Statin treatment increases the clinical risk of tendinopathy through matrix metalloproteinase release - a cohort study design combined with an experimental study.

Recent experimental evidence indicates potential adverse effects of statin treatment on tendons but previous clinical studies are few and inconclusive. The aims of our study were, first, to determine whether statin use in a cohort design is associated with tendinopathy disorders, and second, to experimentally understand the pathogenesis of statin induced tendinopathy. We studied association between statin use and different tendon injuries in two population-based Swedish cohorts by time-dependent Cox regression analysis. Additionally, we tested simvastatin in a 3D cell culture model with human

Imaging of calcific tendinopathy around the shoulder: usual and unusual presentations and common pitfalls.

Rotator cuff calcific tendinopathy (RCCT) is a very common condition, characterized by calcium deposition over fibrocartilaginous metaplasia of tenocytes, mainly occurring in the supraspinatus tendon. RCCT has a typical imaging presentation: in most cases, calcific deposits appear as a dense opacity around the humeral head on conventional radiography, as hyperechoic foci with or without acoustic shadow at ultrasound and as a signal void at magnetic resonance imaging. However, radiologists have to keep in mind the possible unusual presentations of RCCT and the key imaging features to correctly

Correlations between gene expression and mineralization in the avian leg tendon.

Certain avian tendons have been studied previously as a model system for normal mineralization of vertebrates in general. In this regard, the gastrocnemius tendon in the legs of turkeys mineralizes in a well defined temporal and spatial manner such that changes in the initial and subsequent events of mineral formation can be associated with time and specific locations in the tissue. In the present investigation, these parameters and mineral deposition have been correlated with the expression of several genes and the synthesis and secretion of their related extracellular matrix proteins by the

Nano-layered tendon patch to orchestrate timely interactions between tendon stem/progenitor cells and macrophages.

Tendon regeneration remains limited by the unmet challenge of precisely controlling the intensity and duration of local inflammation, often resulting in scarred healing. Here, we present a novel, micro-thin, nano-layered patch that employs Layer-by-Layer (LbL) self-assembly for the precisely controlled release of small molecules, Oxo-M and 4-PPBP, to address this technical challenge. This novel combination of small molecules exhibits distinct functions in modulating the polarization of macrophages and the tenogenic differentiation of tendon stem/progenitor cells (TSCs). The customized, tempora

Calcific tendonitis of the rotator cuff: From formation to resorption.

Calcific tendonitis of the rotator cuff is due to apatite deposits in the shoulder tendons. Patients affected by calcific tendonitis have chronic shoulder pain and disability. Although the disease is frequent, about 10 to 42% of painful shoulders, mechanisms leading to this pathological mineralization are still largely unknown. Research reported in the 1990s suggested that the formation of calcific deposits is linked to cells looking like chondrocytes identified around calcium deposits within a fibrocartilage area. They were considered to be derived from tenocytes but more recently, tendon ste

Allogenous tendon stem/progenitor cells in silk scaffold for functional shoulder repair.

Tendon stem/progenitor cells (TSPCs) were recently identified within tendon tissues. The aim of this study was to investigate TSPC-seeded knitted silk-collagen sponge scaffold for functional shoulder repair. The multidifferentiation potential, proliferation, and immune properties of TSPCs were investigated in vitro, while the efficacy of TSPC-seeded knitted silk-collagen sponge scaffolds in promoting rotator cuff regeneration was evaluated in vivo within a rabbit model. TSPCs, which exhibited universal stem cell characteristics (i.e., clonogenicity, high proliferative capacity, and multidiffer

Adamts5 (aggrecanase-2) is widely expressed in the mouse musculoskeletal system and is induced in specific regions of knee joint explants by inflammatory cytokines.

ADAMTS5 (aggrecanase-2) is an extracellular matrix-degrading protease implicated in cartilage destruction in arthritis. Our goals were to determine expression sites of Adamts5 in the murine musculoskeletal system and in an ex vivo joint inflammation model. In mice with an intragenic LacZ reporter controlled by the Adamts5 promoter, β-galactosidase staining was used to identify Adamts5 expressing cells. Mice expressing one wild-type Adamts5 allele were used to determine distribution of Adamts5 mRNA, cleaved aggrecan and versican, and the ADAMTS5 activating enzymes furin and PACE4. Quantitative

Disruption of Intrinsic Healing Elements Does Not Alter Early Functional or Mechanical Outcomes After Zone II Flexor Tendon Repair in a Large-animal Model.

BACKGROUND: Loss of finger motion after zone II flexor tendon repair remains common and is largely caused by adhesion formation. Although intrinsic vascular and cellular contributions to tendon healing have been examined in canine, avian, and primate models, prior studies have emphasized biochemical or perfusion-based endpoints without comprehensive assessment of functional and mechanical outcomes after clinically relevant repair. Consequently, the influence of intrinsic healing elements on early postoperative function remains unclear. In this study, we employed a novel avian model of intrasyn

Effects of Mechanical Loading on the Structure and Function of the Achilles Tendon: From Homeostatic Adaptation to Pathological Degeneration.

The Achilles tendon, the largest and strongest tendon in the human body, exhibits dynamic adaptive changes in its structure and function through mechanobiological regulation. This review synthesizes the dual regulatory effects of mechanical loading on Achilles tendon homeostasis and pathology: Moderate mechanical stimulation activates integrin-mediated signaling pathways (including PI3K/Akt and MAPK/ERK cascades), promoting tenocyte proliferation/differentiation, collagen biosynthesis, and orderly remodeling of extracellular matrix (ECM), thereby enhancing tendon stiffness, elastic modulus, an

Effect of Diabetes on Tendon Structure and Function: Not Limited to Collagen Crosslinking.

Diabetes mellitus (DM) is associated with musculoskeletal complications-including tendon dysfunction and injury. Patients with DM show altered foot and ankle mechanics that have been attributed to tendon dysfunction as well as impaired recovery post-tendon injury. Despite the problem of DM-related tendon complications, treatment guidelines specific to this population of individuals are lacking. DM impairs tendon structure, function, and healing capacity in tendons throughout the body, but the Achilles tendon is of particular concern and most studied in the diabetic foot. At macroscopic levels,

Biological Treatments for Tendon and Ligament Abnormalities: A Critical Analysis Review.

The poor intrinsic healing potential of tendon and ligamentous tissue has driven the search for biological agents that can improve healing, most notably platelet-rich plasma (PRP) and autologous stem cells. Current evidence best supports the use of PRP as treatment for chronic, degenerative tendinopathies such as lateral epicondylitis of the elbow or patellar tendinopathy. It does not support the use of PRP to promote tendon or ligament-to-bone healing in rotator cuff repair or anterior cruciate ligament (ACL) reconstruction. Clinical evidence regarding the use of autologous stem cells as trea

Neovascularity in chronic posterior tibial tendon insufficiency.

Insufficient posterior tibial tendons in 28 specimens from patients with clinical Stage II or III disease were examined to clarify the etiology of adult-acquired flatfoot deformity. Hematoxylin and eosin and Masson trichrome-stained sections of formalin-fixed tissue were viewed in plain and polarized light. We performed a qualitative analysis for abnormalities in collagen orientation, degree of vascularization, tenocyte cellularity, mucinous change, and chondroid metaplasia. Tendons were divided into three zones: tenosynovial lining cell layer, subtenosynovial lining cell layer, and tendon pro

Optimizing Vancomycin Soaking Protocols for Anterior Cruciate Ligament Reconstruction.

Background/Objectives: Although current guidelines recommend soaking anterior cruciate ligament autografts in 5 mg/mL vancomycin for 20 min to reduce postoperative infections, practical constraints often limit soaking to 5 min. This study aimed to evaluate the bacterial eradication efficacy and cytotoxicity of various vancomycin concentrations and application methods within a 5 min soaking period. Methods: Human semitendinosus tendons were inoculated with one of four bacterial pathogens, including Staphylococcus aureus, Staphylococcus epidermidis with biofilm-producing and non-biofilm-producin

iTRAQ-Based Proteomic Analysis of Spontaneous Achilles Tendon Rupture.

Spontaneous Achilles tendon rupture (SATR) predominantly affects middle-aged and elderly individuals with chronic injuries. However, the exact cause and mechanism of SATR remain elusive, and potential therapeutic intervention or prevention is still insufficient. The present study aimed to uncover the key pathological molecules by using iTRAQ proteomics. The results identified 2432 candidate proteins in SATR patients using iTRAQ proteomic analysis. A total of 307 differentially expressed proteins (DEPs) were identified and linked to 211 KEGG signaling pathways including Coronavirus disease (COV

MSC in Tendon and Joint Disease: The Context-Sensitive Link Between Targets and Therapeutic Mechanisms.

Mesenchymal stromal cells (MSC) represent a promising treatment option for tendon disorders and joint diseases, primarily osteoarthritis. Since MSC are highly context-sensitive to their microenvironment, their therapeutic efficacy is influenced by their tissue-specific pathologically altered targets. These include not only cellular components, such as resident cells and invading immunocompetent cells, but also components of the tissue-characteristic extracellular matrix. Although numerous in vitro models have already shown potential MSC-related mechanisms of action in tendon and joint diseases

Editorial Commentary: Platelet-Rich Plasma Shows Promise for Improving Shoulder Tendinopathy.

Platelet-rich plasma (PRP) injections continue to be used at increasing rates to treat common musculoskeletal conditions. PRP has a low-risk profile and emerging in vitro evidence to support its positive effects on soft-tissue healing. PRP has been shown to be of benefit for knee osteoarthritis, but less has been published regarding the shoulder. PRP delivers a high concentration of growth factors, cytokines, and other important inflammatory modulators. Its use is appealing for treating partial-thickness rotator cuff tears, subacromial bursitis, and rotator cuff tendinopathy since rotator cuff

Lysyl Oxidase Activity Is Required for Ordered Collagen Fibrillogenesis by Tendon Cells.

Lysyl oxidases (LOXs) are a family of copper-dependent oxido-deaminases that can modify the side chain of lysyl residues in collagen and elastin, thereby leading to the spontaneous formation of non-reducible aldehyde-derived interpolypeptide chain cross-links. The consequences of LOX inhibition in producing lathyrism are well documented, but the consequences on collagen fibril formation are less clear. Here we used β-aminoproprionitrile (BAPN) to inhibit LOX in tendon-like constructs (prepared from human tenocytes), which are an experimental model of cell-mediated collagen fibril formation. Th

Primary Cilia Are Required for Efficient BMP Signaling in Traumatic Heterotopic Ossification.

Background/Objectives: Heterotopic ossification (HO), the aberrant formation of bone within soft tissues, arises either from rare genetic mutations or more commonly from traumatic insults. It is a major cause of morbidity not only in individuals harboring causative mutations, but also in those undergoing musculoskeletal surgery or trauma and in soldiers sustaining blast or burn injuries. Bone morphogenetic protein (BMP) signaling is a central driver of both hereditary and acquired forms of HO. Primary cilia are nonmotile, antenna-like organelles that extend from the cell surface and serve as c

MRI and histological features of supraspinatus tendon degeneration in rotator cuff tears: An underexplored perspective.

BackgroundThe supraspinatus muscle and tendon are an integral part of shoulder joint function and stability. The supraspinatus tendon is one of the four tendons forming the rotator cuff in the shoulder. Supraspinatus tendon tear is a frequent lesion that occurs due to various causes like lifting heavy objects, falls onto the shoulder, a dislocation of the shoulder, swimming, trauma, sports injury, occupational activity and degenerative change. Smoking, hypercholesterolemia, and hereditary predisposition are all established risk factors for developing RC tears, which affect the age dependent te

Achilles Tendinopathy.

Achilles tendinopathy is a common cause of disability. Despite the economic and social relevance of the problem, the causes and mechanisms of Achilles tendinopathy remain unclear. Tendon vascularity, gastrocnemius-soleus dysfunction, age, sex, body weight and height, pes cavus, and lateral ankle instability are considered common intrinsic factors. The essence of Achilles tendinopathy is a failed healing response, with haphazard proliferation of tenocytes, some evidence of degeneration in tendon cells and disruption of collagen fibers, and subsequent increase in noncollagenous matrix. Tendinopa
