Impact of Physiotherapy on Shoulder Kinematics in Swimmers with Swimmer's Shoulder Pain.

Swimmer's shoulder is a common condition among elite swimmers, often leading to pain and reduced performance. Fatigue can exacerbate this condition by affecting shoulder strength, proprioception, and range of motion, potentially increasing the risk of overuse injuries. This preliminary study aimed to evaluate the impact of physiotherapy treatment and the effects of fatigue on shoulder kinematics using inertial and magnetic measurement units (IMUs). Five male swimmers (aged 21-27) with at least 3 years of training and suffering from swimmer's shoulder pain participated in the study. The protoco

Primary synovial osteochondromatosis of the shoulder: a rare cause of shoulder pain.

Synovial osteochondromatosis is a benign neoplastic condition which manifests as multiple cartilaginous or osteocartilaginous nodules within a joint, bursa and tendon sheath. It typically results in pain and stiffness of the affected joint, and is thought to arise as a result of synovial membrane proliferation and metaplasia. 1 We report the diagnosis and management of a 36-year-old man who presented with a long history of shoulder pain, which had been managed conservatively prior to referral to orthopaedics. Following investigation and subsequent diagnosis, he underwent successful surgical tr

Shoulder giant schwannoma - a diagnosis to be considered in painless shoulder masses.

Tumours formed from peripheral nerve sheaths are rare in the shoulder. We present the case of a 80-year-old female referred to an orthopaedic surgery clinic because of a large painless mass of the right shoulder, with one year of evolution. There was no history of trauma, involvement of any other joint or systemic complaints. Given the clinical and imagiological suspicion of a giant synovial cyst, the patient underwent complete surgical resection of the mass, which took place without any complications or functional impairment. Histological diagnosis - schwannoma - was unexpected given the lesi

The effectiveness of shoulder stretching and joint mobilizations on posterior shoulder tightness.

CLINICAL SCENARIO: It has been suggested that posterior shoulder tightness is a common contributor to shoulder impingement in overhead-throwing athletes. The incidence of shoulder pain in the general population has been reported to be as high as 27%, and as many as 74% of the patients who were seen for shoulder issues had signs of impingement. Particularly regarding physically active adults, shoulder impingement is frequent among overhead-throwing athletes and may lead to lost participation in sport, as well as other injuries including labral pathologies. Therefore, finding an effective mechan

Shoulder arthroscopy: its role in evaluating shoulder disorders in the athlete.

We retrospectively reviewed 100 initial shoulder arthroscopies from a general sports medicine orthopaedic practice to determine if arthroscopy is helpful diagnostically and to see if specific lesions correlate well with specific diagnoses. Seventy-two percent of the patients in the review had glenoid labral tears. Ninety-two percent of patients who had a diagnosis of either recurrent anterior dislocation or recurrent anterior subluxation demonstrated a tear of the anterior inferior margin of the glenoid labrum. Sixty-eight percent of patients with a diagnosis of impingement demonstrated tears

Does Medicaid payer status affect patient's shoulder outcomes after shoulder arthroplasty?

BACKGROUND: Low socioeconomic status and Medicaid insurance as a primary payer have been shown to influence resource utilization and risk-adjusted outcomes for total joint arthroplasty. The purpose of this study was to assess the effects of Medicaid primary payer status on outcomes following shoulder arthroplasty (SA). MATERIALS AND METHODS: A retrospective review of shoulder surgery patients was undertaken to identify a matched cohort of 51 patients who underwent SA and were stratified based on insurance type into two cohorts: 28 Medicaid (M) patients and 23 non-Medicaid (NM) patients. [Chronic Locked Posterior Shoulder Dislocation Treated by Anatomical Total Shoulder Arthroplasty].

The aim of this study was to present the results of surgical treatment in two male patients with chronic locked posterior dislocation of the shoulder, who underwent total shoulder arthroplasty due to destruction of more than 45% of the humeral head articular surface and glenoid impairment. The deltopectoral approach was used in both cases. Total shoulder arthroplasty (Epoca-Synthes) was carried out in the standard manner. Case one: A 29-year-old, extremely obese patient suffered injury to his shoulder during an epileptic seizure. At 11 months after injury, surgery was performed using an inters

Bilateral Shoulder Dislocation Presenting as a Unilateral Shoulder Dislocation: Case Report.

Shoulder dislocations are a common condition seen by orthopedic surgeons and emergency medicine physicians. Shoulder dislocations typically present unilaterally and are anterior in nature. Bilateral simultaneous posterior shoulder dislocations are typically associated with seizure or electrocution and are more common than bilateral simultaneous anterior dislocations, which occur because of trauma or fall. In a traumatic setting, a shoulder dislocation may be accompanied by a distracting injury, and a thorough examination must be conducted to avoid overlooking concomitant diagnosis. The authors

Detailed shoulder MRI findings in manual wheelchair users with shoulder pain.

Shoulder pain and pathology are common in manual wheelchair (MWC) users with paraplegia, and the biomechanical mechanism of injury is largely unknown. Establishing patterns of MRI characteristics in MWC users would help advance understanding of the mechanical etiology of rotator cuff disease, thus improving the logic for prescribed interventions. The purpose of this study was to report detailed shoulder MRI findings in a sample of 10 MWC users with anterolateral shoulder pain. The imaging assessments were performed using our standardized MRI Assessment of the Shoulder (MAS) guide. The tendon m

Moment arms of the shoulder musculature after reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty is known to increase the moment arm of the middle subregion of the deltoid during shoulder abduction; however, at present, comprehensive data regarding the shoulder muscle moment arm through the full range of abduction and flexion are not available. The purpose of this study was twofold: (1) to measure the instantaneous moment arms of thirteen subregions of major muscles spanning the glenohumeral joint during abduction and flexion of the shoulder after reverse total shoulder arthroplasty and (2) to compare these data with the muscle moment arms p

[Shoulder arthroplasty following rotator cuff tear: acquired arthropathy of the shoulder].

The treatment of cuff tear arthropathy (CTA) of the shoulder is based on the patient's subjective complaints, functional deficits, and the treatment goals that result from both. Only in the second line is the radiologic morphology of joint destruction important for the surgeon's decision. Based on these aspects it is possible to discriminate three types of CTA that obviously have similar functional deficits and radiological characteristics. This symptom-based classification can offer decision guidelines for the choice of an appropriate joint replacement. For the most frequent, stable, and osteo

Swimmer arm-to-shoulder test for early differentiation between shoulder and cervical spine pathology in patients with shoulder pain.

BACKGROUND: Several tests have been suggested for screening and diagnosis of cervical spine and shoulder girdle conditions underlying shoulder pain with variable degrees of clinical accuracy. The present study aimed to test the reliability, clinical benefit and screening value of the Swimmer Arm-to-Shoulder (SAS) test; a new clinical test developed to differentiate shoulder impingement from cervical radiculopathy in patients with shoulder pain of ≤ 12 weeks. METHODS: The study included 718 patients aged 40-65 years, with unilateral and localized shoulder girdle pain lasting for ≤ 12 weeks. Di

Which shoulder functions correlate with patient satisfaction after primary shoulder arthroplasty?

BACKGROUND: A principal goal of shoulder arthroplasty is to achieve patient satisfaction. While patient self-assessment of shoulder function is commonly used to assess operative success, it is not known how improvements in shoulder function correlate with patient satisfaction after primary anatomic total shoulder arthroplasty (aTSA), reverse total shoulder arthroplasty (rTSA), and ream-and-run (RnR) arthroplasty. The objectives of this study were (1) to determine the preoperative to postoperative improvement in the individual functions assessed by the Simple Shoulder Test (SST) for patients ha

Arthroscopic shoulder suspensioplasty in painful hemiplegic shoulder subluxation-a case series.

BACKGROUND: Inferior glenohumeral subluxation (GHS) can cause disabling pain in hemiplegics. Conservative treatments have not been proven to be effective or maintained over time. A few studies have shown the benefits of surgical treatment. The objective of our study was to evaluate the medium-term clinical and radiological results of arthroscopic glenohumeral suspensioplasty surgery by biceps tenodesis in the setting of painful GHS in hemiplegics. METHODS: We conducted a retrospective study of patients who underwent arthroscopic glenohumeral suspensioplasty. The assessment, at a minimum of 1

Atypical Shoulder Instability Patterns in Adolescents Following Traumatic Anterior Shoulder Dislocation.

BACKGROUND: Traumatic anterior shoulder instability is common in the adolescent athlete, and when it is untreated, the recurrence rate is high. Atypical lesions-such as anterior glenoid periosteal sleeve, humeral glenohumeral ligament, and insertional tendon avulsions-may occur within this population, and accurate diagnosis and appropriate lesion management are key to treatment success. PURPOSE: To evaluate the age, skeletal immaturity, bone loss, and uncommon soft tissue lesions as correlates of posttraumatic anterior shoulder instability lesion patterns in an adolescent population. Pain After Anatomic Total Shoulder Arthroplasty Versus Reverse Total Shoulder Arthroplasty.

As anatomic total shoulder arthroplasty (aTSA) and reverse total shoulder arthroplasty (rTSA) become increasingly common, differences between the 2 procedures are of importance. The purpose of this study was to determine differences in acute pain levels between patients undergoing aTSA and rTSA. Sixty patients undergoing aTSA or rTSA were assessed for participation. The primary outcome was postoperative daily visual analog scale scores, which were initially collected for 4 days postoperatively. Secondary outcomes included opioid consumption, length of stay, and complications. Comparisons betwe

Shoulder Reduction Bench Project: improving care for patients with shoulder dislocations.

This paper presents the background, methodology and results of a quality improvement project undertaken at a district general hospital. The project was launched in response to the concerning results from audit data which showed significant delays in the treatment of patients with shoulder dislocations and a high percentage of patients receiving procedural sedation. Using 'Plan-Do-Study-Act' cycles involving training sessions, written protocols and an online video, we were able to train a large cohort of nurse practitioners in the use of the Shoulder Reduction Bench. This is a relatively novel,
