Minimizing H.R.1-related Medicaid coverage disruptions for high-risk patients.

Forthcoming H.R.1 Medicaid eligibility changes requiring documentation of work, school, or volunteer activities are projected to result in significant disenrollment. High-risk beneficiaries, including those experiencing homelessness, are more likely to experience barriers in navigating the eligibility process, such as qualifying for work requirement exemptions. We utilized an existing set of linked data across the medical safety net, state Medicaid agency, housing services providers, and justice agencies as a case study to understand the prevalence of select potential medical and social exempt

Outcomes from a trainee-run ovulation induction program at a large urban safety net hospital.

PURPOSE: To improve access to fertility care at the largest safety net hospital in New York City, fellows and residents run a reproductive endocrinology and infertility clinic that supports an ovulation induction (OI) program under attending physician supervision. Our objective was to evaluate OI pregnancy outcomes to describe the program's efficacy and guide quality improvement. METHODS: We performed a descriptive study of patients who completed at least one OI cycle from 6/1/2019 to 4/1/2023. Fellows and residents managed patient care, including the prescription of an OI agent (clomiphene c

Sociodemographic Factors Associated with Opioid Agonist Treatment Receipt and Retention at an Integrated Safety-Net Health System.

INTRODUCTION: Sociodemographic differences in opioid use disorder (OUD) treatment engagement have implications for population health. Continuum of care models can guide the opioid epidemic response by identifying differences in treatment patterns. We evaluated one such model at an integrated, safety-net health system in Denver, Colorado and examined the association of patient characteristics with opioid agonist treatment (OAT) receipt and retention. METHODS: We identified a cohort of patients with OUD from 2023 to 2024 from electronic health records. We used multivariable regression to estima

National Outcomes of Increasing Cervical Cancer Screening in Federally Qualified Health Centers.

IMPORTANCE: Federally qualified health centers (FQHCs) deliver health care to more than 30 million people across the US and often face structural constraints that affect their ability to provide cervical cancer screening (CCS). OBJECTIVES: To estimate the number of underscreened individuals served by FQHCs and the outcomes of improving CCS in FQHCs to meet the Healthy People 2030 goal of 79.2%. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional analysis used data from the Uniform Data System reported in 2023 by 1352 FQHCs across the US, self-reported survey-weighted CCS prevalence from

Designing equitable telehealth solutions for outpatient surgical care in a safety-net population: a human-centered design approach.

BACKGROUND: The SARS CoV-2 (COVID-19) pandemic catalyzed a dramatic shift in healthcare delivery, with telemedicine emerging as a common mode of care provision. While pre-pandemic telemedicine services were more commonly used for preventive visits and had better adherence among younger and more affluent demographics, the landscape of telehealth in the post-pandemic period has shifted significantly to include surgical visits and publicly-insured patient populations. Without specific insights from patients and clinicians to guide this transition, telehealth delivery risks exacerbating disparitie

Predictors for spontaneous pleurodesis in patients with indwelling pleural catheters for malignant pleural effusion: a safety net hospital experience.

BACKGROUND: Malignant pleural effusion (MPE) affects approximately 150,000 patients in the United States each year and usually signifies advanced-stage cancer. The optimal treatment remains a challenge but indwelling pleural catheters (IPC) offer several advantages and may help achieve spontaneous pleurodesis (SP) in some patients. OBJECTIVES: We aim to investigate the predictors of SP among patients with MPE, particularly in a resource-limited community-based safety net hospital. DESIGN: This is a retrospective cohort study done at a community-based safety net hospital. METHODS: Adults dia

Cross sectional analysis of an addiction consultation service, substance co-use patterns, and receipt of medications for opioid use disorder during hospitalization.

INTRODUCTION: Despite effective medications for opioid use disorder (MOUD), treatment engagement remains low. As the overdose crisis is increasingly characterized by opioids co-used with other substances, it is important to understand whether existing models effectively support treatment for patients who use multiple substances. Hospital-based addiction consultation services (ACS) have shown promise at increasing MOUD initiation and treatment engagement, but the effectiveness for patients with specific co-use patterns remains unknown. METHODS: Using 2016-2023 admissions data from a large safe

Depression rates among African American inflammatory bowel disease patients at a large safety net hospital.

AIM: Major depressive disorder is a prominent psychiatric illness in the United States. It has been found to be higher among patients with inflammatory bowel disease. However, few studies have focused on depression among minority populations with inflammatory bowel disease. Our study determined the prevalence of depression in minority patients with inflammatory bowel disease at our safety-net hospital, which serves a predominantly African American patient population. METHODS: We conducted a single centre retrospective cohort study at a large, urban outpatient centre. We retrieved the electron

Patterns of social determinants of health screening and health-related social needs among patients with newly diagnosed breast cancer in a safety-net system.

PURPOSE: Various social determinants of health (SDOH) have been shown to negatively impact breast cancer outcomes. Though there are recommendations for comprehensive SDOH assessment, little is known about the optimal frequency of reassessments and change in health-related social needs (HRSN) over time for specific patient populations. We sought to characterize SDOH screening and HRSN patterns among patients with breast cancer in a safety-net system. METHODS: A retrospective study was conducted to evaluate SDOH survey responses in the electronic medical records and screening frequency from pat

A mixed methods evaluation of a six-year primary care community safety net-academic partnership model.

To expand the tripartite mission to advance health equity, academic medical centers (AMCs) need to look outside their walls. In 2018, Stanford's primary care division launched the Community Partnership Program (CPP) between their AMC and local safety-net community health centers (CHCs) to support the goals of meaningful community engagement and addressing local clinic capacity needs. The CPP embeds Stanford physician faculty in local CHCs through service agreements. This study aimed to characterize the experiences of CHC leaders and faculty involved in the CPP to better understand barriers and

Breast cancer care quality and survival are not associated with hospital safety-net burden.

BACKGROUND: Safety-net hospitals anchor access for Medicaid/uninsured patients but operate under persistent financial pressure. Whether a higher safety-net burden influences breast cancer care quality or survival is unclear. METHODS: We conducted a retrospective cohort study using the National Cancer Database (2018-2022) for women aged ≥40 years with stage 0-IV breast cancer. Hospital safety-net burden (proportion of Medicaid/uninsured patients) was categorized as low (low-burden hospital), medium (medium-burden hospital), or high (high-burden hospital). The primary outcome was guideline-conc

Safety and Feasibility of Early Initiation of Peritoneal Dialysis at a Safety-Net Hospital.

IntroductionTraditionally peritoneal dialysis (PD) is initiated >3 weeks post-placement for tract maturation. The pandemic spurred a need for earlier initiation. We hypothesized that there is no significant difference between outcomes of early start (<2 weeks) vs standard (>2 weeks).MethodsA retrospective review was conducted between 2020 to 2022 at a safety-net hospital and included patients who received a PD catheter ("PDC"). Demographics, technique, dialysate volume, and outcomes were analyzed.Results83 patients met criteria. 47/83 (56.6%) were male, the average age was 49.9 [41-60] years,
