Endovascular treatment of traumatic thoracic aortic rupture combined with diaphragmatic rupture--a case report.

The purposes of this report are to describe the immediate repair of a diaphragmatic rupture and subsequent endovascular treatment of a thoracic aortic rupture secondary to blunt chest and abdominal trauma and to discuss the outcome in the light of the trauma scores. A 29-year-old man was injured in a car collision. There were signs of a left diaphragmatic rupture and spleen injury. After urgent laparotomy, the diaphragm was repaired. The spleen appeared uninjured; however, there was a dissection with a transmural tear of the thoracic aorta, which was subsequently treated intraluminally by usin

Postinfarct cardiac free wall rupture: the relationship of rupture site to papillary muscle insertion.

The objective of this study was to examine for any relationship between the sites of papillary muscle insertion in left ventricular free wall (LVFW) and the site of LVFW rupture postinfarct. Twenty-five consecutive patients with LVFW rupture (12 men and 13 women, mean age 72.3 yr, range = 48 to 93) at the University of Ottawa Heart Institute during the period of July 1, 1988 to 1992 were studied. Clinical charts were reviewed, and the Formalin-fixed hearts were re-examined grossly and microscopically. Each patient died of tamponade or after surgery for tamponade. Eight (32%) of the 25 infarcts

Ventricular free-wall rupture, ventricular pseudoaneurysm, and papillary muscle rupture complicating acute myocardial infarction.

The prevalence of mechanical complications following acute myocardial infarction has steadily declined in recent years owing to advances in prompt coronary revascularization, and they now occur in <1% of acute myocardial infarction cases. Nevertheless, significant haemodynamic impairment may already be present at hospital admission, requiring immediate diagnostic evaluation and urgent intervention. Until recently, surgical repair was the only treatment option, with non-negligible in-hospital mortality rates, particularly among patients with acute cardio-circulatory failure. Advances in transca

Left ventricular pseudoaneurysm rupture following prosthesis infective endocarditis after closure of ventricular septal rupture.

BACKGROUND: Left ventricular pseudoaneurysm (LVP) is a rare but life-threatening complication of myocardial infarction or cardiac surgery, typically arising within the first few years after ventricular septal rupture (VSR) repair. Delayed rupture associated with prosthetic infection is extremely uncommon and carries high mortality. CASE PRESENTATION: We report the case of a 47-year-old man with a history of post-infarction VSR repair using the infarct exclusion technique. The patient remained stable for nearly nine years before undergoing transcatheter closure of a residual VSR with an Amplat

Rupture mechanics of blood clots: Influence of fibrin network structure on the rupture resistance.

Embolization is a leading cause of mortality, yet we know little about clot rupture mechanics. Fibrin provides the main structural and mechanical stability to blood clots. Previous studies have shown that altering the concentration of coagulation activators (thrombin or tissue factor (TF)) has a significant impact on fibrin structure and viscoelastic properties, but their effects on rupture properties are mostly unknown. Toughness, which corresponds to the ability to resist rupture, is independent of viscoelastic properties. We used varying TF concentrations to alter the structure and toughnes

Delayed Spleen Rupture After Trauma.

The spleen is the most commonly injured solid organ in patients sustaining blunt abdominal trauma. The widespread use of computed tomography (CT) has significantly improved its diagnosis and grading. Delayed splenic rupture (DSR) refers to hemorrhage occurring more than 48 hours after blunt abdominal trauma in a patient who was initially hemodynamically stable. Although uncommon, it remains a recognized complication of splenic injury and can occur days to weeks after the initial trauma and carries significant mortality. We present a case of DSR presented one week after the injury, which was ma

[GASTROCNEMIUS RUPTURE - DIAGNOSIS AND TREATMENT].

Gastrocnemius muscle ruptures are frequent in both recreational and competitive athletes. The superficial posterior compartment of the leg includes the gastrocnemius and soleus muscles, and a tear can occur in either one of them or both. Gastrocnemius injuries usually occur in young adult male athletes in their 40s. Moreover, they are common in sport involving eccentric lower extremity movements such as football, basketball, and tennis. Injuries are more common in athletes who have not warmed up adequately or are fatigued. The diagnosis is based on the patient's history and physical examinatio

Spleen rupture due to brucellosis.

Brucellosis is a common zoonotic infection worldwide; it is caused by infection with the bacterial species Brucella and leads to severe diseases in humans and animals. In Turkey, this bacterial species has not been completely eradicated and is commonly found in animals (such as goats or sheep). Brucellosis can lead to various symptoms, affect multiple systems, and cause splenomegaly in the case of spleen involvement. In contrast to traumatic spleen ruptures, spontaneous spleen ruptures are rare and most commonly occur because of infectious causes. A 52-year-old man was treated at our infectiou

Ossified proximal patellar tendon rupture.

Extensor mechanism injuries are not uncommon in young active individuals. Patellar tendon is a part of extensor mechanism of the knee which is commonly ruptured due to forced eccentric contraction against flexed knee. There have been reports of pathological changes in the patellar tendon which eventually lead to the rupture. The common pathologies include hypoxic tendinopathy, mucoid degeneration, calcific tendinopathy and tendolipomatosis. We report a rare case of ossified proximal patellar tendon rupture in a fit and active skittle player, who sustained indirect injury to knee while playing

Periviable Premature Rupture of Membranes.

Periviable deliveries (less than 26 weeks) are a small percentage of deliveries but account for a disproportionately high number of long-term morbidities. Few studies describe interventions and outcomes for periviable preterm premature rupture of membranes (PPROM). The available reports may include only those neonates who received resuscitation, making interpretation and application difficult. Counseling should consider the impact of oligohydramnios on fetal lung development. This article discusses standard and experimental interventions that may offer neonatal benefit. Antenatal corticosteroi

Splenic rupture after elective cardioversion.

Splenic laceration and rupture are common phenomena among patients in a traumatic setting, especially in blunt trauma. Much more unusual, however, is splenic injury without a known insult. Several case reports and studies have been written about spontaneous splenic injury in patients with viral, haematological or malignant processes. Recently, we encountered a patient with a spontaneous splenic rupture and no preceding trauma apart from semielective cardioversion. Operative decision-making was complicated by the fact that he required systemic anticoagulation for atrial fibrillation. He eventua

Spontaneous bilateral quadriceps tendon rupture.

Spontaneous bilateral quadriceps tendon rupture is a rare finding in emergency departments. Thus, the pathophysiology is not well understood. Imaging for improved speed of diagnosis is rarely considered. We present a case of non-traumatic spontaneous bilateral quadriceps tendon rupture and examine current literature on the pathophysiology and imaging guidelines on the topic. The patient is a 49-year-old male that presented to the emergency department with bilateral thigh pain. He had been seen earlier with similar pain, but now presents with increased difficulty ambulating. The patient was fou

[Idiopathic and Iatrogenic Esophageal Rupture].

Esophageal rupture is a rare emergency disorder. The leakage of gastric contents and foods can cause severe mediastinitis and pyothorax, occasionally resulting in the development of systemic sepsis and shock within hours. Iatrogenic injury to the esophagus is the most frequent cause of esophageal rupture, followed by spontaneous rupture. A plain chest roentgenogram, esophagogram, and chest computed tomography (CT) are necessary for the diagnosis of esophageal rupture, and early diagnosis reduces the rate of complications and mortality. The principle treatment is surgery. Drainage of the conten

Spontaneous splenic rupture in pregnancy.

Splenic rupture during pregnancy is a rare and can frequently be a misdiagnosed pathology. This rupture is associated with a high maternal and fetal mortality rate. A 26 years old Moroccan woman para II gravida II presented at the third stage of pregnancy with acute onset of severe abdominal pain. She developed immediately a hypovolemic shock. After both a physical and sonographical exam, it was revealed that it was due to a massive hemoperitoneum. Therefore, an emergent laparotomy and cesarean delivery with abdominal exploration were performed; also, an active bleeding was identified at the s
