Pedunculopontine-thalamic cholinergic projections in rapid eye movement sleep behaviour disorder.

Cholinergic system degeneration is a hallmark of Lewy body disease, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB), but its involvement in prodromal stages, particularly regarding pedunculopontine-thalamic projections, remains unclear. This study investigated cholinergic pathway integrity in isolated REM sleep behaviour disorder (iRBD) and its relation to phenoconversion risk. We analyzed data from 146 iRBD patients and 102 controls from the Parkinson's Progression Markers Initiative using T1-weighted MRI for basal forebrain volume and diffusion-weighted imaging for thr

The thalamus: Structure, function, and neurotherapeutics.

The complexity and expansive nature of thalamic research has led to numerous interventions for varied disease states. At the same time, this complexity along with siloed areas of study can hinder a comprehensive understanding. The goal of this paper is to give the reader a broader and more detailed perspective on the thalamus. In order to accomplish this goal, the paper begins with a summary of the function, electrophysiology, and anatomy of the normal thalamus. With this foundation, thalamic involvement in neurological diseases is discussed with a focus on epilepsy. Therapeutic interventions

Thalamic pathology in schizophrenia.

The thalamus plays a critical role in the coordination of information as it passes from region to region within the brain. A disruption of that information flow may give rise to some of the cardinal symptoms of schizophrenia. In support of this hypothesis, schizophrenia-like syndromes emerge when illnesses, such as stroke, selectively damage the thalamus while sparing the rest of the brain. Evidence from many sources has implicated thalamic dysfunction in schizophrenia. In postmortem studies, several subregions of the thalamus, including the mediodorsal nucleus and the pulvinar, have been show

Recovery of thalamic microstructural damage after Shiga toxin 2-associated hemolytic-uremic syndrome.

INTRODUCTION: The underlying pathophysiology of neurological complications in patients with hemolytic-uremic syndrome (HUS) remains unclear. It was recently attributed to a direct cytotoxic effect of Shiga toxin 2 (Stx2) in the thalamus. Conventional MRI of patients with Stx2-caused HUS revealed - despite severe neurological symptoms - only mild alterations if any, mostly in the thalamus. Against this background, we questioned: Does diffusion tensor imaging (DTI) capture the thalamic damage better than conventional MRI? Are neurological symptoms and disease course better reflected by thalamic

Topographic patterns of thalamic infarcts in association with stroke syndromes and aetiologies.

OBJECTIVE: To characterise the topographic patterns of thalamic infarcts associated with various stroke syndromes and aetiologies. METHODS: In this study, 168 consecutive patients with acute infarcts involving the thalamus were investigated by use of diffusion weighted MR imaging. Involved thalamic territories were classified into four vascular territories: anterior (polar artery), posteromedial (thalamoperforating artery), ventrolateral (thalamogeniculate artery) and posterolateral (posterior choroidal artery) territory. The distribution of thalamic infarcts involving specific vascular terri

Fatal familial insomnia: clinical and pathologic study of five new cases.

In 1986, we reported two anatomoclinical observations of a familial condition that we called "fatal familial insomnia" (FFI). We now present the pedigree as well as the clinical and neuropathologic findings in five new subjects. The pedigree includes 288 members from six generations. Men and women are affected in a pattern consistent with an autosomal dominant inheritance. The age of onset of the disease varies between 37 and 61 years; the course averages 13 months with a range of 7 to 25 months. Progressive insomnia (polygraphically proven in two cases); autonomic disturbances including hyper

A systematic review of brain imaging findings in neurological infection with Japanese encephalitis virus compared with Dengue virus.

OBJECTIVES: Japanese encephalitis virus (JEV) and dengue virus (DENV) represent important causes of encephalitis in Asia. Brain imaging may provide diagnostic clues about the etiology of infectious encephalitis. We performed a systematic review of brain imaging findings in Japanese encephalitis (JE) and DENV neurological infection (dengue) to identify characteristic lesions. METHODOLOGY: Five databases were searched. We included all study types and imaging techniques. Laboratory methods were categorized using diagnostic confidence levels. Imaging data were synthesized, and focal findings are

Assessment of paramedian thalamic infarcts: MR imaging, clinical features and prognosis.

Considering the highly variable vascular supply of the thalamic nuclei, MRI and clinical syndromes can be heterogeneous in ischemic diseases. We attempt to determine MRI pattern and to analyse neurological features and prognosis of paramedian infarcts. In a prospective case series within 5 years from 1999 to 2003, MRI, MRA and clinical symptoms of 38 consecutive patients were analysed. The inferomedial (posterior thalamoperforating artery) territory was affected in 89%, and lesions in the anterolateral (tuberothalamic artery) territory occurred in 42%. However, definite attribution to anterola

A Novel Combination of Prion Strain Co-Occurrence in Patients with Sporadic Creutzfeldt-Jakob Disease.

Six subgroups of sporadic Creutzfeldt-Jakob disease have been identified by distinctive clinicopathologic features, genotype at polymorphic codon 129 [methionine (M)/valine (V)] of the PRNP gene, and type of abnormal prion proteins (type 1 or 2). In addition to the pure subgroups, mixed neuropathologic features and the coexistence of two types of abnormal prion proteins in the same patient also have been reported. Here, we found that a portion of the patients previously diagnosed as MM1 had neuropathologic characteristics of the MM2 thalamic form (ie, neuronal loss of the inferior olivary nucl

[Evolutive neuroradiological alterations in Sandhoff's disease].

Sandhoff's disease is a severe form of gangliosidosis GM2 which presents in the first year of life, basically as progressive psychomotor retardation and/or a macular red cherry spot. Our patient presented the clinical picture characteristic of the disease. Diagnosis was confirmed by determining the activity of hexosaminidases A and B in serum and of beta-N-acetil hexosaminases in fibroblast culture. In view of the fatal prognosis of the disease, in 1991 a transplant of alogenic bone marrow (TMO) was carried out to try to replace the enzymes. This required exhaustive radiological follow-up to d

Abnormal thalamic functional connectivity correlates with cardiorespiratory fitness and physical activity in progressive multiple sclerosis.

BACKGROUND: Altered thalamic volumes and resting state (RS) functional connectivity (FC) might be associated with physical activity (PA) and cardiorespiratory fitness (CRF) in people with progressive multiple sclerosis (PMS). OBJECTIVES: To assess thalamic structural and functional alterations and investigate their correlations with PA/CRF levels in people with PMS. METHODS: Seven-day accelerometry and cardiopulmonary exercise testing were used to assess PA/CRF levels in 91 persons with PMS. They underwent 3.0 T structural and RS fMRI acquisition with 37 age/sex-matched healthy controls (HC)

Thalamic nuclei atrophy at high and heterogenous rates during cognitively unimpaired human aging.

The thalamus is a central integration structure in the brain, receiving and distributing information among the cerebral cortex, subcortical structures, and the peripheral nervous system. Prior studies clearly show that the thalamus atrophies in cognitively unimpaired aging. However, the thalamus is comprised of multiple nuclei involved in a wide range of functions, and the age-related atrophy of individual thalamic nuclei remains unknown. Using a recently developed automated method of identifying thalamic nuclei (3T or 7T MRI with white-matter-nulled MPRAGE contrast and THOMAS segmentation) an

Venous hypertension associated with a posterior fossa dural arteriovenous fistula: another cause of bithalamic lesions on MR images.

We report a case of a posterior fossa arteriovenous fistula (AVF) with bithalamic hyperintensity of MR images. The thalamic abnormality improved after surgery, suggesting reversible venous hypertension as the pathogenesis of the finding, as opposed to infarction. This manifestation of a posterior fossa AVF should be considered in the differential diagnosis of bilateral thalamic disease.

The association between neurodegeneration and local complement activation in the thalamus to progressive multiple sclerosis outcome.

The extent of grey matter demyelination and neurodegeneration in the progressive multiple sclerosis (PMS) brains at post-mortem associates with more severe disease. Regional tissue atrophy, especially affecting the cortical and deep grey matter, including the thalamus, is prognostic for poor outcomes. Microglial and complement activation are important in the pathogenesis and contribute to damaging processes that underlie tissue atrophy in PMS. We investigated the extent of pathology and innate immune activation in the thalamus in comparison to cortical grey and white matter in blocks from 21 c

Treatment of functional disorders with gamma knife thalamotomy.

Gamma knife (GK) thalamotomy for functional disorders, primarily Parkinson disease and central pain, are described herein. The goal was to extend our present indications for selective thalamotomy. Our target for tremor surgery is about 45% of the thalamic length. Thus, this principle was applied to deciding the GK thalamotomy target. In most of our cases, the protocol was 130 Gy, delivered in one shot with a 4-mm collimator. The time courses of thalamic lesion changes and clinical improvement after irradiation were assessed. Thus, despite thalamic reaction changes being variable, we achieved a
