Injuries of the upper cervical spine-how can instability be identified?

OBJECTIVE: The diagnosis of instability of the injured upper cervical spine remains controversial, due to its complicated anatomical configuration and biomechanical property. Since identifying unstable injuries of the upper cervical spine is essential for immediate stabilizing therapy, this article reviews the current classification systems of upper cervical spine injuries and their statements towards instability. METHODS: A systematic review of literature concerning upper cervical spine injuries was performed on the PubMed database from inception to December 2019. An English literature searc

Finite Element Modeling of Organ of Cortis Throughout the Cochlea During Blast Wave Transmission.

INTRODUCTION: Exposure to blast overpressures in the line of duty causes many service members and veterans to suffer inner ear injuries and the resulting hearing disabilities. The response to blast waves within the cochlea, particularly by the sensory hair cells within the organ of Corti (OC), is challenging to measure experimentally. Recently, 3D finite element (FE) models of the human ear at macroscale and OC models at microscale have been established to examine the blast transmission throughout the ear and the OC response at the middle turn of the basilar membrane (BM). This multiscale mode

The occipitoatlantal capsular ligaments are the primary stabilizers of the occipitoatlantal joint in the craniocervical junction: a finite element analysis.

OBJECTIVE: There is contradictory evidence regarding the relative contribution of the key stabilizing ligaments of the occipitoatlantal (OA) joint. Cadaveric studies are limited by the nature and the number of injury scenarios that can be tested to identify OA stabilizing ligaments. Finite element (FE) analysis can overcome these limitations and provide valuable data in this area. The authors completed an FE analysis of 5 subject-specific craniocervical junction (CCJ) models to investigate the biomechanics of the OA joint and identify the ligamentous structures essential for stability. METHOD

Temporal bone histopathology in connexin 26-related hearing loss.

OBJECTIVE: Mutations in GJB2, a gene that encodes a gap junction protein, Connexin 26 (Cx26), are responsible for approximately one third of sporadic severe-to-profound or profound congenital deafness and half of severe-to-profound or profound autosomal recessive nonsyndromic hearing loss (ARNSHL). Mouse mutants homozygous for knockouts of this gene are nonviable, precluding histopathologic studies of the associated inner ear pathology in this animal model. Therefore, we studied archival temporal bone sections to identify temporal bone donors with Cx26-related deafness. STUDY DESIGN: Temporal

Hair cell regeneration in the chick basilar papilla after exposure to wide-band noise: evidence for ganglion cell involvement.

It has been demonstrated that the auditory epithelium in the chick basilar papilla may regenerate after acoustic or ototoxic damage. Both types of damage may elicit the appearance of new cells that may develop in to the sensory cells. Factors inducing this process and the role of ganglion cells, the first neuron cells in the auditory pathway, are still unknown. The pattern of auditory damage and regeneration, after octave-band and pure-tone noise exposure, has been well established in research studies on chicks, but there are scarce data on wide-band noise effects. The aim of this study was to

A computational model of the auditory periphery for speech and hearing research. II. Descending paths.

The auditory model described in Giguère and Woodland [J. Acoust. Soc. Am. 94, 331-342 (1993)] is applied to the simulation of the descending paths to the peripheral ear. An external feedback unit regulates the average firing rate of inner hair cell afferent fibers via a simplified modeling of the dynamics of the acoustic reflex to the middle ear and of the slow efferent innervation of the outer hair cells. The terminal effector of the acoustic reflex system is the stapedial muscle which stiffens the middle ear and reduces vibration of the stapes up to 15 dB below 1000 Hz. The control function

[Retraction of the endolymphatic membranes in temporal bones of the Wittmaack and Tufts collections].

BACKGROUND: In histologic studies, the volumetric status of the intralabyrinthine fluids is judged by the position of the endolymphatic membranes. Bulging of the membranes, commonly known as endolymphatic hydrops, is assumed to be caused by excess of endolymph. The opposite situation, retraction of the membranes is, however, only incidentally described and relatively little attention has been paid to its significance. Almost one hundred years ago Wittmaack described retraction of the endolymphatic membranes, which has since been considered to be preparation artifact--a concept that essentially

Rheumatoid arthritis in a patient with compound heterozygous variants in the COL11A2 gene and progressive hearing loss: A case report.

RATIONALE: Collagen type XI alpha 2 chain is a component of type XI collagen and is expressed in various tissues including articular cartilage and tectorial membrane of the cochlea. Variants in the COL11A2 gene, which encodes collagen type XI alpha 2 chain, has been reported to cause hearing loss and has been associated with osteoarthritis and ossification of the posterior longitudinal ligament of the spine. Despite the importance of type XI collagen in the joints, association of rheumatoid arthritis (RA) with COL11A2 has not been reported. PATIENT CONCERNS: The patient is a 60-year-old femal

Hearing impairment in hypothyroid dwarf mice caused by mutations of the thyroid peroxidase gene.

Thyroid hormone (TH) is essential for proper cochlear development and function, and TH deficiencies cause variable hearing impairment in humans and mice. Thyroid peroxidase (TPO) catalyzes key reactions in TH synthesis, and TPO mutations have been found to underlie many cases of congenital hypothyroidism in human patients. In contrast, only a single mutation of the mouse TPO gene has been reported previously (Tpo(R479C)) but was not evaluated for auditory function. Here, we describe and characterize two new mouse mutations of Tpo with an emphasis on their associated auditory deficits. Mice hom

A biomechanical study of unilateral posterior atlantoaxial transarticular screw fixation.

The purpose of this study was to investigate the fixation of C1-C2 instability with the use of a unilateral screw. Transarticular screw placement across C1-C2 may be contra-indicated in up to 20% of specimens on at least one side because of anatomic variations or other pathological processes. Hence the current study looks into unilateral screw fixation of C1- C2 instability. Eight cervical spine specimens, C1 through C5, were harvested from fresh human cadavers (4 male and 4 female) of average age 67 years (54-80). C1 and C2-C5 vertebrae were potted to allow motion only at the C1-C2 articulati

Frequency selectivity in the auditory periphery: similarities between damaged and developing ears.

Single fiber tuning curves (stimulus frequency versus neural threshold curves) were obtained from 198 auditory nerve fibers in 24 kittens between birth and the 16th postnatal day and from 74 auditory nerve fibers in adult cats. Three developmental stages during which adult-like frequency-resolving capacity was acquired were identified. During the early stage of postnatal development, all auditory nerve fibers were essentially untuned and responded to a narrow range of low to middle frequency tone bursts presented at intensities exceeding 110 dB sound pressure level (SPL) re 20 muPa. Hearing loss in athyroid pax8 knockout mice and effects of thyroxine substitution.

Pax8-/- mice do not develop thyroid follicular structures and thus provide an ideal animal model to study the consequences of congenital hypothyroidism. Despite their athyroidism, Pax8-/- mice survive up to postnatal day 21 (P21). No auditory brain stem responses (ABR) to sound could be recorded in these animals at 130 dB SPL, even at P21, when hearing reaches adult sensitivity in control mice. Abnormalities in the outer and middle ear structures were found in a considerable percentage of Pax8-/- animals. Maturation of the inner ear appeared delayed by about 1 week with respect to euthyroid co

Spinal cord injury without radiographic abnormality in children, 2 decades later.

OBJECTIVE: Much new research has emerged since 1982, when the original description of spinal cord injury without radiographic abnormality (SCIWORA) as a self-contained syndrome was reported. This article reviews new and old data on SCIWORA, from the past 2 decades. METHODS: This article reviews what we have learned since 1982 about the unique biomechanical properties of the juvenile spine, the mechanisms of injuries, the profound influence of age on injury pattern and outcome, the magnetic resonance imaging (MRI) features, and management algorithms of SCIWORA. RESULTS: The increasing use of

Craniovertebral Junction Instability after Oncological Resection: A Narrative Review.

The craniovertebral junction (CVJ) is a complex transition area between the skull and cervical spine. Pathologies such as chordoma, chondrosarcoma and aneurysmal bone cysts may be encountered in this anatomical area and may predispose individuals to joint instability. An adequate clinical and radiological assessment is mandatory to predict any postoperative instability and the need for fixation. There is no common consensus on the need for, timing and setting of craniovertebral fixation techniques after a craniovertebral oncological surgery. The aim of the present review is to summarize the an
