial and antifungal agents to the sinus cavities. Much clinical work remains to be done to prove the efficacy of currently available treatments. The recent advances in the understanding of allergic and immune mechanisms may allow eventual intervention at the level of cytokines and other immunomodulators of inflammation. The complex cascade of interleukins and proinflammatory agen

BACKGROUND: The purpose of this study was to investigate the impact of serum eosinophilia in the prognosis of chronic rhinosinusitis (CRS). Eosinophils are now considered to play a major role in the pathogenesis of CRS. To date, no study has evaluated the clinical course of patients with CRS and serum eosinophilia.
METHODS: A retrospective chart review of 620 patients who underwent endoscopic sinus surgery, (ESS) for CRS was performed. Thirty-one patients (5%) had elevated serum eosinophil counts and served as the study group. Patient charts of the study group and 34 (5%) controls with normal…

Rhinogenic orbital and intracranial complications occur in 0.8 and 0.01% of patients with rhinosinusitis, respectively. The author recognizes 9 and 8 forms of rhinogenic orbital and intracranial complications, respectively. Conservative treatment is preferable in non-purulent orbital and intracranial complications resultant from acute inflammation of the paranasal sinuses. In the above complications taking place in chronic paranasal inflammation it is valid to conduct a sanitizing operation on the paranasal sinuses. Purulent intracranial and orbital complications dictate the necessity of major…

Rhinosinusitis is a highly prevalent, inflammatory condition affecting the nose and paranasal sinuses, impacting an individual's quality of life with significant health care burden. Sinusitis is more frequent in females, and they typically present with more severe symptoms and worse quality of life scores. Males are more likely to present with nasal polyps and have higher objective scores on imaging studies. Differences in sinus microbiota by sex may play a role in understanding differences in clinical presentations between them, but additional research is required. An improved understanding o…

Discerning the genetics and epigenetics of chronic rhinosinusitis (CRS) may optimize outcomes through early diagnostics, personalized and novel therapeutics, and early prognostication. CRS associated with cystic fibrosis and primary ciliary dyskinesia has well-characterized genetic mutations. Most CRS subjects, however, do not exhibit identifiable monogenic alterations. Clustering in related individuals is seen in CRS with nasal polyps. Spouses of subjects with CRS without nasal polyps also may be at increased risk of the same disease. These observations generate questions on genetic and envir…

Rhinosinusitis is one of the most frequently diagnosed diseases in patients seeking medical consultation. Sinusitis is a heterogeneous group of diseases and can be acute or chronic. The current state of knowledge on rhinosinusitis is presented in the recommendations of the European Position Paper on Rhinosynusitis and Nasal Polyps 2020 (EPOS 2020). More and more attention is paid to the condition of the microbiota in the context of inflammatory changes in the sinuses. There is also a negative effect of excessively prescribed antibiotics on the increase in bacterial resistance to drugs and sign…

Rhinosinusitis is characterized by inflammation of the mucosa involving the paranasal sinuses and the nasal cavity and is one of the most common and significant health care problems, with significant impairment of quality of life. Current standard conventional management of rhinosinusitis commonly uses multiple therapeutic modalities to break the cycle of chronic disease. However, to date, there is no consensus as to the optimal treatment algorithm for patients with chronic rhinosinusitis. There is a growing interest in the use of complementary and integrative medicine for the treatment of rhi…

BACKGROUND: Chronic rhinosinusitis (CRS) is associated with productivity losses exceeding US$13 billion annually. Although pain is well known to significantly affect patient productivity in other diseases, its economic impact on CRS-related lost productivity has not been examined. The objective of this study was to determine whether CRS-related facial pain correlates with lost productivity in patients with CRS.
METHODS: Seventy patients with CRS were enrolled in a cross-sectional investigation. Patients with a history of systemic inflammatory disease, ciliary dysfunction, chronic pain syndrom…

BACKGROUND: Acute rhinosinusitis is an acute infection of the nasal passages and paranasal sinuses that lasts less than four weeks. Diagnosis of acute rhinosinusitis is generally based on clinical signs and symptoms in ambulatory care settings. Technical investigations are not routinely performed, nor are they recommended in most countries. Some trials show a trend in favour of antibiotics, but the balance of benefit versus harm is unclear. We merged two Cochrane Reviews for this update, which comprised different approaches with overlapping populations, resulting in different conclusions. For t…

OBJECTIVE: Considering that all the evidence indicates that chronic rhinosinusitis without nasal polyps (CRSsNP) and chronic rhinosinusitis with nasal polyps (CRSwNP) are distinct entities, the aim of this study was to compare the concentrations obtained in plasma and in sinonasal mucosa with oral and nasal topical ciprofloxacin, in patients with and without nasal polyps, without evaluating the effectiveness of the use of an antibiotic.
METHODS: Prospective clinical study with single-blind randomization. The population consisted of patients with chronic rhinosinusitis with eligible for endona…

Chronic rhinosinusitis (CRS) is one of the most common chronic diseases in adults and severely affects quality of life in patients. Although various etiologic and pathogenic mechanisms of CRS have been proposed, the causes of CRS remain uncertain. Abnormalities in the coagulation cascade may play an etiologic role in many diseases, such as asthma and other inflammatory conditions. While studies on the relationship between asthma and dysregulated coagulation have been reported, the role of the coagulation system in the pathogenesis of CRS has only been considered following recent reports. Exces…

Rhinosinusitis is a very common condition which is normally readily recognizable. Given the intimate anatomic relationship between the antrum and the posterior maxillary teeth, maxillary sinusitis can present as odontalgia. Distinguishing between odontogenic orofacial pain and pain associated with maxillary sinusitis is important to prevent unnecessary dental intervention and to direct patients to medical colleagues. Conversely, odontogenic infection can spread to involve the antrum, termed odontogenic sinusitis, or maxillary sinusitis of dental origin. Odontogenic sinusitis accounts for about…
