INTRODUCTION: Joint-ill in neonatal lambs, primarily caused by Streptococcus dysgalactiae subspecies dysgalactiae (SDSD), results in increased mortality and morbidity. An effective vaccine is needed to prevent the disease.
METHODS: A blinded randomised controlled trial was carried out using an autologous SDSD vaccine in a commercial sheep flock in the UK. Two doses of vaccine were administered to 481 pregnant ewes, with 509 ewes left unvaccinated. The SDSD antibody titres of the ewes and their lambs were then measured. Any adverse effects or cases of joint-ill were also reported.
RESULTS: Te

Aquaculture industry frequently encounters disease outbreaks, high mortalities, as well as emergence of new pathogens due to its intensification. Streptococcus agalactiae (Lancefield's group B Streptococcus) is an important pathogen extensively causing infectious disease in tilapia resulting in huge economic loss and mortality. To date, vaccination has proved to be successful in defending against infectious diseases prevailing among farmed fish species. This study aimed to develop an S. agalactiae inactivated vaccine (SAIV) using molecular adjuvants, flagellin and tilapia interferon gamma (IFN

Streptococcus pyogenes (Strep A) infections result in a vastly underestimated burden of acute and chronic disease globally. The Strep A Vaccine Global Consortium's (SAVAC's) mission is to accelerate the development of safe, effective, and affordable S. pyogenes vaccines. The safety of vaccine recipients is of paramount importance. A single S. pyogenes vaccine clinical trial conducted in the 1960s raised important safety concerns. A SAVAC Safety Working Group was established to review the safety assessment methodology and results of more recent early-phase clinical trials and to consider future

Narcolepsy is a chronic neurologic sleep disorder. Type 1 narcolepsy (narcolepsy-cataplexy) is associated with the destruction of lateral hypothalamic hypocretin neurons. It is thought that the loss of hypocretin neurons is autoimmune mediated. This is because of the close relationship between type 1 narcolepsy and HLA DQB1∗0602 and the onset of narcolepsy at a young age. Evidence suggests that streptococcal and H1N1 influenza infections (and H1N1 vaccination) may be involved in the pathogenesis of narcolepsy. There are suggestions from genetic and immune studies that the immune system plays a

Studies have used medical record discharge data as coded by the International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9-CM) to estimate pneumococcal pneumonia incidence and vaccine efficacy. However, the accuracy of coding data to identify laboratory-confirmed pneumococcal pneumonia is not known. With the use of information collected in Ohio for a community-based pneumonia incidence study, the authors calculated the sensitivities, specificities, positive predictive values (PPV), and negative predictive values (NPV) of specific codes for pneumococcal pneumonia among

BACKGROUND: Acute rheumatic fever and rheumatic heart disease cause a high burden of disease in Fiji and surrounding Pacific Island countries, but little is known about the epidemiology of group A streptococcal (GAS) pharyngitis in the region. We designed a study to estimate the prevalence of carriage of beta-hemolytic streptococci (BHS) and the incidence of BHS culture-positive sore throat in school aged children in Fiji.
METHODS: We conducted twice-weekly prospective surveillance of school children aged 5 to 14 years in 4 schools in Fiji during a 9-month period in 2006, after an initial pha

PURPOSE OF REVIEW: We describe the epidemiology of the recent global surge in invasive group A streptococcal (GAS) disease and consider its proximate and distal causes. We highlight important knowledge gaps regarding clinical management and discuss potential strategies for prevention.
RECENT FINDINGS: Rates of invasive GAS (iGAS) disease were increasing globally prior to the COVID-19 pandemic. Since mid-2022, following the worst years of the pandemic in 2020 and 2021, many countries with systems to monitor GAS syndromes have reported surges in cases of iGAS concurrent with increased scarlet f

Rheumatic heart disease (RHD) continues to be a significant global health burden, particularly in resource-limited regions of the world. This preventable and often neglected disease affects predominantly children and young adults, leading to considerable morbidity and mortality. In this article, we revisit the Cairo Accord that aims to address the challenges of RHD and set policy priorities for its eradication. We highlight the most remarkable progress in the field since the 2021 update, formulating updated recommendations. These include enhancing disease registries and echocardiographic scree

We reviewed 70 years of research defining the specificity of anti-group A carbohydrate (GAC) monoclonal and polyclonal antibodies and antibodies raised against other S. pyogenes components reacting with GAC. While some rheumatic fever associated autoantibodies react with N-Acetyl-β-D-glucosamine sidechains of GAC and cross-react with tissues, these appear to be a consequence, not the cause, of autoimmunity. We propose GAC be considered further as a broadly protective group A streptococcal vaccine. (70 words).

BACKGROUND: Group A streptococci (GAS) cause infections with a high prevalence in most developing countries. A GAS vaccine under trial that is based on the amino-terminus of the M protein provides type-specific immunity, and hence seems ineffective in India because of heterogeneous emm types. However, the conserved C-terminal region of the M protein protects against multiple serotypes. In this paper, the immune response generated against the conserved C-repeat region of the M protein was checked in an Indian population to establish their vaccine candidature.
METHODS: When screened for GAS, pa

The field of infectious diseases saw numerous exciting advances in 2023. Trials of new antibiotics and treatment regimens sought to address rising rates of antimicrobial resistance. Other studies focused on the most appropriate use of currently available treatments, balancing the dual goals of providing effective treatment and impactful antimicrobial stewardship. Improvements in disease prevention were made through trials of both new vaccines and new chemoprophylaxis approaches. Concerning trends this year included increasing rates of invasive group A streptococcal infections, medical tourism-

BACKGROUND Pneumocephalus is a rare occurrence without trauma, neurosurgery, or intracranial pathology. It is an uncommon complication of bacterial meningitis, and it is usually diagnosed with a CT head. Bacterial pneumocephalus in the setting of influenza B virus infection is an extremely rare complication; however, vaccination against influenza and early diagnosis and treatment help prevent mortality. CASE REPORT A 51-year-old woman presented to the Emergency Department in early winter because of worsening mental status over seven days prior to presentation. She was not vaccinated against in

Streptococcus iniae represents an important bacterial agent responsible for streptococcosis in marine fish, leading to substantial economic impacts in aquaculture worldwide. The development of effective vaccines is therefore a critical priority. EsxA, a conserved early-secreted and homolog of antigenic target six (ESAT-6), is involved in bacterial virulence and mediates interactions between the pathogen and its host via the type VII secretion system. In this work, EsxA was examined as a prospective vaccine antigen in golden pompano (Trachinotus anak) using both subunit and DNA vaccination stra

In this study, we engineered two recombinant fusion proteins by conjugating the streptococcal albumin-binding domain (ABD) to either the N-terminus (AP) or C-terminus (PA) of protamine (P). These proteins spontaneously assembled with the CVB3 VP1 DNA vaccine (pVP1) into nanovaccines, leveraging ABD's albumin-binding capacity to target draining lymph nodes (dLNs) and enhance immunogenicity and efficacy. All recombinant AP, PA, and P proteins efficiently bound to and protected the pVP1 plasmid, forming nanoparticles with a diameter of 170-200 nm. In vivo distribution assays showed that AP-pVP1 e

Rheumatic heart disease (RHD) is a serious and long-term consequence of acute rheumatic fever (ARF), an autoimmune sequela of a mucosal infection by Streptococcus pyogenes (Group A Streptococcus, Strep A). The pathogenesis of ARF and RHD is complex and not fully understood but involves host and bacterial factors, molecular mimicry, and aberrant host innate and adaptive immune responses that result in loss of self-tolerance and subsequent cross-reactivity with host tissues. RHD is entirely preventable yet claims an estimated 320 000 lives annually. The major burden of disease is carried by deve

Emm (M protein gene) typing was used to analyze group A streptococcal (GAS) clinical isolates in Thailand from in-patients at Chulalongkorn University Hospital (CUH) between January 1, 1995 and December 31, 1999. Forty GAS isolates were recovered from blood and other sterile sites from 40 patients presenting with different types of infections. A variety of emm sequences (24 types) have been reported in this study including the identification of 2 novel emm variants demonstrating the diverse population of invasive GAS isolates in Thailand. The common emm types include emm1 (5 isolates, 12.5%),

Group B Streptococcus (GBS) remains a leading cause of neonatal sepsis, meningitis, stillbirth, and long-term neurodevelopmental impairment worldwide. Although intrapartum antibiotic prophylaxis (IAP) has substantially reduced early-onset GBS disease in high-income settings and remains an effective and evidence-based intervention, it does not prevent late-onset disease, maternal colonization, or GBS-associated stillbirths, and its implementation is challenging in many low- and middle-income countries (LMICs). Maternal vaccination has therefore emerged as a promising complementary strategy to a

Group B streptococcus (GBS; Streptococcus agalactiae) infection during pregnancy is an important cause of adverse pregnancy outcomes and life-threatening neonatal infections. All GBS strains have surface proteins that belong to the alpha-like protein (Alp) family. Following the demonstration of the safety and immunogenicity of a maternal GBS vaccine containing the fused N-terminal domains of two Alp proteins (αCN and RibN) in healthy adult women, a second-generation vaccine has been developed (GBS-AlpN, GBS-NN/NN2), which also contains the N-terminal domain of the Alp1 and Alp2/3 proteins (Alp

Streptococcus iniae causes invasive infections in fresh and saltwater fish and occasional zoonoses. Vaccination against S. iniae is complicated by serotypic variation determined by capsular polysaccharide. A potential target for serologically cross-protective vaccines is the M-like protein SiMA, an essential virulence factor in S. iniae that is highly conserved amongst virulent strains. The present study determined how SiMA is regulated and investigated potential as a cross-protective vaccine for fish. Electrophoretic mobility shift suggested that SiMA is regulated by the multigene regulator M
