Staphylococcus aureus bacteremia, risk factors, complications, and management.

Staphylococcus aureus and methicillin-resistant S aureus have emerged as the most important nosocomial pathogens. Traditional therapy may be sufficient in most but not all patients, in whom alternatives should be sought. The infection is often complicated with several sites of metastatic foci and is nosocomial frequently. New antibiotics to fight MRSA have been introduced and are equivalent or better than vancomycin.

Splanchnic vein thrombosis: new risk factors and management.

Splanchnic vein thrombosis (SVT) is a rather heterogeneous disease which involve one or more abdominal veins draining from different organs, including small and large bowel, liver, spleen and pancreas, and it may be associated with a wide spectrum of underlying disorders, either local or systemic. The role of new risk factors for SVT, including the JAK2 V617F mutation and the paroxysmal nocturnal hemoglobinuria clone, has been highlighted in recent years. The clinical presentations of SVT are variable and, not uncommonly, may include the concomitant presence of extensive thrombosis and gastroi

Tailored expectant management: risk factors for non-adherence.

INTRODUCTION: Prediction models for spontaneous pregnancy are useful tools to prevent overtreatment, complications and costs in subfertile couples with a good prognosis. The use of such models and subsequent expectant management in couples with a good prognosis are recommended in the Dutch fertility guidelines, but not fully implemented. In this study, we assess risk factors for non-adherence to tailored expectant management. METHODS: Couples with mild male, unexplained and cervical subfertility were included in this multicentre prospective cohort study. If the probability of spontaneous preg

Genetics and dementia: risk factors, diagnosis, and management.

New developments in molecular genetics have improved our understanding on a number of neurodegenerative dementias considerably, especially Alzheimer's disease and frontotemporal dementia. However, this explosion of information can be overwhelming to clinicians, making it difficult to integrate into regular clinical practice. In this article, we briefly reviewed our current understanding regarding causative genetic mutations and genetic risk factors on the major forms of dementia, which provided the background information for discussion in the Third Canadian Consensus Conference on the Diagnosi

Cervical paragangliomas: neurovascular surgical risk and therapeutic management.

Paragangliomas of the neck region arise most commonly in the carotid and vagal bodies. The goal of this retrospective study is to evaluate intraoperative vascular and neurological morbidity and to define the therapeutic strategy. During the period 1990-2004, 32 patients with 42 neck paragangliomas were referred to our institution (Head and Neck Service, Otolaryngology Federation, CHU La Timone, Marseilles, France). There were 29 carotid body tumors and 11 vagal body tumors. There were 14 men and 18 women. Forty paragangliomas were surgically excised. Mean age of patients with family history of

Fournier's gangrene: risk factors and strategies for management.

OBJECTIVE: The prognosis of Fournier's gangrene (FG) depends on early diagnosis and management. In this study, our objective was to identify the distinct features of FG that may influence the clinical outcome. METHODS: A retrospective chart review was performed in patients with a diagnosis of FG between January 1999 and December 2003. Etiological and predisposing factors, causative microbiological organisms, and clinical outcome were investigated. RESULTS: Twenty-five men (71%) and 10 women (29%) were included in the study. Mean age was 59.7 +/- 10.7 (range: 43-88) years. As a predisposing f

Endometrial cancer: the management of high-risk disease.

Patients with endometrial cancer have an overall good prognosis. Patients with tumors invading deep into the myometrium or the cervical stroma or with extrauterine spread and patients with uterine papillary serous carcinoma (UPSC) or clear cell carcinoma (CCC) are at increased risk of relapse and represent a therapeutic challenge. Surgical treatment remains the cornerstone of therapy. Hysterectomy with bilateral salpingo-oophorectomy, washings, and careful assessment for intra-abdominal tumor should be performed with pelvic and para-aortic lymph node dissection when indicated based on grade of

Post-parathyroidectomy hypocalcemia: incidence, risk factors, and management.

The purpose of this study was to evaluate the incidence and severity of hypocalcemia after parathyroidectomy and delineate its risk factors. Data was retrieved from a prospective database. Patients with postoperative hypocalcemia were identified and risk factors were investigated including primary versus renal hyperparathyroidism (HPT), preoperative calcium, parathyroid hormone (PTH) and alkaline phosphatase levels, gland weight, pathology, extent of surgery, and reoperative surgery. Of the 162 patients who underwent parathyroidectomy, 84 (52%) were hypocalcemic postoperatively: 55 (42%) of 13

HIV lipodystrophy: risk factors, pathogenesis, diagnosis and management.

HIV lipodystrophy is a heterogeneous syndrome, which has yet to be objectively defined, comprising peripheral lipoatrophy, central fat accumulation and lipomata, along with hyperlipidaemia, insulin resistance and lactic acidaemia. Both nucleoside analogues and protease inhibitors are involved, but there are also host factors that probably place some patients at greater risk. The pathogenesis is increasingly understood, with evidence of interference of several regulatory proteins such as sterol regulatory enhancer binding protein-1, the proteasome, mitochondrial DNA polymerase gamma and GLUT-4.

[Brugada's syndrome: epidemiology, risk stratification, and clinical management].

Brugada syndrome is an arrhythmogenic disease, characterized by syncope and sudden cardiac death, with a typical electrocardiographic pattern: right bundle branch block and ST segment elevation in the right precordial leads. Only recently, the first gene causing Brugada syndrome has been demonstrated by the identification of mutations in SCN5A, the gene encoding for the cardiac sodium channel, also responsible for the LQT3 subtype of long QT syndrome. Despite the knowledge on Brugada syndrome has dramatically improved in the recent years, the clinical management is still often empirical and li

Risk management when operating in the posterior mandible.

A risk management protocol for the placement of implants in the posterior mandible is a sensible precaution for reducing the risk of nerve damage and the risk of possible litigation. This paper identifies the need for careful treatment planning and the use of relevant tests (e.g., the CT scan). A protocol is presented that advised the use of certain procedures to reduce the risk of nerve damage. These procedures consider the incision, the type of anesthetic, x-rays as you proceed, the type of implant, and use of anti-inflammatories.

Urinary catheter management: minimizing the risk of infection.

With catheterization comes the risk of infection and therefore people should not be catheterized unless their clinical condition dictates that it is absolutely necessary. Nurses are responsible for both inserting catheters and the subsequent management of the catheterized patient. A high level of nursing knowledge and skill is required to achieve effective and safe management. This article continues the infection control series by reviewing the principles of catheter management with regard to controlling infection.

Percutaneous Coronary Interventions: Bleeding Risk Assessment and Management.

Bleeding is one of the most common and feared complications after coronary angiography and percutaneous coronary intervention (PCI). It is associated with longer hospital stays, higher mortality, and worse clinical outcomes. As the number of patients at high bleeding risk (HBR) continues to grow, preventing bleeding has become an important part of PCI management. This practical review summarizes current evidence on how to assess bleeding risk and reduce bleeding before, during, and after PCI. Several tools, including the BARC classification, ARC-HBR criteria, PRECISE-DAPT, and DAPT scores, hel

Drain management after pancreatoduodenectomy: risk-stratified dynamic algorithm.

BACKGROUND: The ability of drain fluid amylase (DFA) to predict pancreatic fistula (POPF) and safely guide early drain removal is unknown in high-risk pancreatoduodenectomy (HR-PD). The aim of this analysis was to develop a new risk-based algorithm for drain management after pancreatoduodenectomy. METHODS: All consecutive patients undergoing pancreatoduodenectomy (2019-2024) at two institutions were included. During surgery, patients were stratified into HR-PD, defined according to the International Study Group on Pancreatic Surgery score (soft gland and duct ≤ 3 mm), and non-HR-PD. Postopera

Weight Management Before Pregnancy to Reduce Cardiovascular Risk.

Cardiovascular disease remains the leading cause of morbidity and mortality among women globally, with growing evidence suggesting that cardiovascular risk can begin developing well before pregnancy, even during the preconception period. Among the most significant contributing factors is excess prepregnancy weight, which has been associated with a higher risk of adverse maternal and fetal outcomes, including hypertension-related disorders, gestational diabetes, preeclampsia, and long-term cardiovascular complications. As obesity rates continue to rise among women of reproductive age, preconcep

Post-stroke seizures: classification, risk prediction, and management.

Stroke is the leading cause of adult-onset epilepsy. The International League Against Epilepsy (ILAE) classifies seizures occurring within 7 days of stroke as acute symptomatic seizures (ASyS); later seizures are classified as unprovoked/remote symptomatic. After a single remote symptomatic seizure, a diagnosis of post-stroke epilepsy (PSE) requires an estimated 10-year recurrence risk of at least 60%. We review three frequently conflated questions: how post-stroke seizures should be classified, how the risk of subsequent epilepsy should be estimated, and how patients should be managed. The 20
