Complications After Total Knee Arthroplasty in Patients With Pigmented Villonodular Synovitis.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a locally destructive histiocytic proliferation most commonly occurring in the knee. Extensive local joint destruction can indicate the need for a total knee arthroplasty (TKA). The objective of this study is to evaluate PVNS of the knee as a risk factor for complication after TKA. METHODS: Patients who underwent TKA with a diagnosis of PVNS of the knee from 2007 to 2016 were identified in a national private payer insurance database. Complication rates for emergency room visits, readmission, revision, stiffness, infection, and death were

Systematic Arthroscopic Treatment of Diffuse Pigmented Villonodular Synovitis in the Knee.

Pigmented villonodular synovitis (PVNS) is a benign aggressive disease that presents in either a localized (LPVNS) or diffuse (DPVNS) form. Arthroscopic synovectomy is the standard operative treatment for LPVNS, and when used to treat DPVNS, it is usually combined with an open posterior procedure. The purpose of this Technical Note is to report the technique that we have refined to allow for arthroscopic synovectomy as the sole treatment for DPVNS. We describe our technique with the factors we have found to be important to ensure adequate arthroscopic synovectomy, while minimizing risks and co

Computed Tomography and Magnetic Resonance Manifestations of Spinal Pigmented Villonodular Synovitis.

OBJECTIVE: To study computed tomography (CT) and magnetic resonance imaging (MRI) findings of spinal pigmented villonodular synovitis (PVNS), with the aim of improving diagnostic accuracy. METHODS: Findings from CT (n = 7) and MRI (n = 5) of 7 cases of spinal PVNS, diagnosed by pathological examination, were retrospectively analyzed. RESULTS: One case presented with a small lesion, whereas the remaining 6 cases showed lobulated soft tissue masses centering on the facet joints, with lytic expansive destruction of the bone. The CT density of lesions in 3 cases was similar to that of the surrou

Acute Knee Pain in a Child Due to Pigmented Villonodular Synovitis.

INTRODUCTION: Pigmented villonodular synovitis (PVNS) is a proliferative condition of the synovium, which is composed of nodules and/or villi and has an abundant number of hemosiderin-laden macrophages. CASE REPORT: A 10-year-old boy presented with an acute irritable knee. Emergency arthroscopy showed a nodular PVNS in the intercondylar notch. The symptoms resolved after resection of the lesion. CONCLUSION: PVNS of the knee in children is a rare entity. It can be one of the causes of acute irritable knee. Complete resection of the nodular PVNS can cure the disease.

Postoperative radiotherapy for diffuse pigmented villonodular synovitis of the temporomandibular joint.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare/benign condition of the synovial joint lining. It most commonly presents in the knee but has also been reported to occur in the temporomandibular joint (TMJ). Although there are several series reporting the use of postoperative radiotherapy (PORT) for extremity PVNS, there is scant literature on the use of PORT for PVNS of the TMJ. METHODS: We conducted a literature review for case reports related to PVNS of the TMJ and discuss two additional cases treated with surgery and PORT. RESULTS: 71 cases were found in the literature. 89%

Response to imatinib in villonodular pigmented synovitis (PVNS) resistant to nilotinib.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare locally aggressive tumor. PVNS is characterized in most cases by a specific t(1;2) translocation, which fuses the colony stimulating factor-1 (CSF1) gene to the collagen type VIa3 (COL6A3) promoter thus leading through a paracrine effect to the attraction of non-neoplastic inflammatory cells expressing CSF1-receptor. Imatinib is a tirosin-kinase inhibitors (TKI) active against CSF1-receptor whose activity in naïve PVNS was already described. We report on two PVNS patients who responded to imatinib after failure to nilotinib, another

[Bone transplantation with total hip endoprosthesis for diffuse pigmented villonodular synovitis].

Pigmented villonodular synovitis (PVNS) is a rare benign but aggressive disease of the synovium. If the hip is involved early destruction of the joint is common due to the tight structure of the capsule and arthroplasty is unavoidable in these cases. We implanted a cemented total hip replacement in a 17-year-old female patient who had histologically confirmed PVNS. Because of massive bony destruction in the acetabulum a reconstruction with homologous bone (two femoral heads) from the bone bank was necessary. After 5 years the bone transplant had become integrated, there were no signs of recurr

Simultaneous anterior and posterior synovectomies for treating diffuse pigmented villonodular synovitis.

BACKGROUND: The current treatment methods for diffuse intraarticular or extraarticular type pigmented villonodular synovitis (PVNS) include arthroscopic synovectomy or staged anterior and posterior open synovectomies. However, it is unclear whether simultaneous anterior and posterior synovectomies achieve local control and recovery of function. QUESTIONS/PURPOSES: We therefore determined the recurrence rate and function in patients with diffuse PVNS treated with anterior and posterior synovectomies and adjuvant radiotherapy. METHODS: We retrospectively reviewed all 19 patients with diffuse P

Arthroscopic management of intra-articular pigmented villonodular synovitis of temporomandibular joint.

This retrospective study reports on four patients with intra-articular pigmented villonodular synovitis (PVNS) of the temporomandibular joint (TMJ) who were managed with arthroscopy between 2002 and 2009. There were three females and one male, with a mean age of 46 years at diagnosis. The common symptoms were trismus and pain. No pre-auricular swelling or mass was detected. Magnetic resonance imaging (MRI) and arthrography showed an anteriorly displaced disc, disc perforation, osteophyte of the condyle, or increased joint effusion. No neoplasm was suspected radiologically. Under arthroscopy, a

Cementless total hip arthroplasty performed in patients with pigmented villonodular synovitis.

The authors retrospectively reviewed all patients with pigmented villonodular synovitis (PVNS) of the hip treated by cementless total hip arthroplasty (THA) combined with synovectomy. Eight patients were reviewed and observed for an average of 8.9 years (range, 4.3-13.5 years). Mean preoperative Harris hip scores improved from 49.3 to 96.6 points, and all patients were able to regularly perform moderate daily living activities. None of the patients had clinical or radiographic evidence of recurrent PVNS. Osteolysis occurred in 4 hips, and 2 revision surgeries were performed during follow-up. C

Localized pigmented villonodular synovitis in the knee associated with locking symptoms.

Pigmented villonodular synovitis (PVNS) occurs in the knee more frequently than other joints. Most cases involve diffuse PVNS while the localized type is relatively rare. This report describes a patient who had PVNS and a lateral meniscus injury which induced locking symptoms in the knee. A closed reduction of the meniscus tear was performed under local anesthesia. However, complete extension of the knee was not achieved and the locking symptoms persisted. MRI examination showed a neoplastic lesion measuring approximately 2 x 2 cm in the intercondylar space together with a lateral meniscus tea

Pigmented villonodular synovitis causing painless chronic knee swelling in an adolescent.

It is uncommon for an adolescent to present with a painless chronic knee effusion without a history of trauma. Acute knee effusions are usually caused by internal derangement, patellar dislocations, or fractures. The differential diagnosis of a chronic knee effusion must include the more uncommon diagnoses of juvenile rheumatoid arthritis, septic arthritis, reactive arthritis, synovial osteochondromatosis, synovial hemangioma, synovial sarcoma, or pigmented villonodular synovitis. Radiographic imaging, plain radiographs, and magnetic resonance imaging in addition to a biopsy specimen are most

Pigmented villonodular synovitis. Monoclonality and metastasis--a case for neoplastic origin?

We report a 48-year follow-up of a case of recurrent pigmented villonodular synovitis of the knee. Subcutaneous metastasis to the contralateral thigh was an unusual finding. Histology demonstrated fibroblastic and histiocytic proliferation, as well as increased mitotic activity in recurrent lesions. Cytogenetic analyses demonstrated monoclonality and chromosomal abnormalities. Our findings support a neoplastic origin of this lesion.

Polyarticular pigmented villonodular synovitis in children: evidence for a genetic contribution.

Pigmented villonodular synovitis rarely occurs in multiple joints, and only once has been noted in siblings. We identified this disorder in 2 generations and found evidence for its presence in a third. It occurred in 2 sibling children at multiple sites. All affected individuals also had a rare autosomal dominant disorder, the multiple lentigines syndrome. Among the developmental abnormalities in this kindred were fibrous dysplasia, hypertelorism, pectus excavatum, high arched palate, and primary lymphedema. The etiology of pigmented villonodular synovitis is uncertain, and suggested causes in

Pigmented villonodular synovitis of synovial joints: clinical, pathologic, and radiologic features.

The clinical, pathologic, and radiologic features of pigmented villonodular synovitis of large synovial joints are presented. The typical plain-film presentation of this unusual entity is a noncalcified capsular soft-tissue mass of the knee, without bony abnormalities. When pigmented villonodular synovitis occurs in other joints, it is often accompanied by cystic bone erosions. This important radiographic finding might mislead one to an improper diagnosis of neoplasia or infection. This review reveals insufficient emphasis in the radiologic literature on the high incidence of bone lesions in j

PIGMENTED VILLONODULAR SYNOVITIS IN THE ANKLE OF A PEDIATRIC PATIENT: A CASE REPORT.

BACKGROUND: Pigmented villonodular synovitis (PVNS), also known as diffuse tenosynovial giant cell tumor, is a rare, benign but aggressive synovial proliferative disorder. It most commonly affects the knee; involvement of the foot and ankle is uncommon (incidence ~1.8 per million), and pediatric cases are especially rare. This rarity often leads to delayed diagnosis, as early symptoms can mimic benign causes (e.g. sprains or "growing pains") and initial radiographs may be unremarkable. CASE PRESENTATION: We report a case of a 12-year-old girl with a four-year history of chronic ankle pain and

Pigmented Villonodular Synovitis of the Shoulder: A Case Report and Literature Review.

BACKGROUND Pigmented villonodular synovitis is a rare clinical entity, with 2-3% of all PVNS cases affecting the shoulder. Diagnosis is challenging and can elude clinicians for years, with definitive treatment involving arthroscopic or open synovectomy. CASE REPORT A 50-year-old woman presented with left shoulder pain persisting for 2 years. She was initially conservatively treated by a rheumatologist, with corticosteroid schemes intra-articularly injected and per os, but no improvement of her symptoms was noted. Two years later, she was referred to the Orthopedics Department of our hospital w
