Treatment of primary snoring using radiofrequency-assisted uvulopalatoplasty.

Laser-assisted uvulopalatoplasty (LAUP) reduces the need for general anesthesia, bleeding and the operation time compared with uvulopalatopharyngoplasty (UPPP), but the postoperative pain is more severe due to the thermal damage to tissues, and the procedure often causes serious scar contracture of the soft palate, with foreign body sensation also occurring occasionally. Radiofrequency-assisted uvulopalatoplasty (RAUP) uses the same surgical method as LAUP, but employs RF instead of laser. RF surgery induces temperatures of 70-85 degrees C, and causes less damage to the surrounding tissues. Th

Can singing exercises reduce snoring? A pilot study.

BACKGROUND: Snoring is not merely a common nuisance but has been identified as a risk factor for poor health. Last resort treatments for palate-based snoring are surgical - reducing the amount of soft palate and/or stiffening it by causing scarring. They carry a burden of risk and expense and have a high recurrence rate. OBJECTIVE: This pilot study was a first step in determining whether singing exercises could be used as a non-invasive treatment to increase muscle tone in the tissues of the throat and thereby reduce snoring. METHOD: The duration of snoring of 20 chronic snorers was recorded

[Surgical treatment of severe snoring. Long-term effects].

AIM: The long-term results of uvulopalapharyngoplasty (UPPP) reported by patients who had been operated for severe snoring but without clinically significant obstructive sleep apnoea were explored. METHODS: Median three years (one to eight years) after the operation follow-up results were obtained from 80 of 85 patients (94%), using a questionnaire. RESULTS: In nine patients (11%) the operation had been without any effect. Eleven patients (14%) indicated that they were completely cured of snoring. In sixty patients (75%) snoring was reduced. In about half of these the initial effect had been

Palatal snoring identified by acoustic crest factor analysis.

The differentiation of palatal from non-palatal snoring is very important for ENT surgeons trying to determine whether palatal surgery would be curative. At present sleep nasendoscopy is the accepted method. Palatal vibration produces marked modulation of sound loudness at low frequency (below approximately 100 Hz). We calculate a crest factor for the sound waveform (ratio of peak to root mean square (rms) value in any given epoch), as a measure of the degree of modulation. Free-field snore sounds were recorded from 11 supine adult patients under intravenous sedation (midazolam), using a digit

Snoring and hypertension: a 10 year follow-up.

In many cross-sectional studies an association has been found between snoring and hypertension. However, differing results have been obtained when confounding factors have been taken into account. To establish whether snoring is a risk factor for developing hypertension, a population-based, prospective survey was performed. In 1984 and 1994, 2,668 males, aged 30-69 yrs at baseline, answered questionnaires concerning sleep disturbances and somatic disease. Of the habitual snorers in 1984, 12.5% reported that they had developed hypertension during the period, compared with 7.4% of the remaining

Pulmonary artery pressure in sleep apnoea and snoring.

There is a renewed interest in pulmonary hypertension (PH) complicating obstructive sleep apnoea (OSA). The prevalence of PH in populations of patients with less severe OSA was documented to be around 10%. The most recent data from both catheterization and echocardiographic studies indicate that as many as 40% of patients with OSA have PH. It has been shown that non-obese patients with normal respiratory function tests can develop pulmonary hypertension. One of the other possible mechanisms involved may be the presence of heightened pulmonary artery pressure response to hypoxia. There are now

Snoring (I). Daytime sleepiness in regular heavy snorers.

Fifteen men, mean (means) age 44 years, were investigated. Their means body mass index was 21.9 kg/m2, and all of them had a respiratory disturbance index below 5 and had good nocturnal oxygen saturation. The subjects were monitored several nights both with and without the following devices: a tight-fitting facial mask, a pneumotachometer, and an esophageal balloon. They were also monitored with and without nasal continuous positive airway pressure. The Multiple Sleep Latency Test was administered after three of the experimental nights (after the baseline nights and after the second nasal CPAP

An absorbable thread suture technique to treat snoring.

We investigated a novel, minimally invasive surgery that uses an absorbable suture technique to treat snoring and mild obstructive sleep apnea. This simple procedure was developed to increase the stiffness of the soft palate and to promote uvula elevation with sutures. Thirty-five snorer and mild obstructive sleep apnea syndrome patients were included in this study. The palate was sutured with the newly developed technique. The results of our surgery were evaluated using polysomnography (PSG), the Epworth sleepiness scale (ESS), and a visual analogue scale (VAS) before surgery and 90 days afte

Managing snoring and obstructive sleep apnoea in childhood.

Snoring assessment and its differentiation from obstructive sleep apnoea are difficult based upon a parent history and physical examination of the size of the tonsils. Not only is the presence of obstructive sleep apnoea important to diagnose, but confirming its severity is the key determinant in prioritising treatment in a resource-limited setting. This review provides current knowledge on the utility of common diagnostic tests, results of treatment options available and implications of treatment and unrecognised or untreated obstructive sleep apnoea.

Obstructive sleep apnoea and snoring - is examination necessary?

This article outlines two cases of snoring and obstructive sleep apnoea (OSA) secondary to parapharyngeal space tumours. Both patients were referred to a specialist sleep clinic where oropharyngeal masses were seen and biopsied. Both underwent surgery and this was curative of both their snoring and their OSA. Parapharyngeal space tumours are an extremely rare cause of OSA and snoring. However, all patients with OSA and snoring should have a full head and neck examination before referral; in rare cases this could enable early detection of a parapharyngeal space tumour.

Snoring surgery with palatal implants after failed uvulopalatopharyngoplasty.

The aim of this study was to assess the efficacy and morbidity in the use of the palatal implant technique (PITs) in patients with failed uvulopalatopharyngoplasty (UPPP) after a 6-month follow-up period. This was a prospective, nonrandomized study. Sixteen patients who underwent UPPP by the same Institution with Fujita's technique with initial success but developed posterior residual apnea or snoring who fulfilled the inclusion criteria were enrolled. Responses of all patients and their partners were assessed using the visual analogue scale (VAS) and the Epworth sleepiness scale (ESS). Object

Uvular edema secondary to snoring under deep sedation.

A 57-year-old male with a documented history of obstructive sleep apnea with loud snoring received deep intravenous sedation with midazolam, fentanyl, ketamine, and propofol infusion and a left interscalene brachial plexus nerve block for a left biceps tendon repair. Loud snoring during the case was noted. On the second postoperative day, he was observed to have significant uvular edema. After due consideration of the various elements in the differential diagnosis, it was concluded that negative pressure trauma from deep snoring during the sedation was the most likely etiology.

Snoring and excessive daytime sleepiness in Parkinson's disease.

Recent recognition of daytime sleepiness in Parkinson's disease (PD) has prompted a search for its causes. Sleepy patients may be more susceptible to sleep attacks after the use of dopamine agonists and the recognition of sleep disturbances in PD may influence important therapeutic decisions. To identify clinical factors influencing excessive daytime sleepiness (EDS) and sleep complaints in PD, we studied 86 consecutive patients with clinical diagnosis of PD using a sleep questionnaire, the Epworth Sleepiness Scale, the Unified Parkinson's Disease Rating Scale and the Montgomery and Asberg Dep

Swallowing dysfunction related to snoring: a videoradiographic study.

Biopsy studies of the soft palatal and oropharyngeal tissues in habitual snorers and patients suffering from obstructive sleep apnoea have shown signs of neurogenic lesions. These lesions might affect the pharyngeal swallowing function, which is dependent on adequate sensitivity. The objective of the present study was to test the hypothesis that snoring is associated with aberrant pharyngeal swallowing function. Forty-one consecutive patients without dysphagia, seeking medical attention because of heavy snoring and various degrees of daytime sleepiness, were prospectively selected. Fifteen non
