BACKGROUND: The epidemiology of transient synovitis is poorly understood, and the aetiology is unknown, although a suggestion of a viral association predominates.

Mycobacterium florentinum is a newly identified, rare, slow-growing species of nontuberculous mycobacteria (NTM). Here, we report a case of M. florentinum-induced synovitis of the wrist in an immunocompromised Japanese patient. Pigmented villonodular synovitis (PVNS) is a relatively rare disease affecting synovium-lined joints. PVNS was first fully described in 1941 by Jaffe et al, who considered it a benign inflammatory state of the synovium of uncertain etiology. Pigmented villonodular synovitis is rare in the younger child. Polyarticular involvement in this condition, regardless of patient age, is distinctly uncommon. The authors describe a case of pigmented villonodular synovitis involving multiple joints in a young boy who also had congenital anomalies of the genitourinary tract. Although rare, pigmented villonodular synovitis should be considered in the differential diagnosis of multiple joint swellings in children with congenital anomalies.

Between 1988 and 1995, 32 knee joints (29 patients) with hemophilic arthropathy underwent arthroscopy. The spectrum of procedures ranged from resection of fibrous plicae to synovectomy. Technical difficulties appeared in cases of scarred fixed patella, pronounced posterior tibia subluxation, and severe fibrous ankylosis. We report seven cases of pathologic fracture in adult patients that were seen an average of 5 years (range from 33 to 114 months) after silicone wrist (three) and trapezial (four) arthroplasty. Fifty-five consecutive children presenting with transient synovitis of the hip were examined with 99mTc-MDP scintigraphy and pin-hole collimator technique. Quantitative assessment was performed along a profile of interest across the hip joint. A 30-year retrospective review of 497 cases of transient synovitis of the hip involving 475 children allowed full characterization of the syndrome. Femoral head measurements of 118 patients at follow-up of 6 months showed no significant dimensional changes. Three cases of Legg-Calvé-Perthes disease (2.5%) were found in follow-up, as well as a case of osteoid osteoma of the femoral neck and two cases of juvenile rheumatoid arthritis.
