All About That Base: Dietary Plasticity, Basicranial Flexion and Sphenoid Form.

OBJECTIVES: The mammalian sphenoid is a morphologically and embryologically complex bone bridging the cranial vault and face. Sphenoid phenotypic diversity has been attributed to the coaction of natural selection and developmental variation in adjacent cranial organs and bones. These associations underscore enduring interest in the evolutionary, ontogenetic, and clinical importance of the sphenoid. We address key gaps regarding several structural and adaptive factors that differentially characterize postnatal ontogeny when brain growth is negligible and has a correspondingly diminished impact.

Sphenovomerine suture: a useful landmark for locating sphenoid sinus ostium.

AIM: The aim of this study was to determine whether the sphenovomerine suture (SVS) can be used as a landmark to localize the sphenoidal sinus ostium. METHODS: Endoscopic imaging was done on 152 skulls to identify ostium of the sphenoidal sinus, the SVS, and the articulation of sphenoidal process of palatine bone between the body of the sphenoid and the sphenopalatine foramen. The variables were as follows: (1) the distance between the medial border of the ostium and SVS (DSO-SVS); (2) the angle between them (ASO-SVS); (3) the distance between the inferior border of the ostium and the horizon

[Technical difficulties in surgical treatment of Cushing disease in children and adolescents].

Cushing disease in children and adolescents is connected with many technical difficulties in operations for removal of pituitary adenoma from the approach through the sphenoid bone which is connected with anatomical conditions specific for that age group. The greatest difficulty is related to incomplete development of the sphenoidal sinus, which is sometimes spongy or presellar. Besides that, the size of the sphenoidal sinus and sella is small. The surgical methods are analysed which make possible overcoming of these difficulties.

[Amaurosis secondary to frontal traumatism].

CASE REPORT: We present the case of a patient diagnosed with amaurosis of the right eye secondary to a right frontal contusion. The energy of the impact was projected from the orbital ceiling to the minor wing of the sphenoid bone. This bone was fractured, thus reducing the optic canal diameter and damaging the optic nerve. DISCUSSION: In our case, we describe a mixed mechanism of injury, that is to say, a frontal contusion indirectly transmitted to the optic canal and a direct lesion of the optic nerve secondary to the movement of the minor wing of the sphenoid bone into the optic canal.

Surgical treatment of transsphenoidal encephaloceles: transpalatal versus combined transpalatal and transcranial approach.

OBJECT: Transsphenoidal encephalocele (TSEC) is a rare developmental anomaly with herniation of neural elements including pituitary gland or optic apparatus, through a defect of sphenoid bone. To prevent obstructive dyspnea, endocrine dysfunction, or visual disturbance, surgical correction is usually performed. However, the optimal operative approach has not been determined yet. The authors present their surgical experiences with transpalatal, transcranial, or combined approach. METHODS: The authors retrospectively analyzed surgical results in patients with TSEC. The charts were reviewed for

The anatomical relations of the sphenoid sinus and their implications in sphenoid endoscopic surgery.

Alongside the past development of endoscopic sinus surgery, knowledge about the anatomy of the sinuses has become crucial for surgeons. The sphenoid sinus is one of the most variable of all sinuses. Its relations to vital vascular and nervous elements make its approach a challenge for endoscopic surgeons. These relations include the internal carotid artery, the optic nerve, the pituitary gland, the Vidian nerve, and other vascular and nervous elements depending on the size of the sinus. This paper will make a review of these anatomical relations and their importance in endoscopic sphenoid surg

Patterns of pneumatization and septation of the sphenoidal sinus.

The sphenoidal bones of 70 adults were removed at post-mortem and were examined for the degree of pneumatization, number of intersphenoidal septa and the relationship of the septa to the lowest portion of the sellar floor. In 85.7%, pneumatization was of the sellar type, in 11.4% of the pre-sellar type and in 2.8% of the conchal type, as defined here. A single septum was present in 61%, two septa in 14%, more than two septa in 12.8% and no septum was present in 11.4%. The insertion of the septum represented the lowest point in the sellar floor in 50% of the bones. In these the septum was locat

Paradoxical evolution of spheno-orbital meningioma after cessation of progestin treatment.

BACKGROUND: Meningioma is the most frequent primary benign intracranial tumor, with a higher incidence in women. Treatment with progesterone acetates, including cyproterone, nomegestrol, chlormadinone, promegestone, medrogestone, and medroxyprogesterone acetate, has been identified as a risk factor of meningioma, particularly in the anterior and middle cranial fossae. Discontinuation of these treatments often leads to volume stabilization or regression of the tumor. CASE DESCRIPTION: A 42-year-old woman undergoing treatment with nomegestrol acetate (NA) presented with headaches and visual los

Sphenoid sinus mucocele: case report of an appearance on a panoramic radiograph.

Mucoceles of the sphenoid bone are rarely seen, although many short reports have appeared in the medical literature since they were first described by Berg in 1889. The sphenoid sinus is located deep in the skull close to sensitive structures such as the optic chiasm and the upper 6 cranial nerves. Most sphenoid lesions are found through a variety of signs and symptoms, the appearance of which depends on the structure involved. A case of an incidental finding of a sphenoid sinus mucocele on a dental panoramic radiograph is described in a totally symptom-free 22-year-old woman. The screening of

Brown tumor of the sphenoid sinus in a patient with secondary hyperparathyroidism: CT and MR imaging findings.

We present a case of brown tumor of the sphenoid sinus in a patient with secondary hyperparathyroidism. CT showed an expansile soft-tissue attenuation mass centered in the sphenoid sinus. CT at bone window setting demonstrated expansile, lytic change and remodeling of the surrounding bone. On MR imaging, the lesion showed iso-intensity to gray matter on T1-weighted images and heterogeneous hyperintensity on T2-weighted images, and showed intense enhancement. The extent of the lesion and its relationship to the surrounding structures were best evaluated by CT and MR imaging.

Giant cell reparative granuloma of the sphenoid: Case report and review of the literature.

BACKGROUND: Giant cell reparative granulomas (GCRGs) are rare lesions in the cranial bones. We present a case of this rare lesion emanating from the clivus and replacing the sphenoid sinus, a highly unusual location for this entity. CASE DESCRIPTION: The case and clinical course of a 29-year-old female who presented with a large sphenoid mass are described here. The patient presented with symptoms of severe headache and diplopia; imaging demonstrated a large sphenoid mass which was completely resected via an endoscopic endonasal approach. It was based on the clivus and was shown to be a GCRG.

Aneurysmal bone cyst at the base of the skull.

Aneurysmal bone cysts have been described as pseudocysts in view of their lack of an epithelial lining. These cysts are uncommon, but when they do occur they typically involve the long bones of the extremities, the membranous bones of the thorax and pelvis, and the vertebrae. Skull involvement is uncommon. We present the case of a 14-year-old girl who presented with nasal obstruction and a swelling of the right cheek. Contrast-enhanced computed tomography detected a heterogeneous cystic mass involving the sphenoid and ethmoid bones. The mass was excised via a lateral rhinotomy approach, and it
