Cervical spinal canal narrowing in idiopathic syringomyelia.

INTRODUCTION: The cervical spine in Chiari I patient with syringomyelia has significantly different anteroposterior diameters than it does in Chiari I patients without syringomyelia. We tested the hypothesis that patients with idiopathic syringomyelia (IS) also have abnormal cervical spinal canal diameters. The finding in both groups may relate to the pathogenesis of syringomyelia. METHODS: Local institutional review boards approved this retrospective study. Patients with IS were compared to age-matched controls with normal sagittal spine MR. All subjects had T1-weighted spin-echo (500/20) an

Syringomyelia associated with inappropriate antidiuretic hormone secretion.

Hyponatraemia is the most common fluid-electrolyte disorder, and the most frequent related aetiologies are syndrome of inappropriate secretion of antidiuretic hormone (SIADH), which accounts for up to 38%. SIADH has been linked to multiple pathologies that affect the central nervous system; these disorders generally originate in the brain and, more rarely, in the spinal cord. It is often observed in patients undergoing neurosurgery and in patients with head injuries or intracranial tumours, and less common in those with spinal pathologies, especially traumatic. We describe an SIADH case associ

A sacral arachnoid cyst causing holocord syringomyelia.

Spinal extradural arachnoid cysts (ACs) have an infrequent predilection for the sacrum. As with their counterparts in other regions of the spine, cysts in this location are mostly asymptomatic. Common presentations in symptomatic cases include pain in the low back or perineum, radiculopathy, and sphincteric dysfunction. The authors report a hitherto undescribed presentation in which the predominant symptoms are those related to an associated holocord syrinx. This 15-year-old boy presented with fluctuating, spastic paraparesis and a dissociated sensory loss in the trunk. Admission MR imaging of

Spinal dural arteriovenous fistula associated with syringomyelia.

The previously undescribed association of a spinal dural arteriovenous fistula with syringomyelia was found in a 60-year-old male, who developed increasing paresis, numbness of both lower extremities and sphincteric dysfunction. Symptoms and signs were attributed to a syringomyelia at T5-L1 and an arteriovenous spinal dural fistula at L1. The fistula was successfully immobilised with N-butyl-cyano-acrylate. Six months after the procedure, all abnormalities had nearly disappeared. Whether the relation between the fistula and the syringomyelia was coincidental or causative could not be determine

[Syringomyelia and normal pressure hydrocephalus (author's transl)].

A case of syringomyelia with coexisting hydrocephalus is reported and a pathogenic relationship between both conditions is discussed. Gardner proposed that anomalies at the exit of the fourth ventricle produce a communicating syringomyelia. Hakim and Adams maintain that normal pressure hydrocephalus can aggravate this condition by a hydraulic pressure effect which can be stopped by the insertion of a ventriculoatrial shunt with improvement in the clinical picture. This has been our experience in the case reported.

Two cases of syringomyelia after ventriculosubgaleal shunt.

Ventriculosubgaleal shunting is an effective treatment for infectious hydrocephalus. We report two pediatric cases of infectious hydrocephalus treated with ventriculosubgaleal shunting, in whom syringomyelia developed during follow-up. Syringomyelia in this setting should not be ascribed directly to the shunt itself, but may be related to persistent infection, inflammation, and disturbed cerebrospinal fluid dynamics.

Cervical syringomyelia associated with cervical disc disease.

BACKGROUND AND PURPOSE: Syringomyelia is a neurological condition in which a longitudinal fluid-filled cavity is formed within the spinal cord. It usually occurs in the cervical region and is associated with Chiari malformation, infections, trauma, and tumors of the spinal cord. However, syringomyelia associated with cervical disc disease (SCD) is very rare and only a few cases have been reported so far. This case report presents the clinical and radiological findings of 13 cases of SCD to describe the properties of SCD and explore the nature of the relationship between syringomyelia and cervi

Clinical-Radiologic Dissociation in Post-traumatic Syringomyelia.

Post-traumatic syringomyelia develops years to decades after spinal trauma in up to 28% of patients. We report a case of a man who developed extensive syringomyelia 14 years after spinal trauma, but had only minimal symptoms. Presumed pathophysiology of this condition and options for surgical management are briefly reviewed.

Cervical central canal occlusion induces noncommunicating syringomyelia.

BACKGROUND: Mechanisms underlying the development of noncommunicating syringomyelia are poorly understood. OBJECTIVE: To assess the influence of focal arachnoiditis and central canal (CC) occlusion (CCO) on the formation of noncommunicating syringomyelia in the adult rat cervical spinal cord. Expression of pericanalicular aquaporin-4 is also examined. METHODS: Sprague-Dawley rats were subjected to circumferential or dorsal arachnoiditis (n = 34). Rats undergoing CCO (n = 69) were divided into 4 groups: group A, kaolin injection at a single site in the dorsal columns near the CC; group B, kao

Acute brainstem symptoms associated with cervical syringomyelia.

OBJECTIVES: Knowledge about the nature and impact of symptoms faced by patients with systemic sclerosis (SSc) is needed to identify targets for research and treatment. The aim of this study was to assess and compare the frequency and impact on everyday activities of SSc symptoms among patients from five European countries. METHODS: European patients with SSc were invited through announcements by patient associations to complete an online survey. The survey included items assessing the frequency of 40 SSc symptoms and the impact on daily activities, if present. Chi-square tests were utilised t

Traumatic cervical syringomyelia related to birth injury.

A rare case of cervical syringomyelia related to breech delivery is reported. The initial diagnosis was bilateral brachial plexus palsy due to birth injury, which was revealed by magnetic resonance imaging (MRI) to be traumatic syringomyelia. The usefulness of MRI in the early diagnosis of cervical cord birth injury, especially in differentiating between brachial plexus palsy due to birth injury and spinal cord trauma due to birth injury in infancy, is emphasized.

Arnold-Chiari type 1 malformation and syringomyelia.

A 12-year-old boy had a long history of headache since the age of 4 years. Numbness and weakness of the upper extremities and chest tightness, especially in the morning, were noted since 2 years ago. The headache became more severe and frequent and attacked even in sleep since one year ago. Arnold-Chiari type 1 malformation, cervical syringomyelia and mild hydrocephalus were demonstrated by MRI scan of the central nervous system. Headache and other symptoms disappeared immediately after surgical intervention. The formation of syringomyelia may be multifactorial, but Arnold-Chiari type 1 malfor

""Non-communicating'' syringomyelia: a non-existent entity.

Encroachment by man's overlarge forebrain is responsible for the anatomic substrate of syringomyelia, i.e. a hindbrain hernia developing in fetal life with persisting hydromyelia. Communication between the syrinx and 4th ventricle is readily disclosed at operation, but because of postmortem shrinkage, almost never at autopsy. Syringomyelia developing in 16 of 864 post-traumatic paraplegics has been classified non-communicating. However, the syrinx, its fluid, and the ultimate clinical picture are the same as in the non-traumatic, and surgical exposure has disclosed the communication. The sympt

Decompression with or without Duraplasty for Chiari I and Syringomyelia.

BACKGROUND: In children with Chiari type I malformation and syringomyelia, neurosurgical posterior fossa decompression (PFD) provides clinical improvement, but whether duraplasty (incising the dura and placing a dural graft) improves outcomes is unclear. METHODS: We conducted a multicenter, cluster-randomized, controlled trial of PFD with duraplasty (PFD-D) as compared with PFD alone. Persons 21 years of age or younger with cerebellar tonsillar ectopia of at least 5 mm and a maximum syrinx diameter of 3.0 to 9.9 mm were enrolled at 38 centers. Centers were cluster-randomized: all the particip

Spinal intramedullary cysticercosis with syringomyelia: a case report.

Intramedullary cysticercosis of the cervical spine is extraordinarily rare; prior reports are limited to single cases. We review cases of intramedullary cysticercosis, and summarize the features and outcome. Herein, we report a 38 year old woman with progressive quadriplegia, paresthesia in bilateral upper limbs, neck pain and headache for 1 month. She had dyspnea for 1 week. A gross total resection was performed, and after the surgery, the patient was given prednisolone per day orally, for 2 weeks. Oral albendazole 400 mg/day was started 2 days after the start of prednisolone. Ag-ELISA was pe

[Treatment of syringomyelia associated with Chiari 1 malformation].

Syringomyelia (SM) develops due to the disturbance of cerebrospinal fluid dynamics, spinal fixation or a spinal tumor. The disturbance of cerebrospinal fluid dynamics in the area of the foramen magnum leads to the progression of SM in Chiari 1 malformation (CM1). CM1 is the most prevalent pathology of craniovertebral junction associated with SM. The prevalence of CM1 varies from 3 to 8 per 100 000 population but SM is diagnosed in 65% of patients with CM1. Clinical symptoms of CM1 and SM include pain in the occipital area, gait disturbances due to sensitive ataxia, dissociated sensory disorder

Syringomyelia associated with cervical spondylosis: A rare condition.

Spinal spondylosis is an extremely common condition that has only rarely been described as a cause of syringomyelia. We describe a case of syringomyelia associated with cervical spondylosis admitted at our division and treated by our institute. It is the case of a 66-year-old woman. At our observation she was affected by moderate-severe spastic tetraparesis. T2-weighted magnetic resonance imaging (MRI) showed an hyperintense signal within spinal cord from C3 to T1 with a more sharply defined process in the inferior cervical spinal cord. At the same level bulging discs, facets and ligamenta fla
