Anatomical basis for impotence following haemorrhoid sclerotherapy.

Impotence has been reported as a rare but important complication of sclerotherapy for haemorrhoids. The relationship between the anterior wall of the rectum and the periprostatic parasympathetic nerves responsible for penile erection was studied to investigate a potential anatomical explanation for this therapeutic complication. A tissue block containing the anal canal, rectum and prostate was removed from each of six male cadaveric subjects. The dimensions of the components of the rectal wall and the distance between the rectal lumen and parasympathetic nerves in the periprostatic plexus were

Saphenofemoral incompetence treated by ultrasound-guided sclerotherapy.

BACKGROUND: Published long-term recurrence rates are higher after sclerotherapy than after surgery for the treatment of varicose veins involving greater saphenous tributaries associated with saphenofemoral incompetence (SFI). Duplex ultrasound has recently become the gold standard for the diagnosis of venous disorders, and over the past 5 years, has been utilized during sclerotherapy to guide injections more precisely. OBJECTIVE: To determine greater saphenous vein recanalization and recurrence rates of varicose veins associated with SFI after duplex ultrasound-guided outpatient sclerotherapy

Hemothorax following uncomplicated sclerotherapy for esophageal varices.

Although pleural effusion as a complication of esophageal manipulation is well described in the literature, the fluid is usually nonhemorrhagic. We describe the first patient who had uncomplicated sclerotherapy with ethanolamine for esophageal varices, and on two occasions this patient developed left-sided bloody pleural effusion within 12-72 h after sclerotherapy. The effusion resolved spontaneously within 4 weeks. This case illustrates that hemothorax should be included in the pulmonary complications of sclerotherapy for esophageal varices.

[Pulmonary circulatory disturbance following endoscopic injection sclerotherapy].

We compared arterial blood gas analysis and 99mTc-MAA lung perfusion scintigraphy before and after endoscopic injection sclerotherapy (EIS) in 17 cases of esophageal varices complicating liver cirrhosis. EIS was performed by the intra-variceal injection method with 5% ethanolamine oleate (EO) as the sclerosant agent. In blood gas analysis, the mean values of partial arterial oxygen tension (PaO2) and oxygen saturation (SaO2) were decreased, and the mean value of alveolar-arterial O2 difference (AaDO2) was increased, compared with before EIS while breathing room air. On lung perfusion scintigra

Sclerotherapy for 'scrotal cysts' using tetracycline instillation.

Aspiration and tetracycline (Achromycin) instillation has been used to sclerose 'scrotal cysts'. Thirteen hydroceles and epididymal cysts were treated. Of the cysts treated, four failed to sclerose, and moderate to severe pain occurred in eight patients. In three patients the pain was severe, necessitating admission to the hospital. We would not recommend this treatment either in terms of efficacy or for patient comfort.

Cardiorespiratory effects of endoscopic esophageal variceal sclerotherapy.

Endoscopic variceal sclerotherapy (EVS) is an effective means of controlling variceal hemorrhage, which develops as a consequence of portal hypertension. While esophageal perforation, ulceration, strictures, and mediastinitis are potential complications associated with this procedure, it is not clear whether isolated pleuropulmonary events such as pleuritis, pneumonitis, and adult respiratory distress syndrome are causally related to the EVS. Endoscopy and sedation with the attendant risk of aspiration, particularly in the background of hepatic encephalopathy, may account for some of these eve

[Esophageal function following sclerotherapy of esophageal varices].

Functional disorders of the esophagus were evaluated in 24 patients 4 +/- 2 months after sclerotherapy following the first hemorrhage. The mean volume of polidocanol required per patient was 181 +/- 111 ml. 9 patients complained of dysphagia and/or reflux symptoms; strictures were found in two cases; 4 patients had esophagitis. Manometrically the lower esophageal sphincter was completely insufficient in 14 patients; only 7 patients still had a competent cardia. The first group had received significantly more polidocanol than the latter. Tubular motility was disturbed by nonpropulsive (18%) and

Repeated endoscopic sclerotherapy for active variceal bleeding.

Emergency endoscopic sclerotherapy (EEST) during continued moderate to severe bleeding was carried out using a flexible endoscope and absolute alcohol as a sclerosant in 38 patients with variceal bleeding. Portal hypertension was due to cirrhosis in 27, noncirrhotic portal fibrosis in eight, extrahepatic obstruction in two, and Budd-Chiari Syndrome in one patient. A technically adequate EEST could be carried out in 36 (95%) patients, with successful control of variceal bleeding in 35 (92%). Thirty-one episodes of rebleeding occurred in 12 (31.6%) patients. Twenty-nine (93.5%) of these episodes

Persistent bradyarrhythmia after sclerotherapy for esophageal varices.

Sclerotherapy for esophageal varices has recently gained wide acceptance in major medical centers for the control of bleeding esophageal varices. We have described two elderly patients with preexisting cardiac disease (right bundle branch block) in whom persistent bradyarrhythmia followed sclerotherapy using 5% sodium morrhuate. Both patients required insertion of a permanent pacemaker. This is the first report associating sclerotherapy with significant bradyarrhythmias.

Treatment of esophageal varices: a randomized controlled trial comparing endoscopic sclerotherapy and EUS-guided sclerotherapy of esophageal collateral veins.

BACKGROUND: Endoscopic sclerotherapy (ES) and band ligation are standard treatments for esophageal varices. Unfortunately, recurrence is common and seems to be related to esophageal collateral vessels, easily identified by EUS. Eradication of these vessels might lead to a more durable therapeutic effect. OBJECTIVE: To compare ES with EUS-guided sclerotherapy of collateral vessels (EUS-ES). DESIGN: Randomized controlled trial. SETTING: Endoscopy Unit, Division of Gastroenterology. Universidade Federal de São Paulo, São Paulo, Brazil. PATIENTS AND INTERVENTIONS: Fifty cirrhotic patients with

Who benefits from endoscopic sclerotherapy of bleeding oesophageal varices? Proposal for differential indications. The Copenhagen Esophageal Varices Sclerotherapy Project.

The value of early sclerotherapy for variceal haemorrhage remains unsettled, possibly because the treatment may be beneficial to some patients and harmful to others. On the basis of a randomized clinical trial of sclerotherapy in 187 patients presenting with their first variceal haemorrhage, we examined the relationship between clinical, endoscopic and biochemical characteristics at admission and the treatment effect on mortality. As previously published, sclerotherapy had no overall effect on the very high mortality during the first 6 weeks (47%), but thereafter the mortality and risk of rebl

Foam sclerotherapy compared with liquid sclerotherapy for the treatment of lower extremity varicose veins: A protocol for systematic review and meta analysis.

BACKGROUND: There is a continued discussion on which is the best sclerosant to treat lower extremity varicose veins. Therefore, we did this meta-analysis to determine that foam sclerotherapy versus liquid sclerotherapy, which could perform better in the treatment of lower extremity varicose veins. MATERIALS AND METHODS: We independently searched 5 databases from inception to February 1, 2019, for randomized controlled trials and prospective controlled trials for comparing foam sclerotherapy and liquid sclerotherapy for the treatment of lower extremity varicose veins. A comparative study between cryo-laser cryo-sclerotherapy and sclerotherapy in the treatment of telangiectasia and reticular veins: A randomized controlled trial.

OBJECTIVE: Telangiectasias, characterized by dilated venules, are frequently observed in the lower extremities. Sclerotherapy stands out as the predominant treatment of these vascular lesions. The integration of laser therapy with a mild sclerosing agent, serving as an osmotic sclerosant, presents an enhanced cosmetic treatment approach, aiming to optimize outcomes and minimize potential adverse effects. This study sought to evaluate the feasibility, efficacy, and safety of cryo-laser and cryo-sclerotherapy (CLaCS) and compare it with injection sclerotherapy for the treatment of telangiectasia
