Estrogen Hormones' Implications on the Physiopathology of Temporomandibular Dysfunction.

Background/Objectives: Temporomandibular dysfunction syndrome consists of several disorders of the masticatory system, namely those of the muscles, the joint itself, as well as the dental and periodontal system. This syndrome is often characterized by pain and an inability to perform functions within the dental-maxillary apparatus, which creates a certain degree of disability in patients. Women are more susceptible to this syndrome than men and hormonal factors, particularly estrogen, are central to its etiology and physiopathology. Methods: A comprehensive literature search was conducted usin

Loose bodies of the temporomandibular joint: arthrographic diagnosis.

We report a series of four patients presenting with symptoms of temporomandibular joint pain and dysfunction, and manifesting calcified loose bodies within the temporomandibular joint. Tomographic and arthrotomographic findings are described. Surgical confirmation was obtained in two patients and the histopathology in one demonstrated that the loose body was calcified cartilage surrounded by synovial tissue. The synovial tissue of the joint space was normal in all four cases by arthrotomography and at surgery in two patients. The findings suggest that these cases may be examples of osteochondr

[Intra-articular injections of hyaluronic acid for anterior disc displacement of temporomandibular joint].

Anterior disc displacement (ADD) of temporomandibular joint (TMJ) is regarded as one of the major findings in temporomandibular disorders (TMD). It is related to joint noise, pain, mandibular dysfunction, degenerative change and osteoarthritis. In the mean time, the pathological changes were found in synovial membrane and synovial fluid. Hyaluronic acid is a principal component of the synovial fluid which plays an important role in nutrition, lubrication, anti-inflammation and cartilage repair. The synthesis, molecule weight, and concentration of hyaluronic acid are decreased during TMD and ca

Association of temporomandibular joint osteoarthrosis with dentoskeletal morphology in males: A cone-beam computed tomography and cephalometric analysis.

OBJECTIVE: The objective of the study was to assess the differences in dentoskeletal morphology in males with and without temporomandibular joint osteoarthrosis (TMJOA). METHODS: Three hundred seventy-one male participants seeking orthodontic treatment were enrolled in this cross-sectional study. Each participant's osseous status was evaluated using cone-beam computed tomography (CBCT) and classified into normal (N = 104), indeterminate for TMJOA (N = 110) and TMJOA (N = 157) groups. The dentoskeletal characteristics were evaluated using cephalograms. Stratified analysis was performed based o

Orofacial pain: diagnosis and treatment.

Orofacial pain can have an inflammatory, neurologic or musculoskeletal cause. Inflammatory diseases include dental abscess, sinusitis, temporal arteritis, sialolithiasis and infections of the parotid gland. Common neurologic diseases that cause facial pain are trigeminal neuralgia, glossopharyngeal neuralgia, paratrigeminal neuralgia and cluster headaches. Musculoskeletal causes include temporomandibular joint syndrome and myofascial pain dysfunction syndrome. A clear understanding of pertinent anatomy and an organized approach to diagnosis will facilitate the evaluation of patients with orofa

Fibromyalgia syndrome and temporomandibular disorders with muscular pain. A review.

OBJECTIVES: Temporomandibular disorders (TMD) refer to a group of clinical picture affecting the masticatory muscles and temporomandibular joint that are characterized by muscular or joint pain, dysfunction (limited or altered functions) and joint noises, as well as other associated symptoms, such as tension headaches, otalgia, dizziness, tinnitus, and others. Fibromyalgia (FM) is a syndrome of unknown etiology involving generalized chronic pain accompanied, in a high percentage of cases, by other symptoms such as asthenia, anxiety, depression, sleep disturbances, and other less frequent sympt

Myofascial pain-dysfunction syndrome: the role of nonmasticatory muscles in 91 patients.

Ninety-one new patients with myofascial pain-dysfunction (MPD) syndrome were studied prospectively. The patients experienced aural fullness, tinnitus, vertigo, odynophagia, and headache in addition to the cardinal symptoms of otalgia, muscle tenderness, temporomandibular joint (TMJ) click, and trismus. Some nonmasticatory muscles were found to be tender as frequently as the masticatory muscles. It is proposed that MPD syndrome as seen clinically involves more than just the masticatory musculature and is a composite of several head and neck myofascial pain syndromes including tensor tympani syn

New TMJ clinical data and the implication on diagnosis and treatment.

A review of the literature and the presentation of 220 patients with acute temporomandibular disorders and a control group are presented. Condylar position in the fossae is related to clinical symptoms. Ten important clinical factors were recorded and the data compared without reference to specific condylar position in the fossae and then in relation to condylar displacement groups. The significantly high incidence of posterior condylar displacement and joint pain in the patients described in this report indicates that elimination of the term joint in our definition of this disorder may be pre

Implant of articular eminence for recurrent dislocation of the temporomandibular joint.

Hypermobility of the temporomandibular joint is often caused by trauma, by opening the mouth too wide, by having a mouth forced open during general anesthesia procedures, or by dental procedures. The capsule may be stretched to an extent that dislocation occurs more easily thereafter. An implant of Vitallium mesh attached to the zygoma to restrict anterior movement of the condyle is used to prevent recurrent dislocation of the temporomandibular joint.

Professional delays in referral of patients with mouth cancer: six case histories.

Professional delay is an important delay in referral of patients with suspected mouth cancer. Missing the possibility of cancer might not only result in worse outcomes in respect to function and survival, but also have medicolegal implications. The aim of this article was to review a consecutive cohort of patients over a two-year period with mouth cancer diagnosis and identify those with professional delay and illustrate the main types of presentations using short case histories. The multi-disciplinary team records were used to identify case notes of a two-year (2019 and 2020) consecutive coho

Prevalence of Temporomandibular Disorders in Orthognathic Surgery patients: A systematic review conducted according to PRISMA guidelines and the Cochrane Handbook for Systematic Reviews of Interventions.

BACKGROUND: Orthognathic surgery is done to treat a variety of dentofacial abnormalities, but a wide gap still remains on how it can result in temporomandibular joint dysfunction (TMD). The primary goal of this review was to assess the effects of various orthognathic surgical techniques on the emergence or exacerbation of TMJ dysfunction. METHODS: A comprehensive search was conducted across several databases using Boolean operators and MeSH keywords related to temporomandibular joint disorders (TMDs) and orthognathic surgical interventions, with no limitation on the year of publication. [Post-traumatic temporomandibular pain dysfunction syndrome and the problems raised by its imputation].

Direct, certain and total imputability of trauma as the cause of mandibular joint dysfunction is often difficult to establish. The theoretically obligatory conditions are rarely met, making the expert's opinion quite difficult to establish. The complexity of the pathophysiology involved in mandibular joint dysfunction confirms however the reality of this sequella. By determining the precise lesional or functional origin of the syndrome and the latent or patent nature of the prior status it is possible to ascertain intermediary situations.

[Myrhaug's operation. Apropos of 8 cases].

Myrhaug's operation was performed in 5 patients with recurrent dislocations, including two after failure of buttress operations, and 3 cases of temporomandibular joint pain and dysfunction syndrome. The particular conditions for choice of patient for this operation are emphasized, these excluding all cases followed up after occlusal reconstruction and requiring rapid and effective solutions apparently supplied by this surgical procedure.
