Healthcare provider experiences integrating mental illness screening and management into hypertension care: qualitative systematic review.

Mental illness is a frequent comorbidity in chronic diseases including hypertension, yet integration of mental health screening into hypertension care remains limited, particularly in low- and middle-income countries. This systematic review synthesizes qualitative evidence on healthcare providers' experiences of integrating mental illness screening and management into hypertension care. Qualitative studies published between 2010 and 2025 were identified through searches of PubMed, Scopus, PsycINFO, Web of Science, and Africa Wide Information. Out of 114 studies, 7 studies met the inclusion cri

Scaling community-based care for conductive hearing loss in low- and middle-income settings.

PURPOSE OF REVIEW: This review examines recent advances in community-based care for conductive hearing loss in low- and middle-income countries (LMICs), focusing on developments that have materially shifted the feasibility of decentralized service delivery across prevencommution, screening and diagnosis, medical management, and rehabilitation. RECENT FINDINGS: World Health Organization guidance now explicitly endorses task-sharing for primary ear and hearing care, including community health worker-facilitated hearing aid provision. Long-term follow-up from pneumococcal vaccination trials demo

The Impact of Manipulating Attentional Shifting Demands on Preschool Children With Specific Language Impairment.

Purpose This study investigated attentional shifting in preschool children with specific language impairment (SLI) compared to their typically developing peers. Children's attentional shifting capacity was assessed by varying attentional demands. Method Twenty-five preschool children with SLI and 25 age-matched, typically developing controls participated. A behavioral task measuring attentional shifting within and across multiple dimensions (auditory, linguistic, and visual) was employed. Demands on attentional shifting were increased based on input dimension (low load: staying within dimensio

Set-shifting among adolescents with anorexia nervosa.

OBJECTIVE: Set-shifting difficulties are documented for adults with anorexia nervosa (AN). However, AN typically onsets in adolescents and it is unclear if set-shifting difficulties are a result of chronic AN or present earlier in its course. This study examined whether adolescents with short duration AN demonstrated set-shifting difficulties compared to healthy controls (HC). METHOD: Data on set-shifting collected from the Delis-Kaplan executive functioning system and Wisconsin card sort task (WCST) as well as eating psychopathology were collected from 32 adolescent inpatients with AN and co

Enhanced saccadic control in young people with Tourette syndrome despite slowed pro-saccades.

Tourette syndrome (TS) is a neurodevelopmental disorder characterized by motor and vocal tics. Tics are repetitive and uncontrolled behaviours that have been associated with basal ganglia dysfunction. We investigated saccadic eye movements in a group of young people with TS but without co-morbid ADHD. Participants performed two tasks. One required them to perform only pro-saccade responses (pure pro-saccade task). The other involved shifting, unpredictably, between executing pro- and anti-saccades (mixed saccade task). We show that in the mixing saccade task, the TS group makes significantly f

Structural brain basis of latent factors of executive functions in childhood.

Executive functions can be classified into processes of inhibition, working memory and shifting, which together support flexible and goal-directed behaviour and are crucial for both current and later-life outcomes. A large body of literature has identified distinct brain regions critical to performing each of these functions. These findings are however predicated on a piecemeal and single-task approach. It is therefore unclear to what extent these associations reflect task-specific features or actual constructs of executive functions. Here, in a sample of 141 children aged 6-13 years, we admin

Neural basis of impaired cognitive flexibility in patients with anorexia nervosa.

BACKGROUND: Impaired cognitive flexibility in anorexia nervosa (AN) causes clinical problems and makes the disease hard to treat, but its neural basis has yet to be fully elucidated. The purpose of this study was to evaluate the brain activity of individuals with AN while performing a task requiring cognitive flexibility on the Wisconsin Card Sorting Test (WCST), which is one of the most frequently used neurocognitive measures of cognitive flexibility and problem-solving ability. METHODS: Participants were 15 female AN patients and 15 age- and intelligence quotient-matched healthy control wom

Executive function impairments in fibromyalgia syndrome: Relevance of clinical variables and body mass index.

BACKGROUND: Several investigations suggest the presence of deterioration of executive function in fibromyalgia syndrome (FMS). The study quantified executive functions in patients with FMS. A wide array of functions was assessed, including updating, shifting and inhibition, as well as decision making and mental planning. Moreover, clinical variables were investigated as possible mediators of executive dysfunction, including pain severity, psychiatric comorbidity, medication and body mass index (BMI). METHODS: Fifty-two FMS patients and 32 healthy controls completed a battery of 14 neuropsycho

Generalized anxiety disorder patients' cognitive control in affective contexts.

BACKGROUND: Growing evidence suggests a relationship between deficits in cognitive control and anxiety. However, studies examining cognitive control within affective contexts (affective control) are limited, and the specific characteristics of affective control in patients with Generalized Anxiety Disorder (GAD) remain unclear. This study investigated whether differences exist in cognitive control under affective contexts. METHODS: We conduct our research in a population of GAD patients (n = 50) and a healthy control group (n = 50). The affective flanker task measured affective inhibition, wh

Effects of alcohol on tests of executive functioning in men and women: a dose response examination.

Alcohol has been shown to affect performance on tasks associated with executive functioning. However, studies in this area have generally been limited to a single dose or gender or have used small sample sizes. The purpose of this study was to provide a more nuanced and systematic examination of alcohol's effects on commonly used tests of executive functioning at multiple dosages in both men and women. Research volunteers (91 women and 94 men) were randomly assigned to one of four drink conditions (alcohol doses associated with target blood alcohol concentrations of .000%, .050%, .075%, and .1

The role of learned irrelevance in attentional set-shifting impairments in Parkinson's disease.

In this study, the cognitive and neurochemical factors underlying learned irrelevance, one of the mechanisms thought to be responsible for attentional set-shifting deficits in Parkinson's disease (PD), were investigated. In a visual discrimination learning task, the extent to which a target dimension was irrelevant prior to an extra-dimensional shift was varied. Twenty patients with PD and 22 healthy participants performed the task twice, with patients tested on and off L-dopa. The patients made more errors than control participants in the condition in which the target dimension was completely

A scoping review of counselling and psychological intervention models in low income nations.

There has been a persistent lack of synthesized knowledge on mental health treatment models that attune to the realities of low-income countries at large. To address this significant mental health knowledge gap, this scoping review tries to identify and synthesize the literature on psychological intervention models developed for or adapted to these contexts. After conducting a thorough search of academic databases, we went through a two-stage screening process to identify relevant articles. From those eligible studies, we collected and organized data on various theoretical and therapeutic coun

Dissociating executive prefrontal substrates following capsulotomy in obsessive-compulsive disorder.

Obsessive-compulsive disorder (OCD) is characterized by widespread executive function impairments linked to disrupted fronto-striatal circuits. Ablative therapies such as capsulotomy show promise efficacy in treatment-refractory OCD, yet their effects on executive neural substrates remain poorly understood. This study explores how OCD and capsulotomy influence executive prefrontal function. Twenty-three post-capsulotomy OCD patients, thirty OCD controls, and thirty-two health controls (HC) were recruited in the study. Post-capsulotomy patients were recruited at least 6 months following surgery

What Skills Could Distinguish Developmental Dyscalculia and Typically Developing Children: Evidence From a 2-Year Longitudinal Screening.

Developmental dyscalculia (DD) is a mathematics learning disorder that affects approximately 5% to 7% of the population. This study aimed to detect the underlying domain-specific and domain-general differences between DD and typically developing (TD) children. We recruited 9-year-old primary school children to form the DD group via a 2-year longitudinal screening process. In total, 75 DD children were screened from 1,657 children after the one-time screening, and 13 DD children were screened from 1,317 children through a consecutive 2-year longitudinal screening. In total, 13 experimental task

Balance Markers and Saccadic Eye-Movement Measures in Adolescents With Postconcussion Syndrome.

CONTEXT: Deficits in both balance and oculomotor function, including impairments in saccadic eye movements, are observed in approximately 30% of patients postconcussion. Whereas balance and saccadic eye movements are routinely assessed separately, growing evidence suggests that they should be assessed concurrently. OBJECTIVE: To compare balance measures and saccades between adolescents 1 to 3 months postconcussion and healthy uninjured adolescents. DESIGN: Case-control study. SETTING: Concussion clinic and 2 private schools. PATIENTS OR OTHER PARTICIPANTS: Twenty-five adolescents (10 boys,

Exploring the neurocognitive signature of poor set-shifting in anorexia and bulimia nervosa.

Poor set-shifting has been implicated as a risk marker, maintenance factor and candidate endophenotype of eating disorders (ED). This study aimed to add clarity to the cognitive profile of set-shifting by examining the trait across ED subtypes, assessing whether it is a state or trait marker, and whether it runs in families. A battery of neuropsychological tasks was administered to 270 women with current anorexia (AN) and bulimia nervosa (BN), women recovered from AN, unaffected sisters of AN and BN probands, and healthy control women. Set-shifting was examined using both individual task score

SPECIFIC EFFECTS OF ANGER RUMINATION ON PARTICULAR EXECUTIVE FUNCTIONS (.).

The effects of two types of rumination on different kinds of executive functions were investigated. Fifty-nine participants (M age = 22.8 yr., SD = 2.5) were assigned to one of three conditions and instructed either to: (1) ruminate in a self-distanced way, (2) ruminate in a self-immersed way, or (3) think about the layout of their campus following anger induction. Afterward, the participants were directed to finish tasks designed to assess three kinds of executive functions: shifting, inhibition, and updating. Results showed that self-immersed rumination impaired shifting ability the most, wh

The effect of dopamine depletion from the caudate nucleus of the common marmoset (Callithrix jacchus) on tests of prefrontal cognitive function.

This study examined the effects of depletion of dopamine from the caudate nucleus of the common marmoset (Callithrix jacchus), on tasks sensitive to prefrontal damage (attentional set-shifting and spatial delayed response). There was a marked impairment in performance on the spatial delayed response task, but performance on the attentional set-shifting task was relatively preserved except for an impairment in re-engagement of a previously relevant perceptual dimension. This pattern of impairment is distinct from that seen after excitotoxic lesions of the prefrontal cortex and in patients with

Electrophysiological mechanism of executive function deficits in obsessive-compulsive disorder: An EEG microstates study.

BACKGROUND: Executive dysfunction is a common feature in patients with obsessive-compulsive disorder (OCD), yet its underlying electrophysiological mechanisms remain incompletely understood. This study aimed to investigate the characteristics of Electroencephalography (EEG) microstates in patients with OCD while they performed executive-function tasks. METHODS: Eighty-five patients with OCD and 86 healthy controls (HC) completed the Task-Switching Paradigm, Stop-Signal Task, and N-back task, which assess shifting, inhibition, and working memory updating functions, respectively. Frontal midline theta reveals temporal dynamics of target amplification and distracter inhibition during mental set-shifting.

When humans shift between tasks, they initially show slower responses in the new task than in the previous one. Persisting attentional settings are increasingly recognized as a source for these shifting costs. However, the extent to which specific mechanisms underlying information selection and interference control contribute to this phenomenon remains less clear. Here, we use time-frequency analyses of human electroencephalogram (EEG) data to explore the aftereffects of two such mechanisms: target amplification and distracter inhibition. Participants completed a set-shifting task in which int
