Does supportive legislation guarantee access to pregnancy termination and postabortion care services? Findings from a facility census in Central Province, Zambia.

INTRODUCTION: Zambia is one of the few countries in Africa to permit termination of pregnancy (TOP) on a wide range of grounds. However, substantial barriers remain to TOP and postabortion care (PAC). METHODS: We conducted a census of 153 facilities between March and May 2016. We defined facilities according to whether they met basic and/or comprehensive signal functions criteria for TOP and PAC. We linked our facility data to census data to estimate geographic accessibility under different policy scenarios. RESULTS: Overall, 16% of facilities reported they had performed a TOP and 39% perfor

Perceived non-ordinary end-of-life experiences witnessed and reported by a palliative care team accompanying terminally ill patients in a hospice: A qualitative study.

This study examines perceived non-ordinary end-of-life experiences (NOEoLEs) witnessed or reported by the hospice palliative care team and explores their influence on the accompaniment of dying patients, the team's work, and their attitudes toward death and afterlife. A qualitative research design was adopted, including 37 members of the interdisciplinary team from an Italian hospice. According to team members, patients and their relatives frequently report perceived non-ordinary end-of-life experiences (NOEoLEs). While patients typically respond positively to such experiences, relatives may e

Association between quality of life of terminally ill patients and caregiver burden in a Saudi palliative home-care service: a cross-sectional exploratory study.

This study was approved by the Institutional Review Board (IRB) at King Faisal Specialist Hospital and Research Center/Jeddah, Saudi Arabia, given number: IRB 2021-80.BackgroundIn palliative care, patient-reported quality of life (QoL) and caregiver burden are related but conceptually distinct outcomes. Patient QoL reflects the lived experience of advanced illness, whereas caregiver burden reflects the practical, emotional, and social demands of providing care. Although both outcomes have been examined separately, dyadic evidence from Saudi palliative care settings remains limited. This study

Reasons why palliative and terminally ill patients do not die at home with end-of-life home care or in a hospice, but in the hospital: a single-centre study.

BACKGROUND: In the final stages of life, patients discuss their wishes regarding their preferred location of death. However, this preferred location is not always the actual place of death. This study explores why palliative and terminal patients die in the hospital instead of their preferred location. METHODS: This is an exploratory retrospective record review of adult palliative and terminal patients who died in 2023 in Gelre Hospitals Apeldoorn, the Netherlands, while waiting for terminal care at home or in hospices. Data were retrieved from electronic patient records, after which the reas

On dying alone in prison and the social responsibility of medicine: a pilot interview study of physicians caring for terminally ill incarcerated patients in Austria and the United States.

BACKGROUND: This group of physicians is difficult to reach and small in number. While studies of palliative care and end-of-life (EOL) issues in prison have increased, especially in the United States and since the coronavirus disease 2019 (COVID-19) pandemic, they are still limited due to the constraints of carrying out research in carceral contexts. At present, there is very little knowledge of the experiences of physicians providing EOL care in prisons. The aim of this pilot study was to examine the experiences of doctors caring for terminally ill patients inside prisons. METHODS: Three exp

The impact of life orientation on advance care planning engagement in surrogate decision-makers of terminal cancer patients: a chain mediation model of social support and uncertainty of disease.

BACKGROUND: This study aimed to test whether surrogate decision-makers’ (SDMs’) life orientation predicts their advance care planning (ACP) engagement, and whether this effect is mediated by perceived social support and uncertainty about the patient’s illness. METHODS: We conducted a cross-sectional survey of 276 adult SDMs of advanced cancer patients at a Chinese medical center. Participants completed validated Chinese scales for life orientation, social support, uncertainty of disease, and ACP engagement. A serial mediation model tested whether SDMs’ life orientation influenced ACP engagemen

Critical Care-Based Decision-Making and Timing of Pregnancy Termination in Hypertriglyceridemia-Induced Acute Pancreatitis Complicated by Diabetic Ketoacidosis and Intrauterine Fetal Demise: A Case Report.

BACKGROUND: Acute abdominal pain accompanied by severe metabolic acidosis during pregnancy is frequently attributed to pregnancy-related conditions. Hypertriglyceridemia-induced acute pancreatitis (HTG-AP) complicated by diabetic ketoacidosis (DKA) is rare in pregnancy and may be overlooked because of overlapping clinical and laboratory features. CASE PRESENTATION: We report a critically ill pregnant woman in the late second trimester who presented with abdominal pain and vomiting. Initial evaluation revealed severe metabolic acidosis and marked hypertriglyceridemia, and the patient was ultim

Is a Combination of Six Clinical Tests Useful as a Measure to Predict Short-Term Prognosis in Terminal Cancer Patients? A Prospective Observational Study in a Japanese Palliative Care Unit.

BACKGROUND: To address the need for short-term prognostic methods using objective measures, we developed a method to predict a 2- or 3-week prognosis using only six clinical tests (known as the WPCBAL score). However, the method has not yet been directly compared with globally accepted prognostic methods. OBJECTIVES: This study aimed to clarify the usefulness of the WPCBAL score by comparing it with other prediction methods. SETTING/SUBJECTS: A prospective observational study was conducted with patients admitted to the palliative care unit of a Municipal Hospital in Japan between November 20

The effect of temporary closure and enhanced terminal disinfection using aerosolized hydrogen peroxide of an open-bay intensive care unit on the acquisition of extensively drug-resistant Acinetobacter baumannii.

BACKGROUND: At Makassed Hospital's open-bay intensive care unit (ICU), enhanced terminal disinfection (ETD) using hydrogen peroxide (H2O2) was performed without a predefined schedule in extensively-drug-resistant Acinetobacter baumannii (XDR-AB) outbreaks. In this study, we aimed to check for the value of the temporary closure of the ICU and the use of ETD with aerosolized H2O2 and Ag+ on minimizing the rate of XDR-AB acquisition in patients admitted to the ICU of our facility, which might consequently help us determine the optimal schedule for such procedure in this unit. METHODS: This is a
