Identification of natural MAO-B inhibitors with enhanced binding and dynamic stability: Implications for Parkinson's disease therapy.

By a comprehensive in silico workflow, this study investigates promising natural inhibitors of monoamine oxidase B (MAO-B) relevant to Parkinson's disease therapy. Selegiline is used as a pharmacophore scaffold to screen a large ZINC15 natural product library by drug-likeness, PAINS, ADMET, filters, followed by pharmacophore mapping, consensus docking, and MD simulations. This technique efficiently leads over 200,000 compounds to four hits (Lig-473, Lig-1781, Lig-2148, and Lig-2698), all of which demonstrated stronger binding affinities than selegiline. Docking studies showed that Lig-473 and

Medical treatment of later-stage motor problems of Parkinson disease.

Parkinson disease progression is associated with the development of levodopa short-duration responses and dyskinesias, as well as gait freezing. Levodopa dose adjustment and adjunctive treatment with dopamine agonists form the major therapeutic strategies. Catechol O-methyltransferase inhibitors are also appropriate considerations, whereas other drugs, including selegiline, amantadine, anticholinergic agents, and propranolol, have a more minor role.

Type-B monoamine oxidase inhibitors in neurological diseases: clinical applications based on preclinical findings.

Type-B monoamine oxidase inhibitors, encompassing selegiline, rasagiline, and safinamide, are available to treat Parkinson's disease. These drugs ameliorate motor symptoms and improve motor fluctuation in the advanced stages of the disease. There is also evidence supporting the benefit of type-B monoamine oxidase inhibitors on non-motor symptoms of Parkinson's disease, such as mood deflection, cognitive impairment, sleep disturbances, and fatigue. Preclinical studies indicate that type-B monoamine oxidase inhibitors hold a strong neuroprotective potential in Parkinson's disease and other neuro

Platelet abnormalities in patients with Parkinson's disease undergoing preoperative evaluation for deep brain stimulation.

Normal hemostatic function is important for reduction of the risk of intracranial hemorrhage during stereotactic neurosurgery including deep brain stimulation (DBS) surgery. This study investigates the hemostatic function in patients with Parkinson's disease (PD) undergoing preoperative evaluation for DBS, with emphasis on the number and function of platelets. In 107 PD patients, only one had abnormal activated partial prothrombin time and normal prothrombin time. Among the other 106 patients, six (5.7%) had only thrombocytopenia, seven (6.6%) only prolonged bleeding time (BT), and 14 (13.2%)

Adolescent nicotine abstinence increases anxiety and depressive-like behaviors, alcohol consumption, oxidative stress and inflammatory response accompanied by attenuated serotonergic/dopaminergic and cholinergic function in rats.

During the last decade adolescent nicotine exposure has become a growing concern worldwide and the significance of this issue has even been further highlighted by recent evidence indicating long-term behavioral complications associated with nicotine abstinence. In addition, nicotine has been suggested to serve as a gate for inclination and susceptibility to other drugs of abuse such as alcohol. The present study was designed to investigate how nicotine abstinence during adolescence may affect the expression of anxiety- and depressive-like signs, as well as the alcohol consumption in adult rats

Ropinirole: a dopamine agonist for the treatment of Parkinson's disease.

The pharmacology, pharmacokinetics, clinical efficacy, adverse effects, dosage and administration, and formulary considerations of ropinirole are reviewed. Ropinirole is a nonergoline dopamine agonist that binds to dopamine D2-receptors; the drug is indicated for use in the symptomatic treatment of early and late Parkinson's disease (PD). Ropinirole is rapidly absorbed after oral administration and undergoes extensive hepatic metabolism to active metabolites. The elimination half-life averages about six hours. Ropinirole has a low potential to interact with other drugs likely to be administere

Niacin improved rigidity and bradykinesia in a Parkinson's disease patient but also caused unacceptable nightmares and skin rash--a case report.

A patient with Parkinson's disease taking levodopa/carbidopa, selegiline, buproprion, aspirin and niacin had decreased rigidity and bradykinesia when his niacin dose was steadily escalated for treatment of high triglycerides, but ultimately the patient could not tolerate niacin because of severe nightmares and skin rash. If further research can reproduce this patient's initial beneficial experience while avoiding the adverse effects, niacin could be a useful adjunctive agent for Parkinson's disease, either population-wide or in a pharmacogenomically defined set of responders.

Zonisamide attenuates MPTP neurotoxicity in marmosets.

MPTP (1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine) induces parkinsonism in humans and animals. The effects of zonisamide on dopamine neurons were studied in MPTP-treated common marmosets (Callithrix jacchus). Groups of animals (n = 3) were treated with MPTP (2.5 mg/kg, every 24 h x 3); MPTP plus zonisamide (40 mg/kg administered 1 h before each MPTP dose); MPTP plus selegiline (a known MAO-B inhibitor) (2 mg/kg administered 1 h before each MPTP dose); and saline controls. An immunohistochemical study of the substantia nigra was performed 14 days after MPTP treatment in each group. MPTP reduce

Monoamine-oxidase Type B Inhibitors and Cognitive Functions in Parkinson's Disease: Beyond the Primary Mechanism of Action.

Symptoms of cognitive impairment are rather common since the early stage of Parkinson's disease (PD); they aggravate with disease progression and may lead to dementia in a significant proportion of cases. Worsening of cognitive symptoms in PD patients depends on the progression of subcortical dopaminergic damage as well as the involvement of other brain neurotransmitter systems in cortical and subcortical regions. Beyond the negative impact on disability and quality of life, the presence and severity of cognitive symptoms may limit adjustments of dopamine replacement therapy along the disease

Orthostatic hypotension: a non-motor complication assessment in 82 patients with idiopathic Parkinson's disease in Phramongkutklao Hospital.

OBJECTIVE: Parkinson's disease (PD) is one of the neurogenic etiologies of orthostatic hypotension, a non-motor symptoms complex, that tends to be under-recognized and under-treated leading to a major cause of disability for PD patients. This complication is associated with one or recurrent falls causing mortality and morbidity. To the authors' knowledge, there is no study about this condition in Thai PD population. The authors therefore aimed to determine the frequency, clinical and risk factors of orthostatic hypotension in PD patients treated in Phramongkutklao Hospital. MATERIAL AND METHO

New considerations in the medical management of early Parkinson's disease: impact of recent clinical trials on treatment strategy.

The best approach to medical management of early Parkinson's disease remains controversial. Recent studies suggest that early use of MAO-B inhibitors may improve long-term outcome. Long-term follow-up to a delayed-start rasagiline study indicated that patients who were treated with rasagiline from the start did better through 5.5-6 years of treatment (with all PD medications) than patients who began rasagiline after a delay of 6 months. In a long-term selegiline study, patients randomized to treatment with selegiline did better over 7 years than patients randomized to treatment with placebo. D

Pharmacological options for the treatment of Tourette's disorder.

Tourette's disorder is a neuropsychiatric disorder characterised clinically by motor and vocal tics, which may be associated to conductual disorders such as obsessive-compulsive disorder (OCD) and attention-deficit hyperactivity disorder (ADHD). Although the neurochemistry of Tourette's disorder is not well known, there are some effective therapies for tics, OCD and ADHD. However, these are not devoid of adverse effects. Tics only require treatment when they interfere with the functioning of the patient. If therapy is needed, monotherapy at the minimal effective dose is desirable, but some pat

NLRP3-GABA signaling pathway contributes to the pathogenesis of impulsive-like behaviors and cognitive deficits in aged mice.

BACKGROUND: Perioperative neurocognitive disorders (PND), such as delirium and cognitive impairment, are commonly encountered complications in aged patients. The inhibitory neurotransmitter γ-aminobutyric acid (GABA) is aberrantly synthesized from reactive astrocytes following inflammatory stimulation and is implicated in the pathophysiology of neurodegenerative diseases. Additionally, the activation of NOD-like receptor protein 3 (NLRP3) inflammasome is involved in PND. Herein, we aimed to investigate whether the NLRP3-GABA signaling pathway contributes to the pathogenesis of aging mice's PND

Newly-diagnosed Parkinson's disease: a therapeutic update.

For patients with newly-diagnosed Parkinson's disease, current research is pointing to new therapeutic approaches. Those that are now available allow the institution of levodopa to be delayed until there is some functional disability requiring treatment. Treating mild symptoms with anticholinergics and other agents may delay the use of levodopa for up to 3 years. And when finally required, levodopa may be used at lower doses when combined with carbidopa and a dopamine agonist. The monoamine oxidase type B inhibitor selegiline is also being used by many physicians to delay the onset of disabili
