Evolving utilization of bariatric surgery since the rise of semaglutide and tirzepatide.

BACKGROUND: Semaglutide and tirzepatide have transformed obesity treatment, but recent changes to bariatric surgery utilization are not well understood. METHODS: Epic's nationwide Cosmos database was queried for patients who underwent primary sleeve gastrectomy or gastric bypass between 2018 and 2025. Patient characteristics including preoperative semaglutide or tirzepatide dispense history were compared using chi-squared and Wilcoxon rank sum tests. Modified Poisson regression was used to identify factors independently associated with pre-surgery GLP-1RA use. Multilevel models were used to e

Euglycemic Ketoacidosis Following the Use of Counterfeit Semaglutide for Weight Loss.

Euglycemic ketoacidosis is a rare but increasingly recognized adverse effect related to the use of glucagon-like peptide-1 (GLP-1) receptor agonists. Reduced oral intake in combination with gastrointestinal side effects can trigger a starvation state and ketone body formation. The growing off-label use of glucagon-like peptide-1 (GLP-1) agents for weight loss, particularly without medical supervision, may further increase the risk of this serious complication. An 18-year-old woman with no medical history presented with persistent nausea, intractable vomiting, and reduced oral intake for three

Kidney and Survival Outcomes with Semaglutide by CKD Severity in the FLOW Trial.

KEY POINTS: Semaglutide reduced kidney outcome risk across broad CKD severity strata (eGFR; albuminuria). Reduced risk for all-cause death also carried through strata of CKD severity. In a population with type 2 diabetes, semaglutide improved kidney and survival outcomes irrespective of CKD severity, including advanced CKD. BACKGROUND: Semaglutide improved kidney and overall survival in participants with type 2 diabetes (T2D) and CKD in the Evaluate Renal Function with Semaglutide Once Weekly (FLOW) trial. The aim of the present analyses was to quantify these benefits across broad strata of C

Skeletal effect of semaglutide and tirzepatide in patients with increased risk of fractures.

CONTEXT: Glucagon-like peptide-1 receptor agonists (GLP1-RA) have potent glucose-lowering and weight loss benefits, but their effects on bone remain unclear. OBJECTIVE: To investigate changes in bone mineral density (BMD) in patients using semaglutide (SEM) and tirzepatide (TIR), a dual agonist of GLP-1/glucose dependent insulinotropic polypeptide. METHODS: Single-center retrospective study. Adult patients using SEM/TIR for ≥6 months with DXA scans before initiation and at least 6 months after were matched by age, sex, BMI, and diabetes mellitus (DM) to nonusers with at least 2 DXA scans ove

Current trends in semaglutide therapy and strategies to improve its bioavailability.

INTRODUCTION: A GLP-1 Receptor Agonist, semaglutide, is given in the management of type 2 diabetes mellitus and obese individuals. However, oral semaglutide exerts very low bioavailability due to multiple gastrointestinal and biopharmaceutical barriers. Delivery of oral semaglutide becomes difficult due to instability in GI fluids, degradation through proteolysis by various enzymes, and mucus diffusion limitation; epithelial permeability restricts the oral absorption of the drug, due to which the oral bioavailability of semaglutide is exceedingly low. This review identifies methods that enhanc

Semaglutide for the Silent Phenotype: A Translational Signal in Obesity-Driven HFpEF.

Obesity-related heart failure with preserved ejection fraction (HFpEF) represents an increasingly prevalent clinical phenotype characterized by exercise intolerance, systemic inflammation, and limited therapeutic options. Semaglutide, a glucagon-like peptide-1 receptor agonist approved for obesity management, has shown favorable effects on symptoms and cardiometabolic markers in patients with HFpEF and obesity, but the overall clinical profile of semaglutide across randomized trials and its relevance within the broader HFpEF therapeutic landscape remain incompletely defined. We conducted a sys

Impact of semaglutide introduction on the use of triptans: an interrupted-time series.

BACKGROUND: Emerging evidence suggests that glucagon-like peptide-1 receptor agonist (GLP-1RA) may be associated with reduced migraine burden. We aimed to evaluate whether semaglutide for weight management initiation is associated with changes in triptan consumption. METHODS: We conducted a nationwide interrupted time series analysis using Danish health registers. All adults initiating semaglutide between December 2022 and December 2024 were included. The first dispensing date served as the index date, establishing a 24-month baseline and a 12-month follow-up period. The primary outcome was m

Effects of semaglutide on vessel morphology: Studies on the chicken chorioallantoic membrane.

Glucagon-Like Peptide-1 (GLP-1) receptor agonists are widely used to manage type 2 diabetes and promote weight loss. Semaglutide (SEM)-a long acting GLP-1-has experienced an extraordinary surge in popularity since its approval in 2017. Between 2021 and 2023, SEM prescription fills in the U.S. climbed to 2.56 million per month. Yet, the uptake of SEM into larger populations has raised safety concerns, with provocative findings now suggesting that SEM could negatively affect ocular and reproductive systems, counter to its beneficial effects on the heart. At least some of these concerns involve S

Finerenone and semaglutide: Role in heart failure with reduced ejection fraction.

Obesity and type 2 diabetes mellitus commonly coexist with heart failure (HF) and may contribute to the pathogenesis of HF with preserved ejection fraction. With progression in management therapies for HF with preserved ejection fraction, the mechanism behind beneficial actions of finerenone and semaglutide remains enigmatic. For decades, the cardiorenal protective effects of aldosterone blockage in patients with chronic kidney disease have been of significant interest. But due to multiple side effects, these trials were likely to stop.

Exocrine Pancreatic Insufficiency During the Use of Semaglutide: A Case Report.

Although acute pancreatitis has been reported as an adverse event in patients treated with glucagon-like peptide-1 (GLP-1) analogues, to date we have not identified a case describing exocrine pancreatic insufficiency related to this drug. Here we present a case of a patient with no history of acute pancreatitis, who was diagnosed with exocrine pancreatic insufficiency during the third year of weekly subcutaneous semaglutide for obesity control. A 68-year-old man with previous unsuccessful therapeutic approaches for weight loss used subcutaneous semaglutide, once a week, for 181 weeks. The phar

Implications of Ozempic and Other Semaglutide Medications for Facial Plastic Surgeons.

Obesity is a growing global health concern, leading to various health issues, including diabetes. Semaglutide-based medications, such as Ozempic, Wegovy, and Rybelsus, have emerged as potential treatments. These medications, belonging to the glucagon-like peptide-1 (GLP-1) receptor agonist class, mimic the action of GLP-1, regulating appetite and promoting weight loss. Clinical trials have shown their effectiveness in reducing body weight and improving metabolic parameters. Ozempic, though Food and Drug Administration-approved for diabetes, is also used off-label for weight loss alone. Rapid w

Potential New Therapeutic Implications of Semaglutide: New Colours of the Rainbow?

Semaglutide is a potent glucagon-like peptide 1 receptor agonist for the management of type 2 diabetes mellitus. In addition to this, it has emerging potential clinical implications. First, there is accumulating preliminary data on its potential role in type 1 diabetes mellitus. In this setting, we need to know which patient subgroups may benefit more. Furthermore, its role in non-alcoholic fatty liver and in non-alcoholic steatohepatitis is emerging. Other potential therapeutic implications of semaglutide include kidney disease, Alzheimer disease and pulmonary diseases. Nonetheless, we still
