Sudden infant death syndrome and complications in other pregnancies.

BACKGROUND: The likelihood of recurrence of sudden infant death syndrome (SIDS) is an issue of biological, clinical, and legal interest. Obstetric complications are associated with an increased risk of SIDS and are likely to recur in subsequent pregnancies. We postulated that women whose infants died from SIDS would be more likely to have had obstetric complications in their other pregnancies. METHODS: We linked national UK databases of maternity-hospital discharges, perinatal deaths, and death certifications. We studied 258 096 women who had consecutive births in Scotland between 1985 and 20

Nasal and intrapulmonary haemorrhage in sudden infant death syndrome.

BACKGROUND: Fresh intrapulmonary and oronasal haemorrhages in cases of sudden infant death syndrome (SIDS) might be markers for accidental or intentional smothering inappropriately diagnosed as SIDS. AIM: To compare the incidence, epidemiological association, and inter-relation of nasal haemorrhage, intrapulmonary haemorrhage, and intrathoracic petechiae in infant deaths certified as SIDS. METHODS: In SIDS cases from a large nationwide case-control study, a wide range of variables were compared in cases with and without reported nasal haemorrhage and, in a subgroup of cases, in those with an

Histopathology of the conduction system in sudden infant death.

Because ventricular fibrillation or other arrhythmia have been suggested as a cause of unexplained sudden infant death, many authors focused their studies on the conduction system especially in the 1970s. These studies were not conclusive and many pathologists have abandoned the study of the conduction system. We have reviewed the conduction system of 87 sudden infant death cases between 15 days and 1 year old: 55 were SIDS cases; 17 had mild lesions insufficient to explain death and 15 cases were sudden explained deaths considered as controls. Fetal dispersion of the AV node and/or His bundle

Sudden infant death syndrome: a search for allergen hypersensitivity.

BACKGROUND: Sudden infant death syndrome (SIDS) remains a diagnosis by exclusion which leaves few if any pathologic clues to its etiology. Previous evaluations for anaphylaxis in SIDS have been few and limited. OBJECTIVE: To analyze forensic blood specimens for evidence of anaphylaxis in 51 (43 boys and 8 girls) children dying of SIDS and 13 (9 boys and 4 girls) age-matched controls who died from defined, nonanaphylactic causes. METHODS: Specimens collected over a 5-year period were assayed for (1) total IgE (IU/mL) by immunoenzymatic assay; (2) latex, cat, dust mite (Dermatophagoides farina

Hypovitaminosis A: A model for sudden infant death syndrome.

The cause of sudden infant death syndrome (SIDS) is unknown. It is the leading cause of death from age one month to one year in North America. The purpose of this essay is to generate some testable hypotheses as to the cause of SIDS by drawing attention to distinct epidemiological parallels between SIDS and a newly recognized form of enamel hypoplasia, termed localized hypoplasia of the primary canine tooth (LHPC), which has been attributed to vitamin A deficiency. LHPC and SIDS share a common epidemiological profile: winter seasonality, occurrence at 3-5 months, and affecting apparently healt

[Sudden death syndrome in the newborn infant and sleep].

It seem increasingly obvious that sudden death is a multifactorial syndrome, the result of respiratory and/or cardiac dysfunction in responses to various causes. Cardiorespiratory and neurophysiological explorations performed during sleep have considerably increased our understanding of the syndrome. They have been at the root of all recent theories, and they make it possible to rule out a likely cause in infants "rescued" from sudden death. Some of these explorations may be used for screening purposes, as they sometimes evaluate the risk of apnea or cardiac accident capable of causing sudden

Ultrastructure of carotid bodies in sudden infant death syndrome.

Recent studies have implicated an abnormality in carotid body structure and function in the pathogenesis of sudden infant death syndrome (SIDS). In the present investigation, the light and electron microscopic findings in carotid bodies from ten victims of SIDS were compared with those in six control infants and five infants dying of congenital heart disease. The cross-sectional area of carotid body chemoreceptor cells and the frequency, distribution, and size of neurosecretory granules were assessed morphometrically. The area of carotid body occupied by chemoreceptor cells (the functional are

Variations in Cause-of-Death Determination for Sudden Unexpected Infant Deaths.

OBJECTIVES: To quantify and describe variation in cause-of-death certification of sudden unexpected infant deaths (SUIDs) among US medical examiners and coroners. METHODS: From January to November 2014, we conducted a nationally representative survey of US medical examiners and coroners who certify infant deaths. Two-stage unequal probability sampling with replacement was used. Medical examiners and coroners were asked to classify SUIDs based on hypothetical scenarios and to describe the evidence considered and investigative procedures used for cause-of-death determination. Frequencies and we

Using death certificates to characterize sudden infant death syndrome (SIDS): opportunities and limitations.

OBJECTIVE: To examine cause-of-death terminology written on death certificates for sudden infant death syndrome (SIDS) and to determine the adequacy of this text data in more fully describing circumstances potentially contributing to SIDS deaths. STUDY DESIGN: With 2003 and 2004 US mortality files, we analyzed all deaths that were assigned the underlying cause-of-death code for SIDS (R95). With the terminology written on the death certificates, we grouped cases into SIDS-related cause-of-death subcategories and then assessed the percentage of cases in each subcategory with contributory or pos

Differences in heart DNA methylation between sudden infant death syndrome and other sudden deaths.

BACKGROUND: Sudden Infant Death Syndrome (SIDS) remains a leading cause of infant mortality worldwide, yet its underlying mechanisms are largely unknown. SIDS etiology is complex and involves the interplay between a vulnerable infant, a critical developmental period, and external stressors. Cardiac function, including cardiorespiratory autonomic control, is considered a major contributor to SIDS. Emerging evidence suggests that epigenetic modifications of heart tissue, particularly DNA methylation (DNAm), may contribute to SIDS risk. We conducted an epigenome-wide association study (EWAS) to d

Sudden Unexpected Infant Deaths: 2015-2020.

OBJECTIVE: Although the US infant mortality rate reached a record low in 2020, the sudden infant death syndrome (SIDS) rate increased from 2019. To understand if the increase was related to changing death certification practices or the coronavirus disease 2019 (COVID-19) pandemic, we examined sudden unexpected infant death (SUID) rates as a group, by cause, and by race and ethnicity. METHODS: We estimated SUID rates during 2015 to 2020 using US period-linked birth and death data. SUID included SIDS, unknown cause, and accidental suffocation and strangulation in bed. We examined changes in rat

Topicalities in the Sudden Unexpected Death Syndrome in Infants.

Sudden Infant Death Syndrome (SIDS) represents the third cause in postnatal mortality. The pathogenesis is multifactorial. SIDS victims can present sign of preexisting chronic asphyxia, persistent increase in dendritic spine and delayed maturation of synapses in medullary respiratory centers, if a decreased reactivity of 5 hydroxytryptamine 1A (5-HT1A) and 5-HT2A in the dorsal nucleus of the vagus, solitary nucleus and ventrolateral medulla. SIDS is an exclusion diagnosis, so that inexplicable SIDS is the accepted term. The objective of this report is to present current data about the pathogen

A functional approach to sudden unexplained infant deaths.

There is a great deal of variation in the methods and wording used by medical examiners in the medicolegal investigation and certification of infant deaths. This paper was created by the NAME Ad Hoc Committee on Sudden Unexplained Infant Death to address several specific issues, namely: * To establish a functional approach to the investigation of sudden unexplained infant deaths; * To outline a "bare minimum" set of recommendations to define the scope of investigation required; * To recommend methods and wording to be used when certifying infant deaths; * To develop a list of potential stresso

Haemoglobin F levels in sudden infant deaths.

Fetal haemoglobin levels have been measured prospectively in 135 autopsy cases of sudden, unexpected infant deaths (31 pre-term, 104 full term) using standard laboratory methods. These results have been compared with Hb F values from a normal control group of 570 living infants (145 pre-term, 425 full-term) with a post-conceptional age < 90 weeks. The gestational age was established for all live controls and sudden infant deaths. The results show that full-term (> 38 weeks gestational age) sudden infant death victims as a group have significantly elevated Hb F levels (chi 2 = 25.20, P < 0.001)

Sudden infant death syndrome and hibernation: is there a link?

Two very large bodies of information seem to overlap, one being the epidemiological and pathological studies on sudden infant death syndrome (SIDS), the other being the research on hibernation. Until now, these two phenomena have been studied independently; however, it will be argued here that the role of the pineal gland, that of brown adipose tissue (BAT) and sleep patterns in both endothermic hibernating mammals and those babies presenting as SIDS provide clues and support for the hypothesis that SIDS is pathoplastic to the hibernatory response.

Homicide as a cause of the sudden infant death syndrome.

The homicidal asphyxiation of a 10 1/2-month-old male infant and the attempted asphyxiation of his 4-month-old sibling, documented by parental confession, is presented as evidence that murder may sometimes be mistaken as sudden infant death syndrome (SIDS). A review of the literature of the relationship between murder and SIDS deaths reveals the suspicions of some physicians but few published cases; this reflects not only the difficulties of making a determination of murder by suffocation, since no injuries may be present, but also a lack of awareness among physicians who must evaluate infant

Apnea and sudden unexpected death in infants with achondroplasia.

Thirteen infants with achondroplasia and sudden unexpected death or unexplained apnea were discovered through nonsystematic retrospective case collection. Most were initially thought to have died from sudden infant death syndrome. However, historical and pathologic findings suggest that many of these infants had apnea and sudden unexpected death secondary to acute or chronic compression of the lower brainstem or cervical spinal cord. Infants with achondroplasia evidently are at considerably increased risk for such deaths between 1 month and 1 year of age. Appropriate intervention, given these
