Lumbar spinal stenosis in elderly patients.

Over a 3-year period in our clinic, surgeons operated on 32 persons over 65 years old with lumbar spinal stenosis. This article presents the retrospective analysis of the clinical, radiological, and short-term surgical outcomes. The stenosis seen most commonly among the elderly develops focally at the intervertebral junctions as a result of a complex process of disc degeneration, facet arthropathy, ligamentum flavum hypertrophy, spondylosis, and sometimes spondylolisthesis. All patients underwent a midline decompressive laminectomy with foraminotomies at the affected levels, and discectomy was

Treatment of degenerative lumbar spinal stenosis.

Spinal stenosis is a narrowing of the vertebral canal that compresses spinal nerves and may cause leg pain and difficulty walking. The symptoms of degenerative lumbar stenosis commonly occur in elderly adults and can be treated conservatively with pain-relieving agents or aggressively with decompressive surgery. Most studies of the effectiveness of treatments are poor in quality; however, there appear to be potential relationships between treatments, patient characteristics, and treatment outcomes. Studies indicate the following: (1) local anesthetic block can reduce symptoms on a short-term b

Spinal stenosis: history and physical examination.

The history and physical examination are an essential component in the assessment of patients with lumbar spinal stenosis. The differential diagnosis is broad, and many conditions may be ruled out with a thorough office evaluation. Peripheral neuropathy, arteriovascular disease, and hip arthrosis are common entities with similar symptoms. Imaging studies provide poor specificity. Clinical decision making should be based on a collection of data, including the history and physical findings, functional status, imaging and electrodiagnostic studies, and other adjunctive studies.

Lumbar spinal stenosis causing congenital clubfoot.

Congenital lumbar spinal stenosis is believed to rarely cause neurologic symptoms during childhood. We present a 16-year-old boy with bilateral congenital clubfeet surgically corrected by tendo Achillis releases at 2 years of age who presented with progressive, bilateral footdrop. Magnetic resonance imaging of his lumbosacral spine showed severe spinal stenosis at the L3-5 vertebrae. Congenital lumbar spinal stenosis is probably an under-recognized cause of lower extremity neurologic abnormalities, including clubfoot deformity. Magnetic resonance imaging has made this eminently treatable disor

Microsurgical techniques in lumbar spinal stenosis.

The rationale for microsurgical decompression is to maximize neural decompression while minimizing the destabilization and resection of uninvolved structures. Microsurgical decompression is appropriate for most degenerative lumbar spinal stenosis because the pathoanatomy tends to be concentrated at the levels of the disk space and foramen. Careful preoperative analysis of the imaging studies will guide the surgeon to the most appropriate method of decompression for a particular patient and help focus the decompression to the appropriate regions. Thorough knowledge of surgical spinal anatomy is

Laminectomy for thoracic spinal canal stenosis.

Stenosis of the thoracic spinal canal is a relatively rare disorder with numerous causes. Clinical manifestations include signs and or symptoms consistent with focal thoracic radiculopathy and/or myelopathy. Several surgical approaches for the decompression of the stenotic thoracic canal have been described. Laminectomy is typically reserved for only those cases in which dorsal compression of the neural elements is demonstrated; it is contraindicated when the epidural compression is primarily ventral in location.

[MR myelography in spinal canal stenosis].

PURPOSE: The purpose of this prospective study was to evaluate the clinical value of 3D-MR-myelography (3D-MRM) in comparison to myelography and intra-operative findings. MATERIAL AND METHODS: 25 patients with suspected lumbar spinal canal stenosis were studied via myelography and 3D-MRM (volume-data set, 3D-FISP sequence, TR 73 ms, TE 21 ms, flip angle 7 degrees, sagittal slices) besides the routinely acquired sagittal and axial T1- and T2-weighted images. Diagnoses were made by two radiologists and one neurosurgeon without knowing the clinical history and symptoms, in two separate sessions.

[Radiology of lumbar spinal canal stenosis].

Lumbar spinal stenosis is a frequent cause of low back pain. In this paper we discuss both the pathophysiology and clinical symptomatology of the disease. We also discuss advantages and limitations of plain film radiography, myelography, CT and MRI in its diagnosis. Following an analysis of the pertinent literature we try to determine the relative merit of the various radiologic procedures as to their true representation of clinical and intraoperative findings and faithful rendition of postoperative results. Until sufficient statistically valid data regarding the clinical significance of radio

Decompressive lumbar laminectomy for spinal stenosis.

A total of 258 consecutive decompressive lumbar laminectomies performed on 244 individuals presenting with spinal stenosis were analyzed retrospectively. Spinal fusion was avoided in all but two patients. Outcome in terms of pain relief and return to normal activity was evaluated in two stages, one derived from patient charts and having a relatively short-term follow-up time (mean 8.4 months) and a second derived from patient responses to a questionnaire (which also scored for satisfaction with the results of surgery), which had a longer follow-up time (mean 4.7 years). More than 20 clinical a

Acrodysostosis associated with spinal canal stenosis.

An adult case of acrodysostosis with striking lumbar spinal canal stenosis is reported. She complained of numbness on the right arm, intermittent claudication with numbness on both legs and pain and weakness on the left leg. Although the reduced lumbar interpedicular distance has been reported, no adult case with neurologic symptoms has been reported.

Functional pathology of lumbar spinal stenosis.

This paper deals with the effect of motion upon the stenotic lumbar spinal canal and its contents. A review is presented of personal investigations and relevant data from the literature. The normal spinal canal and its lateral recesses are naturally narrowed by retroflexion and/or axial loading, as shown by anatomical, myelographic and computerized tomographic studies. Due to enough free play and protection by safety cushions, cauda equina and exiting nerve roots are not endangered. In stenosis this protection is lost, causing clinical symptoms of nervous compression of a posture-dependent cha

Does sublaminar wiring produce spinal stenosis?

Sublaminar wiring provides strong and effective fixation of the scoliotic or unstable spine, but its long-term effects on the spinal canal remain unknown. An animal model was developed to observe these effects on the growth and development of the immature spine over a two-year period. Laminar overgrowth occurred both longitudinally to produce a kyphoscoliosis and in the transverse plane to cause significant laminar thickening and growth into the spinal canal. However, the cross-sectional area of the spinal canal was not significantly compromised.

Lumbar spinal canal stenosis in osteopoikilosis.

Spinal canal stenosis is diagnosed in a 45-year-old man who has osteopoikilosis and anomalies of the hip. Clinical investigations demonstrated a lumbar syndrome, previously described as an early phase of a neuropathic condition, namely, the premyelopathic syndrome. Roentgenographic calculations of canal diameters and computerized tomography proved helpful in the diagnosis of narrowed spinal canal. This case, presenting with unusual aspects of osteopoikilosis, emphasizes the importance of early diagnosis and surgical decompression of the stenosed spinal canal.

The significance of residual stenosis after decompression for lumbar spinal stenosis.

Thirty-one patients with degenerative or combined degenerative and developmental lumbar spinal stenosis diagnosed by myelography and at subsequent surgery were thoroughly examined by computed tomography (CT) to reveal the completeness of the decompression and its correlation with the outcome. Complete elimination of the stenosis was achieved in 10 cases (32%) of whom 9 (90%) showed improvement. Various combinations of residual stenosis (some type of stenosis of the lateral part of vertebral canal at the laminectomy sites and also central stenosis outside the laminectomy field) were observed in
