Upper-Limb Compartment Syndrome Following Rapid Infusion Catheter Extravasation During Orthotopic Liver Transplantation: A Case Report.

Rapid infusion catheters (RICs) provide high-flow peripheral venous access for major haemorrhage but can cause severe tissue injury if pressurised infusion becomes extravascular. We report upper-limb acute compartment syndrome during orthotopic liver transplantation following transfusion through an 8.5 Fr Arrow RIC (Teleflex Incorporated, Morrisville, NC, USA). A female in her late 60s with decompensated cryptogenic cirrhosis underwent urgent transplantation. Four hours before surgery, an attempt to place an 8.5 Fr RIC in the left antecubital fossa was abandoned because of guidewire resistance

Construct validation: simulation of thoracoscopic intrathoracic anastomosis.

BACKGROUND AND OBJECTIVES: We sought to develop a simulation model that accurately replicates the challenges of the thoracoscopic intrathoracic anastomosis. This model is intended to serve as a teaching tool during the introduction to, and development of, the skills required to perform a thoracoscopic intrathoracic anastomosis during an Ivor Lewis minimally invasive esophagectomy. METHODS: The simulation model uses porcine tissue placed within an artificial hemithorax and covered with a synthetic skin. The model is draped to simulate a realistic operative setting, and ports are placed in stan

Maxillomandibular Advancement for Obstructive Sleep Apnea Syndrome: Long-Term Results of Respiratory Function and Reverse Face-Lift.

PURPOSE: Scientific literature considers maxillomandibular advancement (MMA) as the most effective surgical treatment for the management of adult obstructive sleep apnea syndrome (OSAS). Maxillomandibular advancement enlarges the pharyngeal space by expanding the skeletal framework. Moreover, it projects the soft tissue of the cheeks, the mouth, and the nose in the aging face, which is characterized by multiple signs affecting the middle third and the lower third. The potential of orthognathic surgery (double jaw surgical advancement) of expanding the skeletal foundation to increase the facial

One- or two-stage reimplantation for infected total knee prosthesis?

A prosthetic joint infection (PJI) is possibly the most significant potential complication of total knee arthroplasty (TKA) and is associated with substantial morbidity and socioeconomic burden. It is a devastating complication for both the patient and the surgeon alike. A two-stage revision approach for infected TKA has been the standard for surgical management; however, there is growing interest in single-stage revision surgery due to fewer procedures, reduced inpatient hospital stay and reduced costs to healthcare systems. A one-stage exchange is indicated when there is no sign of systemic

Projection geometry and stress-reduction techniques in craniofacial surgery.

Since 1981, we have been able to modify the mathematical patterns of projection geometry to reshape the skull in craniofacial surgery. Unlike burring, morcellization, rotation, and plate switching, this technique actually changes the shape of individual sections of the skull by changing their radius of curvature. The technique is an adaptation of the principles used by engineers to build complex structures such as ships' hulls, airfoils, and domes. The result is a rigid form of the desired shape that becomes permanent with healing. This has several advantages: 1. An increase in the level of sa

Health Care Sustainability: Strategies for Operating Rooms and Urogynecologic Procedures.

In 2023, the World Health Organization (WHO) concluded that climate change is the biggest threat to public health. Health care has a large carbon footprint with operating rooms contributing an inordinate amount to this issue. Surgeons have an opportunity to lead sustainability efforts by changing individual practices and collaborating with other stakeholders. Some of these changes that have been suggested include using reusable devices, including gowns, drapes, and specula; updating instrument trays and surgeon preference cards to minimize opening unused instruments; using clean energy sources

Development of a Surgical Infection Surveillance Program at a Tertiary Hospital in Ethiopia: Lessons Learned from Two Surveillance Strategies.

BACKGROUND: Surgical site infections (SSIs) are a leading cause of post-operative morbidity and mortality. We developed Clean Cut, a surgical infection prevention program, with two goals: (1) Increase adherence to evidence-based peri-operative infection prevention standards and (2) establish sustainable surgical infection surveillance. Here we describe our infection surveillance strategy. PATIENTS AND METHODS: Clean Cut was piloted and evaluated at a 523 bed tertiary hospital in Ethiopia. Infection prevention standards included: (1) Hand and surgical site decontamination; (2) integrity of gow

Reducing the environmental impact of breast cancer surgery: a pilot study on sustainable practices in the operating room.

BACKGROUND: The environmental impact of climate change is unequivocal, and surgical practice, given its substantial carbon footprint, must also move toward sustainability. The primary aim of the study is to assess the environmental impact of surgical practice in breast cancer treatment and to identify interventions that can effectively reduce it. The secondary aim was to explore the feasibility and preliminary clinical performance of indocyanine green (ICG) compared with technetium-99m (99mTc) for sentinel lymph node mapping, within the context of a pilot project on sustainable surgical practi

Is it the technique or wound protection that is key to reducing wound infections in Roux-en-Y gastric bypass procedures?

INTRODUCTION: An unusually high surgical site infection (SSI) rate after Roux-en-Y gastric bypass (RYGB) was noted on routine outcomes review. Surgeon A, who had a rate of 8.9%, utilized the transoral technique (passage down esophagus into gastric pouch) for anvil insertion for the circular-stapled gastrojejunostomy. By comparison, SSI rate was 0% for Surgeon B, who inserted anvil transabdominally (direct passage into stomach via gastrotomy) and used wound protection (wound protector and plastic drape over stapler). We sought to determine if it was the technique for anvil insertion (transoral

Ovine model as a surgical simulator for pediatric laryngotracheal reconstruction.

OBJECTIVES: There are increasing pressures on the health system and training programs to provide adequate training opportunities for surgical trainees. This is particularly true with respect to complex procedures that are performed less commonly. We sought to develop an ovine model for the simulation of pediatric laryngotracheal reconstruction (LTR) and validate its use for training. METHODS: Knee-high lamb specimens were sourced from an accredited Australian abattoir that included structures of the neck, cervical spine and subcutaneous tissues from the thyroid cartilage to the carina. Infection prevention methodologies for lower extremity total joint arthroplasty.

Despite advances in our understanding of surgical site infections following total joint arthroplasty, this serious surgical complication continues to represent a substantial economic burden for the patient, the treating institution and the healthcare system. After increasing for the past decade, infection rates have stabilized at 1.6%; however, the total cost is projected to increase with the total number of revision procedures performed. A systematic review of the literature was performed to identify studies that assess the efficacy of pre-, peri- and post-operative infection prevention strat

Closed hyperthermic intraperitoneal chemotherapy with open abdomen: a novel technique to reduce exposure of the surgical team to chemotherapy drugs.

BACKGROUND: Exposure of the surgical team to toxic drugs during hyperthermic intraperitoneal chemotherapy (HIPEC) remains a matter of great concern. During closed-abdomen HIPEC, operating room staff are not exposed to drugs, but the distribution of the heated liquid within the abdomen is not optimal. With open-abdomen HIPEC, the opposite is true. Although the open-abdomen method is potentially more effective, it has not become a standard procedure because of the risk of exposure of members of the team to drugs. METHODS: We present a new technique (closed HIPEC with open abdomen) which ensures

Clean Joints, Clean Planet: A Review of Literature on Sustainable Practices in Orthopaedic Joint Replacements.

The healthcare sector generates significant global greenhouse gas emissions, and the orthopaedic operating theatre is among the most resource-intensive environments within any hospital. Hip and knee arthroplasty produce considerable waste and carbon emissions with each procedure. This review evaluates global sustainable practices adopted for hip and knee joint replacement surgeries and identifies practical opportunities to reduce environmental impact while preserving patient safety and quality of care. A narrative review rather than a systematic review of literature of major databases was perf

Endophthalmitis Associated with a Retained Cilium in the Clear Corneal Wound after Cataract Surgery.

We report a case of endophthalmitis occurring secondary to a retained cilium in a clear corneal wound after cataract surgery. A 67-year-old female presented to an ophthalmology emergency room with light perception vision in the right eye 5 days after routine cataract surgery. Examination of the right eye demonstrated decreased vision, conjunctival injection, a corneal endoplaque, and anterior chamber fibrin. Additionally, a cilium was visible in the clear corneal incision. A vitreous aspiration and injection of antibiotics and steroid were performed, and the patient underwent a vitrectomy the

Use of biologic mesh for a complicated paracolostomy hernia.

BACKGROUND: Parastomal hernias are among the most frustrating and incapacitating complications of permanent colostomies. Because the traditional surgical options of primary repair with or without ostomy repositioning have led to disappointing results, the use of mesh is indicated, especially in the setting of multiple recurrences. METHODS: After laparoscopic lyses of adhesions, the colostomy is pushed against the lateral abdominal wall, and a bovine pericardium graft is gently stretched and draped over the colostomy (the Sugarbaker technique). Transfascial sutures and tacks are placed along t

[Treatment of priapism with intracavernous injection of etilefrine hydrochloride].

A case of priapism following epidural anesthesia was reported. A 67 year old man received epidural anesthesia for the transurethral resection of a malignant bladder tumor. Epidural anesthesia which he had wanted to receive was given through the L3 to L4 intervertebral space with 18 ml of 2% lidocaine. The penile erection occurred while he was prepared and draped. Additional 8 ml of 1% lidocaine failed to overcome it and the operation cancelled. After this episode, he had anesthesia on 5 occasions in 2 years. General anesthesia was given 3 times without penile erection, but epidural block was t

Effect of a preoperative coating agent on postoperative skin tears in pancreatectomy.

BACKGROUND: Postoperative skin tears are an underrecognized complication following pancreatectomy and often result from the removal of adhesive surgical drapes. Despite a negative impact on recovery, limited strategies are available for their prevention. METHODS: We retrospectively analyzed data for 348 patients who underwent pancreatectomy at our institution from April 2019 to December 2021. In this cohort, 71 patients had received a preoperative sterile coating agent (intervention group), and 277 patients had not (control group). The incidence, severity, and treatment duration of postoperat
