Topical application of nitrates onto the papilla of Vater: manometric and clinical results.

BACKGROUND AND STUDY AIMS: Systemic administration of nitrates inhibits motility of the sphincter of Oddi (SO) but is hampered by a decrease of the arterial blood pressure. We evaluate whether topical application of glyceril trinitrate (GTN) onto the papilla can facilitate cannulation of the common bile duct during routine endoscopic retrograde cholangiopancreatography (ERCP) and compare the effect of topical GTN with that of isosorbide dinitrate (ISDN) on SO motility. PATIENTS AND METHODS: A total of 80 patients undergoing routine ERCP for suspected biliary obstruction received topical appli

Diagnostic and safety value of ultrasound-guided transthoracic core needle biopsy for mediastinal masses: a single-center experience.

US-guided (Ultrasound-guided) percutaneous transthoracic core needle biopsy (CNB) is a favorable method for establishing the correct diagnosis of mediastinal masses. However, studies in this area are scant and often include small samples, thus making it difficult to provide robust evidence regarding the safety and efficacy of this procedure. Thus, we aimed to report on the 20 years of experience at our center with regard to US-guided CNB.We included all consecutive patients referred to our center to undergo US-guided percutaneous transthoracic CNB for a mediastinal mass between 1999 and 2022.

Acceptability of IV iron treatment for iron deficiency anaemia in pregnancy in Nigeria: a qualitative study with pregnant women, domestic decision-makers, and health care providers.

BACKGROUND: Anaemia in pregnancy causes a significant burden of maternal morbidity and mortality in sub-Saharan Africa, with prevalence ranging from 25 to 45% in Nigeria. The main treatment, daily oral iron, is associated with suboptimal adherence and effectiveness. Among pregnant women with iron deficiency, which is a leading cause of anaemia (IDA), intravenous (IV) iron is an alternative treatment in moderate or severe cases. This qualitative study explored the acceptability of IV iron in the states of Kano and Lagos in Nigeria. METHODS: We purposively sampled various stakeholders, includin

Aspects Towards the Anastomotic Healing in Crohn's Disease: Clinical Approach and Current Gaps in Research.

Anastomotic leakage is a major complication in gastrointestinal and colorectal surgery and its occurrence increases morbidity and mortality. Its incidence is even higher in Crohn's disease surgeries. Several authors have identified factors involved in the pathophysiology of anastomotic leak in the literature, aiming to reduce its occurrence and, therefore, improve its surgical treatment. Surgical technique is the most discussed topic in studies on guiding the performance of side-to-side stapled anastomosis. Preoperative nutritional therapy also has been shown to reduce the risk of anastomotic

Expert consensus on oral management in autoimmune bullous diseases, erythema multiforme and SJS/TEN.

BACKGROUND: Autoimmune bullous diseases (AIBD), Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS-TEN) and erythema multiforme (EM) often present with clinically similar features in the oral mucosa: classically widespread mucosal erosions or ulceration. Despite this, there are no formalized guidelines for the specific management of the oral manifestations of these conditions. OBJECTIVES: We sought to establish an consensus on the management of oral involvement of AIBD, SJS-TEN and EM using the Delphi method. METHODS: Participants were sent a survey comprising 62 statements organiz

Life-threatening Carotid Complications Caused by Extraluminal Migration of Ingested Foreign Bodies; a Case Report and Narrative Review of Literature.

Carotid complications resulting from extra-luminal migration of ingested foreign bodies (FB) are rare but potentially life-threatening. Previous data on the topic predominantly comprises isolated case reports, leaving a gap in comprehensive evidence necessary to guide clinical decision-making. In this article, we offer a narrative review alongside a novel case report, aimed at providing a broad, evidence-based perspective on the topic to guide clinical practice. The search strategy employed keywords related to carotid artery complications from ingested FB across the following electronic databa

Updates on Pediatric Optic Neuritis.

Optic neuritis is a central demyelinating disease that can significantly affect vision. Favorable outcomes are contingent on prompt recognition, work-up, and treatment. Understanding key differentiations in the pediatric population compared with adults is necessary for accurate and timely diagnosis. Recent studies highlighting these distinctions have deepened insight into clinical variabilities in presentation, as well as neurological associations, treatments, and prognostic factors. Ongoing collaborative studies continue to shed light on the main considerations guiding research on this topic.

In patients undergoing valve-sparing aortic root replacement, is reimplantation superior to remodelling?

A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was: in patients undergoing valve-sparing aortic root replacement, is reimplantation superior to remodelling? The purpose of this best evidence topic was to re-review the updated evidence that has become available in the near decade since the previous review published in 2011. Altogether more than 300 papers were found using the reported search, of which 8 papers represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, pati

Population Pharmacokinetics of Topical Timolol Maleate Gel in Healthy Volunteers and Infants With Superficial Infantile Hemangioma.

Although timolol ophthalmic solutions or hospital-compounded formulations have been used off-label for the treatment of superficial infantile hemangioma (IH), no topical timolol formulation has been developed specifically for this indication. This study was the first to develop a population pharmacokinetic (popPK) model for topical timolol maleate gel and to perform exploratory exposure-safety analyses. Data from Phase I-III clinical trials in healthy adults and infants with proliferative superficial IH were used to develop the popPK model. Potential covariate effects on the pharmacokinetics w

Prevention and early diagnosis of cancer in the rectal stump of ulcerative colitis patients after colectomy and topical treatment.

Ulcerative colitis with malignant degeneration and dysplasia can be a precancerous lesion. Therefore, it is necessary to prevent or, at least to diagnose as early as possible any development toward neoplasia in the colon or rectum of the colitis patients. The only reliable guide for a risk of malignant tissue degeneration is dysplasia of the mucosa. A group of 31 patients was studied after total colectomy with ileorectal anastomosis and subsequent topical treatment with enemas containing sulphasalazine and corticosteroids. Two of these patients had mild rectal dysplasia before surgery, seen in

Efficacy of Topical Corticosteroid Therapy in Chronic Rhinosinusitis Post-endoscopic Sinus Surgery: A Narrative Synthesis of Randomized Controlled Trials.

Chronic rhinosinusitis (CRS) is a frequent inflammatory disorder that can markedly impair patients' quality of life. Endoscopic sinus surgery (ESS) remains the treatment of choice for refractory cases, but postoperative inflammation and adhesion formation are still major concerns. Corticosteroids are often prescribed to address these complications, although systemic administration carries the risk of adverse effects. This has led to growing interest in localized, topical corticosteroid therapies. This review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISM

Extramedullary Tibial Guide Alignment Is Not Affected by Excess Lower Limb Fat Distribution in Total Knee Arthroplasty.

Background The goal of this study is to investigate whether excess lower limb fat distribution affects tibial guide alignment in conventional total knee arthroplasty (TKA) with extramedullary guides. A thicker soft tissue envelope may affect the accuracy of extramedullary cutting guide placement and subsequent instrumentation. Previous studies have used body mass index (BMI) to stratify patients, a poor proxy of lower limb fat distribution, which may explain conflicting results reported on this topic to date. This study overcomes this issue by using a novel, radiographic anthropometric index t
