Cost comparison of arthroscopic rotator cuff repair with arthroscopic vs. open biceps tenodesis.

PURPOSE: To use a nationwide database to determine differences in cost between patients who underwent arthroscopic rotator cuff tear with open vs. arthroscopic biceps tenodesis (BT). METHODS: The 2014 State Ambulatory Surgical and Services Databases from 6 US states was utilized. All cases with CPT codes 29827 (arthroscopic rotator cuff repair [RCR]) and either 23430 (tenodesis of long tendon of biceps) or 29828 (arthroscopic BT) were selected. Cases that included both 23430 and 29828 were excluded, as were those missing demographic data. Generalized linear models were used to model costs bas

Arthroscopic tenodesis through positioning portals to treat proximal lesions of the biceps tendon.

Arthroscopic biceps tenodesis is a good choice for treating proximal lesions of the biceps tendon. However, there are few descriptions of the surgical approach. We introduce a technique for proximal biceps tenodesis using positioning portals and placing suture anchors. Our patients had a minimum of 12 months of follow-up. Between January 2010 and June 2012, a total of 49 patients (21 men, 28 women) underwent arthroscopic biceps tenodesis. The pathology was mainly associated with proximal lesions of the biceps tendon, with the diagnosis confirmed in all patients. Patients were evaluated preoper

Collateral ligament reconstruction of the chronic thumb injury with bio-tenodesis screw fixation.

We describe a new technique for the reconstruction of chronic lesions of the collateral ligaments of the metacarpophalangeal ligaments of the thumb, using a Bio-Tenodesis screw for the fixation of a tendon graft in a triangular manner with proximal apex and allowing early mobilization, starting 2 weeks after operation. Short-term results after 12 to 43 months (mean, 26 mo), showed no significant differences in grip and pinch strength compared with the opposite thumb and restricted loss of motion in the metacarpophalangeal joints (10%) and the interphalangeal joints (4%). We conclude that this

Biomechanical evaluation of open suture anchor fixation versus interference screw for biceps tenodesis.

Biceps tenodesis provides reliable pain relief for patients with biceps tendon abnormality. Previous cadaver studies have shown that, for biceps tenodesis, an interference screw provides biomechanical strength to failure superior to that of suture anchors. This finding has led some providers to conclude that screw fixation for biceps tenodesis is superior to suture anchor fixation. The purpose of the current study was to test the hypothesis that the strength of a 2-suture-anchor technique with closing of the transverse ligament is equal to that of interference screw fixation for biceps tenodes

Functional electrical stimulation reinforced tenodesis effect controlled by myoelectric activity from wrist extensors.

We demonstrated a method for enhancing the tenodesis grip in individuals with sustained tetraplegia at the 6th cervical vertebra neurological level. Subjects used the myoelectric activity from wrist extensor muscles to directly control the electrical stimulation of the extrinsic finger and thumb flexors (flexor pollicis longus and flexor digitorum superficialis/ profondus) with the use of a specially designed system, Myoelectrically Controlled Functional Electrical Stimulation (MeCFES). We screened 20 medical records and selected 9 subjects. Of the nine subjects, five showed a positive respons

Role of lateral extra-articular tenodesis (LET) in anterior cruciate ligament-injured athletes.

Lateral extra-articular tenodesis (LET) has re-emerged as a significant adjunct to anterior cruciate ligament reconstruction (ACLR), particularly in high-level athletes who are at a heightened risk for re-injury and graft failure. This review synthesizes recent literature examining the biomechanical and clinical outcomes associated with LET in the context of ACLR in high-level athletes. Biomechanical studies indicate that LET can enhance knee stability by providing additional restraint against anterior tibial translation and rotatory instability, thereby reducing rotational laxity and offloadi

The Gripping Biceps Knot: All-Arthroscopic Self-Cinching Knot for Proximal Biceps Tenodesis.

Lesions involving the long head of the biceps tendon are a frequent source of anterior shoulder pain and often coexist with other shoulder pathologies such as rotator cuff or labral tears. Although biceps tenodesis has emerged as a preferred treatment over tenotomy-particularly in younger, active individuals-tissue pullout at the suture-tendon interface remains a critical failure point in various fixation methods. This Technical Note introduces the gripping biceps knot, an adaptation of the gripping sailor's hitch, designed to enhance suture-tendon fixation through a self-cinching, intratendon

Lateral Extra-articular Tenodesis With Cortical Suspensory Femoral Fixation and Suture Tape Augmentation.

This article aims to provide a new surgical technique for rotational instability in the setting of anterior cruciate ligament rupture. Two main groups of surgical procedures can be identified in the treatment of anterolateral knee instability: lateral extra-articular tenodesis and anterolateral ligament reconstruction. Although the importance of anterior cruciate ligament reconstruction in anterolateral complex injuries is well known, the superiority of lateral extra-articular tenodesis over anterolateral ligament reconstruction or vice versa has not yet been shown. Both techniques show improv

Lateral Extra-articular Tenodesis: A Technique With an Iliotibial Band Strand Without Implants.

The main goal in anterior cruciate ligament reconstruction (ACLR) should be to restore normal knee biomechanics so the chances of failure decrease. The persistence of knee instability after ACLR goes from 0.7% to 20%. Several factors have been identified and studied, but there are some selected cases in which it seems that without adding lateral extra-articular tenodesis (LET) it is not possible to control rotational instability. Data exist supporting that LET could reduce pivot shift (PS), without losing flexion/extension range of motion nor adding risk of osteoarthritis. Recently, LET has be

Can Tho Transfer Technique: Extensor Hallucis Longus to Tibialis Anterior Tenodesis for Footdrop.

BACKGROUND: Footdrop, or the inability to actively dorsiflex the foot, may result from numerous pathologic conditions, including poliomyelitis and cerebral palsy. Although the gait of patients with footdrop can be improved by performing an extensor hallucis longus (EHL) to tibialis anterior (TA) tendon transfer, the success rate of this procedure is relatively low. METHODS: Seven paralytic patients with footdrop were surgically treated using a new buttonhole-type technique that involves passing a loop of the EHL through a TA split using umbilical tape and suturing at the four corners of the E

Biceps tenodesis versus tenotomy in isolated LHB lesions: a prospective randomized clinical trial.

INTRODUCTION: Currently there exists no clear evidence concerning the surgical treatment of LHB lesions with either tenotomy or tenodesis. The aim of the study is therefore to evaluate elbow flexion and forearm supination force as well as the biceps muscle distalization according to both techniques in isolated LHB lesions. METHODS: Consecutive patients aged 40-70 years with shoulder arthroscopies for isolated SLAP or biceps pulley lesions were prospectively randomized to arthroscopic suprapectoral intraosseous LHB tenodesis or tenotomy.
