Prevention of self-mutilation in patients with Lesch-Nyhan syndrome: review of literature.

There have been no standard methods established to prevent the self-mutilation seen in LNS. Preventive treatment must be developed, therefore, for each patient based on clinical findings. The first method to be utilized should be a nonsurgical splint or mouth guard. If it proves to be unsuccessful then the benefits of surgical procedures should be considered. In the future, the use of medication, such as carbamazepine, may prove to restructure the current approach advocated by the literature today.

Botulinum toxin as a novel treatment for self-mutilation in Lesch-Nyhan syndrome.

Lesch-Nyhan syndrome (LNS) is an X-linked recessive disorder resulting from a deficiency of the metabolic enzyme hypozanthine-guanine phosphoribosyltransferase (HPRT). This syndrome presents with abnormal metabolic and neurological manifestations including hyperuricemia, mental retardation*, spastic cerebral palsy (CP), dystonia, and self-mutilation. The mechanism behind the severe self-mutilating behavior exhibited by patients with LNS is unknown and remains one of the greatest obstacles in providing care to these patients. This report describes a 10-year-old male child with confirmed LNS who

Covert self-mutilation of oral tissues and skin by mechanical and chemical means.

A case is presented in which a patient performed persistent covert acts of self-mutilation through mechanical and chemical means. The lesions first involved the oral mucosa and then the skin and were misinterpreted as erythema multiforme and pyoderma gangrenosum, respectively.

Male genital self-mutilation due to tomophobia - case report and review of literature.

Self-mutilation is defined as a "deliberate destruction or alteration of body tissue without conscious suicidal intent". The prevalence of self-mutilation is about 1%, however the voluntary cutting of the genital organs remains extremely rare with fewer than 100 cases of genital self-mutilation reported in both sexes in the English literature to date. Genital self-mutilation is most often associated with psychiatric disorders, but it has also been reported in nonpsychotic individuals owing to various reasons. Here, we present the case of a 45-year-old man who was found deceased in his home in

[Conservative therapy after self-mutilation of the scrotum resulting in bilateral testicular underperfusion].

We report the case of a patient with massive penile and scrotal swelling due to use of a compression ring with auto-erotic intent and under the influence of addictive substances. Based on the patient history, the mechanical pressure on the scrotum was about 30 hours (h). The Doppler sonogram showed no central perfusion in either testicle. Conservative treatment with antibiotics and analgesics was initiated with a slight increase in inflammatory parameters and local edema and necrosis formation. Follow-up showed regression of swelling, healing of necrosis and improvement of laboratory parameter

Successfully treated bizarre self-mutilation with disembowelment--case report with review of literature.

Self-inflicted abdominal stab wounds are generally uncommon and there is no published report of survivors after extensive self inflicted disembowelment with mutilation. Here we present a case of 28 year old male who was brought to hospital 2 1/2 h after disembowelment through self inflicted abdominal stab injuries. The patient had hypovolemic shock due to bleeding from the mesentery and a 450 cm segment of small bowel which had been pulled out from 2 abdominal stab wounds and slashed multiple times by him. He had alcohol intoxication and hallucinations and did not seem to be in pain or emotion

Nicotine dependence is associated with suicide attempts and self-mutilation among adolescent females.

OBJECTIVE: There are no studies investigating the degree of nicotine dependence (ND) and suicidality among underage adolescents. Our aim was to investigate the relationship between ND and various forms of suicidal behavior among adolescent psychiatric inpatients in Finland. METHODS: Data were collected from 508 patients (age, 12-17 years) admitted to inpatient psychiatric hospitalization between April 2001 and March 2006. The level of ND was assessed by the modified Fagerström Tolerance Questionnaire. Information on adolescents' suicide attempts and self-mutilation as well as psychiatric Diag

Symmetrical "mirror-image" injuries and the "chessboard" pattern: useful markers of self-mutilation.

The evaluation of injuries due to self-mutilation may be complicated by atypical lesions and fabricated histories of the causative events. Four cases are described where one of the most striking findings was the presence of "mirror-image" injuries and/or a "chessboard" pattern of intersecting parallel lines from self-inflicted incised wounds. Case 1: A 32-year-old man was dead in his burnt-out car, with symmetrical, "mirror-image", circular burns from the car cigarette lighter over his forehead, cheeks, ears, upper and lower arms, the dorsal and palmar aspects of the hands, the anterior chest

Oral self-mutilation by a 17-month-old child with Lesch-Nyhan syndrome.

We report a 17-month-old boy who was a known case of Lesch-Nyhan syndrome and presented with self-inflicted oral ulcerations of his lips and cheeks. He had a normal complement of caries-free deciduous teeth. Initially a conservative approach was planned and a bite plate made, but as a result of poor compliance and persistent ulceration and after consultation with his parents it was decided to extract all deciduous teeth.

Male external genitalia self-mutilation in Burkina Faso: Nationwide study of 13 cases.

INTRODUCTION: Genital self-mutilation is an infrequent uro-psychiatric emergency in urological practice. Many authors have emphasized its rarity in the literature. In Burkina Faso, the incidence and prevalence are poorly due to under-notification and the absence of large-scale studies. We proposed this multicenter study to determine the epidemiological, diagnostic, therapeutic and evolutionary aspects of male external genitalia (MEG) self-mutilation in Burkina Faso. MATERIALS AND METHODS: A retrospective multicenter study was conducted, including all patients treated for male external genital

Self-burning versus self-cutting: patterns and implications of self-mutilation; a preliminary study of differences between self-cutting and self-burning in a Japanese juvenile detention center.

The purpose of the present paper was to examine the differences in clinical features between self-cutters and self-burners, to clarify clinical implications of self-mutilating behaviors other than self-cutting. Subjects were 201 delinquent adolescents consecutively entering a Japanese juvenile detention center from February 2003 to March 2003. The subjects were assessed using a self-reporting questionnaire to evaluate self-mutilation, traumatic events, and problematic behaviors. Beck Depression Inventory-2 (BDI-2) and Adolescent Dissociative Experience Scale (A-DES) were also tested. Subjects

A preventive approach to oral self-mutilation in Lesch-Nyhan syndrome: a case report.

Lesch-Nyhan syndrome is a rare X-linked recessive disorder of purine metabolism, caused by complete absence of the enzyme hypoxanthine-guanine phosphoribosyl transferase. Persons affected with this incurable disease are developmentally and physically delayed, and suffer from self-injurious behavior. The most typical feature results in partial or total destruction of perioral tissues. The purpose of this paper was to discuss a case of Lesch-Nyhan syndrome with self-mutilative behavior and lip injuries, including some suggestions of a preventive approach avoiding the extraction of teeth. A soft

Male genital trauma caused by self-mutilation: A first case series report in Indonesia.

INTRODUCTION AND IMPORTANCE: Genital self-mutilation (GSM) is a rare phenomenon with a spectrum ranging from a superficial laceration to complete amputation of the external genitalia. These could result in severe impairment of genitourinary functions. Psychotic disorder adds to the complexity of the patients. Since GSM is underreported, we report the first case series in Indonesia. CASE PRESENTATION: Four included patients comprised different injuries to the external genitalia, from a superficial laceration to penile amputation. Notably, these patients exhibited psychotic traits. Initial mana

Rudy and Borden technique for penile self-mutilation in Klingsor syndrome: a case report.

Penile self-mutilation is a challenging situation that often jeopardizes sexual and voiding functions. Surgical treatment is currently based on penile replantation, nevertheless its requirements of time, conservation, and quality of amputated phallus and microsurgical expertise are not constantly available. Here, we present a case of penile self-inflicted amputation in a 28-years-old patient suffering from a psychotic disorder, who did not preserve the amputated phallus. In the first clinical examination, we have attested a hemorrhagic total penile section, 6cm from the penoscrotal angle. We p

Investigating the co-occurrence of self-mutilation and suicide attempts among opioid-dependent individuals.

The prevalence and risk factors associated with self-mutilation among opioid dependent cases and controls were determined, and the co-occurrence of self-mutilation and attempted suicide was examined. The prevalence of self-mutilation among cases and controls did not differ significantly (25% vs. 23%, respectively), with gender differences identified among cases only. A number of risk factors were found to be associated with self-mutilation, including borderline personality disorder, alcohol dependence, childhood sexual abuse, and multiple suicide attempts. Not only is self-mutilation a clinica

A novel technique for prevention of self-mutilation in three Harris' hawks (Parabuteo unicinctus).

Feather destructive behavior is an ever-increasing problem in captive birds of prey and any associated self-mutilation can be challenging to manage. This clinical report describes a noninvasive, nonpainful, temporary beak modification technique used to prevent self-mutilation in 3 Harris' hawks (Parabuteo unicinctus), when other methods and husbandry modifications had failed. The technique was successful in all 3 birds, and no adverse effects were observed. Further investigations are required to further evaluate if this beak modification technique causes any psychologic or physical stress to t
