Assessing the severity and improving the understanding of sleep-related breathing disorders in heart failure patients.

In this manuscript we present an overview of novel signal processing techniques developed by our group to reduce scoring time in the assessment of the severity of sleep-related breathing disorders in heart failure patients and to detect sleep/wake fluctuations during periodic breathing. Besides describing these methods, we present the results of validation experiments. Our work shows that novel signal processing techniques can reduce costs and resources needed to screen the patients and can provide relevant information for better understanding the role of wake/sleep transitions in the developm

Orexins and orexin receptors: from molecules to integrative physiology.

Recent studies have implicated the orexin system as a critical regulator of sleep/wake states, feeding behavior, and reward processes. Orexin deficiency results in narcolepsy-cataplexy in humans, dogs, and rodents, suggesting that the orexin system is particularly important for maintenance of wakefulness. Orexin agonists and antagonists are thought to be promising avenues toward the treatment of sleep disorders, eating disorders, and drug addiction. In this chapter, we discuss the current understanding of the physiological roles of orexins in regulation of arousal, sleep/wake states, energy ho

Melatonin and the circadian system: Keys for health with a focus on sleep.

Melatonin (MLT), secreted during the night by the pineal gland, is an efferent hormonal signal of the master circadian clock located in the suprachiasmatic nucleus (SCN). Consequently, it is a reliable phase marker of the SCN clock. If one defines as "chronobiotic," a drug able to influence the phase and/or the period of the circadian clock, MLT is a very potent one. The most convincing data obtained so far come from studies on totally blind individuals. Exogenous MLT administered daily entrains the sleep-wake cycle of these individuals to a 24-h cycle. MLT, however, is not essential to sleep.

Sleep-wake abnormalities in narcolepsy.

To evaluate the degree to which sleep (REM vs. NREM) intrudes into wake and wake intrudes into sleep in narcolepsy, 103 patients with narcolepsy were compared to 105 patients with other diagnoses of disorders of excessive sleep (DOES). Narcoleptic patients had more frequent REM onsets on the multiple sleep latency test (MSLT) and nocturnal polysomnograms. But the MSLT latencies to REM versus NREM in narcoleptic patients did not differ. Nocturnal measures of REM pressure, percentage of REM, and REM latency excluding the REM onsets, did not differ among patient groups. With respect to the intrus

Risk of sleep problems in a clinical sample of children who stutter.

PURPOSE: Previous studies have shown increased prevalence of sleep problems among people who stutter. However, there is a lack of knowledge about what these sleep problems may specifically be. METHOD: Fifty children who stutter (CWS) from 6;0 to 12;9 years of age and 50 age- and gender-matched controls participated in this study. Parents did not report coexisting conditions, excepting stuttering and/or sleep problems. Sleep problems were investigated using a standardized questionnaire answered by parents. The questionnaire shows cut-off scores to identify the risk of sleep problems as a whole

Intracranial electrical stimulation of corticolimbic sites modulates arousal in humans.

BACKGROUND: Humans routinely shift their sleepiness and wakefulness levels in response to emotional factors. The diversity of emotional factors that modulates sleep-wake levels suggests that the ascending arousal network may be intimately linked with networks that mediate mood. Indeed, while animal studies have identified select limbic structures that play a role in sleep-wake regulation, the breadth of corticolimbic structures that directly modulates arousal in humans remains unknown. OBJECTIVE: We investigated whether select regional activation of the corticolimbic network through direct el

Therapeutic potential of targeting the orexin (hypocretin) system in sleep disorders.

Sleep is emerging as a key health topic in modern society. The prevalence of and focus on sleep-wake disorders is increasing, which has led to a large interest from the pharmaceutical industry to develop new treatments targeting sleep biology (that is, drugs that have the potential to mimic normal sleep-wake physiology). The orexin (also known as hypocretin) system is one of the key targets and regulators of sleep and wakefulness. In this Review, we will discuss the mechanisms by which loss of orexin signalling is involved in narcolepsy type 1 and whether loss and/or disruption of orexin signa

Melatonin Ameliorates Abnormal Sleep-Wake Behavior via Facilitating Lipid Metabolism in a Zebrafish Model of Parkinson's Disease.

Sleep-wake disorder is one of the most common nonmotor symptoms of Parkinson's disease (PD). Melatonin has the potential to improve sleep-wake disorder, but its mechanism of action is still unclear. Our data showed that melatonin only improved the motor and sleep-wake behavior of a zebrafish PD model when melatonin receptor 1 was present. Thus, we explored the underlying mechanisms by applying a rotenone model. After the PD zebrafish model was induced by 10 nmol/L rotenone, the motor and sleep-wake behavior were assessed. In situ hybridization and real-time quantitative PCR were used to detect

[Sleep disorders and psychiatry. Keys for trans-nosological symptomatology, genetic epidemiology and dynamic pathogenesis].

Sleep studies in psychiatry collect various disorders in the category of "dysomnia", and sleep EEG studies today can be routinely performed using appropriate autonomous devices. Three major needs emerge from a broad approach of the topic: a transnosological view on symptomatology--dysomnia gets its meaning from the context, a genetic epidemiology--dysomnia appears as a risk factor for psychiatric disorders and a marker of depressive vulnerability, and as a dynamic pathogeny. A wide consensus arises in the field of polysomnographic studies claiming a clear distinction between two opposite sleep

Abnormally abrupt transitions from sleep-to-wake in Huntington's disease sheep (Ovis aries) are revealed by automated analysis of sleep/wake transition dynamics.

Sleep disturbance is a common and disruptive symptom of neurodegenerative diseases such as Alzheimer's and Huntington's disease (HD). In HD patients, sleep fragmentation appears at an early stage of disease, although features of the earliest sleep abnormalities in presymptomatic HD are not fully established. Here we used novel automated analysis of quantitative electroencephalography to study transitions between wake and non-rapid eye movement sleep in a sheep model of presymptomatic HD. We found that while the number of transitions between sleep and wake were similar in normal and HD sheep, t

[Definitions and clinical classifications of sleep disorders].

Sleep disorders are defined as disorders which symptoms or pathophysiology are related with sleep regardless of comorbid physical and/or mental disorders. Sleep disorders are classified into 6 major categories: sleep related breathing disorders which exhibit abnormal breathing during sleep, sleep related movement disorders which show involuntary movements and/or abnormal sensations during sleep and/or nighttime, hypersomnia of central origin not due to other sleep disorders, circadian rhythm sleep disorders due to desynchronization between sleep-wake pattern and required social schedule, paras

Genetic approaches for target identification in sleep/wake systems.

Attempts to therapeutically induce sufficient amounts of sleep have occurred since the advent of the medical arts, but the health implications of failing to maintain a correct balance of sleep/wake has only recently become widely appreciated both in the medical arena and among the general public. Inappropriate amounts of sleep have been linked to cognitive impairment, immune suppression and metabolic changes, as well as to the sequelae of reduced vigilance and increased likelihood of involvement in automobile accidents or other causes of fatalities. Currently approved treatments for insomnia a

Melatonin for primary insomnia?

Melatonin, a hormone produced by the pineal gland, has a key role in regulating circadian rhythms, most importantly, the sleep-wake cycle. Melatonin's action has led to its being tried as a treatment for a wide range of sleep disorders, such as jet lag, primary insomnia, sleep-wake cycle disruption and sleep problems in children with neuro-developmental disorders. Until recently, it had not been licensed in the UK for any indication. Prolonged-release melatonin (Circadin - Lundbeck) has now been licensed as a treatment for primary insomnia. Here we consider whether this product has a place in

Sleep and Rhythmic Profile After Pineal Gland Removal in Humans.

Melatonin, a hormone produced by the pineal gland, is classically described as a central circadian modulator. However, the impact of its absence on circadian rhythmicity in humans remains poorly understood. Pinealectomised patients, in whom melatonin secretion is chronically suppressed, represent a valuable clinical model to investigate the physiological role of this hormone in sleep and temporal organisation. This study evaluated sleep quality and sleep disorders, as well as circadian rhythmicity, in 17 individuals who underwent surgical or radiotherapeutic treatment for pineal tumours and ex

Dopamine transporter function fluctuates across sleep/wake state: potential impact for addiction.

The dopamine transporter (DAT) has been implicated in a variety of arousal-related processes including the regulation of motor activity, learning, motivated behavior, psychostimulant abuse, and, more recently, sleep/wake state. We previously demonstrated that DAT uptake regulates fluctuations in extracellular dopamine (DA) in the striatum across the light/dark cycle with DA levels at their highest during the dark phase and lowest during the light phase. Despite this evidence, whether fluctuations in DA uptake across the light/dark cycle are associated with changes in sleep/wake has not been te

Sleep and sleep disorders.

Sleep is a complex neurological state, with its primary function of providing rest and restoring the body's energy levels. The importance of sleep could be seen from the fact that people spend about one-third of their lifespan in sleep. Normal human sleep is divided into non-rapid eye movement (NREM) and rapid eye movement (REM) sleep, and the alteration between NREM and REM occurs about 4-5 times during a night of normal sleep. Human NREM sleep could be classified into four stages, namely, stage I, II, III and IV, representing successively deeper stages of sleep. Sleep is an active rhythmic n
