A practical clinical approach to the diagnosis and management of morphea (localized scleroderma).

Morphea, also referred to as localized cutaneous scleroderma, encompasses a heterogeneous group of inflammatory and fibrosing skin disorders characterized by variable depth of tissue involvement and different clinical course. The diagnostic delay remains common due to insidious onset, broad differential diagnosis, and lack of validated biomarkers of disease activity. Clinical assessment relies therefore on expert evaluation, supported by clinical scoring systems and selected imaging techniques. Therapeutic strategies depend on disease subtype, activity, depth, and risk of irreversible damage,

Single-Cell Transcriptome Analysis Identifies Subclusters with Inflammatory Fibroblast Responses in Localized Scleroderma.

Localized scleroderma (LS) is an autoimmune disease with both inflammatory and fibrotic components causing an abnormal deposition of collagen in the skin and underlying tissue, often leading to disfigurement and disability. Much of its pathophysiology is extrapolated from systemic sclerosis (SSc) since the histopathology findings in the skin are nearly identical. However, LS is critically understudied. Single-cell RNA sequencing (scRNA seq) technology provides a novel way to obtain detailed information at the individual cellular level, overcoming this barrier. Here, we analyzed the affected sk

Outcomes of Fat Grafting in the Active Versus Quiescent Phase of Localized Scleroderma.

Introduction: Progressive hemifacial atrophy (PHA) and linear scleroderma (LS) are both rare conditions and defined by atrophy and/or sclerosis of the skin and subcutaneous tissue. The ideal timing of reconstructive intervention in these patients is controversial. We compared the outcome and satisfaction of autologous lipofilling performed during active and stable phases of the diseases in adults. Methods: A retrospective chart review was conducted with all patients diagnosed with PHA or LS between 2007 and 2019 in our department. We analysed demographic data, clinical features, and surgical p

Treatment of juvenile localized scleroderma: current recommendations, response factors, and potential alternative treatments.

PURPOSE OF REVIEW: Juvenile localized scleroderma (jLS) is a chronic autoimmune and fibrosing disease associated with a high risk for functional impairment. Antifibrotic options are limited, so current treatment strategies are focused on disease activity control. Pediatric rheumatologists are in consensus on the need to treat with systemic immunosuppressants, in particular, methotrexate. However, more than 30% of patients fail initial methotrexate treatment. This review provides an update on current management and reviews reports on potential alternative treatments. RECENT FINDINGS: An overvi

[Solitary morphea profunda: An unusual form of localized scleroderma in childhood. Case report].

Solitary morphea profunda is an unusual form of localized scleroderma in childhood. It is characterized by a single, poorly defined and indurate plaque often located on the upper trunk near the spine. Solitary morphea profunda is frequently asymptomatic and shows no associated systemic involvement. Histological examination reveals dense sclerosis of collagen and a marked lymphocytic infiltrate in the reticular dermis and subcutis. We report a 7-year-old girl with a solitary asymptomatic and sclerotic plaque on the back. Clinical, ultrasonographic and histological features were consistent with

Esophageal abnormalities in juvenile localized scleroderma: is it associated with other extracutaneous manifestations?

OBJECTIVE: To assess esophageal involvement (EI) in juvenile localized scleroderma (JLS) population and the possible association between this gastrointestinal manifestation and demographic data, clinical features, laboratory exams, treatments and outcomes. METHODS: For a period of 30 years, 5881 patients with rheumatic diseases were followed in our Pediatric Rheumatology Division. EI was defined by the presence of symptoms (solid/liquid dysphagia, heartburn, esophageal regurgitation, nausea/vomiting and epigastralgia) and confirmed by at least one EI exam abnormality: barium contrast radiogra

Localized scleroderma progressing to systemic disease. Case report and review of the literature.

We describe a 15-year-old girl with biopsy-proven morphea who developed progression to systemic disease 2 years after initial presentation. In contrast to other reported patients with localized scleroderma, some of whom have had mild, nonprogressive systemic involvement, this patient developed severe, debilitating disease, with skin tightness, sclerodactyly, esophageal involvement, restrictive pulmonary disease, and myopathy. From the time of her initial evaluation, the patient was positive for antinuclear antibodies (ANA), which were shown to be primarily directed against the Ku antigens. Thi

20-MHz B-mode ultrasound in monitoring the course of localized scleroderma (morphea).

Ultrasonographic methods have recently provided us with the means for objective and non-invasive monitoring of the dynamics of chronic skin diseases. We examined 34 patients with localized scleroderma (morphea) using a 20-MHz B-mode ultrasound scanner (DUB 20, Taberna pro Medicum, Lüneburg). In patients with plaque-type and linear band-type localized scleroderma intraindividual comparison of sclerotic skin with corresponding areas of healthy skin showed thickening of the corium. The increase in corium thickness was between 2% and 251%. The extent of the difference in corium thickness between s

General and local scleroderma in children and dermatomyositis and associated syndromes.

Scleroderma in children virtually always manifests in the form of localized disease, whereas adult scleroderma usually manifests as systemic disease. The recent literature on childhood scleroderma is limited almost exclusively to localized disease. The treatment of this disease remains a dilemma, especially with regard to which patients should be treated, how they should be treated, and how the treatment response should be monitored. Our current therapeutic approach is reviewed. The recent literature regarding juvenile dermatomyositis is discussed. New understanding of the pathobiology of this

[Should watermelon stomach be considered to be an unrecognized localization of scleroderma?].

We reported a typical case of watermelon stomach which occurred during the course of a limited cutaneous systemic sclerosis. Watermelon stomach is an important source of upper gastrointestinal bleeding which requires endoscopic treatment. Such an association has already been described and we suggest that watermelon stomach could be an unrecognized localization of scleroderma stomach involvement. Because gut involvement may precede skin manifestations. A search of progressive systemic sclerosis should be done with clinical examination, antinuclear antibodies research (especially anticentromere

Single-Cell Profiling Reveals Divergent Mechanisms of Fibrosis in Localized Scleroderma and Systemic Sclerosis.

OBJECTIVE: Localized scleroderma (LoS) and systemic sclerosis (SSc) are both fibrotic diseases, but LoS is limited to the skin, whereas SSc involves systemic organ fibrosis. This study aimed to elucidate the mechanisms underlying these differences. METHODS: Skin biopsies from three healthy controls, three patients with LoS, and three patients with SSc underwent immunofluorescence and single-cell RNA sequencing (scRNA-seq). Key molecular functions were validated using in vitro assays and murine models. RESULTS: Immunofluorescence revealed a high prevalence of tertiary lymphoid structures (TLS

Incidence, prevalence, and mortality of localized scleroderma in Quebec, Canada: a population-based study.

BACKGROUND: Localized scleroderma is an understudied autoimmune disease characterized by fibrosis of the skin and/or subcutaneous tissue. To date, only 6 articles reported on the incidence and/or prevalence estimates globally, with significant design limitations and risk of bias. None of the studies originated from Canada or investigated mortality/geospatial epidemiology. We aimed to study the incidence, prevalence, mortality and spatiotemporal trends of localized scleroderma in Quebec, Canada, stratified by sex and age. METHODS: Quebec populational health administrative databases were used t

Unique and shared transcriptomic signatures underlying localized scleroderma pathogenesis identified using interpretable machine learning.

Using transcriptomic profiling at single-cell resolution, we investigated cell-intrinsic and cell-extrinsic signatures associated with pathogenesis and inflammation-driven fibrosis in both adult and pediatric patients with localized scleroderma (LS). We performed single-cell RNA-Seq on adult and pediatric patients with LS and healthy controls. We then analyzed the single-cell RNA-Seq data using an interpretable factor analysis machine learning framework, significant latent factor interaction discovery and exploration (SLIDE), which moves beyond predictive biomarkers to infer latent factors und

Coexistence of Localized and Systemic Juvenile Scleroderma: A Case Report and Review of Literature.

Juvenile scleroderma (JS) is a rare chronic connective tissue disorder characterized by progressive fibrosis of the skin and soft tissues with/without internal organ involvements. Scleroderma manifests itself in both systemic (SSc) and localized (LS) forms. This case report aimed to present an Iranian girl with coexistence of SSc and LS treated by Tocilizumab. A 12-year-old girl was referred to the rheumatology clinic with the chief complaint of asymmetrical facial appearance starting from two years ago and coexisting with restricted phalangeal motion for the last 6 months. Benefits of the Topical JAK Inhibitor Delgocitinib in a Patient With Pediatric Localized Scleroderma.

This is the first report of pediatric linear scleroderma successfully treated with the topical Janus kinase (JAK) inhibitor delgocitinib. JAK inhibitors targeting the JAK/STAT pathway have been used to treat various immune-mediated diseases. In both in vitro and in vivo, JAK inhibitors also block the transforming growth factor (TGF)-β-mediated effects that contribute to skin sclerosis. In the present case, the histological findings of inflammation and fibrosis were considered as conditions that would benefit from the anti-inflammatory and antifibrotic effects of the JAK inhibitor, delgocitinib

An Unusual Case of Deep Localized Scleroderma in a Patient with Chronic Kidney Disease.

Localized scleroderma (LS) is a rare fibrosing disorder of skin and underlying tissues. Although it can affect all races, it has a higher prevalence in whites. Deep LS is the least common among seven LS variants, representing less than 5% of cases, and typically affects areas of pressure such as the hips and waist. We report a unique clinical case of bilateral lower extremity deep LS in a 51-year-old Puerto Rican woman with chronic kidney disease (CKD). In patients with CKD, it is important to distinguish LS from nephrogenic systemic fibrosis (NSF). Both can present with skin fibrosis and cont
