Seronegative Immune-Mediated Necrotizing Myopathy: A Case Report.

Idiopathic inflammatory myopathies (IIMs) are a group of chronic autoimmune disorders characterized by proximal skeletal muscle weakness. One subtype of the IIMs is immune-mediated necrotizing myopathy (IMNM). IMNM can be further classified according to its autoantibody presence, including anti-3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMGCR), anti-signal recognition particle (SRP), and seronegative.  Here, we describe the case of a 61-year-old Caucasian female with a prior history of distant lung cancer and current statin use presenting with a subacute onset of bilateral proximal lower

Protein translocation into proteoliposomes reconstituted from purified components of the endoplasmic reticulum membrane.

We have reproduced the process of protein transport across and of protein integration into the mammalian endoplasmic reticulum membrane by the use of proteoliposomes reconstituted from pure phospholipids and purified membrane proteins. The transport of some proteins requires only two membrane protein complexes: the signal recognition particle receptor, needed for targeting of a nascent chain to the membrane, and a novel complex, the Sec61p complex, that consists of Sec61p and two smaller polypeptides. The translocation of other proteins also needs the presence of the translocating chain-associ

Anti-SRP-positive necrotising myopathy concurrent with breast malignancy.

Anti-signal recognition particle (anti-SRP)-positive necrotising myopathy causes severe progressive proximal weakness with a propensity to involve pharyngeal, laryngeal and respiratory muscles. It is one of the aggressive inflammatory myopathies. First-line treatment is with high-dose steroids followed by other immunosuppressants, but this conventional therapy is often ineffective. Second-line treatment involves use of either rituximab or intravenous immunonoglobulin (IVIG). Anti-SRP-positive necrotising myopathy is frequently treated as refractory myositis due to its poor responsiveness to st

[Myositis-specific autoantibodies].

BACKGROUND: Myositis-specific antibodies (MSA) are autoantibodies that are almost exclusively detected in idiopathic inflammatory myopathies (IIM). This article provides an overview of these autoantibodies and how they can be used clinically to identify subgroups of IIM. MATERIAL AND METHODS: The article is based on a non-systematic literature review and our own experience. RESULTS: MSA can be detected in up to 50 % of patients with IIM. Patients with anti-synthetase antibodies have a constellation of clinical findings termed "the anti-synthetase syndrome", in which interstitial lung disease

[Autoantibodies specifically detected in patients with polymyositis/dermatomyositis].

Polymyositis/Dermatomyositis (PM/DM) is a chronic inflammatory disorder that culminates in injury to the skin and muscle and, sometimes, is accompanied by interstitial lung disease (ILD). A number of autoantibodies are associated with myositis, including those specific for aminoacyl-tRNA synthetase (anti-ARS), signal recognition particle (anti-SRP), and Mi-2. These autoantibodies have proven to be useful in the diagnosis and classification of the diseases and are predictive of prognosis. It has been known that certain patients may have typical DM skin manifestations without clinical evidence o

STING Activation in Dendritic Cells Enhances Antigen Presentation by Live Tumor Cells.

The stimulator of interferon genes (STING) pathway is a key regulator of antitumor immunity. Activation of STING by the natural agonist 2'3'-cyclic GMP-AMP (cGAMP) induces type I interferon and promotes CD8+ T cell responses against tumors. However, how type I interferon modulates tumor cell recognition by T cells remains unclear. Here, using a co-culture model and virus-like particles encapsulating cGAMP (cGAMP-VLP), we show that STING activation in dendritic cells enhances CD8+ T cell recognition of live tumor cells through type I interferon signaling. This effect is independent of enhanced

The spatially resolved transcriptome signatures of glomeruli in chronic kidney disease.

Here, we used digital spatial profiling (DSP) to describe the glomerular transcriptomic signatures that may characterize the complex molecular mechanisms underlying progressive kidney disease in Alport syndrome, focal segmental glomerulosclerosis, and membranous nephropathy. Our results revealed significant transcriptional heterogeneity among diseased glomeruli, and this analysis showed that histologically similar glomeruli manifested different transcriptional profiles. Using glomerular pathology scores to establish an axis of progression, we identified molecular pathways with progressively de

Novel antibody associations in immune-mediated necrotising myopathy without inflammation.

INTRODUCTION: The patient presenting with proximal muscle weakness, elevated serum creatinine kinase and myopathic electromyography and biopsy findings has a wide differential diagnosis that includes toxic, autoimmune, paraneoplastic and congenital myopathies. Autoimmune myopathies are important to identify because they may respond to immunosuppressive therapies. METHODS: We describe two cases of immune-mediated necrotizing myopathy each associated with a novel antibody. RESULTS: Case 1 describes a progressive myopathy in a statin user. Identification, molecular characterization and expression analysis of a mucosal C-type lectin in the eastern oyster, Crassostrea virginica.

Lectins are well known to actively participate in the defense functions of vertebrates and invertebrates where they play an important role in the recognition of foreign particles. They have also been reported to be involved in other processes requiring carbohydrate-lectin interactions such as symbiosis or fertilization. In this study, we report a novel putative C-type lectin (CvML) from the eastern oyster Crassostrea virginica and we investigated its involvement in oyster physiology. The cDNA of this lectin is 610 bp long encoding for a 161-residue protein. CvML presents a signal peptide and a

The C-Type Lectin Mincle Serves as a Pattern Recognition Receptor for the Lipid Envelope of Human Cytomegalovirus.

Enveloped viruses possess a membrane composed of a complex mixture of host-derived lipids, glycolipids, and viral glycoproteins, which play a critical role in host-virus interactions. Viral glycoproteins can be detected by a range of immune lectins, thereby modulating and sometimes subverting the host immune responses to promote immune evasion. Whether viral lipids can directly interact with pattern recognition receptors to modulate antiviral immune responses remains unclear. In this study, we report recognition of the human cytomegalovirus (HCMV) viral envelope by the C-type lectin Mincle. Un

Clinicopathological features of anti-HMGCR and anti-SRP myopathies that do not satisfy the EULAR/ACR criteria of inflammatory myopathies.

OBJECTIVES: The clinicopathological features of immune-mediated necrotizing myopathy (IMNM) sometimes mimic muscular dystrophy, complicating accurate diagnosis. The European League Against Rheumatism (EULAR)/American College of Rheumatology (ACR) classification criteria of idiopathic inflammatory myopathies (IIMs) are superior in terms of sensitivity and specificity; however, the sensitivity is reported to be relatively low in IMNM. We examined the clinicopathological characteristics and the prognoses of anti-3-hydroxy-3-methylglutaryl-CoA reductase (HMGCR) antibody-positive and anti-signal re

Muscle strength recovery in patients with idiopathic inflammatory myopathy with different myositis-specific autoantibodies.

This study aims to assess muscle strength recovery in patients with idiopathic inflammatory myopathy (IIM) with three myositis-specific autoantibodies (MSAs). Forty-eight IIM patients (19 with anti-TIF1-γ Ab, 21 with anti-ARS Ab, and 8 with anti-SRP Ab) were included. Physical exercise began one week after starting medication. Muscle strength was measured using a hand held dynamometer. CK levels, muscle strength recovery, manual muscle test 8 (MMT8), and the Barthel index (BI) scores were evaluated before and after treatment among patients with anti-TIF1-γ, anti-ARS or anti-SRP Abs. CK levels

ER translocation intermediates are adjacent to a nonglycosylated 34-kD integral membrane protein.

We have used the homobifunctional cross-linking reagent disuccinimidyl suberate (DSS) to identify proteins that are adjacent to nascent polypeptides undergoing translocations across mammalian rough ER. Translocation intermediates were assembled by supplementing cell free translations of truncated mRNAs with the signal recognition particle (SRP) and microsomal membrane vesicles. Two prominent cross-linked products of 45 and 64 kD were detected. The 64-kD product was obtained when the cell free translation contained SRP, while formation of the 45-kD product required both SRP and translocation co

Technological advancements and challenges in the analysis of fluoroquinolone antibiotic environmental residues.

Fluoroquinolone antibiotics (FQs) are widely used broad-spectrum antimicrobials, and their incomplete metabolism and continuous discharge have led to their frequent detection in surface water, groundwater, wastewater, and aquaculture systems. Although generally present at ng/L-μg/L levels, environmental FQ residues may affect microbial communities, aquatic organisms, and the spread of antimicrobial resistance. Accurate determination of FQs in environmental waters remains challenging because dissolved organic matter, humic substances, metal ions, proteins, surfactants, and suspended particles c

Diagnostic modelling and therapeutic monitoring of immune-mediated necrotizing myopathy: role of electrical myotonia.

Delayed diagnosis of immune-mediated necrotizing myopathy leads to increased morbidity. Patients with the chronic course without 3-hydroxy-3-methylglutaryl-coenzyme-A reductase-IgG or signal recognition particle-IgG are often challenging to diagnose. Immunotherapy response can also be difficult to assess. We created a statistical model to assist immune-mediated necrotizing myopathy diagnosis. Electrical myotonia versus fibrillations were reviewed as biomarkers for immunotherapy treatment response. Identified were 119 immune-mediated necrotizing myopathy cases and 938 other myopathy patients. I

SRP54-related congenital neutropenia: a multidisciplinary effort.

A previously healthy Caucasian man in his 30s presented with a 9-week history of productive cough, fever and dyspnoea, initially treated as pneumonia with transient improvement. Examination revealed coarse right-sided crepitations and finger clubbing. Laboratory testing demonstrated persistent severe neutropenia and elevated inflammatory markers, while imaging showed persistent right middle lobe consolidation. Further history revealed recurrent respiratory infections, diarrhoea and poor dentition since childhood, with medical records showing possible neutrophil migration defect. The patient re

Human genetic defects in SRP19 and SRPRA cause severe congenital neutropenia with distinctive proteome changes.

The mechanisms of coordinated changes in proteome composition and their relevance for the differentiation of neutrophil granulocytes are not well studied. Here, we discover 2 novel human genetic defects in signal recognition particle receptor alpha (SRPRA) and SRP19, constituents of the mammalian cotranslational targeting machinery, and characterize their roles in neutrophil granulocyte differentiation. We systematically study the proteome of neutrophil granulocytes from patients with variants in the SRP genes, HAX1, and ELANE, and identify global as well as specific proteome aberrations. Usin
