Efficacy and safety of single-trajectory posterior subthalamic area and subthalamic nucleus deep brain stimulation for dystonic tremor: a retrospective pilot study.

OBJECTIVE: To evaluate the feasibility, safety, and preliminary efficacy of a novel single-lead, dual-target deep brain stimulation (DBS) approach targeting the posterior subthalamic area (PSA) and subthalamic nucleus (STN) for dystonic tremor. METHODS: This retrospective pilot study reviewed outcomes of six consecutive patients with medically refractory dystonic tremor who underwent single-lead PSA-STN DBS at our center (June-December 2024). Clinical outcomes were assessed using the BFMDRS and FTMTRS scales. A formal blinded crossover assessment was performed in three patients to compare PSA

Subthalamic nucleus in patients with Parkinson's disease encodes changes and magnitude of applied force.

The subthalamic nucleus (STN), a primary target for Parkinson's disease (PD) neuromodulation therapies, contributes to movement dynamics but it is unknown how STN relates to specific kinematic parameters. We investigated the neuronal activity in the dorsolateral STN during a visually cued isometric grip-force task in the contralateral hand in patients with PD who were undergoing deep brain stimulation surgery. Patients applied various levels of grip force during a task that separated a sustained applied force vs periods of changing force (yank) while we recorded single unit activity with a mic

Impact of subthalamic nucleus deep brain stimulation on voice and speech in Parkinson's disease.

PURPOSE: Subthalamic nucleus deep brain stimulation (STN-DBS) effectively alleviates motor symptoms in Parkinson’s disease (PD), yet its impact on speech and voice remains inconsistent. This study examined acoustic parameters, perceptual features, and speech intelligibility using a large, methodologically standardized cohort. METHODS: A total of 105 PD patients (55 females, 50 males; mean age ≈ 57 years) undergoing STN-DBS were evaluated. All received posterolateral electrode placement, bilateral stimulation, and a postoperative 30–35% reduction in levodopa. Acoustic measures fundamental frequ

Effect of high, low, and asymmetric frequency stimulation of the subthalamic nucleus in Parkinson's disease.

BACKGROUND: This study investigated the clinical efficacy of asymmetric frequency stimulation (AFS), which involved the unilateral low-frequency and contralateral high-frequency stimulation, in the treatment of PD. METHODS: In a randomized controlled design, we compared the efficacy of Bi-LFS and Bi-HFS (n = 33) and explored the efficacy of AFS (n = 20) in the acute setting in one cohort of postural instability gait difficulty (PIGD) motor subtype of patients with PD. Motor evaluations included MDS UPDRS-III, Berg balance scale (BBS), and gait kinematic analysis, and 6 to 12 months of telepho

Beyond motor control: The subthalamic nucleus as a central hub for pain in Parkinson's disease.

The subthalamic nucleus (STN), considered a primarily motor structure within the basal ganglia, is recognized as a key contributor to a wider set of behaviors. Among these, nociceptive processing has gained particular attention, especially given the high prevalence and early emergence of pain in Parkinson's disease (PD). This review brings together anatomical, physiological and translational evidence to examine how the STN integrates motor and nociceptive information. The STN receives ascending afferents from brainstem nociceptive pathways and its activity tracks the intensity and salience of

Piezoelectric neuromodulation of the subthalamic nucleus ameliorates motor and nonmotor symptoms of Parkinson's disease.

Traditional deep-brain stimulation via implanted electrodes can effectively treat neurological disorders, but surgical injury limits its clinical application. Here, we developed ultrasound-responsive piezoelectric nanoparticles for minimal-invasive and wireless neuromodulation. In the 6-OHDA-induced Parkinson's disease (PD) mouse model, these nanoparticles are injected into the subthalamic nucleus (STN) of the mouse brain. After ultrasound stimulation for several days, the motor behavior, particularly gait abnormalities and nonmotor symptoms such as pain and anxiety, in PD mice is alleviated w

Abnormal neuronal activity in the subthalamic nucleus contributes to dysarthria in patients with Parkinson's disease.

BACKGROUND: This study investigated the subthalamic nucleus (STN) function and deep brain stimulation (DBS) effects on single-unit activity (SUA) in Parkinson's disease (PD) patients with dysarthria. METHODS: After presurgical speech analysis, we recorded STN neuronal activities while PD patients (n = 16) articulated Chinese Pinyin consonants. The Pinyin consonants were categorized by the manner and place of articulation for SUA cluster analysis. The cohort was then divided into normal articulation and dysarthria groups based on diadochokinetic (DDK) assessments. The STN SUA patterns, represe

Differential contribution of sensorimotor cortex and subthalamic nucleus to unimanual and bimanual hand movements.

Why does unilateral deep brain stimulation improve motor function bilaterally? To address this clinical observation, we collected parallel neural recordings from sensorimotor cortex (SMC) and the subthalamic nucleus (STN) during repetitive ipsilateral, contralateral, and bilateral hand movements in patients with Parkinson's disease. We used a cross-validated electrode-wise encoding model to map electromyography data to the neural signals. Electrodes in the STN encoded movement at a comparable level for both hands, whereas SMC electrodes displayed a strong contralateral bias. To examine represe

Deep brain stimulation of the subthalamic nucleus and the temporal discounting of primary and secondary rewards.

Although deep brain stimulation of the subthalamic nucleus is an effective surgical treatment for Parkinson's disease, it may expose patients to non-motor side effects such as increased impulsivity and changes in decision-making behavior. Even if several studies have shown that stimulation of the subthalamic nucleus increases the incentive salience of food rewards in both humans and animals, temporal discounting for food rewards has never been investigated in patients who underwent STN-DBS. In this study, we measured inter-temporal choice after STN-DBS, using both primary and secondary rewards

Subthalamic nucleus pathology contributes to repetitive behavior expression and is reversed by environmental enrichment.

Repetitive motor behaviors are common in neurodevelopmental, psychiatric and neurological disorders. Despite their prevalence in certain clinical populations, our understanding of the neurobiological cause of repetitive behavior is lacking. Likewise, not knowing the pathophysiology has precluded efforts to find effective drug treatments. Our comparisons between mouse strains that differ in their expression of repetitive behavior showed an important role of the subthalamic nucleus (STN). In mice with high rates of repetitive behavior, we found significant differences in dendritic spine density,

The effect of unilateral subthalamic nucleus deep brain stimulation on depression in Parkinson's disease.

BACKGROUND: Depression is common in Parkinson's disease (PD) and adversely affects quality of life. Both unilateral and bilateral subthalamic (STN) deep brain stimulation (DBS) effectively treat the motor symptoms of PD, but questions remain regarding the impact of unilateral STN DBS on non-motor symptoms, such as depression. METHODS: We report changes in depression, as measured by the Hamilton Depression Rating Scale (HAMD-17), in 50 consecutive PD patients who underwent unilateral STN DBS. Participants were also evaluated with UPDRS part III, Parkinson's Disease Questionnaire-39, and Pittsb

Effects of varying subthalamic nucleus stimulation on apraxia of lid opening in Parkinson's disease.

Apraxia of lid opening (ALO) is a non-paralytic inability to open the eyes or sustain lid elevation at will. The exact pathophysiological mechanisms underlying the syndrome are still unknown. ALO has been reported in patients with Parkinson's disease (PD) after subthalamic nucleus (STN) deep brain stimulation (DBS), suggesting a possible involvement of the basal ganglia. We aimed to assess the effects of varying STN stimulation voltage on ALO in PD patients. Seven out of 14 PD patients with bilateral STN stimulation consecutively seen in our centre presented with ALO. We progressively increase

Involvement of subthalamic nucleus in the stimulatory effect of Delta(9)-tetrahydrocannabinol on dopaminergic neurons.

The cannabinoid CB1 receptor which is densely located in the basal ganglia is known to participate in the regulation of movement. The present study sought to determine the mechanisms underlying the effect of Delta(9)-tetrahydrocannabinol (Delta(9)-THC) on neurons in the substantia nigra pars compacta (SNpc) using single-unit extracellular recordings in anesthetized rats. Administration of Delta(9)-THC (0.25-2 mg/kg, i.v.) increased the firing rate of SNpc neurons (maximal effect: 33.54+/-6.90%, n=8) without modifying other firing parameters (coefficient of variation and burst firing). This eff

Electric field and stimulating influence generated by deep brain stimulation of the subthalamic nucleus.

OBJECTIVE: The goal of this project was to develop a quantitative understanding of the volume of axonal tissue directly activated by deep brain stimulation (DBS) of the subthalamic nucleus (STN). METHODS: The 3-dimensionally inhomogeneous and anisotropic tissue medium surrounding DBS electrodes complicates our understanding of the electric field and tissue response generated by the stimulation. We developed finite element computer models to address the effects of DBS in a homogeneous isotropic medium, and a medium with tissue conductivity properties derived from human diffusion tensor magneti

[Chronic electric stimulation of the internal globus pallidus and subthalamic nucleus in Parkinson disease].

PATHOPHYSIOLOGY: In Parkinson's disease, the neurodegenerative process of the nigrostriatal dopaminergic pathways induces an increase in activity of the subthalamic nucleus and the medial globus pallidus, which cause inhibition of thalamo-cortical outputs explaining parkinsonism. HIGH-FREQUENCY DEEP BRAIN STIMULATION: The adverse effects induced by lesions of subcortical structures (thalamotomy, pallidotomy) have increased interest in chronic electrical stimulation proposed as a new therapy in Parkinson's disease. This technique is reversible and can be modulated with less adverse effects. TW

High-frequency stimulation of the subthalamic nucleus suppresses experimental resting tremor in the monkey.

The effect of high-frequency stimulation of the subthalamic nucleus on parkinsonian-like resting tremor was investigated in two monkeys (Macaca fascicularis). Unilateral tremor of the arm and leg was induced by electrical coagulation of the brainstem area including the substantia nigra and the red nucleus. The tremor was only seen at rest condition with a very stable frequency of 4.46+/-0.59 Hz (mean+/-S.D.). Apomorphine (0.10-0.4 mg/kg, s.c.) completely blocked the tremor, suggesting that it was a dopaminergic-dependent symptom just like the parkinsonian tremor. When the stimulating frequency

Neuroprotective effect of chronic inactivation of the subthalamic nucleus in a rat model of Parkinson's disease.

Several evidences showed that glutamate can be implicated in the degenerative process of dopaminergic neurons in Parkinson's disease. The treatment with NMDA antagonists have been shown to induce a neuroprotective effect in animal models of this disease. As subthalamic nucleus neurons send direct glutamatergic projections to the substantia nigra, we studied the effects of kainic acid lesion of this nucleus on the degeneration of dopaminergic neurons induced by microinjection of 6-hydroxydopamine in the striatum of rat done one week after the first lesion. Animals were killed 15 days after the

An unusual concomitant tremor and myoclonus after a contralateral infarct at thalamus and subthalamic nucleus.

A 72-year-old woman experienced a sudden onset of spontaneous tremor and myoclonus of right extremities that completely subsided 24 hours after onset. Neuroimaging study revealed an infarct at the left ventral portion of thalamus and subthalamic nucleus. Concomitant dyskinetic movement disorders after stroke are extremely rare and the mechanism is herein discussed.

The effect of subthalamic nucleus deep brain stimulation on speech metrics in Parkinson's disease.

BACKGROUND: Speech impairment frequently occurs in Parkinson's disease (PD). The sequence effect, progressive deterioration in speed or amplitude of ongoing movement, is unique to PD and does not respond well to dopaminergic therapy. While subthalamic nucleus deep brain stimulation (STN DBS) can improve various motor symptoms in PD, its impact on speech and the speech sequence effect remains unclear. Here, we investigated the effects of STN DBS on speech metrics including the sequence effect in people with PD. METHODS: Fifteen individuals with PD who underwent bilateral STN DBS surgery partic
