Construction of a computational mechanical model of bronchi for practical simulation of the optimal positive intrathoracic pressure conditions during general thoracic surgery.

BACKGROUND: Thoracic CO2 insufflation with positive intrathoracic pressure is usually effective during thoracoscopic surgery, however, lung collapse is sometimes insufficient. We hypothesized that inappropriate bronchial collapse might cause this unsuccessful lung collapse. OBJECTIVE: The objective of this study was to construct a computational mechanical model of bronchi for practical simulation to discover the optimal conditions of positive intrathoracic pressure during thoracoscopic surgery. METHODS: Micro-focus high-resolution X-ray computed tomography measurements of lungs from just-sla

From coping to structural abandonment: Reframing self-medication in Yemen's protracted conflict.

The protracted conflict in Yemen has led to profound disruptions in the health system, fundamentally reshaping patterns of healthcare access and utilization. This commentary engages with a recent study that conceptualizes self-medication and informal antibiotic use as forms of coping and agency in constrained settings, and re-examines this interpretation within the context of systemic collapse. Drawing on a structural perspective, the analysis argues that self-directed care practices in Yemen are not temporary responses to episodic constraints but are embedded within enduring structural condit

Avascular Necrosis of the Talus Following Charcot Midfoot Beaming.

Avascular necrosis (AVN) of the talus results from disrupted blood flow, causing bone death, collapse, and joint degeneration. Its vascular structure makes it prone to ischemia, with causes including trauma, corticosteroids, and metabolic disorders. Charcot neuroarthropathy shares pathologic similarities with AVN. A study of 135 Charcot reconstructions found talar AVN in 14%, linked to talonavicular dislocation, renal disease, and high body mass index. Starling resistor modeling explains microcirculatory failure. Management ranges from immobilization to surgical options like arthrodesis and di

Limited Impact of Short-Term Osteoporosis Medication on Vertebral Height Loss in the Acute Phase of Osteoporotic Vertebral Compression Fractures: A 3-Month Longitudinal Analysis.

Background and Objectives: The optimal pharmacological strategy to mitigate progressive vertebral collapse during the acute phase of osteoporotic vertebral compression fractures (OVCFs) remains a subject of debate. This initial 3-month window is the most critical period for evaluating the structural stability of the fracture, as the majority of progressive height loss occurs before solid bone union is achieved, directly influencing the decision to continue conservative management or transition to surgical intervention. Materials and Methods: In this retrospective study, 123 patients were alloc

From information to existence: the Information-Meaning-Existence (IBE) framework as a meta-method for second-order observation.

Sociological systems theory offers a rich vocabulary for meaning, observation, and communication, but that vocabulary has proven difficult to translate into executable, testable, and revisable procedures. This gap has become more consequential as large language models are increasingly used to read, summarize, and simulate social-scientific discourse without any shared standard for distinguishing genuine semantic stabilization from fluent but ungrounded continuation. This paper introduces the Information-Meaning-Existence (IBE) framework, which reconstructs Luhmannian second-order observation a

Management of preeclampsia with severe features in a patient with relapsing polychondritis affecting the tracheobronchial tree.

Relapsing polychondritis is a rare disease that affects cartilaginous structures throughout the body. Progressive destruction of the laryngeal structures and the tracheobronchial tree occurs in 50% of patients, potentially leading to loss of patency and collapse of the airway. Respiratory involvement in relapsing polychondritis includes airway stenosis, tracheomalacia and recurrent lung infections due to chronic inflammation caused by the destruction of upper and lower airway cartilage. Pregnancy and preeclampsia can worsen pharyngolaryngeal oedema, while treatment with magnesium sulphate can

The Prognostic Significance of Pedicle Enhancement from Contrast-enhanced MRI for the Further Collapse in Osteoporotic Vertebral Compression Fractures.

STUDY DESIGN: A retrospective observational study. OBJECTIVE: The aim of this study was to investigate the prognostic significance of contrast-enhanced magnetic resonance imaging (MRI) for detecting pedicle enhancement and predicting future collapse in patients with acute benign osteoporotic vertebral compression fracture (OVCF). SUMMARY OF BACKGROUND DATA: Traditional morphological classification regarding compression versus burst fractures cannot accurately anticipate the prognosis of further collapse. Identifying subgroups with a higher risk for further collapse may be beneficial in deter

Anterior septal deviation and contralateral alar collapse.

INTRODUCTION: Septal deviation is often found in conjunction with other pathological conditions that adversely affect nasal patency. Anterior septal deviation, together with contralateral alar collapse, is a relatively rare type of anatomical and functional incompetence. In our experience, it can often be resolved with septoplasty, without the necessity of surgery involving the external valve. The aim of this paper was to verify this hypothesis prospectively. PATIENTS AND METHODS: Twelve patients with anterior septal deviation and simultaneous alar collapse on the opposite side were prospecti

Upper airway collapsibility and cephalometric variables in patients with obstructive sleep apnea.

Increased pharyngeal collapsibility and abnormal anatomic structures have been postulated to contribute to the pathophysiology of obstructive sleep apnea (OSA) syndrome. It is unclear whether the abnormal craniofacial and soft tissue features may affect the pharyngeal collapsibility and contribute to the apnea density. In the present study we examine the relationship between pharyngeal collapsibility and cephalometric variables in a group of 57 male OSA patients. Pharyngeal collapsibility was measured during the night of nasal continuous positive airway pressure (nCPAP) titration by analyzing

A case of natural hydatidosis in a baboon.

Four large and several smaller cysts of Echinococcus granulosus were found in the liver of a baboon submitted for necropsy. Three of the large cysts had an atypical structure and were packed with a semi-solid mass of collapsed daughter cysts, small cysts containing fluid and involuted membranes. This atypical structure has been reported previously though only in baboons.

Disc collapse aggravates vertebral recollapse following percutaneous kyphoplasty: a clinical review and biomechanical study.

BACKGROUND: Promoting uniform load distribution is an essential biomechanical function of intervertebral discs (IVDs), but disc degeneration (DD) may compromise this and lead to localized stress concentration. However, whether DD can trigger stress concentration on the cement-augmented vertebral body and result in more severe augmented vertebral recollapse has yet to be determined. The aim of this study was to investigate clarify this issue and provide theoretical guidance regarding the mechanism underlying this complication. METHODS: Augmented vertebrae height was measured immediately after

Is the ischemia found in normal pressure hydrocephalus secondary to venous compression or arterial constriction? A comment on Ohmura et al.

BACKGROUND: In a recent study of normal pressure hydrocephalus published by Ohmura et al., there was a progressive reduction in the cerebral blood volume within the cortex, as measured by near-infrared spectroscopy, following an increase in the intracranial pressure from an infusion study. The authors contend that this reduction in blood volume occurred due to the collapse of the venous structures, starting from the smallest veins adjacent to the capillaries and involving the entire venous outflow tract. We wish to outline some problems with this interpretation. MAIN BODY: It has been previou

Obstructive sleep apnoea as a late complication after tracheostomy.

BACKGROUND: Obstructive sleep apnoea occurs consequent to partial or complete upper airway obstruction, caused mostly by the collapse of upper airway musculature. Drug-induced sleep endoscopy represents the 'gold standard' in identifying the obstruction site, from the palatal level to laryngeal entry. Breathing impairment in sleep caused by the collapse of cervical trachea after previous tracheostomy has not yet been described in the literature. METHODS: This report presents two patients with severe obstructive sleep apnoea, in whom pre-operative drug-induced sleep endoscopy revealed upper ai

Segment-specific prediction of vertebral collapse severity in osteoporotic fractures.

BACKGROUND: Osteoporotic vertebral compression fractures (OVCFs) predominantly cluster at the thoracolumbar junction (T11-L1). Recent studies suggest that both bone quality and muscle health significantly contribute to vertebral collapse. However, among numerous multidimensional imaging metrics, which one can most accurately predict the severity of vertebral collapse, and whether the precise load-sharing proportions between bone and muscle exhibit distinct segment-specific differences, remain rarely quantified in the clinical literature. PURPOSE: To identify imaging metrics that best predict

Airway Anatomy and Surgical Management in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a sleep-related breathing disorder characterized by recurrent collapse of a structurally vulnerable upper airway during sleep. Airway caliber and stability are influenced by multiple anatomical structures, including the nasal cavity, soft palate, lateral pharyngeal walls, tongue base, hyoid complex, and craniofacial skeleton. Detailed knowledge of these structures is therefore essential for rational surgical planning. This narrative review summarizes key anatomical features relevant to upper pharyngeal surgery, including nasal obstruction and the internal nasal

Alveolar overdistension is an important mechanism of persistent lung damage following severe protracted ARDS.

It is now widely accepted that mechanical ventilation may damage the lung, but the mechanism of lung damage is not clear. Possible causes include overdistension of aerated alveoli by inappropriately large tidal volumes (volutrauma), shear stresses generated during the recruitment and de-recruitment of lung units at the junction of aerated and collapsed lung, and infective or ischaemic necrosis of persistently collapsed lung. Computerized tomography allows noninvasive assessment of lung structure during and after acute lung injury, and may provide insight into the mechanism of lung damage. Usin

Comparative Study of Functional Nasal Reconstruction Using Structural Reinforcement.

IMPORTANCE: Nasal reconstruction after Mohs surgery is a unique challenge in that it must satisfy both functional and aesthetic goals. Despite some advocacy in the literature for using structural reinforcement to achieve both functional and aesthetic outcomes in soft-tissue reconstruction, no study has validated this claim by comparing reconstruction with and without structural support. OBJECTIVE: To evaluate the effectiveness of and need for structural reinforcement when reconstructing the nasal alar and sidewall subunits. Development of morphologically restored and deproteinized Juglans regia pollen-derived sporopollenin exine capsules: A multi-analytical verification study.

Sporopollenin exine capsules (SECs) are attractive natural microcarriers, but allergenic protein residues remain a critical limitation for biomedical applications. In this study, the development of morphologically restored and deproteinized JSECs from Juglans regia pollen was verified. High-purity capsules were isolated via sequential chemical treatments, followed by a PEG-4000-assisted stabilization process to resolve structural collapse observed during acidolysis. Protein elimination and morphological recovery were validated using SDS-PAGE, solid-state ¹³C CP/MAS NMR, and BET analysis. The a

Porous Anisometric PNIPAM Microgels: Tailored Porous Structure and Thermal Response.

The porous structure of microgels significantly influences their properties and, thus, their suitability for various applications, in particular as building blocks for tissue scaffolds. Porosity is one of the crucial features for microgel-cell interactions and significantly increases the cells' accumulation and proliferation. Consequently, tailoring the porosity of microgels in an effortless way is important but still challenging, especially for nonspherical microgels. This work presents a straightforward procedure to fabricate complex-shaped poly(N-isopropyl acrylamide) (PNIPAM) microgels wit

Left coronary artery anomaly: an often unsuspected cause of sudden death in the military athlete.

More than 300,000 cases of sudden cardiac death (SCD) occur in the United States each year. Left coronary artery anomaly (LCAA), although rare, is second only to hypertrophic cardiomyopathy as the most common cause of SCD associated with structural cardiovascular abnormalities. This case illustrates SCD secondary to LCAA in a military athlete. A 19-year-old soldier collapsed after an 8-km run. On arrival at the emergency room, he was unresponsive and in asystole. Despite successful resuscitation and aggressive management, the patient died the next morning. Autopsy revealed an anomalous left co
