Preventing Covert Brain Infarct-Related Cognitive Impairment and Dementia.

Covert brain infarcts (CBIs) are five times more prevalent than symptomatic brain infarcts. CBIs are associated with cognitive impairment and therefore may be a target for preventing cognitive decline and dementia. This review focuses on strategies for preventing CBI-related cognitive impairment, either by preventing incident or recurrent CBI or by enhancing cognitive reserve. CBIs begin to become prevalent during midlife and are highly prevalent in later life. The distribution of vascular pathologies of CBI differs from those that cause symptomatic stroke; therefore, preventive treatments may

HIV-risk reduction intervention for juvenile offenders on probation: The PHAT Life group randomized controlled trial.

OBJECTIVE: Juvenile offenders report high rates of sexual risk taking, increasing the possibility of HIV. This 2-arm group randomized controlled trial tested the efficacy of PHAT Life, a sexual risk reduction program, compared with a time-matched health promotion program for youth on probation. METHOD: Male and female 13- to 17-year-olds (M = 16.08; SD = 1.09) recently arrested and placed on probation at an Evening Reporting Center were eligible for the study. Youth were 66% male, and 90% African American. Teens self-reported their sexual behavior (condom use, number of sexual partners) at ba

Drug treatment as HIV prevention: a research update.

Drug use continues to be a major factor fueling the global epidemic of HIV infection. This article reviews the current literature on the ability of drug treatment programs to reduce HIV transmission among injection and noninjection drug users. Most data come from research on the treatment of opiate dependence and provide strong evidence on the effectiveness of medication-assisted treatment for reducing the frequency of drug use, risk behaviors, and HIV infections. This has been a consistent finding since the epidemic began among diverse populations and cultural settings. Use of medications oth

Beyond the bisexual bridge: sexual health among U.S. men who have sex with men and women.

CONTEXT: Men who have sex with both men and women (MSMW) experience health problems in ways that distinguish them from men who only have sex with men (MSM) and men who only have sex with women (MSW). Historically, an undue focus on MSMW's potential role in transmitting HIV to women has resulted in limited understanding of these men's unique sexual health needs. This article discusses the sexual health of MSMW in the U.S. EVIDENCE ACQUISITION: The author searched PubMed, Sociological Abstracts, PsycINFO, and GoogleScholar to acquire peer-reviewed studies pertaining to MSMW that were published

Reduced Risk-Taking After Prior Losses in Pathological Gamblers Under Treatment and Healthy Control Group but not in Problem Gamblers.

A group of pathological gamblers and a group of problem gamblers (i.e., gamblers at risk of becoming pathological) were compared to healthy controls on their risk-taking propensity after prior losses. Each participant played both the Balloon Analogue Risk Taking task (BART) and a modified version of the same task, where individuals face five repeated predetermined early losses at the onset of the game. No significant difference in risk-taking was found between groups on the standard BART task, while significant differences emerged when comparing behaviors in the two tasks: both pathological ga

Guiding Risk Reduction for Breast Cancer-Relevant Chemicals (BC-Chems): Chemical Production Volumes, Sources, Intake Levels, Emissions, and Regulations.

Breast cancer (BC) among women in all age groups continues to increase. Changes in reproductive risk factors cannot explain these trends, and rising rates among women under 40 cannot be explained by cancer screening, suggesting other factors are involved. We recently identified 920 chemicals whose biological activity suggests they can increase BC risk, the breast cancer-relevant chemical (BC-Chem) list, because they induce mammary tumors in rodents or activate estrogen or progesterone signaling in vitro. To develop exposure reduction strategies and inform BC research, we used US government dat

A framework for ecologically and socially informed risk reduction before and after outbreaks of wildlife-borne zoonoses.

Despite increasing emphasis being placed on the inclusion of upstream ecological and social perspectives for zoonotic disease control, few guidelines exist for practitioners and decision makers to work with communities in identifying suitable, locally relevant interventions and integrating these into public health action plans. With an interdisciplinary group of Kenyan stakeholders, we designed and tested a comprehensive framework for the co-design, evaluation, and prioritisation of beneficiary-oriented, ecologically and socially informed interventions for preventing and controlling outbreaks

Ovarian cancer risk reduction by salpingectomy during non-gynaecological surgery: scoping review.

BACKGROUND: Ovarian cancer is the leading cause of death among gynaecological cancers. The identification of the fallopian tube epithelium as the origin of most ovarian cancers introduces a novel prevention strategy by removing the fallopian tubes during an already indicated abdominal surgery for another reason, also known as an opportunistic salpingectomy. This preventive opportunity is evidence based, recommended and established at the time of gynaecologic surgery in many countries worldwide. To expand interest among surgeons in performing a salpingectomy during non-gynaecological surgery, t

The Synergy of Percutaneous Coronary Intervention and Lifestyle Modification in Reducing Mortality and Blockage Prevention.

In developed countries, cardiovascular diseases are the leading cause of death. Cardiovascular disease risk factors can be categorized as non modifiable (age, sex, race, and family history) or modifiable (hypertension, hyperlipidemia, diabetes, obesity, smoking, poor nutrition, stress, and sedentary lifestyle). Over 50% of cardiovascular events and deaths can be attributed to modifiable risk factors. The American Heart Association developed the "Life's Simple 7" framework, which illustrates how lifestyle modifications-such as increasing physical activity, improving diet, giving up smoking, and

Acute drug administration in epilepsy: a review.

AIMS: To review acute administration of drugs in epilepsy for indications other than status epilepticus. DISCUSSION: This review looks into the application of acute drug administration (ADA) against febrile and prolonged nonfebrile seizures in children, seizure clustering (habitual or at drug withdrawal), catamenial epilepsy, response to seizure "warnings", and prophylaxis of seizures at perceived increased risk (reflex epilepsies, long-distance travel, lifestyle, and social occasions). The drugs most commonly used for ADA are the benzodiazepines diazepam (oral or rectal), clobazam and buccal

Gout and its comorbidities.

Gout is a chronic disease in which excessively high levels of serum urate (hyperuricemia) result in tissue depositions of sodium urate crystals and intermittent inflammatory attacks. Patients who have gout frequently experience a range of comorbidities, which complicates management and affects long-term prognosis. We review some of the more important of these comorbidities and consider the extent to which gout or hyperuricemia may be either a consequence or a cause of these related conditions. In addition, we briefly consider several neurological conditions in which the presence of gout or a h

Association of family history of cardiometabolic diseases (CMDs) and individual health behaviours: Analysis of CARRS study from South Asia.

OBJECTIVES: Family history is considered as an important predictor of cardiovascular diseases (CVDs) and diabetes. Available research findings suggest that family history of chronic diseases is associated with perceived risk of disease and adoption of healthy behaviours. We examined the association between family history of cardio-metabolic diseases (CMDs) and healthy behaviours among adults without self-reported CMDs. METHODS: Cross-sectional data of 12,484 adults, without self-reported CMDs, from the baseline survey of Centre for cArdiometabolic Risk Reduction in South-Asia (CARRS) cohort s

Lifestyle changes are related to reductions in depression in persons with elevated coronary risk factors.

This observational study investigates whether persons with elevated coronary risk factors (CRFs > 3 and/or diabetes) and depression [i.e., ≥ 16 on the Center for Epidemiological Scale - Depression (CES-D)] can make changes in health behaviours over 3 months and improve depressive symptoms and other CRFs. Analyses were based on data from 310 men and 687 women enrolled in the high-risk arm of the Multisite Cardiac Lifestyle Intervention Program, targeting diet (10% fat), exercise (3 h per week) and stress management (7 h per week). As expected, at study entry, depressed persons had a more advers

Validation, Deployment, and Real-World Implementation of a Modular Toolbox for Alzheimer's Disease Detection and Dementia Risk Reduction: The AD-RIDDLE Project.

The Real-World Implementation, Deployment, and Validation of Early Detection Tools and Lifestyle Enhancement (AD-RIDDLE) project, recently launched with the support of the EU Innovative Health Initiative (IHI) public-private partnership and UK Research and Innovation (UKRI), aims to develop, test, and deploy a modular toolbox platform that can reduce existing barriers to the timely detection, and therapeutic approaches in Alzheimer's disease (AD), thus accelerating AD innovation. By focusing on health system and health worker practices, AD-RIDDLE seeks to improve and smooth AD management at an

Advancing Australian public health initiatives targeting dementia risk reduction.

Public health initiatives aim to improve health outcomes for populations by preventing disease and ill-health consequences of environmental hazards and natural or human-made disasters. Whilst public health initiatives have been used successfully to modify behaviours for chronic diseases, many initiatives targeting reduced dementia risk in older adults suffer from conceptual and statistical flaws that greatly limit their usefulness. The limited success in modifying lifestyle dementia risk factors has led us to fall short in building a successful roadmap to dementia risk reduction. Here we argue

Gambling as social practice: a complementary approach for reducing harm?

BACKGROUND: Gambling is now a well-recognised public health issue and forms the focus of extensive harm reduction initiatives. Recent developments in policy, practice and technology, such relaxation of regulations, the increasing influence of global gambling corporations, and the development of devices such as mobile phone apps and fixed odds betting terminals (FOBTs) mean that the landscape is a complex, dynamic, and fast moving one. Gambling is now practiced using new technologies, in various spaces and places, and features in a range of social surroundings. Therefore, research is needed to
