Effect of solvents on apically extruded debris and irrigant during root canal retreatment using reciprocating instruments.

AIM: To evaluate the effect of several solvents on the weight of apically extruded debris and irrigant during retreatment using a novel agar gel model. METHODS: Orange oil, turpentine oil and chloroform were used as solvents. Eighty single straight-rooted extracted human mandibular premolar teeth with a single root canal were divided into four groups (n = 20). All specimens were root canal-filled and weighed prior to their insertion into a prepared 1.5% agar gel model. The mean initial weights were measured by subtracting the weight of the specimen from the weight of the test apparatus and re

A new approach to the basic principles of endodontics.

The basic principles on which successful endodontic therapy depends have been commonly accepted as (1) debridement, (2) sterilization and (3) obturation. Each of these principles is subjected to critical evaluation. Because of the morphological complexity of most root canals complete debridement is not possible. No sampling technique is yet available to ensure total sterility of the prepared root canal nor is it possible to ensure that the root canal system is hermetically sealed by the filling material. Although it has been shown that 43 per cent of biopsies from the periapical area of endodo

Laser as an Accessory Treatment in Dentistry: A Narrative Review on Its Efficacy in Complex Root Canal System Infections, Pain Control, and Dental Tissue Regeneration.

Conventional root canal therapy relies on mechanical instrumentation and chemical irrigation; however, the complex anatomy of the root canal system often compromises complete disinfection, particularly of anaerobic bacterial biofilms, resulting in persistent or recurrent infections. Laser-assisted endodontic therapy has emerged as an effective adjunct, enhancing antimicrobial efficacy through photothermal effects, photodynamic action, and cavitation, thereby improving disinfection in anatomically inaccessible regions. In addition, laser therapy has been shown to stimulate stem cell proliferati

Clinical aspects of pulp stones: A case report series.

Pulp stones (PSs) are calcified masses that can be found in the pulp cavity of any deciduous or permanent tooth. They can be observed in healthy, diseased, and even unerupted or impacted teeth. Calcifications within the pulp may lead to a poorer outcome of a root canal procedure, as they can block access to the root canals, and hinder their subsequent cleaning and shaping. The paper describes 4 clinical cases of denticles located in the coronal and radicular pulp. Therapeutic methods of PS removal with different techniques and instruments are discussed. Specialist root canal treatment (RCT) wa

[Microbial decontamination of the root canals of devitalized teeth].

The primary goal of endodontic therapy is the reduction or elimination of microorganisms and their by-products from the root canal system. Although a number of instrumentation and irrigation techniques exist, debris is often left behind in the root canal system and proper canal cleaning, shaping, and irrigation are needed to reduce significantly or sometimes even eliminate microorganisms from the canals. Residual microbes in the root canal system are the primary cause of post-treatment apical periodontitis that may persist in both poorly and properly treated cases. Apical periodontitis is a se

Comprehensive Analysis of Mandibular First Molar Root and Canal Morphology in Saudi Patients Using Cone Beam Computed Tomography (CBCT).

BACKGROUND The goal of conventional endodontic therapy is to clean the root canal system thoroughly, restore tooth function, and prevent re-infection. Success depends on understanding root canal morphology, resolving apical periodontitis, and using proper techniques. Studies highlight variations in root canal anatomy, with cone-beam computed tomography (CBCT) being crucial for accurate diagnosis despite its high cost and limited availability. MATERIAL AND METHODS This retrospective study reviewed CBCT images of 1820 (907 male, 913 female) patients aged 18 to 78 years. Analysis included 2081 ma

Conventional retreatment versus surgery in failed root canal treatment--the importance of canal anatomy.

This case report highlights two important aspects of root canal treatment, knowledge of root canal anatomy and inappropriate use of surgery to correct failure of conventional treatment. Initially, two mandibular incisors were root treated conventionally but only one of the two canals present in both teeth was prepared and filled in each case. Symptoms persisted and a surgical approach was adopted but, again, only one of the root canals in each tooth was located and sealed with a root-end filling. Surgery was performed on a second occasion but again failed to resolve the patient's problems. On

[Minimal intervention in pulpal and periapical pathology].

Root canal treatment has been shown to be a predictable procedure with a favourable outcome in the case of pulpitis and apical periodontitis. The most important outcome measure is retention of an asymptomatic and functional tooth. When teeth are lost after root canal treatment, this is often the result of fracture due to weakening. By preventing or postponing root canal treatment, the long-term prognosis of the tooth may be better. Minimal interventions in endodontics are vital pulp therapy (indirect pulp therapy and pulpotomy), postponing and monitoring, and partial revision.

Minimally Invasive Endodontic Treatment through the Labial/Buccal Cervical Approach - Protocol and Case Series.

INTRODUCTION: Traditional access cavity preparation involves removing the roof of the pulp chamber and smoothing the dentin bulges at the root canal orifice, thereby creating straight-line access. However, this may damage more healthy dental tissue and reduce the tooth's fracture resistance. This case series presents a novel minimally invasive endodontic protocol for 1 maxillary canine and 4 mandibular premolars, which required root canal therapy, due to labial/buccal cervical decay that caused pulpitis or periapical periodontitis. METHODS: Our treatment protocol includes the following steps:

Computational fluid dynamics investigation on the irrigation of a real root canal with a side-vented needle.

BACKGROUND: Root canal therapy is one of the main treatments for root canal diseases, and effective irrigation is the key to successful treatment. Side-vented needle is one of the commonly used needle types in clinic. In the real root canal, due to the influence of the curvature of the root canal, the irrigation flow field in different needle directions shows obvious differences. At the same time, changes in root canal curvature and working depth will lead to changes in irrigation efficiency and the flow field. Both the mainstream of the irrigation flow and the shear stress near the wall chang
