Management of Thoracolumbar Spinal Fracture Caused by an Aircraft Accident.

INTRODUCTION: Aircraft crashes are infrequent but often associated with high mortality, morbidity, and only a limited number of survivors. These events are classified as high-energy traumas as aircraft typically maintain significant vertical and horizontal speeds at impact. The forces generated can produce a wide range of injuries, from soft tissue damage to fractures, with particular concern for neurological injury. Due to the low number of survivors, few studies in the literature specifically address spinal injury patterns. METHODS: We retrospectively evaluated thoracolumbar spinal injuries

Spinal anesthesia in the percutaneous fixation of fragility fractures of the pelvis.

INTRODUCTION: The number of geriatric patients with a pelvic ring fracture is rising and minimal invasive fixation techniques are increasingly popular. The patient characteristics of these fragile patients are similar to those of patients with a proximal femur fracture. In the field of proximal femur fracture surgery spinal anesthesia is a very commonly used anesthetic technique in this more fragile patient population. METHODS: All patients were treated between January 2022 and May 2022 in the Amsterdam UMC location AMC in The Netherlands. The operations were performed by a surgeon who specia

Thoracolumbar spinal fracture in children: Conservative or surgical treatment?

PURPOSE: Spinal thoracolumbar fractures represent 10-14% of pediatric fractures. Most children concerned by such fractures are above 10 years of age. No guidelines presently exist. Analysis of spine pathophysiology and of the various common therapeutic attitudes led us to conduct a review of the different therapeutic approaches in pediatric thoracolumbar fracture. METHODS: A review of the literature was carried out using the Medline and Embase databases with the search-term "pediatric thoracic lumbar spine fractures". RESULTS: The systematic review identified 44 studies, 24 of which were sel

Traumatic spinal fracture treated by vertebroplasty: a case report.

UnlabelledIntroductionSurgical treatment for lumbar burst fractures is complex and typically involves either a retroperitoneal corpectomy and/or a posterior pedicle screw fixation. We describe the case of a patient with a lumbar burst fracture who was cured via a less invasive approach that has not been previously reported as standalone treatment.Case presentationThis 25-year-old Caucasian man presented with excruciating axial low back pain exacerbated by any attempt to elevate the head of the bed after a motor vehicle accident. Computed tomography demonstrated a burst L4 fracture without spin

Spinal intraosseous arteriovenous fistula in the fractured vertebral body.

A 68-year-old woman presenting with progressive paraparesis was diagnosed with an AVF involving a previously fractured L1 vertebral body on which feeders from multiple segmental arteries converged. The most distinctive feature in our patient was that the fistula was located in the vertebral body. Transarterial embolization of the segmental arteries with coils and glue resulted in total obliteration of the fistula, which in turn resulted in symptom improvement.

Skeletal fracture demographics in spinal cord-injured patients.

INTRODUCTION: The demographics of skeletal fractures found in patients presenting with a spinal cord injury to a modern level-one trauma center have not been reported. MATERIALS AND METHODS: A retrospective review was performed of 1290 patients presenting between January 1997 and December 1999 with an acute vertebral fracture and spinal cord or cauda equina injury to determine the incidence, fracture type, and mechanism of all fractures of the extremities and pelvis. RESULTS: Overall, 128 (10% of 1290) of these patients sustained 203 associated skeletal fractures including 16 open fractures

Analysis of a retrieved Isola spinal system fractured in service.

A 1/4-in.diameter two-column Isola spinal system implanted in a 13-year-old girl suffering from cerebral palsy and a severe neuromuscular scoliosis of 120 degrees, was replaced 22 months after primary implantation. The system was removed due to fracture of the lower left (concave) bar, between its two cross connectors (i.e., the distal bypass connector and a transverse cross link), as a result of a postoperative infection and a subsequently developed pseudarthrosis. The retrieved implants were analyzed with the use of a multitechnique characterization procedure involving macroscopic and micros

Femoral fracture and iatrogenic hyperthyroidism in spinal cord injury.

Analysis of the clinical case of a male aged 30 years, presenting with T6 complete paraplegia in 1991, demonstrated the existence of aggravating factors for disuse osteoporosis of spinal cord injury, possibly leading to bone density values below the fracture threshold with the risk of spontaneous fractures. This patient was admitted to hospital for multiple pressure sores requiring prolonged local dressings before and after plastic surgery performed in July 1995. In January 1996, following exercise, he developed a fracture of the femoral diaphysis with a cystic demineralized appearance of the

[Social and work rehabilitation of patients with spinal fractures and their sequelae].

In order to study possible social-working rehabilitation of people with fractures of the backbone and their sequellae complex, mainly dynamic, clinico-laboratory investigations were carried out in 224 people with non-complicated fractures of the thoracic and lumbar sections of the vertebral column within 1--15 and more years after trauma. More than 30% of the patients were considered to be stable invalids at the primary examination. The social-working rehabilitation of people with sequellae of the backbone fractures is based on the state of compensation of the disturbed functions of the verteb

The progression of the vertebral body bruise associated with a spinal fracture.

BACKGROUND: Advances in magnetic resonance imaging (MRI) have made it possible to find the vertebral body bruise (VBB), which was not found in computed tomography (CT) after trauma. There has been only one study with adult patients about whether traumatic VBB will cause a collapse of the vertebral body or not. The purpose is to elucidate the progression of VBB in non-osteoporotic adult patients and to identify the possible factors influencing the progression. METHOD: The VBB was defined on MRI as band-like or diffuse zones of high signal intensity on T2-weighted sequences without fracture of

[The coexistence of spinal canal stenosis in fragility fractures of the spine].

BACKGROUND: In trauma care of fragility fractures of the spine, degenerative stenosis is often seen as an accompanying pathology. This may lead to a possible compression of neurogenic structures. The stenosis of the spinal canal can manifest itself with clinically significant complaints before the fracture occurs. This coexistence may have an impact on the injury itself or may provoke a complicated treatment of the fracture. AIM: The aim of this work is to differentiate these pathophysiologies and their merger in terms of clinical diagnostics and treatment options. DIFFERENTIAL DIAGNOSIS: Th

Spinal cord injury associated with thoracic osteoporotic fracture.

This report details a case of sudden neurologic deficit attributable to acute thoracic fractures associated with senile osteoporosis. A 73-yr-old female patient with a history of occasional back pain during the past 4 mos had sudden thoracic vertebral fracture with spinal cord injury. The patient, who had a benign past medical history, had not been evaluated for osteoporosis. Thoracic spine radiographs showed a compression fracture at T8. Thoracic magnetic resonance imaging exposed a compression fracture at T7-T8. She was treated operatively. She was found to have spinal cord injury with Ameri

Flattening of sagittal spinal curvature as a predictor of vertebral fracture.

UNLABELLED: In a 12-year prospective study of 100 adult volunteers, incident vertebral fractures were analyzed for potential predictors. Decreased sagittal spinal curvature and pre-existing fractures were identified as independent predictors. The risk of vertebral fracture increased to 8.6-fold with a 1 SD decrease in both thoracic and lumbar curves. INTRODUCTION: Physiological spinal curvature acts as a shock absorber: however, it has not been demonstrated whether reduced spinal curvature is a risk of vertebral fracture. The aim of this prospective study is to assess the relationship between

Spinal cord injury without osseous spine fracture.

Sixteen patients with spinal cord injury without osseous spine fracture and 55 patients with spinal cord injury with osseous spine fracture aged from birth through 18 years were studied. Those without osseous fracture were younger (mean age 6 years) than were those with osseous fracture (mean age 16 years). Extravasation of myelographic dye from the spinal canal was a poor prognostic sign. All three in the group with this finding without osseous fracture had complete spinal cord lesions. Those without osseous fracture should be followed for the development of late spinal deformity that may req

Spinal ankylosis and sex-specific predisposing factors in type II odontoid fractures: a comparison with sub-axial fractures.

STUDY DESIGN: Retrospective cohort study. PURPOSE: We assessed the predisposition to type II odontoid fractures (T2OFs) in elderly people by comparing patients who had T2OFs with those who had sub-axial fractures. OVERVIEW OF LITERATURE: T2OFs are common among elderly people; osteoporosis and degenerative changes in the upper cervical spine are major risk factors. However, the role of spinal ankylosis remains unclear. METHODS: We analyzed data from 45 patients with T2OFs and 79 with sub-axial fractures, all aged ≥75 years. Hounsfield unit (HU) values at C2-C4, the prevalence of ankylosis, a
