Mechanical Axis Method to Determine First Intermetatarsal Angle and Tibial Sesamoid Position.

Utilizing the mechanical axis can decrease load on the joint and be beneficial when analyzing bony deformities and planning surgical correction with osteotomies. The aim of this study was to identify the normal mechanical axes of the first and second metatarsals and use them to obtain the first/second mechanical intermetatarsal angle (mIMA). The mechanical axis of the first metatarsal was used to obtain the mechanical tibial sesamoid position (mTSP), which provides a mechanical relationship with the sesamoid apparatus. The angular difference between the anatomic and mechanical axis lines (anat

Gout Causing Isolated Sesamoid Destruction Mimicking a Neoplastic Process.

The hallux metatarsophalangeal joint (MTPJ) is a common location affected by gouty inflammatory arthritis. Generally, this condition is successfully treated with medication and regular surveillance. Occasionally, gout can lead to advanced arthritic changes and chronic discomfort of the hallux MTPJ, necessitating surgical intervention to help alleviate symptoms. Rarely is the condition isolated to the sesamoid bone, which may lead to a diagnostic dilemma in patients without a history of gout. We report the case of a patient in whom isolated sesamoid changes associated with gout mimicked an aggr

Intraoperative checking of the first ray rotation and sesamoid position through sonographic assistance.

INTRODUCTION: Hallux valgus (HV) deformity affects the orientation of the metatarsophalangeal (MTP) joint in three planes. Displacement in the coronal plane results in axial rotation of the first metatarsal, with progressive subluxation of the first MTP joint. Multiple techniques have been described to correct the malrotation itself. However, none of them have checked intraoperatively the final position of the first metatarsal head and sesamoids previous to the fixation of the Lapidus procedure or first metatarsal bone osteotomies. The aim of this article is to describe a novel technique to ch

Low-Field Magnetic Resonance Imaging Features of Axial Proximal Sesamoid Osteomyelitis and Intersesamoidean Ligament Rupture in a Horse.

A 7-year-old Warmblood mare used for show jumping was evaluated for progressive left hindlimb lameness of 5 weeks' duration, localized clinically to the metatarsophalangeal region. Radiography revealed ill-defined osteolysis of the axial apical regions of both proximal sesamoid bones (PSBs). Due to rapid clinical deterioration, low-field magnetic resonance imaging was performed and demonstrated aggressive axial osteomyelitis of both PSBs, partial rupture of the intersesamoidean ligament and proximal scutum with herniation of synovium of the plantar joint recess, and an occult penetrating tract

Current Trends in Fixation Techniques.

Hallux valgus is an extremely common and often disabling deformity. In addition to valgus deformity of the hallux, varying degrees of varus and supination of the first metatarsal and instability in the metatarsophalangeal and metatarsocuneiform joints are frequently present. Because of the complexity and multiplicity of deformities, surgical techniques and fixation methods continue to be developed to obtain better results. Recent studies have focused on correcting pronation of the first metatarsal as a way of correcting and equalizing the metatarsal sesamoid bones in a more horizontal and stab

Developmental pathogenesis of chromosome disorders: report on two newly recognized signs of Down syndrome.

Testing the hypothesis of amplified developmental instability in autosomal trisomies as proposed by Shapiro we predicted and found an increased frequency of symphalangies in the toes of patients with Down syndrome. In our X-ray study of the feet of 71 adults with trisomy 21 we also detected a greater than normal number of sesamoid bones. A corollary to Shapiro's hypothesis is a dependence on ethnic origin of the frequency of symptoms in Down syndrome. Compared to data from Europe, toe symphalangies are more prevalent in Japan. We predict this anomaly to occur even more often in Japanese patien

Intra-articular ossicle in interphalangeal joint of the great toe and clinical implication.

Failure of closed reduction for an interphalangeal joint dislocation of the great toe resulted from an intra-articular ossicle interposed between the articular cartilages of the phalanges. The knowledge concerning the intra-articular ossicle is unclear. A study was thus carried out on 100 fresh great toes to document the appearance, number, size, and location of the intra-articular ossicle found in the interphalangeal joint of the great toe. Roentgenographic studies of the joint revealed 86% of bony mass representing either the sesamoid bone or the intra-articular ossicle. Anatomical studies r

Establishing a common instantaneous center of rotation for the metatarso-phalangeal and metatarso-sesamoid joints: a theoretical geometric model based on specific morphometrics.

BACKGROUND: Previous research has identified separate sagittal plane instantaneous centers of rotation for the metatarso-phalangeal and metatarso-sesamoid joints, but surprisingly, it does not appear that any have integrated the distinctive morphological characteristics of all three joints and their respective axes into a model that collectively unifies their functional motions. Since all joint motion is defined by its centers of rotation, establishing this in a complicated multi-dimensional structure such as the metatarso-phalangeal-sesamoid joint complex is fundamental to understanding its f

Ultrasound-based prediction model for years to peak height velocity using multiple secondary ossification centers.

PURPOSE: Determining peak height velocity age (PHVA) is crucial for understanding child growth and development and preventing injuries. Previous studies have used anthropometric measurements or X-ray evaluation to predict the timing of PHV, whereas ultrasound provides a radiation-free and portable alternative. This study aimed to predict the years to PHV by assessing multiple secondary ossification centers using ultrasound. METHODS: A total of 12 sites across eight bones were evaluated using ultrasound in 181 children aged 6-12 years between June and December 2019. Height data were tracked fr

Stress fractures of the medial great toe sesamoids in athletes.

The purpose of this study was to determine whether specific symptoms and findings are present in patients with symptomatic stress fractures of the sesamoids of the great toe and, if so, whether partial sesamoidectomy is sufficient for successful treatment. Five consecutive athletes (five females; mean age 16.8 years [range, 13 to 22 years]) with six feet that were treated for symptomatic stress fractures of the sesamoids of the great toe were included in this study. Four athletes (five feet) performed rhythmic sports gymnastics; the fifth athlete was a long jumper. Some swelling to the forefoo

Subchondral bone cysts of the phalanges.

Subchondral phalangeal bone cysts frequently are encountered when reviewing foot x-rays. Radiologically, these lesions appear as a focus of subchondral radiolucency surrounded by a rim of sclerotic bone. Radiologic and histologic examinations demonstrate that these lesions are identical to the subchondral bone cysts associated with osteoarthritis of the hip and the more recently described cysts of the hallucal sesamoids and the head of the first metatarsal.

Computed tomographic anatomy of the equine foot.

This study describes a detailed computed tomographic reference of the normal equine foot. Ten forefeet of five adult cadavers, without evidence of orthopaedic disease, were used. Computed tomography (CT) was performed on all feet. Two-millimetre thick transverse slices were obtained, and sagittal and dorsal planes were reformatted. The CT images were matched with the corresponding anatomic slices. The phalanges and the distal sesamoid bone showed excellent detail. The extensor and flexor tendons (including their attachments) could be clearly evaluated. The collateral (sesamoidean) ligaments co

Asymptomatic foot and ankle structural injuries: a 3D imaging and finite element analysis of elite fencers.

BACKGROUND: Fencing is a highly asymmetrical combat sport, that imposes high mechanical demands over repeated exposures on the musculoskeletal structures, a primary cause of injuries in fencers. However, there are limited epidemiological studies on the structural injuries of the foot and ankle in fencers. This study aimed to investigate foot and ankle structural injuries, and explore how metatarsophalangeal joint structural changes may affect the mechanisms of foot and ankle injuries in asymptomatic fencers. METHODS: 3D images of foot and ankle morphology using computed tomography were obtain

Histomorphometric analysis of the variability of the human skeleton: Forensic implications.

In the last decades, the histomorphometric analysis of bone tissue has been utilized to develop equations for species discrimination of fragmentary bone. Although this technique showed promising results, its main limitation concerns the lack of knowledge on the histomorphometric variability which may exist between different bones of the skeleton. In a previous study, we demonstrated a significant histomorphological variability in different bones of the same individual and even in different sections of the same bone. The present study aimed at investigating the extent of intra-individual variab

The blood supply to the great toe sesamoids.

The purpose of this study was to define the intraosseous and extraosseous blood supply of the hallucal sesamoids by studying a total of 10 fresh-frozen, below-knee specimens with no evidence of vascular disease. Most specimens were injected with high grade India ink, cleared using a standard Spalteholz technique, and processed to delineate the extraosseous and intraosseous blood supply to include soft tissue dissection and coronal sectioning. Two additional specimens were injected with blue Mercox acrylic solution to further define the extraosseous vasculature. The major extraosseous blood sup

Bone scanning of the foot for unexplained pain.

This paper reports the results of bone scans on 78 painful feet. Scanning helped in the diagnosis of persistent foot pain following injury and it enabled stress fractures, fractures of the sesamoids and subtalar arthritis to be diagnosed earlier. It reliably excluded bone infection and was useful as a screening test when radiographs were normal.
