Multiple Musculotendinous Variations in the Limbs: A Cadaveric Case Report.

Muscle and tendon variations in the upper and lower extremities are common but often go unnoticed due to their asymptomatic nature, typically being incidental findings on radiological imaging. These anatomical variations, widely documented in scientific literature, can impact clinical procedures and surgeries. Particularly, the accessory muscles have been implicated in different forms of entrapment neuropathy syndromes. This case report details the muscle and tendon variations observed during the short-term dissection project. The variations included additional muscle in the flexor aspect of t

Injury Pattern Recognition and Surgical Technique of Pilon Fracture Reduction With Posterior Tibial Tendon Incarceration.

Pilon fractures occur over a broad spectrum of injury severity and soft-tissue compromise. This article will review a distinct pilon injury variant characterized by posterior tibial tendon incarceration in posteromedial pilon fracture propagation. This injury pattern is vital to recognize preoperatively because failure to mobilize the entrapped posterior tibial tendon (PTT) will result in fracture malreduction and postoperative loss of PTT excursion. In addition, the authors' preferred surgical technique for PTT mobilization and anatomic fracture reduction is described.

Chronic Posttraumatic Dislocation of Radial Head With Ulnar Nerve Entrapment in a Child: A Case Report and Literature Review.

We present an unusual case of chronic posttraumatic anteromedial dislocation of radial head with direct ulnar nerve entrapment in a child. Ulnar nerve decompression, open reduction of the radial head, and annular ligament reconstruction using a palmaris longus tendon graft were performed, and a satisfactory functional outcome was achieved at the 15-month follow-up. Through a review of literature, we conclude that early diagnosis and management for radial head dislocation are recommended to avoid nerve symptoms. Besides, open reduction and annular ligament reconstruction with a palmaris longus

Tendon and nerve sonography.

The refinement of high frequency transducers has improved the ability of ultrasound (US) to detect fine textural abnormalities of tendons and nerves and to identify a variety of pathologic conditions. Characteristic echotextural patterns, closely resembling the histologic ones, are typically depicted in these structures using high US frequencies. In tendon imaging, US can identify tendon dislocations, degenerative changes, differentiate partial from complete tears and determine whether the patient has to be treated surgically or conservatively. A spectrum of findings in inflammatory conditions

Plantar Flexion-Induced Entrapment of the Dorsalis Pedis Artery in a Teenaged Cross-Country Runner.

BACKGROUND: Symptomatic peripheral artery disease of the lower extremity rarely affects young adults and, when present, typically has a nonatherosclerotic etiology. Anatomical variants have manifested as symptomatic foot ischemia in four cases in the literature. We describe the case of a 17-year-old girl presenting with foot pain upon plantar flexion due to dynamic dorsalis pedis (DP) artery entrapment by fibrous bands and the extensor hallucis brevis (EHB) tendon. METHODS: The patient was a 17-year-old girl who presented with right foot pain upon plantar flexion, which resolved upon returnin

Accessory muscle of the flexor digitorum superficialis and its clinical implications.

Anatomical variations of the flexor digitorum superficialis (FDS) muscle and tendon unit are frequently reported by anatomists and clinicians. Anatomical muscle variations of the FDS and its tendons may include variations of muscle belly, presence of accessory or duplicate tendons, abnormal tendon connections, and absence of muscle or tendon components. Such variations may or may not have clinical implications. This report presents a case not described previously: a unilateral accessory muscle of the flexor digitorum superficialis which was connected by a thick tendon to the flexor digitorum s

Longitudinal Ultrasonographic Appearance of an Atypical Tendinous Variant of Popliteal Artery Entrapment Syndrome in an Adolescent.

A 15-year-old boy presented with left fifth-toe ulceration, claudication, and rest pain. Computed tomography angiography demonstrated proximal-to-mid popliteal artery occlusion. Duplex ultrasonography revealed thrombotic occlusion and a slender tendon crossing the artery, which appeared more conspicuous longitudinally than transversely. Magnetic resonance imaging showed apparent continuity with the main tendon of the medial head of the gastrocnemius, supporting an atypical variant of popliteal artery entrapment syndrome. Surgery was declined. At 3 months, the ulcer had healed, and symptoms imp

Incidence and Association of CT Findings of Ankle Tendon Injuries in Patients Presenting With Ankle and Hindfoot Fractures.

OBJECTIVE: Tendon injuries are a commonly encountered finding in ankle CT examinations performed for fractures. This study was designed to identify the incidence and associations of tendon injuries in ankle CT examinations performed for fractures. MATERIALS AND METHODS: A retrospective review was performed of 410 patients who underwent ankle CT during a 6-year period. RESULTS: Tendon injuries were common, seen in 25% of all ankle CT examinations. Tendon subluxation-dislocation accounted for most of the tendon injuries (77 of 196 total injuries). Pilon fractures carried 2.2 times increased ri

Open Complete Medial Talar Dislocation without Fracture of the Talus or the Malleoli: A Case Report.

CASE: A twenty-three-year-old man presented with an open complete medial talar dislocation without fracture of the talus or the malleoli; this was associated with an injury to the posterior tibial artery and the tibial nerve. It was difficult to reduce because of entrapment of the tendons of the flexor digitorum, the flexor hallucis longus, and the tibialis posterior, as well as entrapment of the posterior tibial artery and the tibial nerve. At the twelve-month follow-up, the talus did not show osteonecrosis. CONCLUSION: After a complete medial talar dislocation, the patient was found to have

Common nonarticular syndromes in the elbow, wrist, and hand.

Nonarticular elbow, wrist, and hand discomfort commonly results from periarticular or tendon disorders or nerve entrapment. Physicians with a practical knowledge of upper extremity anatomy and mechanics and an awareness of the typical features of disease are best equipped to treat these often-encountered conditions. Therapy is based on an accurate diagnosis and should be tailored to each disorder.

Peroneal tendon injury associated with calcaneal fractures: CT findings.

Injury to the peroneal tendons is one of the major long-term complications of intraarticular calcaneal fractures and heretofore has been difficult to diagnose by noninvasive radiography. Retrospective review of CT scans of 24 intraarticular calcaneal fractures, obtained shortly after injury, identified 22 cases (92%) of acute peroneal tendon abnormalities. In most of these cases, multiple findings were present. These included lateral displacement in 14 (58%) of 24, impingement by bony fragments in eight (33%), subluxation or dislocation in six (25%), soft-tissue masses around the tendons repre

Neuromuscular ultrasound in common entrapment neuropathies.

Neuromuscular ultrasound involves the use of high-resolution ultrasound to image the peripheral nervous system of patients with suspected neuromuscular diseases. It complements electrodiagnostic studies well by providing anatomic information regarding nerves, muscles, vessels, tendons, ligaments, bones, and other structures that cannot be obtained with nerve conduction studies and electromyography. Neuromuscular ultrasound has been studied extensively over the past 10 years and has been used most often in the assessment of entrapment neuropathies. This review focuses on the use of neuromuscula

Arthroscopic all-inside ATiFL's distal fascicle transfer for ATFL's superior fascicle reconstruction or biological augmentation of lateral ligament repair.

PURPOSE: Tendon grafts are often utilized for reconstruction of the lateral ligaments unamenable to primary repair. However, tendon and ligaments have different biological roles. The anterior tibiofibular ligament's (ATiFL) distal fascicle may be resected without compromising the stability of the ankle joint. The aim of this study is to describe an all-arthroscopic and intra-articular surgical technique of ATiFL's distal fascicle transfer for the treatment of chronic ankle instability. METHODS: Five unpaired cadaver ankles underwent arthroscopic ATiFL's distal fascicle transfer using a non-ab

Workplace ergonomic factors and the development of musculoskeletal disorders of the neck and upper limbs: a meta-analysis.

In the past 10 years there has been growing awareness among occupational health professionals worldwide of the large burden of illness associated with musculoskeletal disorders of the neck and upper limbs. It has been suggested that these disorders are associated with highly repetitive work and are due, at least in part, to ergonomic factors. This review examines the epidemiologic evidence of the relationship between workplace ergonomic factors such as repetition, force, static muscle loading, and extreme joint position and the development of muscle, tendon, and nerve entrapment disorders of t

The carpal tunnel.

The carpal bones are deeply convex anteriorly. This bony gutter is converted by the flexor retinaculum into a tube - the carpal tunnel, which conveys the median nerve, together with the long flexor tendons of the fingers and thumb, into the hand. It is of special interest to the surgeon because it is the site of a common nerve entrapment, the carpal tunnel syndrome.

Calcaneal fracture-dislocation with entrapment of the medial neurovascular bundle: a case report.

The calcaneus is the most commonly fractured of all the tarsal bones. Soft-tissue interposition, usually involving the peroneal tendons, has been reported to block reduction of calcaneal fractures and subtalar dislocations. To our knowledge, no case of entrapment of the neurovascular bundle has been reported. This is the subject of our report.

High resolution real time ultrasonography of the sural nerve after percutaneous repair of the Achilles tendon.

BACKGROUND: Percutaneous Achilles tendon repair has been developed to minimise soft tissue complications following treatment of tendon ruptures. However, there are concerns because of the risk of sural nerve injury. Few studies have investigated the relationship between the Achilles tendon, the sural nerve and its several anatomical course variants. METHODS: We studied 7 cadaveric limbs (7 Achilles tendons) in which a percutaneous repair of the Achilles tendon was performed. On each tendon, high resolution real time ultrasonography examination was performed by an experienced musculoskeletal r

[Revision of distal biceps tendon rupture secondary to malpositioning. Case report].

INTRODUCTION: distal biceps tendon rupture is an uncommon injury, with an incidence of two to three cases per 100,000 people annually. It predominantly affects men between the ages of 40 and 60, commonly in the dominant arm and during eccentric contractions. Surgical treatment is preferred as it allows better restoration of supination and flexion strength. Proper anatomical positioning has been shown to optimize forearm supination strength. CLINICAL CASE: a 45-year-old male developed an injury to the posterior interosseous nerve secondary to improper positioning of the distal biceps tendon du
