Schizophrenia among Surinamese in the Netherlands: high admission rates not explained by high emigration rates.

BACKGROUND: A high risk of schizophrenia has been found among Caribbean immigrants in the Netherlands and Great Britain. One hypothesis to explain these findings is that patients with a diagnosis of schizophrenia or patients with symptoms of psychosis not specifically diagnosed, emigrate more than the general population. Such selection might account for high rates of Surinamese patients with a diagnosis of schizophrenia in Netherlands psychiatric hospitals. We examined this hypothesis. METHODS: The files of patients with schizophrenia or patients suffering from unspecified psychosis were sele

European Aedes caspius mosquitoes are experimentally unable to transmit Zika virus.

BACKGROUND: Aedes caspius (Pallas, 1771) is a floodwater mosquito species widely distributed in the Western Palaearctic. As an anthropophilic species, its role as an arbovirus vector may be the key for understanding the transmission cycle of certain diseases in Europe such as Zika virus (ZIKV). Concerning vector competence for ZIKV, studies related to Ae. caspius are still scarce. ZIKV is an arbovirus that has provoked a widespread epidemic in the Pacific region (2007-2013) and in the Americas (2015-2016). ZIKV is associated with serious neurological injuries (e.g. microcephaly) and Guillain-B

Yaws in the Americas.

Until the middle of this century, yaws was a major public health problem in the Americas Region of the World Health Organization. All countries located between the tropics of Cancer and Capricorn reported cases of yaws. From 1950 to 1957, major programs for the eradication of yaws were implemented throughout the region, and yaws rapidly ceased to be a threat. As of 1983, sporadic cases continued to occur in limited geographic areas of Colombia, French Guyana, Guyana, and Suriname.

Deposition of bauxite dust and pulmonary fibrosis.

In a 70 years old worker in the bauxite industry in Surinam, who died of myocardial infarction, bauxite deposits were found in the lung in areas of mild pulmonary fibrosis. The identification and the localization within these areas of the bauxite deposits was determined by a histochemical mineralization technique and by scanning electron microscopy and electron microprobe analysis. The possible relationship between the presence of bauxite in the lung tissue and the pulmonary fibrosis is discussed.

The burden of dengue in South American countries, 1990-2019: Estimates from the Global Burden of Disease study 2019.

OBJECTIVES: To describe the main dengue metrics generated by the Global Burden of Disease (GBD) Study 2019 for South American countries between 1990 and 2019. STUDY DESIGN: A descriptive study was carried out using the metrics obtained by the GBD Study 2019. METHODS: The metrics of incidence, years of life lost due to premature death (YLLs), years lived with disability (YLDs) and disability-adjusted life years (DALYs) were evaluated, grouped by sex and country, between 1990 and 2019. The metrics were presented with age-standardized rates per 100,000 inhabitants and their respective uncertain

Monitoring the response of patients with cutaneous leishmaniasis to treatment with pentamidine isethionate by quantitative real-time PCR, and identification of Leishmania parasites not responding to therapy.

BACKGROUND: Leishmania (Viannia) guyanensis is believed to be the principal cause of cutaneous leishmaniasis (CL) in Suriname. This disease is treated with pentamidine isethionate (PI), but treatment failure has increasingly been reported. AIM: To evaluate PI for its clinical efficacy, to compare parasite load, and to assess the possibility of treatment failure due to other infecting Leishmania species. METHODS: Parasite load of patients with CL was determined in skin biopsies using real-time quantitative PCR before treatment and 6 and 12 weeks after treatment. Clinical responses were evalua

TRYPNET. New hemoparasite information network.

Although Guyana, Suriname, and French Guiana share borders and climatic and geographic similarities, the countries have maintained little contact, due to language, political, and administrative differences. In 1993, two international organizations involved in the improvement of animal health, the Inter-American Institute for Cooperation on Agriculture (IICA) and CIRAD-EMVT (Centre de Cooperation Internationale en Recherche Agronomique pour le Developpement-Elevage et Medecine Veterinaire des Pays Tropicaux), jointly developed a collaborative project between the veterinary services of the three

[A patient with fever and pain in the extremities].

During holidays in Suriname a six year old boy fell ill with high fever, general malaise, pain in the extremities and abdomen and vomiting. Because of a tentative diagnosis of acute rheumatism or bacterial enteritis amoxicillin and salicylates were started. After his return to the Netherlands an increased ESR and impaired renal and hepatic functions were found. The history revealed contact with sewage water. Further analysis established the diagnosis of Leptospirosis. This comparatively rare clinical entity, seldom mentioned in paediatric literature, is discussed.

[Malaria in Guiana. II. The characteristics of different foci and antimalarial control].

In French Guiana, the distribution of malaria in foci inhabited by quite different ethnic groups calls for specific studies. Along the Oyapock on the Brasilian border and along the Litani on the Surinam border, incidence among American Indians and Creoles ranges from 300 and 900 per thousand; Plasmodium falciparum accounts for 65% and P. vivax for 35%. Along the middle and lower Maroni on the Surinam border, the Boni and Ndjukas Negroes move freely through the frontier and since the civil strife Surinamese used to attend health centres of Guiana. Therefore it is difficult to find the sources o

Tuberculous meningitis: treat upon suspicion.

A 58-year-old man from Surinam was referred because of nausea, vomiting, weight loss, ascites and an altered mental state. Tuberculous meningitis was suspected upon examination of the cerebrospinal fluid and antituberculous treatment was initiated. However, the patient did not recover but developed haemiplegia with recurrent aspiration pneumonias. This case illustrates that empiric antituberculous treatment is warranted upon clinical suspicion, since no fast, sensitive diagnostic tests are available to date.

Therapeutic plasma exchange in critically ill patients in low-income and lower-middle-income countries: medical need and feasibility.

BACKGROUND: Therapeutic plasma exchange (TPE) is a blood purification technique designed for the removal of large molecules such as pathogenic antibodies and lipoproteins. The procedure involves removing plasma from the patient in exchange for replacement fluid, and it can be performed either by membrane separation or centrifugation. These conventional techniques are expensive and require the training of skilled personnel. This severely limits their use in low-income countries (LICs) and lower-middle-income countries (LMICs), leading to morbidity and mortality for patients in LICs and LMICs su

The case for investment in tobacco control: lessons from four countries in the Americas.

OBJECTIVE: To synthesize learnings from four national tobacco control investment cases conducted in the Americas (Colombia, Costa Rica, El Salvador, Suriname) under the World Health Organization Framework Convention on Tobacco Control (WHO FCTC) 2030 project, to describe results and how national health authorities have used the cases, and to discuss implications for the role of investment cases in advancing tobacco control. METHODS: We draw on findings from four national investment cases that included 1) a cost-of-illness analysis calculating the health and economic burden of tobacco use, 2)

Modelling the effect of the COVID-19 pandemic on violent discipline against children.

BACKGROUND: The COVID-19 pandemic could increase violence against children at home. However, collecting empirical data on violence is challenging due to ethical, safety, and data quality concerns. OBJECTIVE: This study estimated the anticipated effect of COVID-19 on violent discipline at home using multivariable predictive regression models. PARTICIPANTS: Children aged 1-14 years and household members from the Multiple Indicator Cluster Surveys (MICS) conducted in Nigeria, Mongolia, and Suriname before the COVID-19 pandemic were included. METHODS: A conceptual model of how the COVID-19 pand

Imported Mayaro virus infection in the Netherlands.

A Dutch couple, presenting with persisting arthralgias, temporary fever and rash after a stay in Surinam were diagnosed with Mayaro virus infection. Mayaro virus is a relatively unknown South American Alphavirus responsible for dengue-like clinical features and persisting arthralgias. An important, but probably underappreciated cross-reactivity with other Alphaviruses like Chikungunya virus is present, which may become of clinical importance in the event the various Alphaviruses will have overlapping geographical distributions and in seroprevalence studies.

[A young man with parasites during surgery].

An 18-year-old man came to the Emergency Room at the Academic Hospital Paramaribo (Suriname) with a solitary supra-umbilical gunshot-wound. The patient was hypovolemic, with dropping blood pressure and rising pulse, surgery was indicated. After incision of the abdominal midline 1.5 l of blood drained, immediately followed by the appearance of 2 long thin worms. During inspection of the abdominal cavity a perforation of the duodenum and sigmoid and a mesenterial bleeding was seen and treated. Inspection of the small intestine showed presence of Ascaris lumbricoides (roundworm).

Life-threatening anaemia caused by B19 parvovirus infection in a non-immunocompromised patient.

A 21-year-old Surinam boy suffering from a combined heterozygous sickle-cell beta-thalassaemia presented with an extreme anaemia (haemoglobin concentration 0.7 mmol/l) and severe haemolysis without signs of increased erythropoiesis. An acute B19 parvovirus infection was diagnosed by a transient increase of IgM anti-B19 parvovirus titres and a positive polymerase chain reaction for B19 parvovirus. The patient recovered spontaneously after a short period of supportive care with red cell transfusions and mechanical ventilation.

[Q-fever as a cause of myocarditis in childhood].

Q-fever occurs in The Netherlands in childhood more often than used to be believed. We treated an 8-month-old infant with congestive myocarditis associated with a seroconversion against Coxiella burnetii. The infection was probably caused by the father, who a few weeks earlier had visited his native Surinam, where he frequently was surrounded by domestic animals and drank raw cow's milk. After 2.5 years the child's general and cardial conditions are good although digoxin therapy is still necessary.

Core principles of Malakit intervention for transferability in other contexts.

To eliminate malaria, all populations must be included. For those who are not reached by the health care system, specific interventions must be tailor-made. An innovative Malakit strategy, based on the distribution of self-diagnosis and self-treatment kits, has been evaluated in the Suriname-French Guiana- Amapá (Brazil) region. The results showed effectiveness and good acceptability. The Malakit intervention is complex and has many components. Its transferability requires adaptation to other populations and regions, while retaining the main features of the intervention. This article provides
