Homelessness, mental illness and the law.

One major difficulty that faces both researchers and practitioners who work with the homeless is the lack of definitions and frameworks that assist better understanding of such people's problems and society's response to them. This article reviews the legislation dealing with homeless people's social, civil and legal problems. We tried to give this legislation's historic and social background, including a section on psychiatric disorders in this group because of their relevance to current debate on homeless offenders. We then reviewed the legislation relevant to homeless people in general and

Validity of psychotraumatic reactions.

To determine whether posttraumatic reactions occurred in injured individuals not confronted with litigation or compensation (as reckoned in the U.S.), a parallel study was undertaken in a social-democratic country (Austria) between ski- and work-injured groups. Although each group had a significantly different way of regarding the injury, there appeared to be a comparable incidence of posttraumatic psychiatric reaction in each. The absence of compensation (in contrast to disability maintenance) did not inhibit the appearance of the posttraumatic reaction. A discussion of the effects of compens

Anxiety symptoms and disorders among adults living with HIV and AIDS: A critical review and integrative synthesis of the empirical literature.

There are over 35 million people worldwide infected with the Human Immunodeficiency Virus (HIV) and its progression to Acquired Immunodeficiency Syndrome (AIDS; WHO, 2014). With the advent of combined antiretroviral therapy (i.e., cART) in 1996, persons living with HIV/AIDS (PLWHA) now have much longer life expectancies. However, living with HIV remains challenging, as it is associated with a number of significant and recurrent (chronic) stressors including physical pain, side effects of cART, social stigma, and discrimination, among other social stressors. Presumably, as a result of these typ

Alcohol and anxiety: subtle and obvious attributes of abuse in adults with social anxiety disorder and panic disorder.

Previous research has found a relation between social anxiety disorder and alcoholism but recent work found no differences in drinking levels among socially anxious individuals, dysthymics, and normal controls. Using a more sophisticated measure of substance abuse may further explicate the relation between social anxiety and drinking. We examined aspects of substance abuse in treatment-seeking individuals with social anxiety disorder or panic disorder (psychiatric control group) as well as nondisordered individuals (normal control group). We used the Substance Abuse Subtle Screening Inventory-

Impairment and distress associated with relationship discord in a national sample of married or cohabiting adults.

The clinical importance of relationship discord was examined through evaluating the association between relationship discord and multiple measures of impairment and psychological distress in a population-based sample of married and cohabiting adults (N=2,677). In comparison to people that were not in discordant relationships, individuals in discordant relationships reported greater social role impairment with relatives and friends and greater work role impairment. They also reported higher levels of general distress and poorer perceived health and were more likely to report suicide ideation. W

[Expectations and service performance of social psychiatry services in free Saxony. An analysis of viewpoints of psychiatric practice and clinical specialists and coworkers of social psychiatric services].

After the "Polyclinic system" that had predominated in the GDR had been dismantled, a far-reaching restructuring of the complementary psychiatric care sector became necessary. In the State of Saxony, comprehensive establishment of a homogeneous home-visit outpatient service is the first building block in an interlinked system of regionalized community psychiatry. Based on about 20 years of experience in the Federal States of the FRG, this function is fulfilled by social psychiatry services (SPS). The present study investigates the expectations of free-practising psychiatrists (n = 165), doctor

The impact of social phobia on quality of life.

Although social phobia is a prevalent mental disorder in the general population, it has only recently received clinical and public attention and very few and rather general data are currently available on the general and disease-specific impairments, disabilities, handicaps and economic burdens associated with social phobia. This report summarizes findings from a case-control study in which quality of life and other indices of impairment in 65 subjects with social phobia but without significant comorbidity of other psychiatric disorders (pure social phobia) were compared to those in a matched

Prevalence of Social Media Addiction and Associations With Usage Patterns, Burnout, and Health Conditions Among Medical Trainees in China: Cross-Sectional Study.

BACKGROUND: Medical residency is a demanding training stage characterized by high levels of stress and burnout. As digital natives, current medical trainees (ie, residents) are frequent users of social media; however, little is known about how their personal (nonprofessional) use relates to burnout and social media addiction (SMA). OBJECTIVE: This study aims to characterize the prevalence of SMA among Chinese medical trainees and explore its complex relationships with social media use patterns, occupational burnout, and related risk and protective factors. METHODS: A nationwide cross-section

Trends in referrals to psychosocial services in a pediatric emergency department before and after the onset of the COVID-19 pandemic.

The COVID-19 pandemic upended healthcare in the United States, profoundly impacting the mental and physical health of children and adolescents. This study examined how referrals to psychosocial services in a freestanding children's hospital emergency department changed during the initial COVID-19 state of emergency period in Massachusetts when compared to a previous period of similar length. While the total number of emergency department encounters decreased during the COVID-19 period, there were significant increases in rates of referrals for social work support across general, psychiatric, a

Quality of Life and Functioning Impairments Across Psychiatric Disorders in Adults Presenting for Outpatient Psychiatric Evaluation and Treatment.

OBJECTIVE: Psychiatric disorders are associated with impairments in quality of life (QOL) and functioning. What remained to be investigated was the comparison of these constructs across psychiatric disorders in treatment-seeking adults. It was hypothesized that mood disorder patients would endorse worse QOL and functioning at entry into psychiatric outpatient treatment compared to patients with other disorders, and that regardless of diagnosis, severe impairments in QOL and functioning would be endorsed by the majority of the sample (>70%). METHODS: Data were collected for 2114 adults. Diagno

Neuropsychiatric disorders in Cushing's syndrome.

Endogenous Cushing's syndrome (CS), a rare endocrine disorder characterized by cortisol hypersecretion, is associated with psychiatric and neurocognitive disorders. Major depression, mania, anxiety, and neurocognitive impairment are the most important clinical abnormalities. Moreover, patients most often complain of impairment in quality of life, interference with family life, social, and work performance. Surprisingly, after hypercortisolism resolution, despite the improvement of the overall prevalence of psychiatric and neurocognitive disorders, the brain volume loss at least partially persi

Work and social adjustment in patients with anorexia nervosa.

OBJECTIVE AND METHODS: The Work and Social Adjustment Scale (WSAS) assesses patients' perceptions of impairment in everyday functioning and has been reported as a simple and reliable self-report measure in different psychiatric disorders. This study compared WSAS data from an anorexia nervosa (AN) patient group with that from healthy controls (HCs) and published data from other patient groups. A total of 160 female participants (AN, 77; HC, 83) completed the WSAS as well as measures of eating disorder symptom severity and brief assessments of anxiety and depression. RESULTS: Work and Social A

[Factitious diseases in oncology].

Factitious diseases and pathomimias and particularly Munchausen's syndrome, due to their rarity, are poorly diagnosed by medical teams working in oncology. Consequences can be serious and result in unadapted surgery or non justified implementation of chemotherapy and radiotherapy regimens. These patients simulate diseases in order to attract medical attention. They might become belligerent and are likely to promptly discharge themselves from hospital if they do not get the desired attention or are unmasked. With two following case reports and literature review, we would like to alert clinician

Transforming the legacies of childhood trauma in couple and family therapy.

A multi-theoretical couple/family therapy clinical social work practice model synthesizes various social, family, trauma, and psychodynamic theories to inform a biopsychosocial assessment that guides clinical interventions. The client population involves adult partners who have negotiated the impact of childhood trauma, i.e., physical, sexual, and emotional abuses, including culturally sanctioned trauma. Couples may also be dealing with the aftermath of acute trauma related to interpersonal violence, political conflict, and/or the dislocations related to refugee or new immigrant status. Clinic

Work-Anxiety Rather Than Cognitive Performance Contributes to Workability Decisions in Patients With Mental Disorders.

BACKGROUND: Mental workability decisions must be based on information on person's symptom load, cognitive performance, and workplace conditions. This investigation explores in which way these factors contribute to workability in persons with common mental disorders (CMDs). METHODS: One thousand five hundred seventy CMD-patients underwent multimodal medical diagnostic. Participants filled in questionnaires on work-phobic-anxiety and general mental symptom load. They were also investigated concerning their cognitive performance. Sick leave duration, workplace problems, and subjective workabilit
