Direct current electrical bone growth stimulation for spinal fusion.

An electronic bone growth stimulator (EBGS) had been devised to facilitate lumbosacral fusion in the early 1970s, using a totally implanted device which delivers a steady 5 microA at each of four cathodes. The use of the device is reserved for patients in whom painful and abnormal motion is demonstrated to be the major cause of low-back pain. An initial series (I-A) of 82 patients demonstrated a successful fusion rate of 91.5% compared with a 80.5% fusion rate in 159 patients treated without the EBGS. A second prospective randomized controlled trial series (II) was begun using only "difficult

[Results of posterolateral spinal fusion on spondylolisthesis (author's transl)].

44 patients with spondylolisthesis (type I Meyerding) which had had posterolateral fusion could be followed up for an average of five years. The operation had been performed for low back pain with or without sciatica when conservative treatment was not successful. In 42 patients (95%) fusion was solid and in 35 patients (80%) the symptomes could be distinctly deminished. Posterolateral fusion is a successful treatment with few complications in spondylolisthesis which does not respond to conservative measures.

Spinal fusion limits trunk movement, but improves gait in patients with adult spinal deformity.

BACKGROUND: Adult spinal deformity (ASD) alters posture and gait, leading to greater functional impairment and lower quality of life than other chronic conditions [1, 2]. Coronal deformities (scoliosis) and sagittal imbalance impact gait differently [3]. Spinal fusion is associated with improvements in pain and physical function, yet little is known about its effects on gait [4-6]. RESEARCH QUESTION: How do spatiotemporal (SPT) parameters and joint kinematics differ in ASD patients with either coronal or sagittal imbalance compared to controls, and does spinal fusion improve SPT and joint kin

Application of"Spinal cord fusion" in spinal cord injury repair and its neurological mechanism.

Spinal cord injury (SCI) is a severely disabling and catastrophic condition that poses significant global clinical challenges. The difficulty of SCI repair results from the distinctive pathophysiological mechanisms, which are characterised by limited regenerative capacity and inadequate neuroplasticity of the spinal cord. Additionally, the formation of cystic cavities and astrocytic scars after SCI further obstructs both the ascending and descending neural conduction pathways. Consequently, the urgent challenge in post-SCI recovery lies in repairing the damaged spinal cord to reconstruct a fun

Patient-Specific Lattice Cage Design for Cervical Spinal Fusion.

AIM: To propose a patient-specific interbody cage with graded stiffness distributions analogous to the Young's modulus of the cervical spinal bone interface in order to improve mechanical compatibility, promote physiological load sharing, and enhance osseointegration. MATERIAL AND METHODS: A synthetic database of spinal bone Young modulus values was used, incorporating anatomical regions (cervical, thoracic, lumbar) and patient-specific factors (age, bone density, health status). A parametric generative design approach allowed dynamic modification of lattice unit cell geometry to achieve targ

Surgery for spinal deformity with osteoporosis: Achieving successful fusion.

The objectives of fusion surgery for spinal deformities include decompressing neural elements and achieving balanced spinal alignment. Particularly, in cases where spinal deformities coexist with osteoporosis, successful surgery requires careful consideration due to the susceptibility to fixation failure and non-union. Various efforts are being made to restore spinal alignment through surgery in osteoporotic patients. The administration of osteoporosis medications before and after surgery is effective for bony union. Additionally, appropriate selection of fusion range, rigid internal fixation,

Immune response to foreign materials in spinal fusion surgery.

Spinal fusion surgery is a common procedure used to stabilize the spine and treat back pain. The procedure involves the use of foreign materials such as screws, rods, or cages, which can trigger a foreign body reaction, an immune response that involves the activation of immune cells such as macrophages and lymphocytes. The foreign body reaction can impact the success of spinal fusion, as it can interfere with bone growth and fusion. This review article provides an overview of the cellular and molecular events in the foreign body reaction, the impact of the immune response on spinal fusion, and

Biomechanical Analysis of Multilevel Posterior Cervical Spinal Fusion Constructs.

STUDY DESIGN: Controlled Laboratory Study. OBJECTIVE: To compare multilevel posterior cervical fusion (PCF) constructs stopping at C7, T1, and T2 under cyclic load to determine the range of motion (ROM) between the lowest instrumented level and lowest instrumented-adjacent level (LIV-1). SUMMARY OF BACKGROUND DATA: PCF is a mainstay of treatment for various cervical spine conditions. The transition between the flexible cervical spine and rigid thoracic spine can lead to construct failure at the cervicothoracic junction. There is little evidence to determine the most appropriate level at whic

Failure in Cervical Spinal Fusion and Current Management Modalities.

Failed fusion in the cervical spine is a multifactorial problem stemming from a combination of patient and surgical factors. Patient-related risk factors such as steroid use, poor bone quality, and smoking can be optimized preoperatively. Age, prior radiation, prior surgery, and underlying genetics are nonmodifiable patient-centered risk factors. Surgical risks for failed fusion include the number of segments fused, anterior versus posterior approach for fusion, the type of bone graft, and the instrumentation utilized. Many symptomatic cases of failed fusion (pseudarthrosis) result in pain, ne

Prevalence of Occult Infections in Posterior Instrumented Spinal Fusion.

STUDY DESIGN: This is a prospective observational study. OBJECTIVE: The aim of this study is to determine the rate of occult infection after instrumented spine surgery in presumed aseptic patients. SUMMARY OF BACKGROUND DATA: The reported incidence rate of delayed/occult infection determined by positive culture swabs after instrumented spine surgery in prospective studies is 0.2%-6.9%. However, this rate may be higher as delayed infections are challenging to diagnose. Fever can be absent and inflammatory markers are often normal. If indolent organisms exist in low concentrations surrounding

Preoperative Hyponatremia and Perioperative Complications in Cervical Spinal Fusion.

OBJECTIVE: Preoperative patient optimization is increasingly recognized as key to good surgical outcomes. Preoperative hyponatremia is a modifiable risk factor linked to poorer postoperative outcomes in other surgical fields. We provide the first investigation of the association of preoperative hyponatremia with morbidity and mortality in patients undergoing cervical spine surgery. METHODS: We queried the National Surgical Quality Improvement Program registry for patients who underwent cervical spine fusion. Preoperative serum sodium levels were classified as normal (135-145 mEq/L) or hyponat

Composite piezoelectric spinal fusion implant: Effects of stacked generators.

Spinal fusion surgeries have a high failure rate for difficult-to-fuse patients. A piezoelectric spinal fusion implant was developed to overcome the issues with other adjunct therapies. Stacked generators were used to improve power generation at low electrical load resistances. The effects of the number of layers on average maximum power and the optimal electrical load resistance were characterized. The effects of mechanical preload, load frequency, and amplitude on maximum power and optimal electrical load resistance were also characterized. Increasing the number of layers from one to nine wa

Pain prevalence and trajectories following pediatric spinal fusion surgery.

UNLABELLED: Factors contributing to pain following surgery are poorly understood, with previous research largely focused on adults. With approximately 6 million children undergoing surgery each year, there is a need to study pediatric persistent postsurgical pain. The present study includes patients with adolescent idiopathic scoliosis undergoing spinal fusion surgery enrolled in a prospective, multicentered registry examining postsurgical outcomes. The Scoliosis Research Society Questionnaire-Version 30, which includes pain, activity, mental health, and self-image subscales, was administered
