Considerations on the role of buprenorphine in recovery from heroin addiction from a UK perspective.

The United Kingdom Drug Strategy emphasises recovery as a key focus in the treatment of drug dependence. A framework for recovery is defined in the Recovery-Orientated Drug Treatment report, written by an expert working group, and comprises four key phases: engagement and stabilisation, including the establishment of treatment goals; preparation for change, involving engagement in psychosocial and pharmacological interventions; active change, including detoxification and medical withdrawal; and completion, including interventions that strengthen community integration. A body of evidence suppor

Increased risk of left heart valve regurgitation associated with benfluorex use in patients with diabetes mellitus: a multicenter study.

BACKGROUND: Benfluorex was withdrawn from European markets in June 2010 after reports of an association with heart valve lesions. The link between benfluorex and valve regurgitations was based on small observational studies and retrospective estimations. We therefore designed an echocardiography-based multicenter study to compare the frequency of left heart valve regurgitations in diabetic patients exposed to benfluorex for at least 3 months and in diabetic control subjects never exposed to the drug. METHODS AND RESULTS: This reader-blinded, controlled study conducted in 10 centers in France

Adjunctive memantine for opioid use disorder treatment: A systematic review.

Memantine is commonly used for the treatment of moderate-to-severe Alzheimer's disease. Due to its antagonism of the N-methyl-d-aspartate (NMDA) receptor, which has been shown to block rewarding and reinforcing effects of morphine, memantine has been investigated for potential utilization in opioid use disorder (OUD). The objective of this systematic review is to assess the evidence available to determine the safety and efficacy of memantine as treatment for OUD. Pubmed (1946-August 2019) and Embase (1947-August 2019) were queried using the following search terms: opioid-related disorders, opi

Is uveitis associated with topiramate use? A cumulative review.

Occasional reports of uveitis following topiramate use necessitated an investigation of relevant cases from safety databases and published biomedical literature. Data mining of the Food and Drug Administration Adverse Event Reporting System and cumulative review of cases from the global safety database (sponsor database) and published literature were conducted to assess association between topiramate use and uveitis. The Food and Drug Administration Adverse Event Reporting System search identified disproportional reporting of uveitis (n=23) and related terms (choroidal detachment, n=25; iridoc

Intelligent evaluation system for anticoagulant and antiplatelet therapy in surgical patients.

BACKGROUND: Preoperative evaluation of bleeding and thromboembolic risks in patients receiving anticoagulant or antiplatelet therapy remains a clinical challenge. Decisions regarding drug discontinuation, withdrawal timing, and resumption must carefully balance the risks of hemorrhage and thromboembolism. We developed an innovative, integrated intelligent evaluation system to assist clinicians in minimizing intraoperative bleeding, postoperative ischemic stroke, myocardial infarction, and diabetic ketoacidosis. METHODS: The system's functionality was enhanced through the following: (1) Integr

An overview of intravenous amisulpride as a new therapeutic option for the prophylaxis and treatment of postoperative nausea and vomiting.

Introduction: Current therapies of postoperative nausea and vomiting (PONV) are based on a combination of antiemetics from different pharmacological classes. Dopamine receptor antagonists are one of the cornerstones of such multimodal antiemetic approach, with droperidol being the best studied representative of this group. Droperidol's use has significantly declined after the FDA's black-box warning in 2001 due to its QT-prolonging properties. Amisulpride is a promising antiemetic agent which could fill this gap.Areas covered: In this review, the authors discuss the pharmacological profile as

Effects of CYP2C19*17 genetic polymorphisms on plasma and saliva concentrations of diazepam in patients with alcohol withdrawal syndrome.

INTRODUCTION: Diazepam is one of the most commonly prescribed tranquilizers for the therapy of alcohol withdrawal syndrome (AWS). However, diazepam therapy often turns out to be ineffective, and some patients experience dose-dependent adverse drug reactions. Previous studies have shown that the metabolism of diazepam involves the CYP2C19 isoenzyme, whose activity is highly dependent on polymorphism of the encoding gene. OBJECTIVE: The study aimed to investigate the effects of CYP2C19*17 genetic polymorphisms on plasma and saliva concentrations of diazepam as well as its impact on the efficacy

Efficacy and safety of alginate formulations in patients with gastroesophageal reflux disease: a systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: Alginate formulations are increasingly being used for treating gastroesophageal reflux disease (GERD). However, the benefits of alginate versus control or proton pump inhibitors (PPIs) are somewhat unclear. We performed a systematic review and meta-analysis to summarize data from recent randomized controlled trials (RCTs) comparing the efficacy and safety of alginate-based formulation with PPIs or control for the treatment of GERD. MATERIALS AND METHODS: PubMed, Embase, Scopus, BioMed Central, CENTRAL, and Google scholar databases were searched from 1st January 2000 to 15th June 20

In-depth understanding of the structure-based reactive metabolite formation of organic functional groups.

Idiosyncratic drug-induced liver injury (DILI) is a leading cause of drug attrition and/or withdrawal. The formation of reactive metabolites is widely accepted as a key factor contributing to idiosyncratic DILI. Therefore, identifying reactive metabolites has become a critical focus during lead optimization, and a combination of GSH-/cyano-trapping and cytochrome P450 inactivation studies is recommended to identify compounds with the potential to generate reactive metabolites. Daily dose, clinical indication, detoxication pathways, administration route, and treatment duration are the most cons

Gemcitabine-Induced Drug Rash With Eosinophilia and Systemic Symptoms (DRESS Syndrome): A Case Report.

Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) is a rare, severe, and potentially life-threatening hypersensitivity reaction characterized by fever, diffuse cutaneous eruption, hematologic abnormalities, most notably eosinophilia, and multiorgan involvement, predominantly hepatic. While classically associated with anticonvulsants, allopurinol, or sulfonamides, this syndrome may occasionally be triggered by antineoplastic agents. Gemcitabine, a nucleoside analogue with a generally favorable safety profile, is widely used in the treatment of various solid tumors. However, exceptio

Physiologically-based pharmacokinetic modeling of belimumab for systemic lupus erythematosus patients to predict the withdrawal time in pregnancy and vaccine time in infants.

Belimumab (BEL) is a monoclonal antibody that targets B cell activating factor (BAFF) and has been approved for treating systemic lupus erythematosus (SLE). However, its optimal withdrawal time during pregnancy and the appropriate vaccine timing for infants remain unresolved. This study aimed to analyze variations in the plasma concentrations of BEL in pregnant SLE patients and infants using a physiologically-based pharmacokinetic (PBPK) model. Additionally, it provided evidence-based guidance on drug withdrawal timing and infant vaccination schedules. A PBPK model was developed and validated

"Anything can happen": experiences of people using opioids in a xylazine market.

BACKGROUND: Xylazine is associated with a number of negative health outcomes, such as severe wounds and withdrawal. As xylazine increasingly emerges in drug markets across the United States, there is an urgent need for a national response to reduce related harm. As such, healthcare systems, harm reduction organizations, and community members continue to work to identify and develop the best approaches for harm reduction interventions to address xylazine challenges. This study engaged people who use opioids (PWUO) in Philadelphia to explore current experiences with and responses to xylazine adu

Improving hospital care for people who use drugs: deliberative process development of a clinical guideline for opioid withdrawal management.

BACKGROUND: Management of opioid withdrawal in hospital settings is crucial to improve treatment completion and health outcomes among patients who use opioids, such as heroin. Evidence-based clinical guidelines can support responsive provision of opioid substitution therapy (OST). In England there is no standardised application of guidance for substance dependence management across National Health Service (NHS) Hospitals. A recent review of NHS hospital policies identified varying approaches to managing opioid withdrawal and procedural barriers to timely medication. OBJECTIVE: To develop a cl
