MR imaging of pineal region neoplasms.

Twenty-eight patients with tumor of the pineal region underwent magnetic resonance (MR) examination. Gadolinium DTPA was given to 13 patients. Histologic confirmation was obtained in all patients, including 10 germinomas, 4 pineocytomas, 4 pineoblastomas, 5 astrocytomas, 2 teratomas, 1 choriocarcinoma, 1 mixed germ cell tumor, and 1 metastasis from breast carcinoma. Pineal region neoplasms have remarkable heterogeneity in both histopathologic and MR appearances. Gadolinium DTPA enhanced MR increases the specificity of diagnosing pineal region neoplasms. In addition, Gd-DTPA provides better del

Standardized indices of mortality among persons with spinal cord injury: accelerated aging process.

We conducted this study to compare survival rates and morbidity of persons with spinal cord injury (SCI) versus general population, and to clarify the risk of SCI persons. The subjects of this study were 960 men with SCI who had been accommodated in the eight Labor Accident Rehabilitation Centers in Japan during the period of 1965-1995. The surveyed items were the year of birth, the year of injury, level of spinal cord injury and survival status. The classification of cause of death was taken from ICD-10. The cumulative survival rate by life table method was calculated. In order to compare the

Primary neoplasms of the thoracic and lumbar spine. An analysis of 29 consecutive cases.

Primary osseous neoplasms of the thoracic and lumbar spine are uncommon lesions. Between 1965 and 1982, of 1971 patients with musculoskeletal neoplasms, only 29 (1.5%) had primary osseous tumors in these locations. There were eight children and 21 adults. Back pain was the most common complaint in 25 patients (86%), and neurologic symptoms or deficits were present in 16 patients (55%). All lesions were visible on routine spine roentgenograms, while computed tomography and myelography demonstrated spinal canal encroachment and extradural spinal cord compression in 19 patients (66%). The histolo

Primary intramedullary malignant melanoma: can imaging lead to the correct diagnosis?

Melanoma is a malignant neoplasm of melanin-producing cells. Melanoma usually occurs in the skin, but can also arise in any anatomical site that contains melanocytes, such as mucous membranes, the eyes, and the central nervous system (CNS). Primary CNS malignant melanoma most often develops in the leptomeninges. We report a case of a rare intramedullary melanoma of the thoracic spinal cord. A 78-year-old man was treated with surgery, radiotherapy, and immunotherapy for leptomeningeal spread. We also discuss the role of imaging methods in diagnosis and follow-up. Medullary melanoma occurs more

[Imaging of extradural spinal lesions].

There is a wide variety of spinal extradural tumors. In addition to real neoplasms, degenerative diseases, congenital abnormalities and inflammatory disorders can be causes of extradural masses. Due to the bony boundary of the spinal canal, both benign as well as malignant masses can cause progressive neurological deficits including paraplegia. Most of the spinal tumors are benign (hemangioma of the vertebral body, degenerative diseases). In younger patients congenital abnormalities and primary tumors of the spine have to be considered, whereas in adults the list of differential diagnoses shou

Hemorrhagic intramedullary solitary fibrous tumor of the conus medullaris: case report.

The differential diagnosis of spinal tumors is guided by anatomical location and imaging characteristics. Diagnosis of rare tumors is made challenging by abnormal features. The authors present the case of a 47-year-old woman who presented with progressive subacute right lower-extremity weakness and numbness of the right thigh. Physical examination further revealed an extensor response to plantar reflex on the right and hyporeflexia of the right Achilles and patellar reflexes. Magnetic resonance imaging of the lumbar spine demonstrated an 8-mm intramedullary exophytic nodule protruding into a h

Surgical treatment of vertebro-epidural neoplasms of the cervical tract via the anterior approach.

Fifteen cases of vertebral-epidural neoplasms of the cervical tract, treated surgically by the technique of longitudinal median somatotomy with inlay arthrodesis via the anterior approach are presented. Compression of the spinal anterior artery is eliminated by the wide anterior longitudinal exposition, which also creates a support for a possible second operation, either laminectomy or foraminotomy.

Spinal lesions, paraplegia and the surgeon.

Thirty-six patients with spinal cord lesions and varying degrees of paraplegia were seen by the surgical team at the Angau Memorial Hospital, Lae, over a thirty month period. Because the continued presence of a spinal lesion may lead to progressive cord destruction and ischaemic myelopathy, prompt treatment is advocated. The depressing results that have followed treatment of fracture dislocations of the cervical spine and secondary neoplasm with paraplegia is recorded and some suggestions are made that may improve the outlook in future cases. Early and major surgery is advocated in the treatme

[Primary extra-osseous Ewing sarcoma of the dorsal spine with spinal cord compression at diagnosis: Case report].

Extraosseous primitive neuroectodermal tumors are very rare neoplasms. Only a few cases have been published in the literature. This report is about a 9-year-old female patient whose clinical manifestations showed spinal cord compression, so different diagnosis should be considered, including oncology disease. The patient showed a mass of the dorsal spine with spinal cord compression. She was diagnosed with Ewing Sarcoma after surgical intervention and pathology study. It is necessary to highlight the importance of the present case due to the low incidence of the extraosseous Ewing Sarcoma, and

Exophytic intramedullary meningioma of the cervical spinal cord.

Intramedullary spinal cord neoplasms are relatively uncommon. The most common intramedullary tumors are astrocytomas and ependymomas. Meningiomas can occur as an intradural tumor; however, they are typically in the extramedullary compartment. A 42-year-old male presented with progressive sensory loss in the upper extremities and lower extremity weakness. Pre-operative imaging suggested an intramedullary cervical lesion. To treat the progressive neurological abnormality, surgical resection was planned. At surgery, it was noted that the tumor originated in the cervical spinal cord and extended i

Tumor or hematoma? : An unusual case of an extradural lesion of the lumbar spine.

PURPOSE: Spinal epidural hematoma is a rare clinical entity. We present a case of atypical contrast enhancement pattern in a chronic epidural hematoma of the lumbar spine mimicking an extradural tumor. CASE REPORT: A 76-year-old man on treatment with oral anticoagulants presented with a 1-month history of lower back pain radiating into his right upper thigh accompanied by spinal claudication. Preoperative MRI showed a posterior epidural lesion compressing the cauda equina with almost homogeneous contrast enhancement. Surgery was performed under the presumptive diagnosis of spinal extradural n

Chronic mycotic meningitis with spinal involvement (arachnoiditis): a report of five cases.

Five patients developed mycotic spinal arachnoiditis-meningitis causing signs and symptoms of spinal cord neoplasm. Four had cryptococcal infection, the fifth had aspergillosis. In three patients, diagnosis was made at surgery; all three developed acute fungal meningitis postoperatively and two died. The diagnosis was made nonsurgically in two patients and was followed by medical cure. These five and twelve other reported patients with mycotic spinal arachnoiditis shared features that suggested the diagnosis. In contrast to most patients with spinal tumors, those reported here tended to be you

The incidence of vertebral body metastases.

Over 10 years, 842 consecutive autopsies on patients dying from a malignant neoplasm were evaluated microscopically to determine the incidence of spinal and skeletal metastases. Vertebral wedging, sclerosis and areas of osteolysis were defined as features of spinal metastases. The incidence was 30.6%, 65% of which arose from lung, breast or lymphoma and myeloma. The frequency of extraspinal metastases was 9.8% with 66% arising from the lung, liver, lymphoma or myeloma; they were mainly situated in the ribs, skull and clavicle.

Use of the extreme lateral approach in the surgical treatment of an intradural ventral cervical spinal cord vascular malformation: technical case report.

Vascular malformations on the ventral aspect of the spinal cord are difficult to access surgically. Recently, selected lesions have been treated with endovascular embolization. However, embolization using currently approved agents may not be permanently effective, and recanalization rates range from 25 to 83% in the literature. Additionally, many of these lesions are not amenable to endovascular treatment because of the inadequate diameter, tortuosity, or lack of collateral flow of the anterior spinal artery from which the feeding arteries arise. Surgical approaches to these lesions have been

Giant cervicothoracic extradural meningioma involving the brachial plexus: A case report.

BACKGROUND: Spinal meningiomas account for 25% of spinal tumors, with the majority being intradural neoplasms. Extradural spinal meningiomas, however, represent only 2-3.5% of cases. These tumors tend to be more aggressive, often invading neural structures, particularly the brachial plexus. This results in faster progression, higher recurrence rates, and challenges in achieving complete resection. CASE DESCRIPTION: A 52-year-old male presented with right-hand muscle hypotrophy and motor deficits. Magnetic resonance imaging (MRI) revealed a large extradural mass from C7 to D3, compressing neur
