Differentiation of atypical atrioventricular node re-entrant tachycardia from orthodromic reciprocating tachycardia using a septal accessory pathway by the response to ventricular pacing.

OBJECTIVES: The purpose of this study was to determine whether the response to ventricular pacing during tachycardia is useful for differentiating atypical atrioventricular node re-entrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT) using a septal accessory pathway. BACKGROUND: Although it is usually possible to differentiate atypical AVNRT from ORT using a septal accessory pathway, a definitive diagnosis is occasionally elusive. METHODS: In 30 patients with atypical AVNRT and 44 patients with ORT using a septal accessory pathway, the right ventricle was paced at a c

[Atrioventricular reciprocating tachycardia with QRS type left branch block in patients undergoing radiofrequency catheter ablation: analysis of the substrate and mechanism of tachycardia].

BACKGROUND: Among patients (pts) with atrioventricular accessory pathway (AP), some cases show wide complex arrhythmias with different QRS morphology. In a subset of these pts, an atrioventricular reentrant tachycardia with left bundle branch block morphology (LBBBM-AVRT) is observed. The aim of this study is: 1) to identify the substrate and the reentrant mechanism underlying the LBBBM-AVRT in pts undergoing radiofrequency catheter ablation (RFCA) of AP; 2) to report the results achieved by RFCA of the identified substrate. METHODS: From May 1991 to April 1993, among the 168 pts who underwen

A case of a permanent form of junctional reciprocating tachycardia with negative linking on atrial differential pacing.

This case of PJRT shows negative linking on ventriculoatrial intervals after atrial differential pacing. Interpreting the results of atrial differential pacing may be challenging in cases where the pacing site affects retrograde conduction or when retrograde conduction is unstable.

Decremental accessory pathway conduction after ablation and antidromic atrioventricular reciprocating tachycardia 8 years after successful radiofrequency ablation.

INTRODUCTION: This is a rare case of antidromic reciprocating tachycardia developing 8 years after successful catheter ablation. RESULT: A 15-year-old girl had recurrence of palpitations 8 years after the ablation of manifest right posteroseptal accessory pathway. Atrial burst pacing revealed Wenckebach atrioventricular conduction with preexcitation. Wide QRS tachycardia with identical morphology to sinus rhythm associated with retrograde His potential recorded immediately after the V-wave was induced by isoproterenol infusion. Atrial premature stimulus applied at the identical timing of His

Functional bundle branch block and orthodromic reciprocating tachycardia cycle length: do not bet on accessory pathway location.

AIMS: To show 2 examples in which the analysis of the effect of bundle branch block on orthodromic reciprocating tachycardia cycle length was misleading. METHODS AND RESULTS: We performed an electrophysiological study in two patients with orthodromic reciprocating tachycardia showing a transition from wide to narrow QRS during tachycardia. Our two cases of left bundle branch block during reciprocating tachycardia using infero-septal pathways show that ventricular to atrial conduction time prolongation may be larger than 30 ms and may be concealed by a shortening of atrial to ventricular condu

Antidromic reciprocating tachycardia in patients with paraseptal accessory pathways: importance of critical delay in the reentry circuit.

Previous studies in patients with antidromic reciprocating tachycardia (ART) have observed a critical anatomic requirement (> 4 cm) between an antegrade bypass tract limb and a retrograde AV nodal limb. We report two patients with ART utilizing a paraseptal accessory pathway. In both cases, a critical degree of slow conduction within the circuit provides unusual electrophysiologic substrate to overcome the expected anatomical constraints.

[Unusual mechanism of reciprocating tachycardia in a case of ventricular preexcitation resistant to preventive anti-arrhythmia therapy].

A patient with a previously asymptomatic W.P.W. syndrome had, at the age of 60, frequent runs of paroxysmal tachycardia, which occurred particularly after meals. The paroxysms were easily controlled by pharmacological therapy in the acute stage, but could not be prevented by prophylactic therapy because of a large oesophageal diverticulum which, through right vagal reflexes, caused sinus arrests with junctional escapes that triggered the re-entry mechanism. Resection of the diverticulum abolished the tachycardia spells.

Permanent junctional reciprocating tachycardia due to an atypical retrograde slow conducting decremental left posterolateral accessory pathway and tachycardia-induced cardiomyopathy in a pregnant adolescent.

Permanent junctional reciprocating tachycardia (PJRT) is a rare form of supraventricular tachycardia (SVT) due to a retrograde slow conducting decremental accessory pathway (AP) which is often incessant and can lead to tachycardia-induced cardiomyopathy (TIC). We report the challenging clinical course of a pregnant adolescent with PJRT due to an atypical retrograde slow conducting decremental left posterolateral AP. Pregnancy is known to exacerbate supraventricular arrhythmias and can lead to adverse maternal and fetal outcomes. To the best of our knowledge, there are no prior reports on the c

[Transesophageal atrial stimulation in the oral treatment of supraventricular reciprocal paroxysmal tachycardia in infants].

INTRODUCTION: The aim of the study was the evaluation of the usefulness of transesophageal atrial pacing in predicting chronic oral treatment efficacy of symptomatic reciprocating supraventricular tachycardia in infants and in avoiding the risk of very dangerous recurrences at home. METHODS: We studied 13 infants (11 males, 2 females, mean age 43 +/- 31 days) with symptomatic reciprocating supraventricular tachycardia and no structural heart disease. All patients had chronic oral therapy, using the drug effective in acute i.v. somministration. Each patient was discharged when supraventricular

[Arrhythmogenic effect of anti-arrhythmia agents in patients with paroxysmal supraventricular reciprocal tachycardia].

A retrospective study of the results of consecutive testing of antiarrhythmic agents was carried out in 82 patients with paroxysmal reciprocal supraventricular tachycardia. Arrhythmogenic effects of anti-arrhythmic drugs were detected in 22 (6.1%) of 359 pharmacologic tests. The electrophysiologic basis of arrhythmogenic effect is delayed impulse conduction at some point of the blood flow pathway, conducive to the emergence or maintenance of the re-entry phenomenon.

[Junctional and atrioventricular paroxysmal reciprocating tachycardias: clinical and electrocardiographic characteristics (author's transl)].

Some clinical and electrocardiographic characteristics are reviewed in two groups of patients with paroxysmal junctional reciprocating tachycardia (JRT) (21 cases) and atrioventricular reciprocating tachycardia involving an atrioventricular bypass tract with unidirectional anterograde block (AVRT) (26 cases). Mechanisms of tachycardia were established in all cases by intracavitary electrophysiologic study. Mean age (+/- S.D.) was 46 +/- 18 yrs (range: 14-75) in the JRT group and 32 +/- 16 yrs (6-67) in the AVRT group (p < 0.02). Male/female ratio was 11/10 in the former and 15/11 in the latter

[Mechanism of action and the effectiveness of ethacizin in patients with paroxysmal atrioventricular nodal reciprocal tachycardia].

An electrophysiologic study of ethacizin's mechanisms of action was carried out in patients with paroxysmal atrioventricular nodal tachycardia. Tachycardia was controlled by 0.5 mg/kg ethacizin in all patients. No patients demonstrated induced tachycardia in the presence of the drug, and 55% developed a complete retrograde atrioventricular block. The assessment of the preventive effect of oral ethacizin administration showed that paroxysms of tachycardia could not be provoked by esophageal electrostimulation of the heart in 87% of the patients. In the same patients, stable antiarrhythmic effec

[Concept of the polyfascicular structure of the atrioventricular junction in patients with reciprocal paroxysmal nodal tachycardias].

Intracardiac electrophysiologic studies were carried out in 41 patients with paroxysmal supraventricular tachycardias. Reciprocal atrioventricular tachycardia due to dissociated conduction through the atrioventricular junction was diagnosed in 29 patients. Electrophysiologic data suggested trifascicular atrioventricular conduction in 4 patients, and quadrifascicular conduction in 2. A concept of polyfascicular impulse conduction through the atrioventricular junction in some cases of reciprocal nodal tachycardia is proposed. It is demonstrated that combinations of congenital cardiac conduction

[Paroxysmal ventricular tachycardia with retrograde Wenckebach block and reciprocal beats (author's transl)].

A case of recurrent paroxysmal ventricular tachycardia with retrograde Wenckebach block and reciprocal beats was studied with His bundle electrogram and right ventricular stimulation. Reentry was reproduced whenever retrograde ventriculo-atrial conduction time reached a critical value. Concealed reentry was also demonstrated and shown to be responsible for the dropped beats of the Wenckebach sequence.

Atrial depolarization electrocardiographic features during orthodromic atrioventricular reciprocating tachycardia mediated by right-sided accessory pathway in the dog.

INTRODUCTION/OBJECTIVES: Electrocardiographic features of atrial depolarization (AD) during orthodromic atrioventricular reciprocating tachycardia (OAVRT) have been widely studied to localize accessory pathways (APs) in humans. The aim of this study was to determine whether AD characteristics might also be used to determine AP anatomical sites in dogs. ANIMALS, MATERIALS AND METHODS: Clinical records of 83 dogs that underwent electrophysiologic studies for AP ablation were retrospectively analyzed. Accessory pathways were classified into three groups according to localization: right anterior

[Association of AV reciprocating tachycardia, using a concealed bundle of Kent, and paroxysmal atrial fibrillation in neurocardiogenic syncope].

A case of neurocardiogenic syncope is reported in which both AV-reciprocating tachycardia due to a concealed retrogradely conducting posteroseptal bypass tract and paroxysmal atrial fibrillation were observed. In connection with this case, attention is paid to the difficulties of differential diagnosis and to the pathophysiological correlations concerning the occurrence of neurocardiogenic/vasovagal reflex syncope and cardiac arrhythmias. A prolonged period of freedom from syncope and tachycardia was achieved by means of drug treatment, through the combined administration of disopyramide, biso
